Human herpesvirus 8 open reading frame 26 and open reading frame 65 sequences from multiple myeloma patients: a shared pattern not found in Kaposi's sarcoma or primary effusion lymphoma.

Human herpesvirus 8 (HHV-8), also known as Kaposi's sarcoma-associated herpesvirus, has been implicated in the pathogenesis of Kaposi's sarcoma (KS), primary effusion lymphoma (PEL), multicentric Castleman's disease, and recently multiple myeloma (MM). DNA sequence analyses of HHV-8 suggest that multiple HHV-8 strains exist. We extracted DNA from 24 patients with MM and 3 patients with monoclonal gammopathy of undetermined significance and compared HHV-8 open reading frames (ORFs) 26 and 65 sequences with those derived from patients with KS, PEL, and two HHV-8-positive PEL cell lines KS-1 and BC-1. ORF26 sequence data suggest that MM patients are consistently carriers of HHV-8 strain subtype C3. All MM patients also consistently revealed either a single bp deletion or substitution at position 112197 in ORF65. This unique alteration is not present in patients with KS or PEL or in PEL cell lines. It occurs in the portion of ORF65 that is known to be responsible for a serological response to HHV-8.

[Study on the relationship of alteration and expression of p16 gene to pancreatic carcinoma].

OBJECTIVE: To directly investigate the effect of genetic alteration(homozygous deletion and point mutation) and expression of p16 gene on pancreatic carcinomas. METHODS: Thirty-five cases were analyzed for genetic alteration and expression of p16 gene by polymerase chain reaction(PCR), single strand conformation polymorphism(SSCP), DNA sequencing and immunohistochemical method. RESULTS: The analysis of pancreatic carcinoma for p16 gene revealed alteration in 19 of 35 cases, among which 12 pancreatic carcinomas had 522 bp homozygous deletion at least and 7 cases had two point mutations at the same site. One of them is 126th codon GTC --> AAT (Val126Asn); the other is 127th codon GCA --> GCG (A127A). The 3D (three-dimensional) structure of P16 protein determined by computer techniques according to PDB indicated that Val126Asn influenced the space structure of P16 protein and affected the function of P16 protein. Twelve cases revealed no P16 protein and 9 cases showed low level expression of P16 protein. CONCLUSION: The alteration of p16 gene and abnormal expression of P16 protein are significantly correlated with the biological behavior and clinical staging of pancreatic carcinoma and may hence be helpful to prognostication

[The expression of platelet-derived endothelial cell growth factor in liver cancer].

OBJECTIVE: To investigate the expression of platelet-derived endothelial cell growth factor (PD-ECGF) in liver cancer and explore its related intervention with Capecitabine for the growth and metastasis of liver cancer. METHODS: The protein level of PD-ECGF was determined using immunohistochemical method in 61 HCC samples and the mRNA level was detected using Northern blot analysis. Capecitabine was administered orally in 24 nude mice bearing LCI-D20 tumor. All treatments lasted 3 weeks. The tumor size was calculated by the following formula: V = a x b2 x 0.5. Lung metastasis was evaluated by HE staining in lung samples. RESULTS: The PD-ECGF expression rate in 61 HCC and paratumoral liver tissues was 70.5% and 47.5%, respectively. The rate was higher in HCC tissues with advanced TNM stage than in those with early TNM stage (81.1% vs 54.2%, P < 0.05). The mRNA level of PD-ECGF was related well to its protein expression. The tumor size on day 3 after last treatment measured in the control, and low dose (1.05 mmol.kg-1.day-1), moderate dose (2.10 mmol.kg-1.day-1) and high dose (3.15 mmol.kg-1.day-1) of Capecitabine treatment groups was 447 mm3 +/- 159 mm3, 414 mm3 +/- 97 mm3, 240 mm3 +/- 119 mm3 and 209 mm3 +/- 150 mm3, respectively. High doses of Capecitabine increased the inhibitory effect on the growth and lung metastasis of HCC implant. CONCLUSION: PD-ECGF is highly expressed in HCC and correlates with the TNM staging. Capecitabine may inhibit the growth and metastasis of HCC.

[Progress in researches on anti-tumor agent L-asparaginase for treatment of leukemia].

L-asparaginase is a very effective anti-tumor agent. To take the most advantage of enzyme treatment method, the natural L-asparaginase could be chemically modified, entrapped or immobilized into/onto other carriers. The extracorporeal shunt system with immobilized enzyme reactor deserves to receive special attention. It provides an entirely new idea for leukemia treatment and is proved to be very prospective in the future leukemia cure.

[Changes of maternal and umbilical serum nitric oxide in patients with the intrauterine growth retardation].

