Contact dermatitis in hairdressers.

18 cases of hand dermatitis in hairdressers seen over a 5-year period are reviewed. The diagnoses in these patients are discussed with reference to other studies of hand dermatitis in hairdressers. Contact allergy due to paraphenylenediamine and related hair dyes was the presenting complaint in younger hairdressers, while formaldehyde allergy occurred in those who were older. The prognosis in the former group of workers with respect to continued employment in the trade tended to be poorer than the latter. Follow-up revealed that hand dermatitis often resulted in the worker not continuing to work in the hairdressing trade.

Allergic contact dermatitis due to a biocide containing 5-chloro-2-methyl-4-isothiazolin-3-one.

3 cases of contact allergy due to the biocides 5-chloro-2-methyl-4-isothiazolin-3-one and 2-methyl-4-isothiazolin-3-one are reported. 2 workers exposed to a soluble oil containing these 2 biocides experienced allergic contact dermatitis from them. The 3rd case was that of the chemist who separated the 2 materials for the execution of this investigation. These compounds are chemically related to the biologically active components of Ectimar, Skane M-8 and Proxel CRL. Further instances of contact allergy due to these and related compounds are likely because of the apparently increasing use of such substances as antimicrobial agents in various commercial products.

Contact dermatitis in printing tradesmen.

During a 2-year period in Toronto, Canada, 21 printing tradesmen with contact dermatitis were evaluated. 67% had allergic contact dermatitis; 29% due to ultraviolet-cured ink components. Irritant contact dermatitis accounted for 37% of the cases. The prognosis in printing tradesmen with contact dermatitis is guarded, except for those with allergic contact dermatitis due to UV-cured components, as the tradesmen who were sensitized to other contactants eventually left the trade. Offset lithography was associated with the problem in 18 of the 21 cases. A brief outline is given of the printing processes in common use.

Erythema multiforme (Stevens-Johnson syndrome)--chart review of 123 hospitalized patients.

The charts of 123 patients hospitalized with erythema multiforme were reviewed. Antecedent infections and the administration of drugs were associated with 15 and 32% of the cases, respectively. Pyrexia arthralgia, photosensitivity, abnormal liver function tests and abnormal urine sediment were common findings. The length of stay in hospital was 12.6 +/- 0.7 days (mean +/- SE), time to initial improvement after onset was 10.3 +/- 0.6 days and time to complete resolution was 23.0 +/- 2.4 days. The presence of bullous skin lesions and greater overall severity were associated with longer hospitalization. Antecedent respiratory tract symptoms were related to shorter hospitalization. Steroid therapy was not associated with a shorter time for initial improvement to occur, while the average length of stay in hospital was 4.2 days longer than that for the patients not treated with corticosteroid.

Respiratory function and exposure-effect relationships in wood dust-exposed and control workers.

The effect of wood dust exposure on 50 cabinet makers was examined. Woodworkers reported more nasal and eye symptoms and more cough, sputum and wheezing than did 49 control workers. More irritated cells were present in the woodworkers' nasal cytological smears. In contrast to the control workers, the woodworkers had a significant decline in lung function over the workshift. An inverse correlation between baseline lung function and an exposure index (mean area dust level multiplied by length of exposure) was demonstrated in the woodworkers. Greater dust exposure was not associated with larger falls in lung function over the work shift.

Airborne contact urticaria due to sodium benzoate in a pharmaceutical manufacturing plant.

Three workers exposed to airborne contact with sodium benzoate (SB) in a pharmaceutical plant developed transient urticaria related to skin contamination with SB. Patch test responses to SB and benzoic acid (BA), without occlusion, were similar to those of three previously unexposed controls in keeping with the nonimmunologic nature of the reaction. Sweating, which lowers skin pH and increases topical BA concentration, appeared to increase the susceptibility to urticaria in two of the three workers. Ventilation and hygiene control methods designed to reduce SB skin contamination eliminated the problem in the workplace.

Allergic contact dermatitis due to nonylphenol ethoxylate (nonoxynol-6).

The non-ionic emulsifier "nonoxynol-6" found in an industrial waterless hand cleanser induced allergic contact dermatitis on the upper extremities of a uranium mill maintenance worker. The chemical is an irritant for the rabbit. It was not shown to be a cutaneous sensitizer for the albino guinea pit using the guinea pig maximization test.

