IL-8 and IFN-gamma in tear fluid of patients with cystic fibrosis.

Cystic fibrosis (CF) is inherited as an autosomal recessive disorder. It is caused by mutations in the protein-coding gene of chromosome 7, resulting in chronic pulmonary disease and pancreatic insufficiency. The disease affects all secretory epithelia, including the eye. The pathogenesis of ocular changes in CF is still unknown, but the involvement of immunologic processes in patients with CF has been studied in recent years. We measured interleukin-8 (IL-8) and interferon-gamma (IFN-gamma) levels in tears in a group of patients and a group of normal controls to determine if the levels of these cytokines are elevated in CF. The levels of these cytokines in tears and the clinical severity of CF and eye disease were compared. Tear samples were collected from 24 patients with CF at the department of pediatric diseases, Medical University of Bialystok, Poland. Cytokine levels were determined by ELISA. Ophthalmic examinations, including tests for keratoconjunctivitis sicca (dry eye), were used to study the ocular surface. The tear levels of IL-8 and IFN-gamma in the CF patients were significantly higher than those in controls. The clinical severity of CF correlated significantly with the IL-8 and IFN-gamma levels. We found positive correlation between the tear levels of IFN-gamma and dry eye findings in CF patients. Our results suggest that the inflammatory cytokines IL-8 and IFN-gamma may play key roles in the regulation of ocular surface inflammation and the immunologic reaction in patients with CF. The tear levels of IL-8 and IFN-gamma may be candidate markers for evaluation of the clinical status of CF and eye disease. These findings help to provide a new insight into the pathogenesis of dry eye in patients with CF and provide potential targets for therapy.

[The evaluation of the expression of intercellulare adhesion molecule (ICAM-1) by conjunctival epithelial cells of patients with cystic fibrosis].

PURPOSE: To investigate the expression of intercellulare adhesion molecule (ICAM-1) by conjunctival epithelialcells of patients with cystic fibrosis. MATERIAL AND METHODS: 15 patients with cystic fibrosis and 15 control subjects were included in this study. Impression cytology specimens were collected and analyzed by flow cytometry and analyzed by flow cytometry. RESULTS: A significant increase of ICAM-1 expression by epithelial cells was found in patients with cystic fibrosis compared with normal eyes. CONCLUSIONS: Increase of ICAM-1 expression by epithelial cells in cystic fibrosis patients suggests, that the inflammation appears to have a role in the pathogenesis of the ocular surface changes and may be a marker of the inflammatory status in cystic fibrosis.

[Concentration of interleukine-8 in tears in patients with cystic fibrosis].

PURPOSE: To evaluate IL-8 concentration in tears fluid in cystic fibrosis patients. MATERIAL AND METHODS: Tears samples were collected from 18 CF Patients and 18 normal controls. Cytokine level was determined by ELISA. RESULTS: A significant increase of IL-8 concentration in tears fluid was found in CF patients compared with controls. CONCLUSIONS: Increase IL-8 concentration in tears fluid in CF patients suggests a role of immunologic processes in the pathogenesis of the ocular changes, and may be a marker of the inflammatory status in ocular surface in cystic fibrosis.

[Biophysical and biochemical intrapartum fetal assessment in relation to hematological varaiables in umbilical cord blood].

UNLABELLED: The analysis of the hematological variables of umbilical cord blood can be useful in broadening our knowledge of both physiological and pathological processes undergoing during prenatal period. OBJECTIVES: The aim of our study was to evaluate the hematological parameters of umbilical cord blood in cases of either the abnormal fetal heart rate or the confirmed fetal acidosis. MATERIAL AND METHODS: A study group consisted of 458 neonates, live-born term of 38 to 41 completed weeks of single gestation. In each case, the hematological cord blood parameters were assessed. Fetal acidosis was diagnosed when the umbilical cord blood pH was below 7,15. Fetal heart rate (FHR) was assessed according to FIGO criteria. Statistical analysis was conducted using Mann-Withney tests, analysis of variance, p-value < 0.05 was considered as significant. RESULTS: The values of WBC (16,5 vs. 13,8x109), NRBC (25,1 vs. 7,3 /100 WBC) and HCT (51,8 vs. 46,5%) were higher in cases of fetus acidosis compare to non-acidemic fetuses. The significant differences in RBC, PLT, NRBC, HCT, HG were observed between group with abnormal and normal FHR. CONCLUSIONS: Fetal distress resulted in significant changes of the hematological variables in umbilical cord blood. Fetal hemopoetic system responds in specific manner to asphyxia. A precise analysis of fetal hematological variables may help to establish an exact onset and degree of fetal hypoxia.

