Diagnosis and treatment of small intestinal bleeding: retrospective analysis of 76 cases.

AIM: To investigate the causes of small intestinal bleeding as well as its diagnosis and therapeutic approaches. METHODS: A retrospective analysis was conducted according to the clinical records of 76 patients with small intestinal bleeding admitted to our hospital in the past 5 years. RESULTS: In these patients, tumor was the most frequent cause of small intestinal bleeding (37/76), followed by Mecke's diverticulum (21/76), angiopathy (15/76) and ectopic pancreas (3/76). Of the 76 patients, 21 were diagnosed by digital subtraction angiography, 13 by barium and air double contrast X-ray examination of the small intestine, 11 by 99mTc-sestamibi scintigraphy of the abdominal cavity, 6 by enteroscopy of the small intestine, 21 by laparoscopic laparotomy, and 4 by exploratory laparotomy. Although all the patients received surgical treatment, most of them (68/76) received part enterectomy covering the diseased segment and enteroanastomosis. The follow-up time ranged from 1 year to 5 years. No case had recurrent alimentary tract bleeding or other complications. CONCLUSION: Tumor is the major cause of small intestinal bleeding followed by Meckel's diverticulum and angiopathy. The main approaches to definite diagnosis of small intestinal bleeding include digital subtraction angiography, 99mTc-sestamibi scintigraphy of the abdominal cavity, barium and air double contrast X-ray examination of the small intestine, laparoscopic laparotomy or exploratory laparotomy. Part enterectomy covering the diseased segment and enteroanastomosis are the most effective treatment modalities for small intestinal bleeding.

Application of laparoscopy in diagnosis and treatment of massive small intestinal bleeding: report of 22 cases.

AIM: To investigate the diagnostic and therapeutic value of laparoscopy in patients with massive small intestinal bleeding. METHODS: Twenty-two patients with massive small intestinal bleeding and hemodynamic alteration underwent laparoscopic laparotomy in our unit from December 2002 to April 2005. Post pathologic sites were found, laparoscopy- or laparoscopy-assisted part small intestinal resection including pathologic intestinal site and enteroanastomosis was performed in all these patients. RESULTS: The bleeding sites were successfully detected by laparoscopy in all these 22 patients. Massive small intestinal bleeding was caused by jejunum benign stromal tumor in 8 cases, by jejunum potential malignant stromal tumor in 5 cases, by jejunum malignant stromal tumor in 1 case, by Mechel's diverticulum in 5 cases, by small intestinal vascular deformity in 2 cases, and by ectopic pancreas in 1 case. A total of 16 patients underwent laparoscopy-assisted enterectomy and enteroanastomosis of small intestine covering the diseased segment and 6 patients received enterectomy of the diseased segment under laparoscope. No surgical complications occurred and the outcome was satisfactory. CONCLUSION: Laparoscopy in diagnosis and treatment of massive small intestinal bleeding is noninvasive with less pain, short recovery time and definite therapeutic efficacy.

Expression of estrogen receptors-alpha and -beta in bladder cancer cell lines and human bladder tumor tissue.