Tetraethylene glycol diacrylate. A cause of delayed cutaneous irritant reaction and allergic contact dermatitis.

A woman who worked as a materials handler in a plant manufacturing acrylated chemicals developed allergic contact allergy to tetraethylene glycol diacrylate (TTEGDA). Four co-workers experienced skin irritation 12 to 36 hours after skin contact with TTEGDA but had no immediate reaction at the time of exposure. The chemical sensitized the albino guinea pig in the guinea pig maximization test. By varying parameters such as the administration of Freund's complete adjuvent, the intradermal concentration injected for induction, and the diluent used for the injections, it was possible to alter the apparent sensitization potential of TTEGDA.

The effect of the guinea pig maximization protocol on the irritant response to deodorized kerosene - the excited skin syndrome.

Deodorized kerosene in a concentration of 50 g/100 g in pet. did not cause cutaneous inflammation in 10 control guinea pigs. The proportion of 19 guinea pigs exhibiting inflammation following exposure to deodorized kerosene in the guinea pig maximization test increased, but the increase was not significant. The response in a group of 40 animals, half exposed to tetraethyleneglycol diacrylate and half to nonylphenol polyethoxylate-6, increased (z = 3.505, p = 0,004). The inflammatory response was related to concentration in both groups of animals which exhibited inflammatory responses to deodorized kerosene. The relevance of this alteration and irritant response to the interpretation of predictive tests in animals for sensitization, routine patch testing and repeated insult contact dermatitis is discussed.

Allergic contact dermatitis due to urethane acrylate in ultraviolet cured inks.

Seven workers exposed to ultraviolet printing inks developed contact dermatitis. Six cases were allergic and one irritant. A urethane acrylate resin accounted for five cases of sensitisation, one of which was also sensitive to pentaerythritol triacrylate and another also to an epoxy acrylate resin. One instance of allergy to trimethylpropane triacrylate accounted for the sixth case of contact dermatitis in this group of workers. An irritant reaction is presumed to account for the dermatitis in the individual not proved to have cutaneous allergy by patch tests. In this instance trimethylpropane triacrylate was thought to be the most likely irritating agent. Laboratory investigation proved urethane acrylate to be an allergen. The results of investigations of the sensitisation potentials of urethane acrylate, methylmethacrylate, epoxy acrylate resins, toluene-2,4-diisocyanate, and other multifunctional acrylic monomers in the albino guinea pig are presented. The interpretation of such predictive tests is discussed.

Histiocytosis X in two adults. Treatment with topical mechlorethamine.

Two adult patients with skin lesions typical of histiocytosis X were treated topically with mechlorethamine hydrochloride. In the patient whose disease was limited to the skin, a sustained remission was induced, but in the patient with extensive systemic disease, cutaneous remission lasted only six weeks.

Cutaneous granulomatous vasculitis associated with Crohn's disease.

A thirty-nine-year-old man with Crohn's disease of fourteen years' duration developed papules and nodules on his lower extremities which, not infrequently, became purpuric or pustular, and healed with pigmented depressed scars. Histologic examination showed fibrinoid necrosis of the superficial dermal vessels associated with an inflammatory infiltrate consisting mainly of epithelioid histiocytes. Activity of the cutaneous lesions was not correlated with activity of the patient's underlying visceral disease. The cutaneous lesions improved with dapsone therapy combined with prednisone and sulfasalazine. The role of circulating immune complexes in the pathogenesis of vasculitis in Crohn's disease is reviewed.

Airborne irritant contact dermatitis due to sewage sludge.

An outbreaks of cases of airborne irritant contact dermatitis developed among incinerator workers employed in a sewage treatment facility. Contamination of the workplace and workers' clothing by sludge from the interstices of an incinerator exhaust fan proved to be the cause of the problem. The irritancy of the sludge was determined by tests in rabbits. There was no recurrence of the problem after the institution of hygienic measures designed to control contact with the sludge during subsequent fan maintenance procedures.

Allergic contact dermatitis due to an epoxy acrylate.

A printer developed allergic contact dermatitis due to epoxy acrylate resin. Incomplete cross-reactivity between four epoxy acrylate resins and a lack of cross-reactivity to epoxy resin was demonstrated. The epoxy acrylate resin has been shown to be a potent cutaneous sensitizer for the guinea-pig.
