[Genetic analysis of the UGT1A1 gene mutation sites in a Chinese family suffered from Gilbert's syndrome].

To learn the variation in the gene for UGT1A1 enzyme, the genetic mechanism in a Chinese Han nationality family suffered from Gilbert's syndrome was studied. At first, genomic DNA from peripheral blood of the sufferer in this family was used for amplifying all of the five exons of the UGT1A1 gene by PCR, and then direct sequencing of the PCR product was applied to analyze gene mutation. The results showed that there existed a G-->A homozygous transition at nucleotide 211 leading the substitution of arginine for glycine at position 71 of corresponding protein product (G71R) and a T-->G homozygous transition at nucleotide 1456 leading the substitution of aspartic acid for tyrosine at position 486 of corresponding protein product (Y486D). No mutation was detected in promoter region and the splicing junction sites. The relevant mutation sites of the other family members were sequenced and identified to be heterozygous in the two above-mentioned mutation sites and in the TA repeat mutation in the promoter region. Furthermore, fresh blood samples were collected from all of the members to detect the serum bilirubin levels to determine the sufferer. The result was consistent with the mutation analysis. It could thus be inferred that this family was caused by mutation in the open reading frame of the gene UGT1A1.

[Study on vascular endothelial injuries and inflammation related cytokines in patients with multiple organ dysfunction syndrome].

OBJECTIVE: To investigate the relationship between the circulating endothelial cell (CEC), coagulation, fibrinolysis indexes and inflammation related cytokines in patients with multiple organ dysfunction syndrome (MODS). METHODS: Thirty-five patients with MODS (MODS group) and 20 healthy individuals (control group) were enrolled for study. CECs were harvested by isopycnic centrifugation method. Plasma prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (FG), D-dimer, and serum tumor necrosis factor-alpha (TNF-alpha), interleukin-6 (IL-6) were determined simultaneously. The relationship between dynamic changes in CEC, TNF-alpha, IL-6 and that in PT, APTT, FG, D-dimer was analyzed. RESULTS: Compared to the control group, CEC numbers, serum TNF-alpha, IL-6 levels and plasma D-dimer level significantly increased, PT and APTT were prolonged, and FG significantly decreased in MODS group (all P<0.05). The quantity of CEC and serum levels of TNF-alpha and IL-6 elevated with the lapse of time. Correlation analysis revealed a significant positive correlation between CEC and serum levels of TNF-alpha and IL-6 (r(1)=0.536, P(1)=0.000; r(2)=0.412, P(2)=0.002), and the same correlation between serum TNF-alpha, IL-6 levels and PT, APTT. CONCLUSION: Elevation of serum inflammation related cytokines and vascular endothelial cell damage occur in MODS patients. Serum inflammation related cytokines play an important role in endothelial damage, and an increase in CEC number may be attributed to vessel injury.

[Implementation of sector imaging mode with high signal-to-noise ratio in the high frame rate ultrasonic imaging system].

The high frame rate (HFR) ultrasonic imaging system, which is developed with limited diffraction beams, constructs images at a high frame rate. However the rectangular imaging area, to some extent, restricts the far field imaging information. At the same time, because of one transmission for constructing image, the system suffers from low SNR. In this paper we present a computationally efficient method to construct sector mode image and to increase the SNR in HFR system. The method uses Golay complementary sequence as excitation to realize two transmission events. Each event simultaneously transmits two plane waves with different transmission angle. Then the received echo signals related to different angle are separated according to orthogonality of Golay complementary sequence and used to construct two images of different area by HFR method. Finally the two images are synthesized to one frame of sector mode image.

[Gene expression profiles on three kinds of genotype hepatitis C virus core protein in Huh-7 cell line with microarray analysis].

OBJECTIVE: To analyze three kinds of genotype hepatitis C virus (HCV) core protein expressed in human hepatoma (Huh-7) cell line and to recognize HCV core proteins biological function and its pathogenic mechanism. METHODS: The Huh-7 cell expressed three kinds of core proteins were established respectively. Affymetrix human gene chip was used for identifying the gene expression dependently on Affymetrix's protocol. All genes changed by 3 or 1.5 folds between the transfected cells and a control cells were further analyzed, and annotated by using NetAffx analysis through Affymetrix website and were categorized based on their biological processes. RESULTS: The HCV-1b core protein caused 16 genes up/down-regulated expression, of which the immune response genes of PF4V1 and SPP1 were up-regulated 3.4 or 4.4 folds respectively. The HCV-2a core protein had caused the immune response gene CXCL5 and apoptosis gene BTF a down-regulated expression of 3.4 and 3.1 folds respectively, but caused the apoptosis genes of HRK and LZTS1 an up-regulated expression of 3.2 and 3.4 folds respectively. As compared with HCV 1b or 2a core protein, HCV-4b core protein caused 111 genes expression changing and it had more obvious effects on gene expression. If we applied 1.5 fold change for a comparison gene expression, a few of the same gene expression profiles might be caused by these two core proteins. CONCLUSION: The three kinds of HCV core protein should have its own expression character and be mainly shown in immune responses, signal transduction, apoptosis, etc. It should be helpful for our recognizing the HCV core protein biological function and its pathogenic mechanism.