BACKGROUND: Estrogen receptors (ERs) are known to mediate important physiologic responses as well as the growth of some tumors in response to estradiol stimulation. In a previous study the selective ER modulator raloxifene was shown to induce apoptosis in an ERbeta-positive bladder cancer cell line. However, the expression of ERbeta in human bladder cancer has not been thoroughly investigated. METHODS: ERalpha and ERbeta expression in 224 bladder tumor samples was evaluated using tissue microarray and immunohistochemistry. Levels of ERalpha and ERbeta protein and mRNA expression were determined in several bladder cancer cell lines using quantitative reverse-transcriptase polymerase chain reaction (RT-PCR) and Western blot analysis. The effect of estradiol and antiestrogen treatments on RT4 bladder cancer cell growth was determined by cell proliferation assays. RESULTS: Analyses revealed that only 2 human bladder cancers weakly expressed ERalpha. In contrast, the expression of ERbeta was detected in 141 tumors (63%). ERbeta was expressed in 58% of WHO Grade 1 and 2 tumors, whereas 70% of Grade 3 tumors demonstrated expression (P = .085). Importantly, although only 53% and 55% of Ta and T1 tumors demonstrated ERbeta expression, 80% of T2, 81% of T3, and 75% of T4 tumors showed ERbeta expression. The differences in ERbeta expression between Ta/T1 and T2/T3/T4 tumors were found to be highly significant (P < .001). Metastatic transitional cell carcinomas had ERbeta expression (80%) comparable to that of muscle invasive bladder cancers. Western blot analysis detected ERbeta protein expression in each of the 5 bladder cancer cell lines tested, whereas no or very low levels of ERalpha were found. Quantitative RT-PCR revealed that higher levels of ERbeta than ERalpha mRNA were present in 5637, T-24, TSU-Pr1, and TCC-Sup bladder cancer cells, whereas ER-alpha mRNA levels were greater than ERbeta in RT4 cells. Treatment with 17beta-estradiol modestly increased RT4 cell growth, whereas the antiestrogens, 4-hydroxtamoxifen, raloxifene, or ICI 182,780 inhibited the growth of RT4 cells. CONCLUSIONS: ERbeta is the dominant receptor expressed in bladder cancer cell lines and in the majority of human bladder tumors. Moreover, the degree of ERbeta expression increases with increasing stage and grade of differentiation. Antiestrogens have an inhibitory effect on the growth of bladder cancer cells in vitro.

Procedures for a dynamical system on {0,1}n with DNA molecules.

In this paper, an improved form of DNA representations of elements in {0,1}n, which was first proposed by Fujiwara et al. [Fujiwara, A., Matsumoto, K., Chen, W., 2004. Procedures for logic and arithmetic operations with DNA molecules. Int. J. Found. Comput. Sci. 15, 461-474], is given. Using this improved representations, a procedure for cycling shift is proposed, and this procedure can be implemented in O(1) lab steps theoretically. Based on the operation for cycling shift, dynamic behavior of an operator on {0,1}n is investigated by DNA molecules.

Randomized study of single instillation of epirubicin for superficial bladder carcinoma: long-term clinical outcomes.

PURPOSE: Intravesical instillation of epirubicin (EPI) is one of the most effective adjuvant therapies for nonmuscle-invasive bladder cancer postoperation. We evaluated the long-term efficacy of single dose intravesical epirubicin for superficial bladder carcinoma recurrence. METHODS: Between June 1997 and May 1998, a total of 47 patients with resectable superficial bladder carcinoma (Ta-1, Grade 1-2), primary or recurrent with no recurrence during last one year, were enrolled in this study. All patients were randomized into 3 study groups: Group A-single epirubicin (80 mg/40 mL of normal saline) was administered into the bladder within 6 hours postoperation; Group B-40 mg Epirubicin consecutively; Group C-40 mg mitomycin C, consecutively. In Group B and C, instillation were given every week for 6- 8 weeks and then every one month for 10 months. Patients were followed up at 3, 6, 9, 12, 18, 24, 36, 48, and 60 months of treatment. The analyzed background factors were the therapeutic method, tumor recurrence, and side effects. RESULTS: Of the 47 patients, 43 (91.5 percent) were eligible and were followed up for 5 years postoperation. The disease free intervals of the three groups were found to have no significant differences (F = 10.28, p > 0.05). The recurrence rates were 35.7 percent (5/14), 33.3 percent (5/15), and 40 percent (6/15), respectively (chi(2)= 0.83, p > 0.05). Side effects of group A (13.6 percent) was lower than that of Group B or C (53.3 percent and 46.7 percent, respectively) significantly (chi(2) test, p < 0.01). CONCLUSIONS: These data indicate that single dose of epirubicin instillation postoperation can reduce the recurrence of superficial bladder carcinoma and has low side effects.

[Effect of co-expressing M-bcr/abl and m-bcr/abl fusion gene transcripts on the clinical features of patients with chronic myelogenous leukemia at diagnosis].