OBJECTIVE: To determine whether the levels of maternal and umbilical serum nitric oxide (NO) were changed in cases of intrauterine growth retardation (IUGR). METHODS: Fifteen pregnant women with fetuses of IUGR (IUGR group) and 25 normal pregnant women in late trimester (normal control group) were studied. Maternal and fetal umbilical venous blood were collected from all the cases. Serum NO2-/NO3-, the end products of NO, were measured with the Griess reaction after reduction with nitrate reductase. RESULTS: Mean NO2-/NO3- values in maternal serum were (74.22 +/- 28.99) mumol/L in IUGR group and (56.71 +/- 22.81) mumol/L in the control. Umbilical serum NO2-/NO3- values were (39.73 +/- 24.65) mumol/L in IUGR group and (21.49 +/- 6.69) mumol/L in the control. Compared with the control, maternal as well as umbilical serum NO2-/NO3- in IUGR group were significantly higher (P < 0.05 and P < 0.001, respectively). The total nitrite levels in fetal circulation were lower than in maternal circulation in both groups (P < 0.05). CONCLUSIONS: Changes of NO in the pregnant women and their fetuses were due to the result of secondary complementary response in the patients with IUGR. The metabolic disorder of nitric oxide in maternal and fetal circulation may play an important role in pathogenesis of IUGR.

[Phytotoxic effect of soil cadmium pollution on ramie].

A microplot locating experiment was conducted to study the phytotoxic effect of adding Cd in soil on ramie plants. The results showed that the contents of chlorophyll a and b of leaf, and the photosynthesis productivity of ramie plant were significant reduced by the addition of Cd. As the content of soil Cd reached to 14 mg.kg-1, the biomass yield of the above-ground part of ramie plants was 20% lower than the control, and was 50% lower when soil Cd content was at 100 mg.kg-1. The number of effective tillers was a main factor affecting the yield of ramie raw fibers, and Cd addition had a relatively small effect on the quality of raime fibers. It is suggested that a synthetical technology of soil treatment and agronomic measures should be applied to eliminate the toxic effect of soil Cd and to enhance the resistance of plant against soil Cd pollution when employing ramie as the alternative of food and vegetable crops in a Cd-polluted region, which is also important for a higher yield of ramie.

[Percutaneous tubing and drainage for the diagnosis and treatment of malignant pericardial effusion].

OBJECTIVE: To insert a tube into pericardial cavity as an emergent measure of diagnosis and treatment in patients with malignant pericardial effusion. METHODS: Pericardial puncture was followed by insertion of drainage tube (diameter = 1.8 mm) through the puncture needle. The effusion collected was examined for cancer cells. After drainage, chemotherapeutic agents were administered. Four hours later, drainage was continued for 2 days (< or = 30 ml pericardial fluid drained in 24 hr) and the tube was removed. RESULTS: In 34 cases with malignant pericardial effusion, tube draining was successful to relieve cardiac temponade within 15-60 minutes. Clots were present in 91.2% of the cases and cancer diagnosis was confirmed in all of them. The cytologic diagnosis of effusion was positive in 61.8%, and the cyto-pathologic typing of clots was 81.0%. The difference was statistically significant. When the results of the 2 examinations were put together, the positive rate increased to 94.1%. CONCLUSION: Tube drainage of malignant pericardial effusion is useful in diagnosis and emergency treatment.

[Comparison of several therapeutic methods for hepatocellular carcinoma with tumor thrombi in portal vein].

OBJECTIVE: To compare the therapeutic effect and significance of different treatment methods for hepatocellular carcinoma (HCC) with portal vein tumor thrombi (PVTT). METHODS: One hundred and forty-seven HCC patients with tumor thrombi in the main portal vein or the first branch of portal vein were divided into four groups. A, conservative treatment group (n = 18); B, hepatic artery ligation (HAL) and/or hepatic artery infusion (HAI) group (n = 18), periodically received postoperative chemoembolizations; C, excision of HCC with removal of PVTT group (n = 79); D, transcatheter hepatic arterial chemoembolization or portal vein infusion (PVI) or HAI after operation group (n = 32). RESULTS: The median survival period was 2, 5, 12, and 16 months in group A, B, C, D, respectively. Their 1-, 3- and 5-year survival rates was 5.6%, 0 and 0 in group A; 22.2%, 5.6% and 0 in group B; 53.9%, 26.9% and 16.6% in group C; 82.8%, 48.8% and 41.3% in group D, respectively. The survival rates differed significantly between the 4 groups (P < 0.05). CONCLUSION: Resection of cancer with removal of tumor thrombi for HCC with PVTT significantly improves the curative effect and quality of life. Local hepatic chemotherapy or chemoembolization after tumor resection with removal of tumor thrombi may further prolong survival period.

[Effects of the occlusal splint on stress distribution of the mandible with temporomandibular joint disorders].

