[Carcinoembryonic antigen in pleural effusion. Adenocarcinoma versus mesothelioma].

One hundred and thirty six patients presenting pleural effusion and whose diagnosis was obtained by clinical, biochemical, bacteriological, cytological and histological methods have been studied and classified into seven groups: 20 transudates, 29 inflammatory, 25 tuberculosis, 25 non adenocarcinoid neoplasias, 24 adenocarcinomas, 10 mesotheliomas and 3 miscellaneous. Carcinoembryonic antigen was determined in all of them, evaluating as positives those values above 10 ng/ml, value which we consider discriminatory in our series and which agrees with the literature, finding positive values in 21 out of 24 adenocarcinomas and 3 out of 25 non adenocarcinomas. All benign pleural effusions and mesotheliomas had values lower than 10 ng/ml. The specificity to determine adenocarcinomas in neoplastic effusions was 91.48% and the sensibility was 87.5%. We consider that the CEA determination is useful to differentiate adenocarcinomas from other neoplasias and that a positive value rules out mesothelioma.

Genetic differentiation among natural European populations of Atlantic salmon, Salmo salar L., from drainages of the Atlantic Ocean.

Previously published allelic frequencies at four polymorphic protein coding loci were used as a basis for examining genetic relationships among 19 European populations of Atlantic salmon, Salmo salar L.--exclusive of Baltic drainages--from the Barents Sea to Spain. The data did not support a model of distinct ancestral (e.g. Boreal and Celtic) origins, but were consistent with all populations descending from a single ancestral group within this region with genetically diverged populations drawn together through limited local migrations.

Thromboembolism in patients with pericardial valves in the absence of chronic anticoagulation: 12 years' experience.

Between January 1977 and January 1989, 465 pericardial bioprostheses were implanted in 424 patients. The mean age of patients was 59.1 years (range 16-81 y.) At the time of surgery, 68% of the patients suffered from chronic atrial fibrillation. Mitral valve replacement was performed in 167 patients, aortic valve replacement in 216, multiple replacement in 40 (36 mitral and aortic, 3 mitral and tricuspid, and 1 mitral, aortic and tricuspid), and 1 pulmonary valve replacement. The different types of pericardial valve used were: Ionescu-Shiley 408, Mitral Medical 23, Bioflo 30, and Hancock 4. Hospital mortality was 10.1% with an attrition rate of 1.8 episodes per 100 patients/year. The 12-year actuarial survival rate was 65.1%. No patient underwent long-term anticoagulant treatment. The first 144 patients undergoing mitral and multiple valve replacements received temporary anticoagulation for the first 8 weeks after surgery. There was no valve thrombosis observed. Altogether 19 thromboembolic events (6 early and 13 late) were clinically documented. One patient died after an embolic event. The linearized rates of thromboembolism were 1.64 episodes per 100 patients/year for mitral and multiple valve replacements and 0.33 episodes per 100 patients/year for aortic valve replacement, with an overall rate of 1.0 episodes per 100 patients/year. Excluding early thromboembolism, the linearized rate was 1.02 episodes per 100 patients/year overall. The actuarial freedom from embolism was 92.4% overall, 88.2% for the mitral and multiple valve replacement group, and 97.6% for the aortic valve replacement group at a maximum follow-up of 12 years.(ABSTRACT TRUNCATED AT 250 WORDS)

The new Labcor-Santiago pericardial bioprosthesis.

The Labcor-Santiago pericardial valve has been designed to minimize mechanical stress and to avoid abrasion wear, at the same time preserving the concept of mounting the pericardium outside the posts for full orifice opening. The new design has preserved the traditional superior hemodynamic performance of pericardial valves, avoiding abrasion by careful padding of the stents and introducing a new concept of cusp attachment in order to increase mechanical durability. Since June 1990, 40 patients received 46 Labcor-Santiago valves: 29 in the aortic position and 17 in the mitral position. Mean age was 65.6 years. Eighty percent of the patients were women. New York Heart Association Functional Class III-IV was present in 76% of patients. Hospital mortality has been 12.5%, with 31% undergoing concomitant procedures. No anticoagulation was administered and there has been one thromboembolic event. Echo-Doppler assessment yielded the following in the aortic position: effective orifice area for 19-mm size = 1 cm2; for 21-mm size = 1.2 cm2. Mean systolic gradient for 19-mm size = 14.7 mmHg; for 21-mm size = 11.2 mmHg. In the mitral position: effective orifice area for 27-mm size = 2 cm2; for 29-mm size = 2.3 cm2. Mean diastolic gradients for 27-mm size = 4.6 mmHg; for 29-mm size = 3.3 mmHg. The low-profile flexible stent, the streamlined shape without outside prosthetic material, and the anatomical sewing rim allow a comfortable implantation of the Labcor-Santiago valve, particularly in the small aortic annulus. It is hoped that this new design will contribute to an increase in in vivo mechanical durability.

Intrapopulational variation of olfactory responses in Drosophila melanogaster.

The existence of genetic variability for olfactory response within natural populations of Drosophila melanogaster was studied in two newly collected natural populations from two very different habitats, the Sandiche population, from a very heterogeneous environment, and the Los Areneros population, from a very homogeneous one. Intrapopulational variability was estimated over approximately 50 isofemale lines derived from each population. Results confirm significant differences in olfactory response to ethyl alcohol and acetaldehyde in both populations and to ethyl acetate in the Sandiche population. The differences were due partially to common components of the olfactory responses to different chemical (nonspecifics) in the Sandiche population, but they were specific for the stimulus in the Los Areneros population.

