[Effect of antibiotic therapy on phagocytosis and oxidative metabolism of polymorphonuclear leukocytes in patients with Lyme arthritis].

The purpose of this work was to evaluate phagocytosis and oxidative burst changes in neutrophils of patients with Lyme arthritis before and after antibiotic treatment. The examined patients were divided into three groups: group 1-18 patients with diagnosed Lyme arthritis, group 2-12 patients with Lyme arthritis after 14-day treatment with cefuroxime and control group--25 healthy individuals. The phagocytic activity of peripheral blood neutrophils (percentage of phagocytic cells and fluorescence) was measured using standard cytometric flow assay (Phagotest-kit, Orpegen Pharma). The oxidative metabolism (percentage of bursting cells and fluorescence stimulated by Escherichia coli and PMA) was measured using cytometric flow assay (Burst-test, Orpegen Pharma). The results were statistically analysed using Mann-Whitney test. Evaluation of phagocytosis demonstrated no changes in the percentage of phagocytic cells. But fluorescence increased significantly after treatment. The comparison of oxidative metabolism activity showed no changes despite E. coli and PMA stimulation. Parameters of phagocytosis and oxidative metabolism did not correlate with neutrophil count in analysed groups of patients.

[Bacterial flora of hypertrophied tonsils in children with secretory otitis media].

The potentially pathogenic bacteria: Haemophilus influenzae, Moraxella catarrhalis, Streptococcus pneumoniae are the major bacterial flora of hypertrophied tonsils. In children suffering from otitis media with inflammatory effusion an unfortunate decreased physiological flora of Streptococcus viridans and Neisseria is observed.

Lymphocyte subpopulations in hypertrophied adenoid in children with otitis media with effusion.

Otitis media with effusion (OME) usually coexist with the adenoid hypertrophy. The aim of this study was a quantitative evaluation of the lymphocyte subpopulations in hypertrophied adenoid in children with persistent (ove three months) otitis media with effusion. We used flow cytometry for study the subpopulations of the lymphocytes in the pharyngeal adenoid during otitis media in the children, alternative group consist of children with simple adenoid hypertrophy. The monoclonal antibodies selected were specific markers for T lymphocytes (CD3), T-helper cells (CD4), T-suppressor cells (CD8), B cells (CD19) and NK cells (CD56). There were also analyzed T activation markers: HLA-DR and CD25. The percentage of NK cells was significantly decreased in the group of younger children with otitis media with effusion (0.81 +/- 0.51) in comparison to older ones (1.40 +/- 0.59%). Also, in the group of younger children with adenoid hypertrophy, the NK cells percentage was significantly lower (0.85 +/- 0.37%) than in the group of older children--(1.46 +/- 0.83). In both subgroups of younger children the percentage of lymphocytes T with HLA-DR activation marker was close, but it was significantly lower in the group of older children with otitis media with effusion (4.28 +/- 3.40%) in comparison to the group of older children with adenoid hypertrophy (5.03 +/- 3.00%).

[Influence of hemodialysis on expression of glycoprotein lb platelets reactivity in patients with the end stage renal failure].

Complex disturbances in hemostasis characterise chronic renal insufficiency. Defect of primary hemostasis is a common cause of bleeding complications. The hemodialysis procedure (HD), by itself, influences platelet function (the key part of primary hemostasis) which is compromised after the procedure. Many functional defects of thrombocytes have been described in end stage renal failure (ESRF) patients; one of them is adhesion defect where the pivotal role plays the complex of glycoprotein Ib (GP Ib) and IX. The aim of the study was to determine the extent of activation, reactivity of platelets and expression of GP Ib in ESRF patients. The flow cytometry method was used, and thrombin was used for stimulation of thrombocytes. Membrane expression of GP Ib and selectin P was assessed in 14 hemodialysed patients before and after stimulation with thrombin, before and after HD and in 10 healthy persons. The patient group had lower expression of GP Ib on resting platelets and significantly lower expression of selectin P after stimulation with thrombin when compared to the control group. After HD, thrombocytes had much lower expression of GP Ib; however there was no difference in expression of selectin P when compared to the state before HD. A lower reduction of GP Ib expression after stimulation with thrombin was observed after and before HD. On the basis of the results the following conclusions may be drawn: hemo-dialysed ESRF patients have lower expression of platelet GP Ib and reactivity of thrombocytes; the HD results in further depression in expression of GP Ib and reactivity of thrombocytes. It is plausible that nitric oxide (NO) released during HD modulates the expression of selectin P, but it remains to be confirmed.