OBJECTIVE: To study the effect of co-expressing M-bcr/abl and m-bcr/abl fusion gene transcripts on the clinical features of the patients with chronic myelogenous leukemia (CML) at diagnosis. METHODS: m-bcr/abl fusion gene transcripts were detected by nested PCR. RESULTS: The percentage of the CML patients who co-expressed M-bcr/abl and m-bcr/abl was 51.4% at diagnosis. Comparison of the patients who co-expressed and those who expressed M-bcr/abl alone showed there was no difference in their hemoglobin concentrations, WBC counts, neutrophilic alkaline phosphatase (NAP) stains, Ph chromosomes and hepatosplenomegaly; however, significant difference in platelet count was seen. In the patients with b3a2 who co-expressed m-bcr/abl, an increased platelet count was noted, compared with that of patients with M-bcr/abl alone. But in patients with b2a2, the two groups showed no difference in all clinical features. Two patients had priapism, their m-bcr/abl being negative. CONCLUSIONS: CML patients who co-expressed b3a2 and m-bcr/abl showed a tendency to have an increased platelet count at diagnosis. Co-expressing m-bcr/abl has no the effect on the clinical features of CML patients with b2a2.

[Learning curve of laparoscopic resection for rectal cancer].

OBJECTIVE: To evaluate the surgical outcomes of laparoscopic resection for rectal cancer and to investigate how surgeons without previous training of laparoscopic cholecystectomy(LC) can quickly learn laparoscopic resection for rectal cancer. METHODS: Clinical data of 105 cases of laparoscopic rectal resection performed by a group of surgeons without previous training of LC were reviewed. The cases were divided equally into 3 groups (groups A, B and C) according to the sequence of the operations. The operating time, blood loss, lymph node harvest, length of specimen, conversion rate to open surgery, intra- and postoperative complications and hospital stay were compared between the 3 groups. RESULTS: There were no significant differences between the 3 groups with respect to age, gender, Dukes'stage or surgical approach (P>0.05). The operating time in group A was 196.1+/-30.3 min, significantly longer than that in group B (164.8+/-22.7 min) and group C (158.7+/-20.9 min) (P<0.001), but the operating time did not vary significantly between groups B and C (P>0.05). The blood loss was significantly greater in group A than in groups B and C (72.4+/-21.5, 48.2+/-16.3, and 46.6+/-15.4 ml, respectively, P<0.001), but showed no significant difference between the latter two groups (P>0.05). The rate of conversion to open surgery decreased from 11.4% in group A to 2.9% in group B and group C, but the difference was not statistically significant (P>0.05). The rate of intraoperative complications declined from 17.1% in group A to 5.7% in group B and group C, showing no significant difference either. The lymph node harvest, length of specimen, and postoperative complications showed no significant variation between the 3 groups (P>0.05), but group C had significantly shorter mean hospital stay in comparison with groups A and B (P<0.001). The 35 patients in group A received the operation within a time period of 17 months (2.1 cases per month), and operations in groups B and C were done in 7 months (5 cases per month). CONCLUSION: The learning curve of laparoscopic rectal resections is approximately 35 cases, and the surgeons without previous experience of laparoscopic cholecystectomy can learn the surgical skills after performing 35 laparoscopic resections for rectal cancer at the monthly frequency of 2.1 cases.

[Effect of arsenic trioxide on bone marrow stromal cells of patients with multiple myeloma].

To explore the effect of arsenic trioxide (As2O3) on growth and secretion of interleukin-6 (IL-6) and vascular endothelial growth factor (VEGF) of bone marrow stroma cells (BMSC) from the patients with multiple myeloma (MM). Specimens of bone marrow aspiration from MM patients were used to establish BMSC cultures. BMSC and human MM cell line CZ-1 were cultured together or alone in the absence or presence of As2O3 at various concentrations (1-20.0 micromol/L). Cell growth inhibition was assessed by MTT assay, cytokines in the culture supernatants were measured with ELISA. The results showed that As2O3 had cytostatic effect on CZ-1 with fifty percent growth inhibition (IC50) for 48 hours at 2.3 micromol/L. As2O3 did not inhibit the growth of BMSC. High levels of IL-6 and VEGF have been found in the culture supernatants of BMSC from MM patients. Cytokine production of BMSC treated with As2O3 significantly decreased as compared with controls (P < 0.05). Excitingly, even the increased cytokine production triggered by adhesion of MM cell and BMSC was also inhibited by As2O3. It is concluded that As2O3 has no inhibitory effect on cell growth of BMSC, but inhibit the production of IL-6 and VEGF by BMSC.