[The cytofluorometric opinion of expression of adhesion molecules CD11a and CD54 on lymphocytes T and B in children's hypertrophied adenoids, which are sick on the otitis media with effusion].

INTRODUCTION: Otitis media with effusion (WZU) is highly prevalent among young children. The hypertrophied adenoid favorables to development (WZU). The pharyngeal tonsil delivers the essential lymphocytes for developed immunological answer. The migrations of lymphocytes to pharyngeal tonsil and to places inflammatory proceed with part of adhesive molecules (CD11a, CD54) presented on lymphocytes. The aim of this study was examination the percentage of the lymphocytes CD4+, CD8+ and lymphocytes B with expression of adhesion molecules ICAM-1 (CD54) and LFA-1 (CD11a). We examined their expression in hypertrophied adenoid by children with otitis media with effusion. MATERIAL AND METHODS RESULTS: In investigations was showed statistically significant decrease of percentage of lymphocytes CD4+CD11a+ and CD8+CD11a+ and increase of percentage of lymphocytes CD19+CD54+ in otitis media with effusion in comparison to group with hypertrophied adenoid. CONCLUSIONS: Molecule adhesive ICAM-1 and LFA-1 fulfills important part in many immunological processes proceed to pharyngeal tonsil. The considerably lower percentage of lymphocytes T CD4+ and CD8+ with expression LFA-1 in hypertrophied adenoids is possibly of causes of incorrect their immunological answer, and the same factor favourables to prolonging the otitis media with effusion at children.

[Platelet satellitism].

Platelet satellitism is an infrequent phenomenon of platelet adhesion to leucocytes. Twelve-month-old patient was hospitalized because of bronchopneumonia. In peripheral blood smear prepared from EDTA-anticoagulated blood sample taken from this patient platelet satellitism was observed. As time went by, a gradual, small decrease of platelet count was observed. Cytometric analysis showed the following proportions of platelet aggregates: with granulocytes--88,7%, with monocytes--5,0%, with lymphocytes--17,0%. In spite of using modern hematological analyzers still microscopic assessment of peripheral blood sample is necessary. Microscopic examination of blood smear is necessary not only to assess erythrocytes and leucocytes systems but also blood platelets.

[Inflammatory mediators and hematological parameters in cord blood in labor complicated by meconium-stained amniotic fluid].

OBJECTIVE: Meconium-stained amniotic fluid at term gestation is a predictor for adverse perinatal outcome and is associated with increased peripartum infections, independent of other risk factors. The aim of our study was to evaluate concentrations of inflammatory mediators (such as cytokine IL-6 and intracellular adhesion molecule ICAM-1) and values of hematological parameters of cord blood in presence or absence of meconium in amniotic fluid in term labor. MATERIAL AND METHODS: Cord blood samples were obtained from 66 term normal neonates immediately after birth, Soluble ICAM-1 and IL-6 concentrations were measured with ELISA R&D Systems kits. The umbilical blood specimen was analyzed using an automated hematology cell analyzer. Blood films were stained using May-Grünwald-Giemsa method. RESULTS: There were no difference in concentrations of ICAM-1 and IL-6 in cord blood in groups with or without meconium-stained amniotic fluid. The mean count of umbilical nucleated red blood cells and white blood cells was significantly higher in meconium group. There was no correlation between the cord blood hematological values and ICAM-1 or IL-6. There was also no correlation between IL-6 and ICAM-I and duration of labor. The mode of delivery influenced cord blood IL-6 levels. CONCLUSIONS: There was no influence of meconium-stained amniotic fluid on cord blood IL-6 and ICAM-1 levels. Changes in hematological parameters in cord blood in meconium passage can suggest either fetus hypoxia or infection. Significant differences of concentrations of fetal IL-6 were associated with the mode of delivery.

