Proprioceptive sensibility in women with normal and osteoarthritic knee joints.

The objective of this study was to quantify the effect of degenerative joint change on the proprioceptive acuity of women with osteoarthritis (OA) of the knee. Middle-aged women with OA of the knee, age-matched healthy women, and younger healthy women were studied. Tests examined the ability of subjects to correctly reproduce knee angles in weightbearing and non-weightbearing situations. The knee angles were photographed and measured in degrees. The absolute error (AE) of each matching test was calculated and the mean AE of two tests was used as the criterion variable in a three-way analysis of variance (ANOVA). The relationship between the reproduction error and the osteoarthritic subjects' self-paced walking speed was also examined. Under both test conditions the AE of the osteoarthritic subjects was greater than that of the healthy subjects (p < .05). There was, however, no significant relationship between the AE measurements of the patients and their walking speed. These data indicate that while women with OA may have poorer proprioception than healthy controls, this impairment may not affect their walking ability.

[Cesarean section of the second twin. Is this unusual mode of delivery justifiable?].

Within a ten-year period 1983-1992 eleven Caesarean sections were performed on the second twin after the first twin had been born vaginally. The reasons leading to this rather unusual mode of delivery are described here. Predominantly, multiple risk factors were present. One first twin and one second twin died in the early post natal period. In a comparative study of the literature, it is shown, that this, disputed mode of delivery has increased during the last twenty years and that, under special circumstances, it is a justifiable method of management of gemini deliveries.

Muscle function in chronic compartment syndrome of the leg.

Chronic compartment syndrome (CCS) is a recognized cause of recurrent leg pain in the exercising patient. Decreased muscle function has been implied in this condition. This study compared the ankle dorsiflexion torque of 10 CCS patients with that of 18 control subjects during 20 repeated, maximal, isokinetic contractions at 60 degrees/sec. Peak torque, relative peak torque, and endurance data were collected. Results showed significantly lower peak torque and relative peak torque in the CCS group (p < or = 0.05), supporting the implication of muscle weakness in CCS. Paradoxically, endurance was significantly higher in the CCS group (p < or = 0.01), and there was a significant (p < or = 0.01), negative correlation (r = -0.50) between peak torque and endurance. The relationship between the findings and CCS is discussed. Strengthening may be useful in very mild cases or in postfasciotomy patients.

[Recent results regarding indications for the "morning after pill"].

Results of interrogating 150 women who visited our department because of postcoital contraception are demonstrated. 80% did actually use one method of contraception. The rate of "condom accidents" was 70 per cent. Only 21 per cent did not use any method of contraception. Taking the facts of sexual habits into consideration we can conclude that women who wish to take the "morning after pill" have a more active sexual life. The intention to prevent an unwanted pregnancy through this postcoital method of contraception indicates a more responsible attitude towards sexuality. The popularity of this method may be increased.

[The course of labor in intrauterine fetal death].

The results of the course of birth in 71 intrauterine cases of foetal death are presented. There were spontaneous deliveries in 82% of the cases and in 4% a vacuum extraction was performed. The relatively high rate (14%) of Caesarean sections was in most cases, due to maternal indication. The introduction of prostaglandins was a significant progress factor in birth induction of these disturbed pregnancies. All 36 patients (51%) were treated with sulprostone (mostly intravenously). Furthermore, intrapartal, directly postpartal complications and complications of early puerperium are discussed.

[Mucinous metaplasia of fallopian tube mucous membrane as a cause of pseudomyxoma peritonei].

A woman aged 49 years was relaparotomized at the Gynaecological Department of the "Rudolf Virchow" Medical Centre of the Free University of Berlin, following diagnosis of metastatic mucinous adenocarcinoma. Proliferative mucinous metaplasia of the Fallopian tube and consecutive peritoneal pseudomyxoma were established as clinical findings and were hypothetically attributed to mucinous metaplasia of the salpingofimbrial mucosa. Neither mucocele of the appendix nor ovarian tumour were found. This possible site origin has been rarely mentioned in any case report in the past.

Measurement of torque of trunk flexors at different velocities.

The purpose of this study was to determine the test-retest reliability of recording isometric and isokinetic torque of the trunk flexors and to examine the effect of velocity on the torque curves. Thirty healthy subjects were tested on two occasions for isometric torque of trunk flexion at four angles and eccentric and concentric torque at three velocities. Two subjects repeated these tests in the passive mode to determine the torque produced by the trunk when there was no active flexion effort. Intraclass correlation coefficients were above 0.85 for all isometric and isokinetic measures. Standard errors of measurement ranged from 6.9 to 19.5 Nm. Student t-tests indicated no significant differences between occasions for all outcome measures. Examination of passive and active torque curves indicated that the torque produced by the mass of the trunk increased with increasing velocity. It is concluded that both isometric and isokinetic testing of the trunk flexors are reliable, but that testing at higher velocities may not provide a valid measure of muscle performance.

[Clinical aspects and pathology of granulosa cell tumors. A single case report].

Case report on a granuloma cell tumour in a 82 year old patients. Because of estradiol production postmenopausal bleedings from a adenomatous-hyperplastic endometrium had been in the history. Hormonal values became normal following removal of the benign tumour in stage I. The most important pathologic, endocrinologic and therapeutical principles of the tumour have been described.