[Gene mutation analysis of coagulation factor VIII from a female patient with hemophilia A].

Hemophilia A affects male, whereas females are carriers and generally spared from this disease. However, we here reported a 65-year-old female with Hemophilia A while screening the gene mutation of coagulation factor VIII. The female went to hospital because of tripping to lead her right chest to be injured with subcutaneous hematoma. She had historically a hemorrhagic diathesis. The physical examination discovered her hip limited to bend and move, but no discrepancy length between her two legs. The initial laboratory tests showed that the activated partial thromboplastin time (APTT) was 61. 3 seconds (20-40 seconds), and the APTT corrected by mixing with normal plasma was 41.3 s, but the levels of PT, FIB and TT were normal. The plain radiographs revealed the hip joints to suffer from the acetabular dysplasia and osteoarthritis. The level of FVIII:C was 2%, F IX:C 200%, vWF:Ag 120%, vWF:Rcof 100%, vWF:CBA 128%, and the F VIII binding assay to vWF was normal. The primers for exon 14 of F VIII gene were designed according to the NM - 000132 gene sequence. DNA was abstracted from the patient blood. PCR were carried out and the DNA sequence was followed. A new mutation of 4111A-->C was discovered, which caused the amino acid sequence changed (T 1314 P). The mutation of T 1314 P may be the cause of this female patient to get the hemophilia A. This mutation was a novel one which has never been reported before.

[A preliminary observation of bone marrow angiogenesis in patients with myelodysplastic syndromes].

OBJECTIVE: To observe the change of angiogenesis in the bone marrow of patients with myelodysplastic syndromes, and to explore its effect on this disease. METHODS: The sera of 20 patients with myelodysplastic syndromes and 20 normal controls were collected; the level of serum vascular endothelial growth factor (S-VEGF) was measured by ELISA; and the microvessel density (MVD) of marrow biopsy specimens was examined with the streptavidine-biovatidine immunoperoxidase method; the count of MVD was evaluated with image analysis system. RESULTS: The level of S-VEGF and count of MVD were higher than that of normal control, and both significantly elevated in MDS/MPD and RAEB-t subtypes. The S-VEGF was positively related with MVD (P<0.05), and S-VEGF and MVD were closely related with the percentage of blast in marrow and in peripheral blood. CONCLUSION: The result shows that there is abnormal angiogenesis in MDS bone marrow, and the elevated levels of S-VEGF and MVD are closely related with a progressive status in myelodysplastic syndromes.

Analysis of organochlorine pesticides in water by novel activated carbon fiber-solid phase microextraction coupled with gas chromatography-mass spectrometry.

This study describes a fast activated carbon fiber-solid phase microextraction (ACF-SPME) method for determining organochlorine pesticides (OCPs) in water. The pesticides in this study consist of Hexachlorobenzene (HCB) and alpha-, beta-, gamma-hexachlorocyclohexanes (HCHs). The optimal experimental procedures for the adsorption and desorption of four OCPs were evaluated. The linearity was obtained with a RSD of 20% for the OCPs studied over a range from 1.0 to 100 microg/L. The limits of detection at ng/L level were achieved with GC-MS under selected ion monitoring (SIM) acquisition mode. The proposed method was applied to the determination of OCPs concentration in tap water. The results have demonstrated the suitability of the ACF-SPME-GC-MS approach for the analysis of multi-residue OCPs in water. Compared to the commercial fiber, ACF has shown its advantages in solvent-resistance, thermal stability, and the cost. The results obtained in this study suggest that ACF is a promising choice in solid phase microextraction.

[Detection of IgH gene rearrangement in Reed-Sternberg cells microdissected from classical Hodgkin's lymphoma].