Phagocytic and oxidative burst activity of neutrophils in the end stage of liver cirrhosis.

AIM: To evaluate the phagocytic activity and neutrophil oxidative burst in liver cirrhosis. METHODS: In 45 patients with advanced postalcoholic liver cirrhosis (aged 45+/-14 years) and in 25 healthy volunteers (aged 38+/-5 years), the percentage of phagocytizing cells after in vitro incubation with E. coli (Phagotest Kit), phagocytic activity (mean intensity of fluorescence, MIF) and the percentage of neutrophil oxidative burst (Bursttest Kit), and the level of free oxygen radical production (MIF of Rodamine 123) were analyzed by flow cytometry. The levels of soluble sICAM-1, sVCAM-1, sP-selectin, sE-selectin, sL-selectin, and TNF-alpha were determined in blood serum. RESULTS: The percentage of E. coli phagocytizing neutrophils in liver cirrhosis patients was comparable to that in healthy subjects. MIF of neutrophil -- ingested E. coli was higher in patients with liver cirrhosis. The oxidative burst in E. coli phagocytizing neutrophils generated less amount of active oxygen compounds in liver cirrhosis patients (MIF of R123: 24.7+/-7.1 and 29.7+/-6.6 in healthy, P<0.01). Phorbol myristate acetate (PMA) -- stimulated neutrophilsproduced less reactive oxidants in liver cirrhosis patients than in healthy subjects (MIF of R123: 42.7+/-14.6 vs 50.2+/-13.3, P<0.01). A negative correlation was observed between oxidative burst MIF of PMA-stimulated neutrophils and ALT and AST levels (r -0.35, P<0.05; r-0.4, P<0.03). sVCAM-1, sICAM-1, sE-selectin concentrations correlated negatively with the oxygen free radical production (MIF of R123) in neutrophils after PMA stimulation in liver cirrhosis patients (r-0.45, P<0.05; r-0.41, P<0.05; r-0.39, P<0.05, respectively). CONCLUSION: Neutrophil metabolic activity diminishes together with the intensification of liver failure. The metabolic potential of phagocytizing neutrophils is significantly lower in liver cirrhosis patients, which can be one of the causes of immune mechanism damage. The evaluation of oxygen metabolism of E. coli-stimulated neutrophils reveals that the amount of released oxygen metabolites is smaller in liver cirrhosis patients than in healthy subjects.

Renal function during and after treatment for acute lymphoblastic leukemia in children.

Renal function tests (cystatin C, serum and urine creatinine, creatinine clearance, serum and urine beta(2)-microglobulin, microalbuminuria, osmolality) were performed in 21 children at the diagnosis and during the treatment for acute lymphoblastic leukemia (ALL) (group I) and in 37 children (group II) treated for ALL 3.9+/-3.7 years before the study. The results were compared to 20 healthy children. Mean values of renal tests were in normal range at all points of analysis in groups I and II compared to the control group. Transitory higher cystatin C values (but in normal range) were observed after methotrexate administration and after the end of treatment. Deteriorated renal function was observed in one child during the treatment (after each protocol) and in five children treated previously for ALL. In conclusion, combined treatment for ALL is not associated with severe or long-term impairment of renal function.

Influence of perinatal factors on hematological variables in umbilical cord blood.

OBJECTIVE: The purpose of our study was to investigate any possible relationship between the duration of labor, the mode of delivery and the duration of rupture of membranes and hematological parameters in cord blood. MATERIAL AND METHODS: We studied 298 pregnant women who delivered term normal infants. The patients were divided into three groups according to the route of delivery: vaginal (n = 165), cesarean section after labor (n = 27) and elective cesarean section (n = 106). Immediately after delivery, umbilical cord blood samples were collected. RESULTS: The mode of delivery influenced white blood cells, hemoglobin, hematocrit, red blood cell distribution, platelets count and nucleated red blood cells. There was no correlation between the cord blood hematological values and the duration of labor, as well as the duration of rupture of membranes before delivery. CONCLUSION: The influence of mode of delivery, duration of labor and duration of ruptured membranes on hematological parameters in umbilical cord blood is limited.

[Lymphocyte subpopulations in the surface secretion on the adenoid].

