The treatment and prognosis of patients with phyllodes tumor of the breast: an analysis of 170 cases.

BACKGROUND: The study addresses the controversial prognostic and therapeutic aspects of phyllodes tumor of the breast. METHODS: Records of 170 women with phyllodes tumor of the breast were reviewed. On the basis of the criteria proposed by Azzopardi and Salvadori et al., including estimation of tumor margin, growth of the connective tissue component, mitoses, and cellular atypia, the entire series was divided into three histotypes of phyllodes tumor, i.e., benign (92 cases, 54.1%), borderline (19 cases, 11.2%), and malignant (59 cases, 34.7%). Ninety-eight patients (57.6%) were treated by wide local excision (79 benign, 15 borderline, and 4 malignant), 43 (25.3%) by simple mastectomy (13 benign, 4 borderline, and 26 malignant), and 29 (17.1%) by radical mastectomy (all malignant). RESULTS: Of the 170 treated patients, 141 (82.9%) survived 5 years without evidence of disease. In the Cox multivariate analysis the histotype of the tumor was the only independent prognostic factor: 5-year NED survival was observed in 95.7% of the patients with benign phyllodes tumor, 73.7% with borderline phyllodes tumor, and 66.1% with malignant phyllodes tumor. After a wide local excision 98.7% of the patients with benign tumor, and 80% with borderline tumor, were cured. Local recurrence was found in 14 patients (8.2%) (4 benign, 3 borderline, and 7 malignant); 10 of these underwent reoperation (7 wide local excision, 3 radical mastectomy) and survived 5 years NED. CONCLUSIONS: The histotype of phyllodes tumor (benign, borderline, and malignant), assessed on the basis of the criteria proposed by Azzopardi and Salvadori et al., was the only prognostic factor in our group of patients. Based on the data from literature and our own observations, we observed that a wide local excision, with an adequate margin of normal breast tissue, is the preferred initial therapy for phyllodes tumor of the breast.

Expression of multidrug-resistance-associated protein gene in human soft-tissue sarcomas.

We examined the mRNA expression of the multidrug- resistance-associated protein gene (MRP) in soft-tissue sarcomas and compared it with the expression of the multidrug resistance gene (MDR1), using the reverse transcriptase/polymerase chain reaction. We investigate 39 samples from 33 cases of soft-tissue sarcomas (11 liposarcomas, 9 malignant fibrous histiocytomas, 6 leiomyosarcomas, 4 malignant schwannomas, 3 fibrosarcomas, 3 synovial sarcomas, and 3 epithelioid sarcomas) and 7 benign soft-tissue tumors. All samples were obtained prior to chemotherapy. An expression of MRP mRNA was noted in 56% of soft-tissue sarcoma specimens. The co-expression of MRP and MDR1 was recognized in 15 samples (38%) (5/11 liposarcomas, 5/9 malignant fibrous histiocytomas, 3/6 leiomyosarcomas, 2/3 fibrosarcomas) and significantly correlated with histological grade (P=0.0165). A positive and significant correlation was found between MRP and MDR1 expression in soft-tissue sarcomas(P=0.0013). In benign soft-tissue tumors, 1 chemodectoma and 1 neurothekeoma showed low MRP expression; however, no case showed co-expression of MRP and MDR1.

Reverse transcriptase-polymerase chain reaction amplification of MDR1 gene expression in adult soft tissue sarcomas.

Expression of the multidrug resistance gene MDR1 is reported to be an important determinant of the response to chemotherapy and survival in some cancers. We compared three methods for determining the intrinsic MDR1 expression in soft tissue sarcomas. We studied MDR1 gene expression in 39 samples from 33 cases of soft tissue sarcomas comprising 11 liposarcomas, nine malignant fibrous histiocytomas, six leiomyosarcomas, four malignant schwannomas, three fibrosarcomas, three synovial sarcomas, and three epithelioid sarcomas, and seven cases of benign soft tissue tumors in adult patients. To detect MDR1 mRNA, reverse transcriptase-polymerase chain reaction (RT-PCR) was performed in all samples. Furthermore, RNA dot-blot analysis with digoxigenin-labeled RNA probe and immunohistochemistry with JSB-1 and C-219 antibodies for P-glycoprotein were employed in 34 and 37 samples in soft tissue sarcomas, respectively. We compared these three detection techniques. Of the 39 specimens, 18 (46%) showed MDR1 PCR products. Liposarcomas (six of 11), malignant fibrous histiocytomas (six of nine), leiomyosarcomas (four of six), fibrosarcomas (two of three) revealed high or intermediate MDR1 expression at high frequency. No MDR1 expression was detectable in malignant schwannomas, synovial sarcomas, or epithelioid sarcomas. Of seven benign soft tissue tumors, one ganglioneuroma and one lipomatosis showed low levels of MDR1 expression. By RNA dot-blot analysis, MDR1 transcripts were detectable in 12 of 34 specimens (35%). Four samples were negative by dot blot despite positivity with RT-PCR. Concordance between MDR1 expression by RNA level with RT-PCR and dot blot and at the protein level with immunohistochemistry using C-219 was found in 16 (47%) of the 34 comparable specimens. Eight samples showed positive immunoreactivity for C-219 despite negative results in RT-PCR and dot-blot analysis. The intrinsic MDR1 expression in soft tissue sarcoma seemed to depend on certain tumor types, such as liposarcoma, malignant fibrous histiocytoma, leiomyosarcoma, and fibrosarcoma. For the evaluation of MDR1 expression, RT-PCR is useful because of its relative simplicity and sensitivity. However, the clinical significance of such low levels of MDR1 expression detected only by RT-PCR must be discussed within systematically treated patient groups.

