Percutaneous pericardial biopsy: technique, efficacy, safety, and value in the management of pericardial effusion in children and adolescents.

This study describes our technique of nonsurgical percutaneous pericardial biopsy (PPB), its efficacy and safety, and its diagnostic and therapeutic usefulness in the management of pericardial effusion (PE) in children and adolescents. In an 8-year period, 19 selected patients, aged 2 to 20 years, had PPB for evaluation and treatment of PE. Using this procedure we procured multiple biopsy specimens from various areas of the thickened parietal pericardium, and also procured pericardial fluid for investigation. The biopsy material helped establish a specific etiologic diagnosis in 63% and helped exclude tuberculosis and malignancy in the rest (37%). The histopathologic diagnoses were tuberculous pericarditis (37%), uremic pericarditis (16%), radiation pericarditis (5%), connective tissue disease (5%), and idiopathic pericarditis (37%). In tuberculous pericarditis, the biopsy specimen provided an immediate histopathologic diagnosis, and a higher culture positivity (71%) than pericardial fluid (29%). In addition, the procedure was therapeutically effective in relieving tamponade and in treating effusion. Our experience indicates that the PPB procedure has both diagnostic and therapeutic value, and it can be performed easily, safely, and without complications even in small children.

Lipoid proteinosis: report of four siblings and brief review of the literature.

Lipoid proteinosis (Urbach-Wiethe disease) is a rare autosomal recessive disorder associated with deposition of periodic acid-Schiff (PAS)-positive hyaline material in various tissues including skin, mucous membranes, and internal organs. A family is reported in which four siblings (two boys and two girls) born to nonconsanguineous parents had lipoid proteinosis. All had the characteristic hoarseness of voice and three had skin lesions. The diagnosis was confirmed by the presence of typical features on light and electron microscopy.

Leiomyosarcoma complicating chronic inflammation of the testis.

OBJECTIVE: To report on a case of leiomyosarcoma of the testis that appeared to have arisen from a background of chronic testicular inflammation. CLINICAL PRESENTATION: A 65-year-old man with a 15-year history of diabetes mellitus and low-grade discomfort and swelling in the right testis presented as an emergency with exacerbation of the pain and swelling of the testis. Repeated ultrasound examination of the testis in the past 5 years had suggested a chronic testicular inflammatory disorder. Ultrasound during the current emergency case admission revealed a normal left testis, but a large heterogeneous solid mass with a moderate intratesticular calcification in the right testis and the presence of a moderate hydrocele. Serum alpha-fetoprotein and beta-human chorionic gonadotropin were normal. A right radical orchidectomy was performed. Histopathology and immunohistochemistry revealed primary leiomyosarcoma of the right testis. There was no spermatic cord or venous involvement. One year after orchidectomy there was no sign of metastasis. CONCLUSION: Radical orchidectomy followed by surveillance appears to be the treatment of choice for this testicular leiomyosarcoma, which seemed to have run an indolent course compared to other testicular tumours.

Giant cell arteritis of the breast.

OBJECTIVE: To report a rare case of giant cell arteritis of the breast in a 56-year-old woman. CLINICAL PRESENTATION AND INTERVENTION: The patient presented with tenderness in the upper outer quadrant of the left breast, in the absence of any constitutional symptoms. A diagnosis of giant cell arteritis was made only after surgical wedge excision of the affected breast quadrant. She returned 6 weeks later with tenderness in the upper inner medial quadrant of the same breast, with a raised erythrocyte sedimentation rate. She has since been referred to the rheumatologist for further management. CONCLUSION: The diagnosis of giant cell arteritis of the breast was made only after pathological examination of the excised breast tissue. This study focused attention on the need to be aware of systemic conditions that may present as localised lesions of the breast and to maintain a high index of clinical suspicion in order to adequately investigate them and perhaps avoid unnecessary radical surgical intervention such as mastectomy.

BRCA1 gene expression in breast cancer in Kuwait: correlation with prognostic parameters.