The significance and protective efficacy of surface secretions on mucosal membranes in the upper airways are well recognized. The aim of our study was to reveal the role of the adenoids as a source of cellular components in the mucosal secretion. The adenoid removed because of its hypertrophy and the samples of surface secretions taken by "imprint method" described by Ebenfelt et al. from the group of 38 children were examined. By flow cytometry the lymphocyte subsets with following antigens: CD3, CD4, CD8, CD19, CD23, CD16+56, CD45RA, CD45RO, HLA-DR were analyzed. The results were compared between the groups of younger and older children and in the group of concomitant otitis media with effusion (OMS) and pure adenoid hypertrophy. The percentage of lymphocyte subsets in adenoid and secretion were similar, except for lymphocytes Th with memory phenotype which were more numerous in the adenoid and lymphocytes B CD23+ which were more numerous in the secretion. In the adenoid the percentage of T and Th lymphocytes increased with age and the percentage of B lymphocytes decreased. In the secretion the percentage of lymphocytes Th and B CD23+ was higher among older children. In cases of otitis media with effusion higher percentage of lymphocytes B (CD19+ and CD19+CD23+) and lower of lymphocytes Ts and Th with naive phenotype were observed in the secretion. In adenoid however lower percentage of lymphocytes B and higher of lymphocytes Ts was observed in cases of OMS.

[Expression of receptor CD23+ on B lymphocytes in hypertrophied adenoids of children with otitis media with effusion].

Tonsil pharyngeal is stimulated chronic by pathogens can be causes this hypertrophied; whose often is companions to otitis media with effusion. Subpopulation dominates in tonsil pharyngeal are lymphocytes B, and their markers activation are expression of antigen CD23+. The aim of this study was finding dependence between otitis media with effusion and coexistent hypertrophy adenoids, and percentage of lymphocytes CD19+ with expression of antigen CD23+ in hypertrophy adenoids. In the study showed higher significant percentage of lymphocytes CD19+ CD23+ at children in otitis media with effusion (20.08+/-2.93) with reference to comparative group, which was only hypertrophy adenoid (18.16+/-2.25). Percentage lymphocytes B (CD23+) were the highest (20.01+/-5.81) in children subgroup above 5 years old with otitis media with effusion, and lowest (17.36+/-2.78) in children comparative subgroup above 5 years old. As regards on different functions of antigen CD23+ the assessment of percentage lymphocytes B with expression of CD23+ can be additional marker in course immunological and inflammatory processes to occur in hypertrophy adenoids at children are sick otitis media with effusion.

[IFN-gamma in tear fluid in patients with cystic fibrosis].

PURPOSE: To evaluate IFN-gamma concentration in tear fluid in CF patients. MATERIAL AND METHODS: Tear samples were collected from 15 CF patients at the age 10-21, and from 15 patients in control group at the age 11-20. Cytokine levels were determined by ELISA. RESULTS: The concentration of IFN-gamma in tear fluid in CF patients was 10.75+/-2.23 pg/ml, and 4.06+/-0.57 pg/ml in control group. CONCLUSIONS: The results of this study indicate that increased concentration of IFN-gamma in the tear fluid may be an important factor in the pathogenesis of dry eye in CF patients. They also open new perspectives related to therapeutic management.

Nitric oxide and platelet energy metabolism.