The effects of radioiodine therapy on peripheral blood lymphocyte subpopulations in patients with Graves' disease. Preliminary report.

BACKGROUND: Treatment of Graves' disease patients with radioactive iodide ((131)I) is becoming the standard therapy in an increasing group of cases but can induce alterations in immune response, like increasing levels of thyroid autoantibodies, and, in part, exacerbation of ophthalmopathy. The aim of this study was to assess the changes in peripheral blood (PB) lymphocyte subpopulations after (131)I treatment of patients with Graves' disease. MATERIAL AND METHODS: The study was carried out in a group of 30 patients with Graves' disease (23 f; 7 m) 49.5 +/- 10.0 years of age, 26 with different subjective ocular signs like gritty sensation, increased lacrimation, orbital pain, and exophthalmos. PB lymphocyte subsets were analysed by cytofluorometry, serum concentration of TSH and fT4 were evaluated before and 6 weeks after radioiodine treatment. RESULTS: After (131)I treatment a significant increase in CD3+, CD4+, CD3+HLA-DR+ and a decrease in CD19+ percentages of lymphocyte subsets were found in comparison with the initial evaluation. No significant changes in percentage of CD8+ and NK (CD3-CD16+ CD56+) cells were observed during this study. A significant increase in TSH and a slight decrease in fT4 concentration concentration took place in the 6th week after (131)I application. The patients without subjective improvement of ocular signs during the therapy initially had a percentage of CD3+, CD8+ lymphocytes which was significantly lower compared with those with regression of ocular signs observed after 131I treatment. CONCLUSIONS: The changes in PB lymphocyte subsets caused by (131)I treatment of Graves' disease confirm the involvement of acquired cellular immunity after radiation damage of the thyroid gland. The decreased initial percentage of CD8+ and CD3+ lymphocytes could help make a prediction of ocular symptoms persisting after radioiodine treatment in some patients with ophthalmopathy.

[Cord blood reticulocytes and reticulocyte subtypes in normal and complicated pregnancy].

Reticulocyte analysis using flow cytometry increases the precision of reticulocyte enumeration and can evaluate the maturation of reticulocytes. Reticulocytes count and their subtypes may reflect function of hematopoesis, especially during hypoxia. This study was aimed to establish reticulocyte count and reticulocyte maturation profile in cord blood during normal pregnancy and pregnancy complicated by chronic intrauterine hypoxia. For this purpose we analysed 233 cord blood specimens derived from uneventful pregnancy and 58 cord blood specimens from complicated pregnancy. The gestational age ranged from 26 to 42 weeks. The following measurements were obtained (mean +/- standard deviation): reticulocyte absolute count - 268 +/- 63 10(9)/l, reticulocyte percentage - 6.4 +/- 1.5; high fluorescence reticulocyte HFR - 11.3 +/- 3.8%; medium reticulocyte fraction MFR - 18.1 +/- 4.7%; low reticulocyte fraction LFR - 70.7 +/- 7%; immature reticulocyte fraction IFR - 29.3 +/- 7%. The percentage values of reticulocytes decreased in cord blood according to gestational age increase, but maturation subpopulations did not change significantly. Reticulocyte fractions in normal pregnancy were different compared to complicated pregnancy.

Peripheral blood T, B, and NK cells in relation to histological hepatitis activity and fibrosis stage in chronic hepatitis C.