Physiological adaptations to concurrent endurance training and low velocity resistance training.

This study investigated the effects of concurrent endurances and low velocity resistance training (LVR) on measures of strength and aerobic endurance. One group (ES) performed concurrent endurance training 3 days a week and LVR training on alternate days, 3 days a week for 12 weeks. The other group (S) performed only LVR training 3 days a week for 12 weeks without any endurance training. Measurements and increases in training volume were made every three weeks in both groups. Group ES exhibited increases in submaximal exercise responses after 3, 9 and 12 weeks (p less than 0.05). Knee extension peak torque and total work as well as cross-sectional area of quadriceps femoris were significantly increased after 6 and 9 weeks of training in both groups. These findings indicate that no significant differences in strength gains were observed between subjects performing concurrent endurance and resistance training or resistance training only. However, the time-course of adaptations between groups was somewhat different.

[Reduction-plasty of the breast in symmastia].

Medial confluence of the breasts, known as symmastia, has received little attention in plastic surgery literature. By reporting on one case, we present our surgical technique. The chosen method of the reduction mammoplasty is considered to be particularly important. Liposuction is integrated into the surgical procedure. Suggestions made by other authors are discussed.

Adaptations to endurance and low velocity resistance training performed in a sequence.

The purpose of this study was to examine the effects of two different sequences of the same two training programs on strength and endurance adaptations. One group completed 5 weeks of low velocity resistance (LVR) training prior to 5 weeks of endurance training (Group SE) while the other group (ES) completed the opposite sequence. Endurance training and resistance training resulted in anticipated increases in aerobic endurance measures and strength, respectively, regardless of the sequence followed. Some maintenance of strength was observed during subsequent endurance training in Group SE. However, the improvements in peak VO2 and submaximal exercise responses that occurred after endurance training in Group ES were not maintained with subsequent LVR training. This investigation suggests that conducting LVR training prior to endurance training may be more beneficial if resistance and endurance training are performed in a sequence.

[Pseudomyxoma peritonei caused by proliferating mucinous adenoma of the fimbria mucosa].

Omentectomy was conducted on a 49 year old patient at the Dept. of Surgery of the UFK Berlin-Charlottenburg because of a mucinous adenocarcinoma, without prior detection of the primary tumour site. Relaparotomy revealed proliferative mucinous adenoma of the Fallopian tube and a consecutive pseudomyxoma peritonei. Up to now, this seems to be the first case report of a mucinous adenoma of the Fallopian tube.

[Primary reconstruction in breast-saving cancer surgery].

Breast-conserving therapy in mammary carcinoma is an alternative to the more radical forms of therapy when the tumors are small. The cosmetic results obtained are not always satisfactory. Correction of deformities of this kind in breasts which have been subjected to post-operative radiation is difficult and dangerous. Consequently, reconstruction of the mammary gland should be included in the primary operation. The technical possibilities for doing so are listed. It seems to be justifiable from an oncological point of view.

Influence of concentric resistance training on concentric and eccentric strength.

This study investigated the effects of isokinetic concentric training on isokinetic concentric and eccentric torque outputs. Sixteen female subjects (mean age in years +/- SE = 21 +/- 1) were randomly assigned to either a training or a control group. Concentric and eccentric torques of the right knee extensors were assessed at 1.05rad.s-1 using a KinCom isokinetic dynamometer system. Each test consisted of a set of four maximal concentric and four maximal eccentric contractions of the knee extensors. The peak and average torques for each contraction were calculated using the computer software supplied by the KinCom manufacturer. On a separate day, after abstaining from heavy exercise for at least 12 hours, a single cross-sectional image of the thigh at midfemur was obtained using computer tomography (CT) scanning. From this image, cross-sectional area of the quadriceps femoris group was calculated using the computer software associated with the General Electric 9800 CT Scanning System. Training group subjects trained three days weekly for six weeks on a Cybex II+ isokinetic dynamometer, completing five sets of ten maximal effort knee extensions at an angular velocity of 1.05rad.s-1. Each set of exercise was separated by two minutes of self-selected recovery. Torque outputs were monitored daily to ensure that adequate recovery was provided between sets of exercise, and to document changes in strength as the program progressed. Significant (p less than .01) increases in peak and average concentric torque (11% and 12%, respectively), peak and average eccentric torque (18% and 21%, respectively), and muscle cross-sectional area (3.2%) were observed for training group subjects.(ABSTRACT TRUNCATED AT 250 WORDS)

Intrauterine death of a single fetus in twin pregnancies.

The paper reports on nine twin pregnancies in the years 1982-1987 with the intrauterine death of a single fetus. The incidence of 0.10% is consistent with that reported in other comparable studies. Evaluation of the causes of death shows a preponderance of asphyxia. A fetofetal transfusion syndrome occurred in 4 cases. Eight of the pregnancies were terminated by caesarean section. One of the surviving children died in the neonatal period. The course of the neonatal period in the other babies was determined mainly by the state of maturity. No increased maternal morbidity was observed. On the basis of this and other experience described in the literature, some general guidelines are proposed for the management of the further pregnancy and delivery in such high-risk cases.