This study was undertaken to determine whether nitric oxide (NO) can affect platelet responses through the inhibition of energy production. It was found that NO donors: S-nitroso-N-acetylpenicyllamine, SNAP, (5-50 microM) and sodium nitroprusside, SNP, (5-100 microM) inhibited collagen- and ADP-induced aggregation of porcine platelets. The corresponding IC50 values for SNAP and SNP varied from 5 to 30 microM and from 9 to 75 microM, respectively. Collagen- and thrombin-induced platelet secretion was inhibited by SNAP (IC50 = 50 microM) and by SNP (IC50 = 100 microM). SNAP (20-100 microM), SNP (10-200 microM) and collagen (20 microg/ml) stimulated glycolysis in intact platelets. The degree of glycolysis stimulation exerted by NO donors was similar to that produced by respiratory chain inhibitors (cyanide and antimycin A) or uncouplers (2,4-dinitrophenol). Neither the NO donors nor the respiratory chain blockers affected glycolysis in platelet homogenate. SNAP (20-100 microM) and SNP (50-200 microM) inhibited oxygen consumption by platelets. The effect of SNP and SNAP on glycolysis and respiration was not reduced by 1H-[1,2,4]oxadiazolo-[4,3-a]quinoxalin-1-one, a selective inhibitor of NO-stimulated guanylate cyclase. SNAP (5-100 microM) and SNP (10-300 microM) inhibited the activity of platelet cytochrome oxidase and had no effect on NADH:ubiquinone oxidoreductase and succinate dehydrogenase. Blocking of the mitochondrial energy production by antimycin A slightly affected collagen-evoked aggregation and strongly inhibited platelet secretion. The results indicate that: 1) in porcine platelets NO is able to diminish mitochondrial energy production through the inhibition of cytochrome oxidase, 2) the inhibitory effect of NO on platelet secretion (but not aggregation) can be attributed to the reduction of mitochondrial energy production.

[Evaluation of neutrophils adhesive molecules CD11b/CD18 in the course of photochemotherapy (PUVA) for psoriasis vulgaris].

Psoriasis is a common, chronic dermatological disorder. Inflammatory cells assemble in epidermis in the early stage of acute psoriasis. Accumulation of neutrophils at the site of inflammation depends on the expression of adhesion molecules. A variety of stimuli--mediators or selectins can active CD11b/CD18 molecules on the neutrophils surface. The aim of our study was expression of CD11b/CD18 molecules without and after stimulation by fMLP (formyl-Met-Leu-Phe) in patients with psoriasis vulgaris treated by PUVA. Expression of beta2-integrins was estimated with MFI (mean fluorescence intensity). Neutrophils in patient with acute psoriasis showed an increase in CD11b/CD18 expression. It means about an activation of neutrophils and particular role of those molecules in granulocyte adhesion to epidermis in psoriasis.

[Cytofluorimetric assay for evaluation of CD16 receptor expression and myeloperoxidase (MPO) activity of neutrophils in patients with psoriasis vulgaris treated with PUVA].

Early histological changes indicate the collecting of neutrophils in the stratum corneum, mainly in the acute psoriasis. Fc gammaRIIIB (CD16) is the specific functional neutrophilic receptor, which is responsible for phagocytosis. Myeloperoxidase (MPO) is granulocyte enzyme playing main role in metabolic activity of neutrophils. We have evaluated CD16 expression and MPO activity of polymorphonuclear granulocytes in the acute psoriasis vulgaris and the influence of photochemotherapy PUVA (psoralen plus ultraviolet light of A wavelength) treatment on these parameters. The expression of CD16 and MPO activity were significantly higher in neutrophils of patients after PUVA.

Naive and memory T cells in hypertrophied adenoids in children according to age.

The anatomic location of the adenoid implies that this organ is the first site of contact with inhaled antigens. Depending on the expression of different isoforms of the CD45 molecules, T cells can be divided into naive (CD45RA(+)) and memory (CD45R0(+)) cells, the latter representing T cells that have already been exposed to antigens. The purpose of this study was to analyse the lymphoid cells' subsets in adenoids and relate the findings to the age. The analysed material was adenoid tissue removed on the grounds of hypertrophy from 22 children. The patients were divided into two groups: up to 5 and above 5 years of age. The analyses of the lymphocytes subpopulations in the adenoid were performed in an EPICX XL (Coulter) flow cytometry. The results are expressed as the percentage of positively labeled cells (CD4(+), CD8(+), CD4(+)/CDB(+), CD4(+)CD45RA(+), CD8(+)CD45RA(+), CD4(+)CD45R0(+), CD8(+)CD45R0(+)). The percentage of CD4(+)/CD45R0(+) in children up to 5 years of age was significantly lower than in older children. We found the positive regression between age and the percentage of CD4(+) cells was CD45R0(+) (r=0.64). There were no statistically significant differences between study subgroups for the other parameters. The positive regression for CD4(+)/CD45R0(+) cells and age may result from increased stimulation by bacterial, viral and other antigens. Our results indicate that the adenoid have an important role in the development of an immunological memory among younger children.