Flow cytometric DNA-analysis of cells obtained with fine needle aspiration biopsy of the breast.

In 221 consecutive, unselected palpable breast lesions, evaluated by aspiration biopsy, flow cytometric DNA analysis was performed on cell material remaining in needle and syringe after smears had been prepared. DNA histograms were obtained in 186 cases, whereas in 44 samples the cell number was too low for detailed estimation of cell kinetics. Aneuploidy was observed in 71 of 90 samples where cancer cells were detected by cytology, in four of 20 samples where breast cancer could only be suspected and in 12 of those 76 where no cancer cells occurred in cytologic specimens. The average S-phase and S+G2M values were higher in aneuploid than in diploid cell samples. In the group of 40 cases (34 breast cancers, 6 benign lesions) comparative analysis of DNA histograms obtained on aspirates and tissue fragments revealed a concordance in 88.9%. Modified radical mastectomy was performed in 41 breast cancers and in 33 cases ductal infiltrating carcinomas were recognized by histology and scored according to Bloom-Richardson score. Cell features assessed by cytology in these cases did not correlate with histology. In most cases, however, nuclear polymorphism and numerous naked nuclei were present in cytologic specimens. These alterations, characterized as score 3, reflect values of DNA-index in compartment of 1.4-1.8.

Flow cytometric evaluation of Ki67 proliferative antigen in primary breast cancer.

In the group of forty five primary mammary carcinomas Ki67 reactivity was evaluated with flow cytometry and immunohistochemistry. The expression of proliferative antigen, evaluated with both methods did not correlate with other indicators of proliferative activity. This result is probably associated with the physicochemical properties of breast cancer cells.

Relationship of histology, DNA-values and proliferative activity in unselected breast cancer patients (a preliminary study).

Based upon a group of 108 consecutive mammary carcinomas a comparative analysis of morphological parameters, DNA-ploidy and indicators of proliferation activity was made. A correlation between Bloom-Richardson scale, mitotic index, PCNA-labeling index and DNA-index was shown. The ploidy of mammary carcinomas was significantly related to the values of proliferative fraction, as well as to PCNA-labeling index. Among patients with axillary metastases the tumor size and the value of PCNA-labeling index were significantly higher than those in patients with negative lymph nodes.

Flow cytometric analysis of low grade B-cell non-Hodgkin's lymphoma: correlation between histology, immunophenotyping and cell cycle analysis.

In the present study the immunophenotyping and the DNA parameters in the group of 38 patients with B-cell non-Hodgkin's lymphoma of low grade malignancy were evaluated in relation to the Kiel Classification. The study was performed with flow cytometry. The evaluation of the FSC/SSC light scatters enables selective immunophenotyping and also corresponds to tumor histology. The lymphoma cells reactivity with selected anti-lymphocyte antibodies depended on their histological type, however no characteristic phenotype, except for lymphocytic lymphoma, for each lymphoma type could be distinguished. Low grade lymphomas were characterized, in general, by low proliferative activity. The differences in percentage of cells in S-phase fraction within selected groups of lymphoma could be a valuable indicator of the clinical outcome.

Flow cytometric analysis of high grade B-cell non-Hodgkin's lymphoma: correlation between histology, immunophenotyping and cell cycle analysis.

The immunophenotype and the DNA parameters of 21 high grade B-cell non-Hodgkin's lymphomas considered in terms of the Kiel Classification were studied by flow cytometry. The centroblastic and immunoblastic lymphomas differed in the expression of lymphocyte markers, however no characteristic immunophenotype for each lymphoma type could be distinguished. Assessment of the FSC/SSC dot plots with histology enables to distinguish subpopulations of the lymphoma cells. The immunoblastic and centroblastic lymphomas were characterized by high proliferative activity although no significant differences in S-phase values between both groups could be found.

