Case report: Foreign body granuloma mimicking liver metastasis.

In this study, foreign body granuloma mimicking liver metastasis diagnosed on routine follow-up examination in a 41-year-old woman with rectal adenocarcinoma is reported. To our knowledge, this is the first study in English-language literature reporting foreign body granuloma indistinguishable from liver metastasis on radiological examination.

Morphological changes in diseased cementum layers: a scanning electron microscopy study.

The aim of this study was to compare the morphological changes that occurred in root cementum layers due to periodontal disease by using scanning electron microscopy (SEM). Ninety-two periodontally hopeless teeth extracted from 29 patients were studied. Measurements of probing depth (PD) and clinical attachment loss (CAL) were taken prior to extractions. After the longitudinal fracturing process of root specimens, healthy and diseased cementum layers of roots were evaluated by SEM for the thickness of the cementum and the morphological changes in collagen fibers. The result of SEM evaluation revealed a significant ( P < 0.001) decrease in the thickness of cementum layer on the diseased root surfaces compared to the healthy surfaces. There were denser and conspicuous collagen fibers with their interfibrillar matrix in cementum layers on the healthy root surfaces compared to the diseased surfaces. Within the limits of this study, the thickness of cementum layers in diseased areas was found to be significantly less than that in the healthy areas of root surfaces. However, there exist variations in the density and visibility of cemental fibers between individuals and within the individual.

A new MRI method for the quantitative evaluation of extraocular muscle size in thyroid ophthalmopathy.

In cross section, extraocular muscles are more or less elliptical, with short and long diameters. We studied the ratio (R) of short to long diameter and investigated its use in quantitative assessment of the extraocular muscles in patients with Graves's disease. We measured the diameters on T1-weighted axial and coronal MRI and computed R for each extraocular muscle in 80 patients without and 40 with Graves's disease. We compared the measurements and R of the right and left orbits, and of men and women. The short diameter of all extraocular muscles apart from the superior oblique showed significant differences between men and women, and that of the inferior rectus varied significantly with age. R, however, was unrelated to sex or age. All patients with Graves's disease and an increased short diameter also had an increased R, but 6% of the muscles showed an increase in R, even though their short diameter was within the normal range.

Multilocus enzyme electrophoresis analysis of Pseudomonas syringae pv. phaseolicola.

A multilocus enzyme electrophoresis technique was developed to detect variation in seven enzyme loci among isolates of Pseudomonas syringae pv. phaseolicola, representing three races from different geographical locations, the causal agent of the halo blight disease of beans. Cellulose acetate gel electrophoresis of seven enzymes revealed 19 electrotypes (ET) among 21 Pseudomonas syringae pv. phaseolicola isolates. One of the pathovar syringae and one of the pathovar tomato isolates were represented by two different ET. The population of Turkish isolates and three races of the pathovar phaseolicola appeared to be genetically diverse.

Cisplatin and carboplatin combination as second-line chemotherapy in dacarbazine-resistant melanoma patients.

High-dose cisplatin regimens have been shown to be highly active in advanced melanoma patients but are associated with unacceptable side effects. In order to increase the platinum dose but avoid severe side effects, we treated 15 dacarbazine (DTIC)-resistant metastatic melanoma patients with a combination regimen of cisplatin (100 mg/m2) and carboplatin (200 mg/m2), two platinum analogues with a similar mode of action but a different toxicity pattern. After a mean follow-up period of 10.7 months (range 4-18 months), two patients (13.3%) achieved complete remission and two patients (13.3%) showed partial remission, giving an overall response rate of 26.4% (95% confidence interval [CI] 4.2-49%). Furthermore, three patients (20%; 95% CI 0-40.2%) experienced stable disease. The median duration of response was 7.1 months (95% CI 4.2-10.0 months), and the median overall survival was 12.5 months (95% CI 5.8-19.2 months), with eight patients still alive. The main side effects were haematological (leukopenia/thrombocytopenia World Health Organization [WHO] grade I-IV; anaemia WHO grade I-III), gastrointestinal (WHO grade I-III), neurological (WHO grade I-II) and renal (WHO grade I) toxicity. Nevertheless, except in one patient, side effects did not result in discontinuation of therapy. Despite the small number of patients treated in this preliminary study, we believe that combining cisplatin and carboplatin represents a novel, active and well-tolerated therapeutic option as second-line chemotherapy in DTIC-resistant advanced melanoma patients.