[Endothelial function evaluation in salt-sensitive normotensive and mild hypertensive subjects and effects of potassium supplement].

OBJECTIVE: Salt-sensitivity plays an important role in essential hypertension and is associated with more severe target organ injury and higher mortality in patients with essential hypertension. However, the pathologic mechanism of salt-sensitivity is poorly understood and endothelial dysfunction might be involved in salt-sensitive hypertension. We, therefore, observed the endothelial function changes by measuring plasma and urine nitric oxide (NO) concentrations in salt-sensitive (SS) normotensive and mild hypertensive subjects underwent various salt loading protocols and the effects of potassium supplement. METHODS: Thirty-nine normotensive and mild hypertensive subjects (< 160/100 mm Hg), aged 16-60, were enrolled and the study protocol is as follows: 3 days baseline investigation, 1 week low-salt loading (3 g/day), 1 week. high-salt loading (18 g/day) and 1 week high-salt loading plus potassium chloride (4.5 g/day). RESULTS: Plasma and urine NO levels were significantly lower in SS (n = 8) subjects at baseline, low-salt and high-salt loading phases compared with salt-resistant subjects (SR, n = 31) and oral potassium supplement to SS subjects with high salt loading significantly increased plasma and urine NO levels. CONCLUSION: Endothelial function is impaired in normotensive and mild hypertensive SS subjects. Oral potassium supplement could improve endothelial function in normotensive and mild hypertensive SS subjects.

Assessment of atrial septal defect area changes during cardiac cycle by live three-dimensional echocardiography.

OBJECTIVES: To investigate the accuracy of measurement of the atrial septal defect (ASD)area and dynamic change by live three-dimensional echocardiography (L3DE). METHODS: L3DE was performed in patients with ASD using a three-dimensional workstation to obtain the en face view of the ASD and measure its area at the peak of P-wave, the peak of R-wave, the initial and the destination point of T-wave, and the period of P-T. Parameters derived from L3DE were compared with intraoperative measurements. RESULTS: The ASD area changed significantly during cardiac cycles (mean change 46.1%, p < 0.0001; range 15.2-72.5%), with the maximal area at endsystole and the minimal area at enddiastole. There was excellent correlation between L3DE and intraoperative measurements for the area of ASD at the peak of P-wave (r = 0.92). There were good correlations between the two methods during the other phases of cardiac cycle (r = 0.81-0.86). CONCLUSIONS: L3DE provides accurate and feasible measurements of the ASD area. Investigation of the dynamic changes during the cardiac cycle may lead to an improved understanding of the hemodynamics of ASD.

[Origin of mesenchymal stem cells in bone marrow of patients after allogeneic stem cell transplantation].

The aim of this study was to investigate the origin of bone marrow-derived mesenchymal cells in 34 patients who had received a sex-mismatched hematopoietic stem cells transplant (HSCT). The mesenchymal stem cells (MSC) from 34 patients were collected for test. The different passage MSC of patients were analyzed by flow cytometry (FCM) to reveal the cell surface antigen expression. The polymerase chain reaction (PCR) analysis of the amelogenin (AMEL) genes was used to detect donor cells from host cells. The cultured MSC were stained by fluorescence in situ hybridization (FISH) probes for chromosomes X (Xp11.1-Xq11.1) and Y (Yq12) to distinguish donor cells from host cells. The results indicated that 31 out of 34 cases showed the confluent stroma and 24 out of these 31 cases were successfully passaged for more than 5 generations which could be used for PCR and FISH analysis. According to FCM results the number of CD14+CD45+ cells, which was regarded as monocyte/macrophage from the cultured MSC (passage 5) was less than 0.04%. In PCR assay, the marrow-derived MSC from the passage 5 were found to be of host origin. FISH assay demonstrated that marrow-derived MSC from the passage 5 were found to be of host origin up to 100%. It is concluded that the origin of MSC in bone marrow of patients after allogeneic stem cell transplantation was confirmed to be derived still from the recipients their own.

[Landmark of facial nerve in middle ear surgery].