BACKGROUND/AIMS: Many data on the pathogenesis of chronic hepatitis C have pointed to host's immune system disorders and a high variety of virus. However, there are no known criteria that could prognose the course of chronic hepatitis C infection. The analysis of T and B lymphocyte subpopulations in the peripheral blood was undertaken in patients with chronic hepatitis C of more than 6 months of duration. METHODOLOGY: Fluorescein isothiocyanate or phycoerythryne conjugated monoclonal antibodies for CD3+, CD4+, CD8+, CD19+, CD3++ HLA DR+, CD16++ CD56+ were used. The correlation between histological hepatitis activity and fibrosis (according Scheuer's scale) and the distribution of lymphocytes in the peripheral blood was sought. RESULTS: All patients with chronic hepatitis showed statistically significant increase in active lymphocytes CD3++ HLA DR+ and CD16++ CD56+ NK cells in peripheral blood. We observed the correlation between these cells and histological hepatitis activity and fibrosis. There was no correlation between the value of CD3+ and CD8+ cells and the stage of liver failure. In the early stage of chronic hepatitis C we noted decrease CD4+ cells with increase B cells CD19+. CD4+/CD8+ ratio was maintained as slightly decreased in chronic hepatitis C in favor of lymphocytes CD8+. CONCLUSIONS: The results show the correlation between peripheral blood value of activated T cell (HLA DR+) and NK cells with histological activity and fibrosis in chronic hepatitis C. Lymphocyte T (CD4+, CD8+) and B (CD19+) did not correlate with grade and stage of hepatitis C.

[Renal function and size after complex treatment of Wilms' tumour].

The aim of our study was to measure renal function and growth in survivors of unilateral Wilms' tumour in 21 children and young adults (7 female). The mean age was 12.6 +/- 4.8 years, mean follow-up time was 7.01 +/- 4.25 years: seven of the group received irradiation (35 Gy). Blood pressure was normal in all patients. Three of them had elevated cystatin C and clearance of cystatin C below referenced normal value. The others had normal renal function tests (cystatin C, creatinine and cystatin clearance, B2 microglobulin, microalbuminuria, osmolality). Compared to the control we found higher cystatin C values in children treated before the age 3 years old (p=0.03) and in children treated more than 5 years before (p=0.03), and lower cystatin clearance in group treated before the age 3 years old (p=0,03). No difference between the irradiated and non-irradiated group was found. We observed a greater increase in volume (155.9% +/- 33.4) than in length (127.9% +/- 6.3). The highest rise of renal volume was in children treated more than 5 years before (174.6% +/- 22.3). In conclusion, our data suggest that after combined treatment for Wilms' tumour compensatory renal hypertrophy and a tendency progressive renal dysfunction takes place.

Evaluation of selected peripheral blood leukocyte functions in patients with various forms of periodontal disease.

INTRODUCTION: Periodontitis (P) is an infectious disease that develops in the supporting tissues of the tooth. One of the risk factors leading to it may be dysfunction of some immune system cells. Therefore, the object of the study was to assess selected functions of peripheral blood leukocytes in patients with various forms of P. As leukocytes are able to secrete interleukin (IL)-4 and IL-6, concentrations of their soluble receptors and the expression of their membrane receptors were investigated. MATERIAL AND METHODS: Twenty generally healthy subjects with aggressive (AP)and chronic periodontitis (CP)were enrolled in the study. The control group consisted of 8 healthy subjects, with no changes in periodontium. Peripheral blood mononuclear cells (PBMCs) were isolated and cultured. Levels of IL-4,IL-6,and their soluble receptors sIL-4R and sIL-6R were determined in the supernatant by ELISA. The expressions of cell surface IL-4R and IL-6R were assayed on PBMC using flow cytometry. RESULTS: No statistically significant differences were found in the selected parameters between people with periodontal disease and healthy controls. However, in subjects with AP, there was an increasing tendency in IL-6 concentration and IL-4R expression on PBMCs. CONCLUSIONS: Our results show that leukocytes play a significant part in P and their activity is probably lesion-dependent. Estimation of the cytokines secreted by leukocytes may facilitate differentiation and prognosis of the disease progression.

