Although several pathogenetic factors have been identified in recent years, the etiology of peptic ulcer disease is yet unknown. During the past few decades several investigators have reported seasonal patterns in peptic and duodenal ulcer disease. A review of the literature reveals vast differences between studies with respect to the type and number of patients selected, diagnostic techniques, the number of examinations and the interval of time between each as well as the method of data analysis. Nevertheless, there is solid evidence to conclude that peptic ulcer disease is lower during the summer than the other seasons of the year. Although many investigators have reported peptic ulcer disease to be more common in the spring and/or autumn, the evidence based on group studies thus far is not persuasive. On the other hand, initial findings on a small sample of patients studied by endoscopy at frequent intervals over at least a one-year period suggest that the season of peptic ulcer disease is a characteristic of each individual patient. Some experience recurrence of disease only in the spring while others experience such only in the autumn. Studies utilizing protocols which call for frequent endoscopic examination at regular (3-month or less) intervals for at least a one-year period are likely to clarify aspects of the seasonality of peptic ulcer disease.
Seasonal accumulation of duodenal ulcer recurrences suggests that maintenance therapy could be limited to the more risky periods. We carried out a 2-year, multi-centre, randomized, double blind study in 250 patients in whom the last ulcer proved to be healed at the endoscopy on entry. One-hundred-and-twenty-six patients in group A were given pirenzepine 2 X 25 mg while 124 in group B had 2 X 50 mg daily from the beginning of January to the end of March, and from the beginning of September to the end of November for two consecutive years. Test endoscopies were performed each year at the end of February, May and November. Both groups proved to be well comparable. Thirty-five patients dropped out from group A in the first and 10 in the second year; in group B 27 and 29, respectively. As the effect did not show any dose relation, the four yearly cycles were summarized. Recurrence rate checked in May was 22.1% while in both pirenzepine protected months it was 11.3% (p less than 0.0005) and 13.4% (p less than 0.001), respectively. We conclude that pirenzepine administered in the risky seasons prevents the peak ulcer incidence and reduces the recurrence rate to a level lower than in the non-risky season. Thus pirenzepine is effective in ulcer prevention even if administered in an interrupted manner.

The medical diagnoses of 217 successively dead patients were compared with their postmortem findings. The rate of correct diagnoses was 67.8%, and that of false-negative diagnoses was 22.1%. In a corresponding earlier study (1964-75) carried out in the same department the rates were 35.1% and 55.6%, respectively, whereas the percentages of false-positive diagnoses remained the same. The considerable difference between older and younger age groups demonstrated in the previous studies was not found now. The improvement is believed to be due to the introduction of the new imaging, computerized tomography, ultrasonography, gammagraphy, and endoscopic retrograde cholangiopancreatography techniques.