OBJECTIVE: To re-evaluate landmarks for facial nerve in middle ear surgery through temporal bone dissection and facial nerve surgery. METHODS: Some relative landmarks were found through 44 facial nerves dissection in cadaver and 106 cases of facial nerve decompression surgery. RESULTS: (1) Landmarks for vertical segment of the facial nerve: the vertical line in combined point between posterior and middle 1/3 horizontal semicircular canal clews the posterior edge of facial nerve; the prolong line of superior radian of incus short process clues to the anterior edge of the facial nerve, the facial nerve and horizontal semicircular canal are almost in the same plane. (2) Landmarks for horizontal segment of the facial nerve: the facial nerve tracks forward inferior to short process of incus and anterior to horizontal semicircular canal carina in 30 angel. The facial nerve, locating posterior and superior to cochleariform process and parallel with it, forms the step of middle-superior tympanic cavity and tracks forward to geniculate ganglion. (3) location of geniculate ganglion: The same distance prolong line of stapes head to cochleariform process clues to geniculate ganglion. (4) Location of the chorda tympani nerve: chorda tympani nerve, leaving tympanic sulcus at 3 clock of bone canaline left ear and at 9 clock of bone canaline right ear, tracks forward along tympani sulcus and then cross between long process of incus and manubrium. It lies in the border of pars tensa and pars flaccid and is about 5 - 8 mm from the stylomastoid foramen to where the chorda tympani nerve leaves the facial nerve. There is no difference of facial nerve structure in temporal bone dissection and in surgery. CONCLUSIONS: The fixed landmarks of middle ear are the frame of reference of facial nerve, in which horizontal semicircular canal is most invariable; and the safety of surgery will be improved by the reference of the facial nerve.

[Observation of auditory brainstem response and distortion product otoacoustic emission on the animal model of autoimmune auditory neuropathy].

OBJECTIVE: To set up an animal model of autoimmune auditory neuropathy and to observe the auditory brainstem response (ABR) and distortion product otoacoustic emission (DPOAE) in guinea pigs. METHODS: The spiral ganglion and the cochlear nerve were obtained and purified by electrophoresis from 250 normal guinea pigs. The purified cochlear nerve antigen was mixed with an equal volume of complete Freunds adjuvant for immunization. Seventy guinea pigs were divided into three groups: experiment group (50 guinea pigs), control group (10 guinea pigs), normal group (10 guinea pigs). ABR, DPOAE, serum IgG levels, and morphological changes of spiral ganglion cells and the cochlear nucleus were observed. The protein expressions of the antigen were examined by immunohistochemistry and the super-structure of the auditory nerve were observed. RESULTS: The threshold of ABR response increased ranged from 10 to 25 dB in 32% (32/100 ears) of the guinea pigs. The peak latencies of waves I , III and the interpeak latency I approximately III were prolonged in the hearing loss group of guinea pigs. Prolonged peak latency of wave III was noted in hearing loss group at 2 and 3 weeks post immunization and slowly decreased to normal peak latency. The amplitude of DPOAE was no difference in the guinea pigs. The levels of serum IgG increased significantly compared with those of the control group. Inflammatory cell infiltration was observed in the cochlear nerve and the number of spiral ganglion cells detected. On the contrary, inflammatory cell infiltration was not observed in the cochlear nucleus. The cell densities and the across-sectional areas of neurons in anteroventral cochlear nucleus and posteroventral cochlear nucleus were no difference in the guinea pigs. The antigen protein distributed strictly in cochlear nerve and the spiral ganglion. Some demyelinated areas in cochlear nerve was observed in this group. The threshold of ABR response in 68% guinea pigs (68/100 ears) did not increase. The data of DPOAE and the serum IgG levels show no difference compared with the control group. There were not pathological observation in spiral ganglion cells, cochlear nucleus and cochlear nerve. CONCLUSION: An animal model of autoimmune auditory neuropathy has been set up successfully and the character of the ABR and DPOAE was observed.

[Construction of eukaryotic expression vector of short hairpin RNA for transforming growth factor-beta1].

OBJECTIVE: To construct the plasmid containing short hairpin RNA (shRNA) of TGF-beta1 expression vector. METHODS: Short chain oligonucleotide was designed according to the TGF-beta1 mRNA sequence provided by Genebank, then DNA segment was gained through annealing after chemosynthesis, and then was cloned to pWH1 vector. The recombinant TGF-beta1 shRNA expression vector was evaluated by using enzyme cutting. At last, the constructed TGF-beta1 expression vector was transfected into salivary gland mucoepidermoid carcinoma (Ms) cells by Lipofectomine TM 2000, and its effect on TGF-beta1 expression was observed by RT-PCR and immunohistochemistry. RESULTS: Successful construction was identified by enzyme cutting and the constructed plasmid was called pWH1-TGF-beta1. The shRNA and it inhibited the TGF-beta1 mRNA and protein expression effectively. CONCLUSION: The constructed TGF-beta1 shRNA expression vector can block the TGF-beta1 expression in salivary gland mucoepidermoid carcinoma cells.