Lymphocyte subpopulations in peripheral blood in primary sclerosing cholangitis.

BACKGROUND/AIMS: Primary sclerosing cholangitis (PSC) is a chronic disease of autoimmunological etiology, leading to inflammation, destruction and atrophy of the bile ducts. The aim of the study was to determine peripheral lymphocyte B, T, and NK cells in PSC. METHODOLOGY: The estimation of peripheral blood lymphocytes in 17 patients (54+/-12 years old) with PSC was carried out; the control group consisted of 27 subjects (38+/-11 years). The following T lymphocyte subpopulations were determined using duo color flow cytometry: CD3+, CD4+, CD8+, CD3++HLA DR+, B cells CD19+, and NK cells CD16+ +CD56+. RESULTS: In PSC we observed doubled increase in activated T lymphocytes of CD3+ +HLA DR+ phenotype as compared to healthy subjects (7.9% vs. 2.7%, p<0.01) and NK cells (12.6% vs. 10.3%, respectively, p<0.05). There were no significant differences in the composition of CD3+, CD4+, and CD8+. In peripheral blood we noted, in patients with PSC, a decrease in B lymphocytes (11.2% vs. 12.3%, p<0.19). CONCLUSIONS: The examinations showed that activated T (HLA DR+) lymphocytes and NK cells played an important role in development of PSC.

Inhibition of activated blood platelets by interferon alpha 2b in chronic hepatitis C.

BACKGROUND/AIMS: Interferon alpha used in treatment of chronic hepatitis C significantly influences the blood platelets. The role of platelets in initiating and developing pathological processes in hepatic diseases is still barely known. We studied the effects of interferon alpha 2b (IFN alpha2b) on blood platelets in chronic hepatitis C. METHODOLOGY: The studies were conducted in 16 patients who underwent IFN alpha2b treatment 3 times a week at 6MU. The examination was carried out before and on the 14th day of the treatment of IFN alpha2b. Morphological parameters of blood platelets were determined by hematological methods and flow cytometry. Expression of receptors on blood platelet surfaces (CD41, CD42a, CD62P) and thrombopoietin, platelet-derived growth factor, soluble form sP-selectin, IL-6, and tumor necrosis factor alpha were also determined. RESULTS: The use of IFN alpha2b in patients with chronic hepatitis C significantly effects blood platelets morphology by causing the decrease in their number, the change in population size, and the increase in large platelet count. Interferon decreases P-selectin expression on platelets, sP-selectin and platelet-derived growth factor concentration in plasma. During interferon therapy we noted increase concentration of thrombopoietin, tumor necrosis factor alpha, IL-6 in chronic hepatitis C. CONCLUSIONS: IFN alpha2b stabilizes activated platelets and probably decreases their participation in inflammatory and fibrotic processes in the liver.

[Diagnostic significance of reticulocyte analysis by flow cytometry, in children with acute lymphoblastic leukaemia (ALL) on chemotherapy].

INTRODUCTION: Reticulocytes and their fractions quickly show changes at the time of chemotherapy, therefore they can be an early indicator of changes arising in the erythrocytic line. THE AIM: of the study was an assessment of percentage, absolute number and immature reticulocyte fraction (IFR) by flow cytometry in children with acute lymphoblastic leukaemia (ALL) and comparing them with classical indexes of the erythrocyte line. MATERIAL AND METHODS: The examined group of 14 children from 2 to 16 years with ALL, on remission support treatment (New York II protocol). RESULTS: The group which was treated by thioguanine and cyclophosphamide showed reduction of reticulocytes from mean value 3,1+/-1:1,4 to 2,5+/-1,8. IFR reduction was not significant statistically. In the group treated with vincristine and methotrexate an average percentage of reticulocytes fell from 3.5+/-O.8 to l,5+/-0,4 and IFR did not change significantly. In children who were treated by daunorubicin and cytosine arabinoside an average percentage of reticulocytes reduced from 3.2+/-O,8 to value 2,1+/-1, 7. The immature reticulocyte fraction (IFR) reduced significantly from 22,0+/-6,0 to 12,0+/-8,0 (p<0,03). CONCLUSIONS: The indicators of reticulocytes were reduced after each cycle chemotherapy: this indicates damage in the erythrocyte line. The indicators examined by cytometric method can show changes earlier than classical indicators used in haematology.