OBJECTIVES: This study assessed the BRCA1 gene expression in breast cancer in Kuwait, and compared it with other known prognostic factors for the disease. MATERIALS AND METHODS: Forty-eight random samples of archival paraffin-embedded breast cancer tissues were studied for BRCA1 gene expression. Immunohistochemical method utilizing antibodies against different epitopes on the BRCA1 protein was used to study BRCA1 protein expression. In addition, for 29 patients, reverse transcription-polymerase chain reaction was used to detect BRCA1 mRNA expression. BRCA1 expression was correlated with age, histological type and grade of breast cancer, estrogen and progesterone receptor status, and C-erbB-2 expression. RESULTS: No demonstrable BRCA1 mRNA and protein expression was found in 79 and 83% of the breast cancer tissues, respectively. A positive relationship was demonstrated between lack of BRCA1 (mRNA and protein) expression and high histological grade, negative estrogen and progesterone receptor status, and overexpression of C-erbB-2 in the breast cancer tissues. CONCLUSIONS: The study demonstrated lack of BRCA1 gene expression (mRNA and protein) in the majority of breast cancers in Kuwait and confirmed the inverse relationship between BRCA1 expression and parameters that determine poor prognosis in breast cancer.

Langerhans cell histiocytosis of the thyroid gland. A case report.

BACKGROUND: The variant of Langerhans histiocytosis commonly encountered in adults is the benign, localized form (eosinophilic granuloma). The more-aggressive or diffuse type (Letterer-Siwe disease) is rare in adults. CASE: A 28-year-old woman presented with enlargement of the thyroid gland three years after she had been diagnosed with and placed on treatment for diabetes insipidus. Thyroidectomy was performed following an initial fine needle aspiration cytology report of either papillary thyroid carcinoma or Langerhans cell histiocytosis. The latter diagnosis was confirmed on histopathology and immunohistochemical staining for S-100 protein. Intracellular Birbeck granules were also demonstrated by electron microscopy. The disease progressed over a 10-week period to involve the kidneys, resulting in renal dysfunction. CONCLUSION: Synchronous or metachronous involvement of the hypothalamus and thyroid gland by Langerhans histiocytosis could not be excluded in the present case, with subsequent progression to involve other organs. This was an unusual presentation of the disease in an adult.

Chondroblastoma of temporal bone: unusual histologic features.

Chondroblastoma of bone only rarely affects the skull and, depending on its location, may have an uncertain prognosis. The histologic spectrum demonstrated by this tumor has been a source of diagnostic confusion, which still persists. A case of chondroblastoma of the temporal bone with unusual histologic features is presented and discussed. Chondroblastoma must be included in the differential diagnosis of bone tumors of the skull containing giant cells. Radical excision is suggested as the treatment of choice in view of unanswered questions regarding its tendency to recur or to progress to frank malignancy.

Nasopharyngeal carcinoma in Accra.

A higher prevalence of nasopharyngeal carcinoma (NPC) among Ghanaian children, adolescents and young adults is demonstrated in this study. In view of the known association between Epstein-Barr virus (EBV) and NPC on the one hand, and the virus and Burkitt's lymphoma (BL) on the other, it is suggested that the occurrence of both malignancies in younger subjects in this study points to the possible role of early infection with EBV in the aetiopathogenesis of NPC.

Nasopharyngeal carcinoma. A comparative study.

A comparative study of Ghanaian and Saudi hospital cases of nasopharyngeal carcinoma (NPC) shows a higher occurrence among younger subjects in the Ghanaian group, with the peak age incidence in the Saudi group being similar to those reported from other parts of the world. In view of the known high prevalence of Burkitt's lymphoma among Ghanaian children, it is suggested that the much higher proportion of younger Ghanaian subjects with NPC may be related to early exposure to the Epstein-Barr virus which is known to be associated with both malignancies.

Histiocytic necrotising lymphadenitis (Kikuchi-Fujimoto disease) in Saudi Arabia.

Five cases of histiocytic necrotising lymphadenitis (Kikuchi-Fujimoto disease) are described. As the condition appears to be poorly recognised in Saudi Arabia, an extensive review of the histological features is presented, in order to improve pathologists' awareness and to minimise the risk of diagnosing prognostically less favourable conditions such as malignant lymphoma. Etiologic and pathogenetic features of the disease are also discussed.

Tuberculosis of the tonsil revisited.

With the advent of effective antituberculous therapy and pasteurisation of cow's milk, there has been considerable reduction in the worldwide prevalence of tuberculosis. Although tuberculosis of the tonsil is now an uncommon finding, tonsillar granulomata are occasionally seen by histopathologists and rarely, some contain tuberculous organisms. One such case has prompted us to examine the tonsils of seventeen cases of open pulmonary tuberculosis complaining of sore throat, dysphagia or other pharyngeal symptoms. No granulomata were demonstrated in fourteen who yielded representative tonsillar tissue, underlining the rarity of association between secondary pulmonary tuberculosis and tonsillar involvement.
