Percutaneous transluminal angioplasty in renovascular hypertension.

The results of percutaneous transluminal angioplasty of renal arteries (PTA) were evaluated in 43 patients followed up for 2-39 months (mean: 14 months). The benefit of PTA was higher (82%) in hypertensives with arterial fibrodysplasia (n = 11) than in atherosclerotic vascular lesions (53%, n = 19). Of 15 patients with reduced renal function before PTA, the glomerular filtration rate rose in seven. A major complication was in one patient arterial dissection and in another one loss of function of the transplanted kidney due to acute tubular necrosis. The results confirm that PTA is an important contribution to the treatment of renovascular hypertension.

Hypertension rate in renal graft recipients operated on at the Transplantation Centre, IKEM, in 1976-1981.

A group of 198 renal transplantations performed et our Centre in 1976-1981 was evaluated. Arterial hypertension within the first two weeks following transplantation developed in 83% of patients, in 43% being severe. Later, towards the end of the first year, less patients suffered from arterial hypertension, especially of severe hypertension. In this period, hypertension occurred in about 63%, in most cases in the mild form (50%). Patients with graft functioning for more than a year had hypertension, mostly mild, in about 43%. The incidence of hypertension in our group was, however, increased if "normotensives" taking antihypertensives were included. We may conclude that in the first year following renal transplantation true normotensive patients formed approximately 10% of the total and their number in the subsequent years raised up to 50%. The values of blood pressure in the 3rd to 4th months had a predictive significance for the further course of hypertension. In this period, patients with hypertension had a markedly worse cumulative graft survival compared with the remaining subjects. This finding supports the importance of energetic antihypertensive therapy.

Lecithin cholesterol acyltransferase activity in chronic nephropathies.

The authors present data from a group of 55 patients suffering from chronic renal diseases. Twenty-two were treated by haemodialysis; the rest had serum creatinine levels ranging from normal to 950 mumol/l. The molar esterification rats [MER] and total cholesterol [TCH] values decreased parallel to the gradual extinction of renal function. A reduced fractional esterification rate [FER] was found in about two thirds of the dialysed patients and in about half of those whose kidney function was still relatively well preserved. It can thus be concluded that reduction of FER values indicative of a disturbance of cholesterol metabolism can already be detected in the early stages of chronic nephropathies.

Effect of renal transplantation on lecithin cholesterol acetyltransferase (LCAT) activity and plasma lipids.

The rate of free cholesterol esterification, assessed as lecithin cholesterol acyltransferase (LCAT E.C. 2.3.1.43) activity, and cholesterol as well as triglycerides plasma levels, were determined in 45 patients who had been operated for kidney transplantation. An increased concentration of cholesterol and triglycerides was found in 25% and 31% of tests, respectively, whereas subnormal molar esterification rate (MER) could be detected in 74%, and a reduced cholesterol turnover in the endogenous pool (fractional esterification rate - FER) in as many as 95% of cases. Examination of different time intervals after transplantation surgery revealed that the lowest rate of esterification, as well as the lowest concentration of cholesterol was consistently found early after transplantation (within 10--15 days). The values subsequently rose but even 6--24 months after a successful kidney transplantation the molar and fractional esterification rates of cholesterol in most of the patients did not attain the levels of the healthy control subjects. If the function of the grafted kidney failed and the patient had to be returned to the haemodialysis treatment programme, all examined parameters remained permanently low. These results suggest that a disturbance in cholesterol metabolism accompanies an apparently satisfactory functioning of the transplanted kidney, even though it need not necessarily be reflected in increased plasma cholesterol concentrations.

Spironolactones and glomerular filtration.

For pharmacologic screening to identify hyperaldosteronism in patients after renal transplantation, spironolactone (Soludactone, Searle) was administered intravenously in 12 subjects (mean dose 1.2 g in the course of 3 days). After its administration a decrease in glomerular filtration rate was assessed when measured by chromogen clearance. Simultaneous measurement of creatinine showed no change, nor was there any change in the plasma level and excretion fraction of urea. The decrease in chromogen clearance was due to an increase in plasma level of the chromogen. Thus the decrease in its clearance was an artifact resulting most probably from the reaction of spironolactone metabolites with alkaline picrate, which was used for the determination of chromogen. In addition to the causes of the glomerular filtration rate decrease after spironolactone which have been reported in the literature - volume depletion and direct influence on the kidneys - the possibility of a fictive decrease must be taken into consideration.