OBJECTIVE: To investigate the biomechanical mechanism of occlusal splint therapy of temporomandibular joint disorders (TMD). METHODS: The changes of stress distribution on the mandible with TMD before and after occlusal splint therapy were simulated and analyzed by three-dimensional finite element method. RESULTS: Occlusal splint influenced the character of stress distribution on the mandible. It might distinctly decrease maximum and minimum principal stresses on each region of the mandible with TMD, among which the stress on condylar surface of ill side was reduced more significantly, and the stress distribution of bilateral condyles was close to equality and balance. Meanwhile, the stress symmetry on every position of the mandible was improved slightly. CONCLUSIONS: Occlusal splint can alleviate even eliminate the injury to the temporomandibular joint, and make unbalance of joint inner environment adjusted and restored by improving the stress distribution. This is primarily thought to be one of the main biomechanical mechanism of occlusal splint treatment.

[Treatment of non iatrogenic injury of retrohepatic inferior vena cava].

OBJECTIVE: To study a repair method for injured retrohepatic inferior vena cava (IVC) associated with liver trauma. METHODS: Laceration of IVC was sutured bloodlessly after the injured retrohepatic IVC was occluded by compression combined with occlusion of the first hepatic hilus. RESULTS: From April 1996 to January 1999, the injured IVC was successfully repaired in 6 patients. The length of laceration was 2.9 cm (1.5 - 4 cm). The occlusion time was 9.2 min (6 - 15 min) for the IVC and 20.5 min (15 - 29 min) for the first hepatic hilus, respectively. All patients recovered uneventfully, and the IVC was patency without stricture as showed by B-mode ultrasonography and CT. CONCLUSION: The injured retrohepatic IVC can be repaired by direct compression when the laceration of the IVC is caused by liver trauma.

[The study of RDS gene mutation and clinical phenotype in a family with primary retinitis pigmentosa].

OBJECTIVE: To investigate retinal degeneration slow (RDS) gene mutation in a Chinese family with primary retinitis pigmentosa (RP) and the association of the mutation with clinical phenotypes and to explore the pathogenesis of RP. METHODS: Blood DNA from 2 patients in the same family with RP and 2 normal persons was analyzed by molecular genetic methods. RDS gene mutation was screened out by polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) analysis. The mutant RDS gene fragment was cloned, then sequenced with an automatic DNA sequencer using a dideoxy chain termination protocol. The phenotype of the patients with the gene mutation were examined and determined by clinical ophthalmologic examinations. RESULTS: The PCR-RFLP analysis of the RDS gene in 2 patients with RP revealed codon 216 mutation of RDS gene. The mutation was heterozygous, and not found in 2 normal persons as controls. The alteration in the DNA sequence was identified as a heterozygous transversional change of C to T at the second nucleotide in codon 216 of RDS gene, resulting in the amino acid replacement of proline residue with leucine residue (Pro216Leu). The ocular finding of the patients with Pro216Leu mutation of RDS gene included severe visual loss and diffuse distribution of pigmentary changes with macular degeneration. CONCLUSIONS: The Pro216Leu mutation of RDS gene is found in Chinese patients with RP. The gene mutation is associated with the ocular phenotype, diffuse RP with macular degeneration.

[A study on the epidemiology and preventive measure on epidemic haemorrhage fever in army barrack area in Shandong].

OBJECTIVE: In order to guide the army and civilians to carry out the prevention on epidemic haemorrhage fever (EHF), it's necessary to elucidate the epidemic feature of natural epidemic foci and the risk factors of EHF in the army - stationed areas. METHODS: Samples were tested by pathogenic and serologic means before data was collected and analyzed by case - control or family - control studies. RESULTS: Cases belong to house - mouse type (91.48%), field - mouse type (7.21%) and unclassified (1.31%) coexist in the stationed area. In the two types of noticed cases, no significant difference in distributions of sex, profession and ages. Most house - mouse type cases occurred in the peak time from March to June and field - mouse type in November to next February. The type of EHF virus antigen of mice and the type of serum were both accordant with their host types. Dwelling in border area, living rooms with mice, dogs raising, injury of mouth tissue, eating food contaminated by mice, and household with mice living around were more frequently seen in cases themselves and in families of the cases than in controls of both groups (P < 0.05). The mice intensity could be under control to 1.0%, if comprehensive measure on mice control can be implemented. CONCLUSIONS: The stationed area is an epidemic area mixed with both house - mouse and field - mouse types, with majority of house - mouse type. It was house - mouse type virus infection that caused the epidemic area expanded and the number of cases increased radically these years. EHF was epidemic in spring and summer season in the stationed area mainly because the infections were among family members. Food contaminated by mice or close contact with mice played an important role. Mice eradication and prevention were the leading measures to prevent EHF, from our more than ten years of experiences.