Flow cytometric analysis of immunophenotypic and DNA values in non-Hodgkin's B-cell lymphomas classified according to the Kiel Classification and Working Formulation.

B-cell non-Hodgkin's lymphomas classified previously according to the Kiel Classification were reclassified according to the Working Formulation. The comparative analysis of flow cytometric data was done for these two classification systems. It was established that flow cytometric data describe defined groups of malignancy more precisely in the Working Formulation, especially in case of the low grade lymphomas.

Comparative flow cytometric DNA analysis on fresh and paraffin-embedded tumor tissue in different neoplasms.

The results of flow cytometric DNA-analysis on fresh and paraffin-embedded tissue were compared in four various groups of tumors. The best correlation of the results was observed in the group of non Hodgkin's lymphomas. Comparative analysis of the ploidy on fresh and paraffin embedded tissue demonstrated discordance in particular in mammary carcinoma and malignant melanoma groups. The values of proliferative fraction assessed on fresh and paraffin embedded tissues were interrelated.

Retrospective analysis of prognostic significance of the estrogen-inducible pS2 gene in male breast carcinoma.

BACKGROUND: The estrogen-inducible pS2 gene, originally isolated from a breast cancer cell line, is correlated with hormone-dependent female breast tumors and its expression is associated with longer overall and disease-free survival. METHODS: The authors have investigated 38 samples of carcinomas of the male breast for pS2 expression by using a monoclonal antibody. The immunostaining was compared with clinical data, in particular, to the progesterone receptor status, to assess a possible prognostic value of this parameter. RESULTS: Although most cases (27 of 38) were immunopositive (i.e., above the 5th percentile of immunoreactive cells), no correlation with tumor grade and survival was notable. CONCLUSIONS: Therefore, on the contrary to the situation in female breast cancer, pS2 activity failed to constitute a new prognostic parameter in male breast carcinomas.

Ultrastructure of diffuse malignant mesothelioma of the pleura. An analysis of ten cases.

An electron-microscopic examination of ten cases of malignant pleural mesothelioma is presented. The most characteristic features for diagnosis are: presence of microvilli and desmosomes, abundant intermediate filaments and direct contact between microvilli and collagen fibres. Microvilli were present not only on the luminal surface of the cells, but also on the abluminal surfaces. Differentiation towards the cells characteristic for rhabdomyosarcoma was found in one case.

Synovial sarcoma: a new approach to the histological classification and a comparison with malignant mesothelioma. Immunohistochemical study with anti-mesothelial cell serum.

Routine paraffin-embedded sections from synovial sarcomas in 70 patients were stained immunohistochemically with monoclonal anti-cytokeratin antibody and serum against mesothelial cells. Positive reactions occurred in the epithelioid cells of biphasic tumors only. On the ground of the reaction with anti-mesothelial serum a hypothetical scheme of cell differentiation in synovial sarcoma was set up.

Alveolar soft part sarcoma: an analysis of 8 cases. Review of the literature.

Eight cases of alveolar soft part carcinoma (ASPS) are presented. The average age of the patients was 28 years. There were 6 women and 2 men. The left sided and right sided tumours were equal in number. Local recurrence was not observed in any of the patient treated, but lung metastases occurred in 6 of them. Three patients died with disease the average 35 months after treatment. Histologically in one case the uniform sheets of cells were observed without alveolar arrangement. The presence of beta glucuronidase (BG), acid phosphatase (AP) and non specific esterase (NE) in tumor cells was seen in one case studied. Immunohistochemical and ultrastructural examination confirm the thesis, that ASPS consist a distinct clinico-morphologic entity, the histogenesis of which remains uncertain.

[Morphological and clinical evaluation of non-epithelial and mixed neoplasms of the female reproductive organs. II. Leiomyosarcoma].

Among 12886 malignant neoplasms of female sex organ 34 leiomyosarcoma were found. Average age of suffering women was 50 years. Metrorrhagia and lower abdominal pain were most common symptoms reported. Surgical treatment was supplemented with radiotherapy. Significant impact on prognosis showed: number of mitoses, number of bizarre and giant cells, massive necrosis and hyalinization. Efficient early diagnosis these neoplasms still awaits improvement.

Pleural mesothelioma. Morphological characteristics of 17 cases.

Pleural mesotheliomas are rare tumors. Thus, only few reports are based on a material sufficient for reviewing the whole spectrum of pathomorphological features. This has stimulated this review of morphological characteristics of 17 pleural mesotheliomas.

Mesenchymoma malignum of the buttock.

A case of a soft-tissue tumor of the left gluteal region in a 33-year-old woman is presented. Immunohistochemical study and electron microscopic examination confirmed multidirectional differentiation of the tumor. The patient died 9 months after surgery, despite complementary multimodal therapy. No autopsy was performed.