[Screening and identification of metastasis-related gene expression in tongue carcinoma with cDNA microarray assay].

OBJECTIVE: To identify metastasis-associated genes in tongue carcinoma and to better understand the mechanism underlying tongue carcinoma metastasis. To compared mRNA expression profiles of two tongue carcinoma cell strains with high and low metastatic potentials using microarray technology. METHODS: Tca8113 and Tb cells were used as model systems to study the molecular mechanism of tumor metastasis. Two fluorescent cDNA probes labeled with Cy3 and Cy5 dyes were prepared from the mRNA samples of Tca8113 and Tb cells by reverse transcription method. The two color probes were then mixed and hybridized to the cDNA chip constructed by double dots of 1 152 human genes, and scanned at two wave lengths. Differential expression genes from the above two cell lines were analyzed using computer. Then six of the different expression genes were further validated by RT-PCR technique. RESULTS: In the 1 152 clones of known genes and expressed sequence tags that were analyzed, 37 showed significantly different (minimum 2 folds) expression levels in two cell lines. Among the 37 genes, 15 were up regulated (with ratio more than 2) and 22 down regulated (with ratio less than 1/2). The results of RT-PCR analysis were coincident with those of microarray assay. CONCLUSION: Some of these genes are known to be involved in human tumor antigen, immune surveillance, adhesion, cell signaling pathway and growth control. It is suggested that the microarray in combination with a relevant analysis facilitates rapid and simultaneous identification of multiple genes of interests and in this study it provided a profound clue to screen candidate targets for early diagnosis and intervention.

[Biodegradation of acetochlor in soil and its persistence against Echinochloa crusgalli].

Gas chromatograph and bioassay were used to study biodegradation of acetochlor and its influence on the persistence against Echinochloa crusgalli in soil. The results showed that half-life of degradation of acetochlor was significantly shorter in unsterilized soil than that in sterilized soil under the same experimental conditions of concentrations, water content and temperature, when acetochlor was added to the soil with concentrations of 1.25, 2.5 and 5.0 mg x kg(-1) respectively, which demonstrated that microorganisms could evidently degrade acetochlor in soil. The experiment on degradation of three main kinds of microorganism cultivated in liquid culture medium gave same results above. The bioassay' s result showed that the period of acetochlor persistence against Echinochloa crusgalli was shorter in unsterilized soil than that in sterilized soil, which indicated that existence of microorganism could accelerate the degradation of acetochlor and shorten remaining time of the herbicide in soil, consequently reduce its persistence against the weed.

[Clinical experiences of breast reconstruction using deep inferior epigastric perforator flaps].

OBJECTIVE: To summarize the experiences of the breast reconstruction using the deep inferior epigastric perforator (DIEP) flaps. METHODS: From March 2000 to March 2005, 18 cases of breast defects were treated. Defect was caused by mammary cancer in 17 cases and by Poland's syndrome in 1 case. Preoperative radiotherapy was given in 7 patients. The size of the chest wall defects ranged from 25 cm X 20 cm to 12 cm X 8 cm. All the breasts were reconstructed with the DIEP flaps. The flap size ranged from 35 cm X 22 cm to 12 cm X 8 cm (mean 9.58 cm X 26.85 cm). The internal mammary vessels were used as the recipient vessels in 15 patients who underwent delayed breast reconstruction. Among these cases, the bilateral deep inferior epigastric vessels were anastomosed with the proximal and distal ends of the internal mammary vessels separately in 13 cases and only unilateral deep inferior epigastric vessels were harvested and anastomosed to the proximal ends of the internal mammary vessels in 2 cases. The recipient vessels were the thoracodorsal vessels and thoracodorsal vessels plus the circumflex scapular vessels in the patients who underwent immediate breast reconstruction. RESULTS: Sixteen flaps survived completely postoperatively and the survival rate was 89%. Flap necrosis occurred in 2 patients; one underwent radiotherapy preoperatively while the other did not. The distal 1/3 portion of the flap necrosed in the patient with Poland's syndrome. Nipple reconstruction and breast remolding were performed in 2 patients. Partial dehiscence of the median abdominal incision occurred in 2 patients at 2 weeks postoperatively and were repaired with resuturing and split-thickness skin grafting separately. CONCLUSION: Breast reconstruction using the DIEP flaps not only can preserve the advantages of the traditional method using the transverse rectus abdominis myocutaneous flaps, but also can retain the maximal function of the rectus abdominal muscle and prevent the occurrence of abdominal weakness and hernia. It is an ideal method of the breast reconstruction.
