[Changes in the blood flow in celiac artery and superior mesenteric artery after stellate ganglion block].

We evaluated the effect of stellate ganglion block (SGB) on the blood flow of celiac artery (CA) and superior mesenteric artery (SMA) by using the ultrasonic pulse Doppler technique. The subjects were 15 healthy volunteers (14 males, one female; average 26 years old; 22-38). Right C7-SGB was performed using 5 ml of 1% mepivacaine at the 7th cervical process. Blood pressure, heart rate, mean blood flow velocity and diameter in CA and SMA, were measured after 5 minute rest, before and after SGB. To measure the flow velocity and the diameter of vessels, ultrasonic diagnostic equipment SSA-270A or SSA-260A (Thoshiba CO, Ltd.) with convex electric-phased 3.75 MHz, probe was used. Doppler shiftgrams of the CA and SMA were recorded. Then, we calculated their flow volume and sectional area of vessels. In CA mean blood flow velocity tended to decrease (P < 0.05) and both flow volume and sectional area of vessels tended to increase (P < 0.001). In SMA, mean flow velocity tended to decrease (P < 0.05) and sectional area increased (P < 0.001). But flow volume did not show significant change. Our results indicate that right C7-SGB leads to an increase in the blood flow of CA.

[Analysis of therapeutic efficacy of combined radio-chemotherapy, pre-radio-therapy in 61 cases of non-Hodgkin's lymphoma].

Treatment results were retrospectively analyzed in 61 cases of non-Hodgkin's lymphoma (stage I: 21 cases, stage II: 40 cases) that were diagnosed between July, 1977 and Oct., 1987. The actuarial five-year survival rates for stages I and II were 84.4% and 50.7% respectively, whereas those were 47.5% and 75.3% respectively for those treated by radiotherapy (XRT) alone and combined radio-chemotherapy including all cases of stage I and II. Also the results of combined radio-chemotherapy were significantly better than those of XRT alone. Particularly, the pre-radio-chemotherapy group had a 5-year survival rate of 85.7%. In conclusion, the results of combined radio-chemotherapy, particularly pre-radio-chemotherapy, was significant between those of XRT alone.

[Effective evaluation of combination chemotherapy in stage I-II of non-Hodgkin's lymphoma--a comparison of treatment with multivariate analysis].

Sixty-one patients with non-Hodgkin's lymphoma (Stage I and II) were treated solely with radiotherapy or in combination with chemotherapy. A multivariate analysis was used to identify the prognostic factors that significantly affected the treatment outcome. The variables examined included: stage of disease, according to the Ann Arbor classification, sex, age, pathological criteria, according to the LSG, the presence or absence of systemic symptoms, the sites of disease involvement, the LDH, the bulk of the disease, and the combination chemotherapy. Variables achieving a p-value in the 0 to 0.10 range with a multivariate analysis for the overall survival were the stage of disease, the sites of the disease involvement, the bulk of the disease, and the combination chemotherapy. Finally, the overall survival was affected by bulk of the disease (p less than 0.01) and the combination chemotherapy (p less than 0.01). The 5-year survival rates were 84.4% for stage I and 50.7% for stage II. The survival rates were 47.5% for radiotherapy treatment alone and 75.3% for combination chemotherapy. It thus was concluded that combination chemotherapy was effective for treatment of stages I and II of non-Hodgkin's lymphoma.

[Effect of treatment with synthetic eel-calcitonin derivative in patients suffering pain due to bone metastases].

Synthetic Calcitonin Derivative obtained from eel (eel-calcitonin) was given to 15 patients suffering from pain due to bone metastases of malignant tumors. The analgesic effect of synthetic eel-calcitonin derivative was evaluated through pain and narcotic score with the Ridit method. With regard to pain score, significant analgesic effect was observed in the first week (p less than 0.001), and after a month there was also a decrease in the narcotic score (p less than 0.05). However, changes in serum Ca, P, ALP values, and the findings of bone scintigrams and roentgenography, rarely occurred following the treatment. The toxicities of eel-calcitonin which were observed in this series were as follows; nausea and vomiting (33%), flush (11%), eruption (6%). These findings suggest that synthetic eel-calcitonin derivative is a very useful agent for pain due to bone metastases.