[Lymphocytes T gammadelta receptors in hypertrophied adenoids in children with otitis media with effusion].

BACKGROUND: The pharyngeal tonsil is the place of immunological system first contact with pathogenic factors in the nasopharyngeal region, which is responsible for local defence from infection. Chronic antigenic stimulation can lead to adenoid dysfunction and hypertrophy (PMG). Significant role in PMG and in the otitis media with effusion (WZU) play lymphocytes T (CD3(+)) and T-TCR gammadelta. THE AIM of the study was a quantitative analysis of lymphocytes T (CD3(+)) and T-TCR gammadelta in adenoid hypertrophy and otitis media with effusion lasting more than 3 months. MATERIAL AND METHODS: An investigation was executed by flow cytometry in hypertrophied adenoids from children after tonsillectomy. The study and control groups were divided in to 2 subgroups: children younger than 5 years and those aged from 5 to 17 years. RESULTS: The study showed a statistically significant reduction from 35.6+/-3.1% to 32.0+/-2.8% (p < 0.04) of lymphocytes T gammadelta CD3(+) in the examined group (WZU) compared to the control group (PMG). There was a higher percentage of lymphocytes with TCR gammadelta receptor expression in children with WZU (2.04+/-1.13%) than in PMG group (0.99+/-0.44%) (p < 0.002). The lowest percentage of lymphocytes T-TCR gammadelta was found in younger children with PMG (0.67+/-0.17%) and the highest in WZU in younger children (2.13+/-1.04%) (p < 0.0001). We observed a statistically significant difference in expression of TCR receptors (p < 0.02) between younger children with PMG (0.67+/-0.17%) and older children with PMG (1.24+/-0.43%). We did not find significant differences in lymphocytes T CD3(+) related to age. CONCLUSIONS: The enlarged percentage of lymphocytes T gammadelta in adenoid hypertrophy in children with WZU create a possibility of a more rapid response to inflammatory factors.

Expressions of selected adhesion molecules on peripheral blood leukocytes in patients with aggressive periodontitis.

INTRODUCTION: Aggressive forms of periodontitis lead to rapid bone destruction resulting in extensive losses in children's and young adults' dentition. Adhesion molecule deficiency syndrome and abnormalities in the expression of various adhesion molecules on peripheral blood leukocytes can be observed in prepubertal and aggressive periodontitis (AP) patients. The aim of the study was thus to assess the expression of selected cell adhesion molecules (CAMs; CD11a, CD11b, CD11c, CD54, and CD62L) on monocytes, neutrophils, and lymphocytes of the peripheral blood in patients with AP. MATERIAL/METHODS: The study involved 16 patients with AP and a control group of 13 generally healthy subjects with healthy periodontium. CAM expressions were determined by flow cytometry and presented as mean fluorescence intensity (MFI) and percentage of cells showing expression of the assessed adhesion molecules. RESULTS: Neutrophil CAM expressions in AP patients were comparable with those of the control group. MFI of CD62L on monocytes in AP patients was significantly lower than that of the controls. Lymphocytes showed increased CD11b expression compared with the control group. The percentage of leukocytes showing CAM expression in both groups was similar. Only the percentage of lymphocytes with CD11b in AP patients was significantly higher than in healthy controls. CONCLUSIONS: Because of the evident lack of differences between patients and controls and the great amount of individual dispersion of the results, the above CAMs on peripheral blood leukocytes in generally healthy patients with AP do not seem to be characteristic markers of this disease.