Development of immunomagnetic separation technique for isolation of Pseudomonas syringae pv. phaseolicola.

Immunomagnetic separation technique was developed for specific detection and selective isolation of Pseudomonas syringae pv. phaseolicola, the agent of halo-blight disease of beans. Whole-cell and exopolysaccharide fraction of the bacterium was used for polyclonal antibody production in rabbits. High specificity of the antisera was determined in agglutination reactions. The optimum immunocapture time for both antisera was determined as 1 h by using 1/nL CFU (i.e. 10(6) CFU per mL). No significant difference was observed in the binding capacity of cells to immunomagnetic particles with different antisera.

Synthesis and antimicrobial activities of some 3-arylamino-5-[2-(substituted 1-imidazolyl)ethyl]-1,2,4-triazole derivatives.

In this study, some 3-arylamino-5-[2-(substituted imidazol-1-yl or benzimidazol-1-yl)ethyl]-1,2,4-triazole derivatives were synthesised by reacting 3-(substituted imidazol-1-yl)propionyl hydrazides, with S-methyl-N'-arylisothiouronium iodide salts. Their structures were elucidated by IR, 1H-NMR and mass spectroscopic data and elemental analyses results. Antimicrobial activities of the compounds were observed against Staphylococcus aureus NRRL B-767, Micrococcus luteus NRRL B-4375, Escherichia coli B, Pseudomonas aeroginosa NRRL B-23 and the fungi Candida albicans and Candida glabrata by using the tube dilution technique. Remarkable activity was obtained.

Evaluation of patients admitted with hypoglycaemia to a teaching hospital in Central Anatolia.

Hypoglycaemia is one of the most common endocrine emergencies in practice. We analysed retrospectively the incidence and causes of hypoglycaemia in patients admitted to Erciyes University Medical School in Turkey between January 1991 and June 1998 because of hypoglycaemia. Charts were reviewed according to multiple variables including age, sex, blood glucose levels, renal and liver functions, diagnoses, symptoms, drugs, complications, sequelae, and survival status. During this period, 13 500 patients were hospitalised and hypoglycaemia was reported in 126 (0.9%) patients. The most common causes were diabetic treatments in 54 cases (42%), endocrine deficiencies in 25 cases (20%), and malignancy in 15 cases (12%), respectively. The leading endocrine deficiency was panhypopituitarism. Sheehan's syndrome was the most common cause of panhypopituitarism (44%). No underlying cause was diagnosed in seven cases. Nine patients died (7%) and neurological sequelae were observed in one patient with diabetes mellitus. We conclude that hypoglycaemia accounts for about 1% of hospital admissions. Although the hypoglycaemia could be attributed to hypoglycaemic agents in the treatment of diabetes mellitus in the majority of cases, Sheehan's syndrome was also found to be an important cause of hypoglycaemia in our hospital.

Synthesis of some 3-arylamino-5-aryloxymethyl[1,2,4]triazole derivatives and their antimicrobial activity.

In this study, some 3-arylamino-5-aryloxymethyl[1,2,4]triazole derivatives were synthesized by reacting S-methyl-N'-arylisothiouronium iodide and 2-(aryloxy)alkanoic acid hydrazide. The structural elucidation of the compounds was performed by IR, 1H-NMR and MS spectroscopic data and elemental analyses results. Antibacterial and antifungal activities of the compounds were examined.

Investigation of the ultrastructurel of different cells in lymph nodules by using zinc iodide osmium tetroxide technique.

The zinc iodide -osmium tetroxide (ZIO) fixation/staining method was applied for neurocytological study and also to examine several other tissue samples including human blood and bone marrow on nerve endings in the median eminence, epidermal langerhans cells of lymphoid tissue. Although precise specificity can not be attributed to the staining reaction. Interesting staining patterns for different cell types in lymph node were observed by one of the ZIO staining solutions. The significance of ZIO positivity is briefly discussed.

