Technique of cervical myelography with metrizamide.

A technically simple modification of the prone posture technique for cervical myelography with use of metrizamide (Amipaque) after lumbar puncture is described. The procedure is well tolerated by the patients.

Myelography with metrizamide in infants and children.

Technical aspects on myelography in pediatric patients with the water-soluble contrast medium metrizamide (Amipaque) are reviewed. Special interest is paid to site of puncture, positioning of the patients and concentration and dosage of the medium.

Liver enzymes in alcohol-discordant twins.

Seventy moderately alcohol-discordant male twin pairs have been investigated with respect to some liver enzymes. No differences were found as regards S-ALAT, while significantly higher enzyme levels were demonstrated for S-ASAT, S-ALP and S-GT among the high alcohol consumers compared to their low consuming co-twins. Especially S-GT was found to be a valuable and sensitive test in the detection of even a moderate alcohol intake in working and socially well adapted subjects. These results were obtained in a subject group in which the influence of genetic factors was kept to a minimum.

Effects of some immunosuppressive procedures on myasthenia gravis.

Clinical Observations. A total of 53 MG patients have been treated with different immunosuppressive methods (alone or combined) with the following effects: Thymectomy was performed in 38 patients. The improvement was excellent in 15, and moderate or uncertain in 20. In three patients severe long-lasting deterioration followed the operation. ACTH treatment (n=32): Initial deterioration during the 5-7 days of heavy ACTH treatment (1000 IU) was followed by an improvement lasting on an average 4 months. The improvement was good or moderate in 78% of the patients. Betamethazone treatment has been tried in six patients where ACTH and azathioprine was ineffective. In four of these patients the results were excellent. Azathioprine treatment has been given to 26 patients for periods up to 7 years. An improvement is measurable after 6-12 weeks and it seems maximal after about 1 year. Of the 26, 80% responded favorable with reduction in the need for cholinesterase inhibitors. Severe complications were seen in three patients with one death. Drainage of thoracic duct lymph was initiated in 14 patients up to 4 weeks with rapid improvement lasting as long as drainage was performed. Long-termed effects of the drainage may be present, however. Retransfusion of homologous cell-free lymph precipitated a return of the myasthenic symptoms. Biochemical Studies on Myasthenic Lymph. Using a membrane preparation from the electric organ from Torpedo marmorata and tritiated Naja naja siamensis neurotoxin we demonstrated a decreasing binding of toxin to the receptor in the presence of MG lymph gamma-globulin fraction. Gammaglobulins from controls showed almost no inhibition of the neurotoxin binding. Immunological Studies. An increased frequency of HL-A1 and 8 was found in female patients. LD typing was also performed. During a period of three weeks of thoracic duct drainage 130X10(9) or about 10% of total number of lymphocytes in the body were removed. In the lymph an initial decrease in the proportion of thymus-derived lymphocytes (T cells) occurred, which was accompanied by a sequent increase in the proportion of bone-marrow-derived lymphocytes (B cells). Towards the end of drainage this effect was reverted. Mitogenic stimulation using lymphocytes from thoracic duct drainage revealed no differences as compared to normal cells. The proportions of T and B cells was studied in the peripheral blood in nine patients treated with ACTH. During treatment there was an initial decrease in the proportion of T cells accompanied by a subsequent rise in the proportion of B cells, which was maximal after 3-10 days. These proportions were reverted to normal 1-5 days after the maximal change. The effect of azathioprine on T and B cells has also been studied.

Preliminary experiences with the EMI scanner.

Preliminary experiences of computed tomography of the head are reported in a material of 213 patients. The majority of examined lesions includes intracranial tumours and cerebrovascular lesions.

Enzymes in intestinal mucosa from patients with rheumatoid diseases.

Jejunal mucosa from arthritic and control subjects was obtained by intraluminal biopsy or at operation and analysed for eight different enzymes. Histidinemethyl-esterase showed 6 to 10 times higher values in patients uith rheumatoid arthritis on comparison with control individuals in respective series. A disturbance in the histidine metabolism in these patients, possible depending on their intestinal flora, is discussed.

Practical aspects on the determination of serum alkaline phosphatase isoenzymes using L-phenylalanine and urea.

Simple methods for estimation of the most frequently occurring alkaline phosphatase isoenzymes in serum are evaluated. Methods based on inhibition by L-phenylalanine or denaturation by heat or urea have been adopted for a reaction-rate instrument (LKB 8600) using conditions recommended by the Scandinavian Enzyme Committee, when applicable. The analytical results are directly evaluated for various groups of patients and also used in a mathematical "three-isoenzyme" model. These calculations did not improve the clinical value of the primarily obtained data. Thus, it was concluded that under certain precautions the results obtained in the presence of L-phenylalanine or urea should be used separately.

Drainage of thoracic duct lymph in twelve patients with myasthenia gravis.

The effect of thoracic duct lymph drainage (5-34 days) in 12 patients with myasthenia gravis on muscular function has been followed for 5-43 months. Among the results obtained were: (1) During the drainage the myasthenic symptoms decreased markedly after 1-4 days and remained so during the drainage. (2) The doses of cholinesterase inhibitors had to be markedly reduced during the lymph drainage in eight patients. (3) Discontinuation of the lymph drainage increased the myasthenic symptoms within a few days. However, after a median observation time of 14 months with conventional treatment all but one of the patients had improved. (4) Retransfusion of the patients own cell-free lymph caused a worsening of the myasthenic symptoms. This effect could also be obtained following infusion of IgG preparations from the patients lymph. Three retransfusions of cell suspensions obtained from the thoracic duct lymph from two patients had no effects on their myasthenic symptoms. (5) It is suggested that thoracic duct lymph drainage can be combined with other forms of treatment in severe cases of myasthenia gravis.

The cranial index according to Cronqvist compared to echo-ventriculography for determination of hydrocephalus.

The Cronqvist cranial index with the normal values for different age groups according to Austin & Gooding is compared with the ventricular index estimated by ultrasound in a series of 166 neuropaediatric subjects aged 0 to 5 years. A large number of hydrocephalic patients has a normal cranial index. The cranial index alone is thus an unreliable method for the screening of hydrocephalus.

Clinical and haemostatic parameters related to thromboembolism and low-dose heparin prophylaxis in major surgery.

In a study of 112 patients undergoing elective major surgery clinical and haemostatic data was followed in connection with a double-blind investigation on the effect of subcutaneous low-dose heparin prophylaxis. None of the patients developed severe thromboembolism but according to lung photoscanning and leg scanning 41 of the patients had deep vein thrombosis and/or pulmonary embolism. Clinically thromboembolism appeared within 4 days after operation. In 22 patients with epidural anaesthesia the incidence of thromboembolism was lower than in the patients with general anaesthesia. The extension of the operation was positively correlated to a higher incidence of thromboembolism. The surgical trauma was reflected in most of the routine haemostatic laboratory parameters, hiding possible minor changes caused by subclinical thromboembolic complications. The low doses of heparin could only be detected with more sensitive methods. A comparison of sodium and calcium heparin administered subcutaneously revealed no significant differences.
