Serum ferritin in non-dialysis patients with chronic renal failure: relation to bone marrow iron stores.

Serum ferritin and bone marrow haemosiderin iron was studied in 50 non-dialysis patients with chronic renal failure, and in 53 healthy subjects. S-ferritin was correlated to marrow iron both in patients with renal failure and in healthy subjects (P less than 0.001). Geometric-mean S-ferritin in patients with 0- (1+) marrow iron was 33 micrograms/l, 1+ marrow iron 166 micrograms/l, and 2+ marrow iron 519 micrograms/l. Healthy subjects with 0- (1+) marrow iron had a mean S-ferritin of 16 micrograms/l and those with 1+ marrow iron a value of 65 micrograms/l. S-ferritin levels were higher in patients than in healthy subjects at all marrow iron grades (P less than 0.001). Healthy subjects with S-ferritin less than 15 micrograms/l had absent or reduced marrow iron, while those with S-ferritin greater than 30 micrograms/l had normal marrow iron. Using a critical S-ferritin value of less than or equal to 20 micrograms/l, the diagnostic efficiency in terms of diagnosing absent or reduced marrow iron was 0.90 (PV pos = 0.85, Pv neg = 0.91). In patients with renal failure S-ferritin less than 60 micrograms/l indicated absent or reduced marrow iron, while values greater than 80 micrograms/l were associated with normal marrow iron. The diagnostic efficiency of S-ferritin using a critical value of less than or equal to 60 micrograms/l was 0.94 (PV pos = 0.93, PV neg = 0.97). S-ferritin is a useful indicator of marrow iron stores in patients with chronic renal failure.

Verrucous carcinoma of the vulval region.

Verrucous carcinoma is a variant of epidermoid carcinoma with distinctive clinical and pathological features. The diagnosis is often difficult. To date, 20 verrucous carcinomas involving the vulva have been reported in the literature. Metastases to the inguino-femoral lymph nodes have not been described. The treatment of choice is wide local excision. Clinical features, histological findings, and therapy are discussed.

Histologic changes following oestradiol treatment of a hormone-responsive human breast carcinoma grown in nude mice.

The histology following oestradiol treatment of a hormone-responsive human breast carcinoma grown in nude mice is described. The treatment induced characteristic histologic changes which included a progressive and pronounced proliferation of connective tissue splitting up the tumour tissue. The number of tumour cell mitoses declined rapidly and an increasing number of tumour giant cells appeared. The significance of the histologic findings are discussed in relation to previously obtained results on the effect of oestradiol on growth and cell kinetics of the same human breast carcinoma. The morphologic features corroborated the results of the kinetic studies.

Clonal evolution demonstrated by flow cytometric DNA analysis of a human colonic carcinoma grown in nude mice.

A spontaneous change in DNA content of a human colonic carcinoma grown in nude mice was observed fortuitously. The tumor initially had a G1 cell DNA content of 1.3 times that of normal cells. Flow cytometric DNA analysis showed in transplant generation 56 the appearance of a new subpopulation which in three passages completely overgrew the original population. The DNA content of the new subpopulation was twice that of the original population. The observation supports the hypothesis of clonal evolution of tumor cell populations. The growth rates of the tumor before and after the change showed no significant difference (p greater than 0.05). Cell kinetic factors, therefore, offer no obvious explanation of how the overgrowth took place. It is not known whether the original population disappeared completely or survived as a small population below the detection limit. The heterogeneity created by clonal evolution of a tumor would be less pronounced if old subpopulations often become extinct as new ones emerge. Heterogeneity of human tumors is of clinical importance because the individual subpopulations may have different sensitivity patterns to antineoplastic drugs.

Diagnostic efficiency of various laboratory tests in the assessment of bone marrow iron stores in patients with chronic uraemia.

Bone marrow haemosiderin iron was correlated with various laboratory parameters of iron status in order to define a reliable laboratory index of marrow iron stores. Marrow iron, serum iron, plasma transferrin, transferrin saturation, iron absorption, red cell iron incorporation and serum ferritin were measured in 21 non-dialysed, 20 peritoneal dialysed and 34 haemodialysed patients with chronic uraemia. Diagnostic accuracy of the laboratory tests in terms of diagnosing reduced marrow iron stores was expressed as the predictive value of a positive test and a negative test, and efficiency. Diagnostic efficiency of serum iron, plasma transferrin and transferrin saturation was too low to be of value in the individual patient. Iron absorption and red cell incorporation had a higher diagnostic efficiency, but are unpractical for routine use. Serum ferritin displayed a high diagnostic efficiency and appears to be a reliable index of marrow iron stores, suitable for sequential monitoring of iron status in uraemic subjects.

Uterine cellular stromal infiltration. A diagnostic problem.

Extreme pleomorphic stromal cellular infiltration of the cervix and the corpus uteri was demonstrated by curettage in a 29-year-old woman with menorrhagia. The histological classification was not decisive and a hysterectomy was performed. The histological picture may be compatible with a malignant disease such as a stromal sarcoma or a lymphoma of Hodgkin or non-Hodgkin type, or with an inflammatory or immunologic disease. The clinical course, however, indicates a benign disease.

Growth curves of three human malignant tumors transplanted to nude mice.

Experimental growth data for three human malignant tumors transplanted to nude mice of BALB/c origin are analyzed statistically in order to investigate whether they can be described according to the Gompertz function. The aim is to set up unequivocal standards for planned therapeutic experiments and to develop an essential part of the determination of proliferation parameters for the tumors. The results indicate that the course of tumor growth can be described with good approximation by the Gompertz function. A transformation of this function depicts the growth rectilinearly and appears to be suitable as a standard, e.g. in therapeutic experiments. The course of tumor growth is independent of the size of the transplant, and whether tumors are transplanted in the right or left or both flanks of the recipient mice. Furthermore, the growth does not vary in a systematic way with the number of passages in nude mice. For tumors whose growth is described according to the Gompertz function, recording of the growth of the tumor size in two dimensions is sufficient for calculating other relevant growth parameters, if the three linear tumor measurements are proportional throughout the growth period. The initial histological development of a transplanted malignant melanoma is described and used to elucidate other tumor growth measurements.

Carcinoma of the parotid gland following sialography with thorotrast. Report of two cases.

Two cases are presented of malignant tumour of the parotid gland following sialography with Thorotrast, 28 and 45 years previously. Both cases were histologically established as squamous cell carcinoma and the presence of Thorotrast in the tumours was confirmed by autohistoradiography. It is suggested that the tumours may have developed from metaplastic ductal epithelium after many years of exposure to the alpha radiation from Thorotrast deposits in the gland.
