[Detection of circulating immune complexes using a laser-nephelometric assay (author's transl)].

Using Laser nephelometric measurement a method is described to detect immune complexes, which seems to be suitable for routine studies as well as for specific complex characterisation. Tetanus-Antitetanus complexes are used as standard thus the result is expressed as tetanus complex units (TCU). The method is simple and permits the direct measurement of complexes in native sera. Furthermore it is possible to characterise the complexes either by the immuno-globuline class of the antibody involved or the antigen respectively. Complexes were found with this method in various frequency in all but one group of patients under study (SLE, RA, other connective tissue diseases, malignomas, myelomas, thyroiditis, Grave's disease and healthy controls). Their was a high correlation with two other common techniques to measure immune complexes, e.g. C1q-deviation and C1q-PEG precipitation.

[Treatment of rheumatoid arthritis with 90yttrium. Follow up studies (author's transl)].

90Yttrium-silicate was injected into 131 knee-joints from patients with rheumatoid arthritis with stadium II-IV according to Steinbrocker. The observation period lasted until two years. After three months about 80% and after 24 months still more than 50% of the patients treated showed complete or partial remission. Side-effects as formerly observed with 198-goldpreparations did not occur. Therefore the treatment with 90Yttrium-silicate offers an alternative to surgical synovectomy.

Differentiation of lymphocytes in cerebrospinal fluid in disorders of the central nervous system.

In 73 patients with variable CNS-disorders Slg-lymphocytes and RF-lymphocytes originating from CF were characterized by demonstration of surface markers. In the CF the Slg-lymphocytes were increased in comparison to the blood whereas the RF-lymphocytes generelly were decreased. Patients suffering from acute or chronic inflammatory diseases had significantly more Slg-lymphocytes while RF-lymphocytes were significantly decreased in those patients as well as in patients with malignancies. A correlation between the number of Slg-lymphocytes and immunoglobulin-levels was not seen. The respective data in blood did not follow the changes in CF. The results suggest a prevailing stimulation of the humoral immune system of the CNS during inflammatory diseases.

[HBs-antigen induced extrahepatic immune-complex disease (author's transl)].

A 62-year old patient was hospitalized with fever, polyarthritis and exanthema. All bacteriological, virological and biochemical findings were normal. The immunological analysis showed initially the HBs-antigen in the serum. No irregular antibody was found. Chemotherapy was failing, but application of corticosteroids was followed by a dramatic improvement. Nine weeks after the onset of the disease the patient was healthy again. Antibodies against HBs-antigen were found 3 months later. The data suggest an immune-complex disease like serum sickness based on an infection with hepatitis-B-virus without involvement of the liver.

HLA - frequencies in intermittent acute porphyria.

Tissue typing was performed in 20 patients suffering from intermittent acute porphyria using the lymphocytotoxicity test in a two-step method. The patients belonged to different families and were not related. Compared with the healthy population, and increased frequency of HLA-A10, HLA-A29, HLA-B13, HLA-B14 and HLA-B27 was observed, whereas HLA-A2, HLA-A3, HLA-A9, HLA-A11, HLA-A28 and HLA-BW15 had decreased frequencies. No conspicuous combination between particular HLA alleles was detected. Regarding the number of differences, a relationship between the patients due to a common ancestor must be considered.

[Intrathecal production of neopterin in meningitis in childhood].

A major activator of antigen presenting cells (APC) is gamma interferon a product of activated T-lymphocytes. CNS is not well studied and represents a unique system with respect to the immune reactions. Neopterin is an indirect marker of gamma interferon deliberation and may give some new information on the role of APC in CNS. Neopterin in serum and cerebrospinal fluid (CSF) was determined by specific RIA in children who were lumbar punctured to exclude meningitis. Neopterin was found in various concentrations in serum and CSF of all patients (n = 47). Bacterial meningitis (group 3) was diagnosed in 12 and aseptic meningitis in 18 children (group 2). CSF was drawn in 17 children with febrile convulsions (group 1). Elevated serum neopterin in childhood was only reported in children with an atypical PKU, while data on CSF neopterin were published only in a few cases of adults with CNS involvement. The results show that the APC is stimulated rapidly in childhood similar as in adults following severe viral or bacterial infections. Furthermore neopterin in CSF is not only explained by alteration of the blood-brain barrier but also it may reflect local intrathecal response with activation of accessory cells (APC) in the CNS itself. Between the stimulation of the cellular immune system indicated by increased levels of neopterin and the severity of the disease seems to be a positive correlation.