Allozyme polymorphism at the alpha Gpdh and Adh loci and fitness in Drosophila melanogaster.

We have studied in Drosophila melanogaster the effects of allozyme variation at the alpha Gpdh and Adh loci on the following fitness components: female fecundity, egg hatchability, egg-to-adult survival under near-optimal and competitive conditions, rate of development under near-optimal and competitive conditions, and mating capacity of males. Significant effects of the alpha Gpdh locus on rate of development under competitive conditions (SS greater than FS greater than SS), and of the Adh locus on egg hatchability (FF = SS greater than FS) and egg-to-adult survival under competitive conditions (FF greater than SS = FS) were revealed. Possible natural selection mechanisms involved in the maintenance of allozyme polymorphisms at the alpha Gpdh and Adh loci are described. These mechanisms and the fitness of In(2L)t may account for the persistence and clinal distribution of the two allozyme polymorphisms in nature.

[Endocarditis with Ionescu-Shiley's bioprosthesis. Long-term results].

With the aim to attesting the ability of treated pericardial tissue to resist infection we analyzed the incidence of prosthetic valve endocarditis in 541 operative survivors with the Ionescu-Shiley (I-S) valve since January 1977. A total of 580 valves were available for the study, 21 (3.8%) sustained I-S endocarditis or 0.65% patients/annum. The mean age was 53.2 years; male/female ratio was 16/5. Native endocarditis was present in 20 cases (3.6%). There were 13 aortic endocarditis (2 in cases of multiple replacement) and eight mitral. The interval between insertion and endocarditis ranged from 2 weeks to 108 months (means 31); 8 cases were early I-S endocarditis (38%) and 13 late endocarditis (62%). The organisms isolated were: Staphylococcus (epidermidis, aureus) in 10 cases. Streptococcus (viridans, fecalis) in 7 cases, gram negative in three and no organism was cultured in 3 cases. The outcome was as follows: specific medical treatment in 4 cases (one early), valve re-replacement in 11 (four early) from whom eight survived, and 6 patients died with unspecific medical treatment. The total mortality rate 9/21 (43%), in early endocarditis 5/8 (62.5%) and in late endocarditis 5/13 (38%). Operative and necropsy findings in 12/21 showed always periprosthetic abscess but no leaflet perforations. The rate of I-S valve endocarditis compares favourably with other valve substitutes. Early re-replacement combined with adequate antibiotic treatment is mandatory according to our experience.

Cerebral hemisphere swelling in severe head injury patients.

The clinical course and the intracranial pressure (ICP) changes in 66 severe head injury patients presenting bulk enlargement of one cerebral hemisphere within a few hours of trauma have been analyzed. These patients represent 11% of a series of 589 severe head injury cases studied with computerized tomography (CT). Cerebral hemisphere swelling, which was associated with an ipsilateral subdural haematoma of variable extent in 58 patients (88%), or a large epidural haematoma in 5 patients (7%), and occurred as an isolated lesion in 3 patients (4%), carried the highest incidence of uncontrollable intracranial hypertension, the highest mortality rate and the shortest survival period after trauma in the authors' severe head injury series. The high incidence of arterial hypotension and/or hypoxaemia at admission (48% of cases), and the severity of clinical presentation (82%) of patients scored 5 patients or less in the Glasgow Coma Scale, 77% had uni- or bilateral mydriasis and 82% initial ICP above normal limits) correlated with the very poor final outcome (85% mortality). Only one of the 12 patients with normal initial ICP continued to have low pressure throughout the course. High dose thiopental failed to control severe intracranial hypertension in 29 patients (44%) who had a fulminant, malignant course. A transient decrease in ICP elevation was achieved in 17 patients (26%) and a definitive control in 12 patients (18%), among them the 10 survivors in this series. In the authors experience once ICP is controlled, and unless haemodynamic instability compells action to the contrary, barbiturate should not be discontinued until a control CT scan shows complete disappearance of the mass effect.

Genetical analysis of intrapopulational variation in olfactory response in Drosophila melanogaster.

Genetical study of olfactory response to ethyl alcohol (8 per cent) has been carried out on inbred lines extracted from a natural population using biometrical and chromosome analyses. A full diallel analysis was made in seven endogamic lines (3 high, 3 low and 1 intermediate in response values). Our results show a highly significant (P less than 0.001) additive component of variance and a less significant (P less than 0.05) directional dominance component suggesting overdominance and some endogamic depression. No significant maternal or reciprocal components of variation appear in our population. Two chromosome substitution assays among 3 inbred lines of extreme olfactory response add up information to the results of diallel analysis. The differences in response are due to chromosome III with an additive effect highly significant (P less than 0.001) in both studies. The other major chromosomes do not show any effect themselves but some interactions with chromosome III are significant. They can explain the directional dominance that we find in the diallel study. Analysed olfactory variation in response to ethanol can be considered as unspecific in relation to various chemical compounds.

[Ring chromosome 18 46,XY,r(18)].

Authors report a ring chromosome 18 (18 r) in a four year old boy, with low birth weight, retarded growth and development, microcephaly and plagiocephaly, horizontal nystagmus, ambiguous genitalia, clinodactyly of the fifth finger, distal axial triradius, whorls pattern in 8 fingers in dermatoglyphic. Serum IgA is lower than 3 mg/dl. Parents karyotype is normal.