OBJECTIVE: To study the origins and clonality of Hodgkin and Reed-Sternberg (H/RS) cells and their relations with the background lymphocytes in classical Hodgkin's lymphoma. METHOD: IgH gene rearrangement was detected in paffin-embedded tissues from 33 patients with Hodgkin's lymphoma and a further analysis of the gene rearrangement was conducted in 6 of the positive cases identified after immunostaining of the sections with B-cell-specific activator protein (BSAP) followed by microdissection of the positivity labeled H/RS cells and background lymphocytes. RESULTS: IgH gene rearrangement was identified in 16 of the 33 cases. Microdissection of the lymphoma tissues was successfully performed in the 6 positive cases, and of the 19 tubes of H/RS cells obtained, 14 presented clonal bands of the rearrangement, and difference in cell numbers did not significantly influence the positive rate (P=0.290); in the 12 tubes of microdissected background lymphocytes obtained, 2 were positive for the rearrangement, and the positive rates for the rearrangement significantly differed between H/RS cells and the background lymphocytes (P=0.002). CONCLUSION: The results appear to support the hypothesis that H/RS cells originates from B cells, and a part of the background lymphocytes may possess neoplastic proliferation potentials to function as the precursors of H/RS cells.

The association between parental alcohol-related behaviors and children's drinking.

Past literature has established an association between children's alcohol-related behaviors and parents' use of and attitudes toward alcohol. However, most studies tend to measure parental use and attitudes through proxy, i.e. children's perception. It is not clear the extent to which actual parental use and attitudes influence children's alcohol behaviors. The current study directly used parents' reports on alcohol use and attitudes toward alcohol and examined their impact on children's alcohol use. Based on a three-stage random sampling design, 642 dyads of parents and children (ages 15-18) were interviewed by telephone in New York State. Study variables include parental alcohol use, children's alcohol use, parental attitudes toward underage drinking and parent-child interaction. While parental use and attitudes do not seem to significantly affect children's alcohol use, the extent to which parents prohibit children from using alcohol at home tends to reduce children's alcohol involvement. In addition, the greater the amount of time spent with alcohol-using parents, the more likely the children are to use alcohol. Findings suggest that, while parents' alcohol use influences children's alcohol use through extensive interaction, parental control of underage alcohol use in the household appears to reduce children's involvement in underage alcohol use.

Attitudes toward seeking treatment among alcohol-using college students.

Past research indicates that many college students engage in heavy drinking and that these drinking patterns oftentimes lead to alcohol-related problems. The current study contributes to the literature on college drinking by investigating treatment-seeking attitudes among students in relation to their alcohol education background, alcohol environment, alcohol consumption, and perceived and actual drinking problems. A total of 878 students from five New York State colleges were randomly selected through the use of complete student telephone directories provided by students' respective college administrations for a telephone interview. The results revealed that alcohol education increases students' positive attitudes toward treatment, which may help them to reduce their heavy drinking and alcohol problems.

[Explore the origin of H/RS cells on protein and gene].

OBJECTIVE: To explore the origin and clonality of H/RS cells. METHODS: Immunohistochemical method was used to detect the expression of B-cell-specific activator protein (BSAP) and CD(20) in 33 paraffin-embedded tissues of classical Hodgkin lymphoma (cHL). IgH gene rearrangement was detected in 33 paraffin-embedded cHL tissue and 6 microsectioned H/RS cell samples. The PCR products of a case of cHL and its microsectioned cells were sequenced. RESULTS: H/RS cells were positive for BSAP in 30 of 33 (90.91%) cHL cases and positive for CD(20) in 10/33 (30.30%) cases. There was a significant difference between the expression of BSAP and CD(20) in H/RS cells (P = 0.000). BSAP and CD(20) were positive in almost all B cells of lymph node reactive hyperplasia and malignant cells in B-cell lymphomas while were negative in all malignant cells of T-cell lymphomas. 16 of 33 cHL were positive for gene rearrangement, and microsectioned H/RS cells in 14 of 19 tubes displayed clonal bands of rearrangement. There was no significant difference among the rearrangement rates in tubes containing different numbers of H/RS cells (P = 0.280). Sequencing analyses of the PCR products from both paraffin-embedded tissue and microsection of the same patient revealed the rearranged V segments, but the sequences were not identical. CONCLUSION: H/RS cells were originated from B cells of different differentiation stage.
