Yersinia enterocolitica and pseudotuberculosis infection in children.

The clinical symptoms of infections with Y. ent. and pseudotub. are of a different nature. Whether the infection becomes apparent in form of a pseudo-appendicitis, enterocolitis or a typho-septic disease does apparently not only depend on the type of pathogens but also on the children's condition with regard to their resistance. All forms mentioned are observed in case of infections with Yersinia pseudotuberculosis as well as in case of infections with Yersinia pseudotuberculosis as well as in case of infections with Yersinia enterocolitica. However, infections with Yersinia enterocolitica seem to have more frequently a chronic course and to be accompanied by conjunctivitis, arthritis and skin diseases. For the subacute and the chronic forms of the disease, a specific treatment is not necessary in most cases. Severe enterocolitis and septicemia, however, need an antibiotic therapy besides the symptomatic therapy. The best antibiotics for an infection with Yersinia enterocolitica still seem to be the tetracyclines and leucomycine, for an infection with Yersinia pseudotuberculosis also the cephalosporines, ampicilline and carbenicilline.

[Regional suppurative lymphadenitis after BCG vaccination (author's transl)].

In the first half of 1975 there occurred in the Federal Republic of Germany an unusual rise in the incidence of suppurative inguinal lymphadenitis after BCG vaccination of newborns, in immediate time relation with change of the vaccine by its manufacturers, Behringwerke. The attenuated daughter strain Göteborg had been replaced by the effective but also rather aggressive strain Copenhagen 1331. The complication rate was 1.5% in West Berlin. Clinical course, operative technique as well as microbiological and histomorphological features of these cases were analyzed in a joint study. Since it is likely that, after re-admission of the Copenhagen vaccine, such complications may again occur despite reduced micro-organism count, vaccination of newborns should in future be restricted to those at risk.

Autonomous adenoma of the thyroid in infancy.

This is a report on two 10-year-old girls with a decompensated autonomous adenoma of the thyroid gland. Both girls were clinically euthyroid. In one girl there was no response of the thyrotropic cells of the anterior pituitary lobe to TRH stimulation for a short time during the first postoperative weeks. The diagnostic and therapeutic procedure in the case of nodular transformations in the thyroid area is discussed.