N-(chroman-4-ylidene)aryloxyacetohydrazones: synthesis and antimicrobial activity.

In this study, some N-(chroman-4-ylidene)phenoxyacetohydrazone and N-(chroman-4-ylidene)phenoxyacetohydrazone compounds were synthesized. Structure of the compounds were identified by using IR, H NMR and MASS spectral data and by elemental analysis. Particularly, some compounds showed interesting antimycobacterial activities.

Prediction of bone marrow involvement in patients with malignant lymphoma by GM-CSF stimulation test.

Recently, a stimulation test with colony stimulation factors was found useful to predict bone marrow cellularity. We examined the usefulness of this test to predict the bone marrow involvement in patients with malignant lymphoma. A single dose of GM-CSF (5 micrograms/kg) was given subcutaneously to 12 patients with malignant lymphoma. Six patients had normocellular bone marrow. Five of these 6 patients showed an increase in neutrophils of more than 5 x 10(9)/l after GM-CSF administration. The remaining 6 patients who had bone marrow lymphoma involvement had a neutrophil count of less than 5 x 10(9)/l. Therefore, the change in neutrophil count with a cut-off value of 5 x 10(9)/l distinguished the marrow lymphoma involvement with a sensitivity and specificity of 83% and 100%, respectively. The nadir neutrophil count was significantly higher in patients with normal marrow than in patients with marrow involvement after chemotherapy (P < 0.02). In conclusion, the GM-CSF stimulation test is useful to predict the marrow involvement in patients with malignant lymphoma.

Carcinoid syndrome due to a malignant somatostatinoma.

Somatostatinoma is one of the rarest tumours of the endocrine pancreas. Cardinal manifestations of a somatostatinoma include gallstones, mild diabetes mellitus, steatorrhoea, diarrhoea and dyspepsia. Like any other pancreatic islet cell carcinoma, a somatostatinoma may also produce several different hormones such as adrenocorticotropic hormone, calcitonin, vasoactive intestinal polypeptide, pancreatic polypeptide, gastrin, insulin, and glucagon. In many cases, the clinical picture is dominated by the effect of these other hormones. We present a patient with somatostatinoma in which an immunocytochemical study of the specimens from pancreas and liver showed a weak positive reaction for gastrin besides a strong positive reaction for somatostatin. Interestingly, this patient also showed the signs of carcinoid syndrome which was successfully treated with octreotide.

Thyroid function tests in non-alcoholic cirrhotic patients with hepatic encephalopathy.

OBJECTIVES: Changes in thyroid hormone levels in non-alcoholic cirrhotic patients with hepatic encephalopathy were investigated in order to determine whether thyroid function tests could be a useful prognostic indicator for acute outcome. METHODS: Patients with hepatic encephalopathy due to non-alcoholic cirrhosis were studied at Erciyes University Hospital, Kayseri, Turkey, between August 1990 and October 1991. Nine of them were males and eight females, with ages ranging between 22 and 69 years. Eleven cirrhotic patients who had no encephalopathy of any degree comprised the control group. Patients with hepatic encephalopathy were divided into two groups: survivors and non-survivors. The levels of serum triiodothyronine (T3), thyroxine (T4), thyrotropin (TSH), free T3 (FT3) and free T4 (FT4) were obtained in both patients with and without encephalopathy. Alterations of thyroid function tests and their relation to acute outcome were evaluated. RESULTS: Serum T3 and FT3 levels were significantly lower in the patients who died of encephalopathy compared with controls (p < 0.05). Serum FT3 and FT4 were also lower in the survivors compared with those of the cirrhotic controls (p < 0.05). Albumin was found to be the unique and most important prognostic factor with a difference between survivors and cirrhotic controls (p < 0.05). CONCLUSIONS: These data suggest that patients with liver disease due to non-alcoholic cirrhosis are under great risk for hepatic encephalopathy when they have low FT3 levels. The decrease of serum T3 and albumin levels could be considered as indicators of poor prognosis for acute outcome.
