Successful treatment of Budd-Chiari syndrome with early gastric cancer.

In a patient with Budd-Chiari syndrome associated with early gastric cancer, the membranous obstruction of the inferior vena cava was removed by Percutaneus Transluminal Angioplasty (PTA) without any complication. Due to continuous poor clinical status and hepato-renal dysfunction, her early cancer stomach lesion was treated with Nd: YAG laser. In follow-up biopsies, malignant change became evident three months after laser therapy. By that time hepatorenal function gradually improved, and a partial gastrectomy was performed successfully. PTA is a safe and effective procedure for clinical management of Budd-Chiari syndrome, and when there is concomitant early stage malignancy, PTA followed by laser therapy paves the way for major surgery.

MR imaging of pituitary dwarfism.

The purpose of this study was to determine whether the MR findings of idiopathic pituitary dwarfism correlated with a clinical history of perinatal abnormalities and abnormal levels of pituitary hormones. MR examinations of 18 patients with pituitary dwarfism were performed; these patients were divided into two groups: those with ectopic posterior lobes (group 1) and those with normal posterior lobes (group 2). Among the seven patients in group 1, MR showed hyperintense signal at the median eminence of the hypothalamus, which was regarded as the ectopic posterior lobe of the pituitary gland. The mean anteroposterior length and height of the pituitary gland in group 1 patients were significantly smaller than those dimensions in the normal control group; the pituitary stalk was not detected in three of seven patients. Six of seven patients were products of breech presentation with perinatal asphyxia. The peak serum growth hormone level was less than 5 ng/ml when assessed by insulin-induced hypoglycemia or the clonidine test. MR findings in the 11 patients with pituitary dwarfism and normal posterior lobes were normal except that the mean size of the pituitary gland was slightly smaller than that of normal controls. The clinical history of these patients was normal except for perinatal asphyxia in one case. Our findings in patients with pituitary dwarfism, with or without an ectopic posterior pituitary lobe suggest that the ectopic lobe, visualized as a bright spot at the median eminence of the hypothalamus, may be common when pituitary dwarfism follows perinatal anoxic/ischemic episodes.

Gd-DTPA administered MR imaging of intracranial mass lesions: a comparison with CT and precontrast MR.

Magnetic resonance (MR) images of 13 intracranial mass lesions taken with the intravenous administration of gadolinium-DTPA were evaluated in comparison with precontrast computed tomography (CT), postcontrast CT, and precontrast MR. In the MR images taken with Gd-DTPA, tumor delineation improved in eight of 13 cases (62%) in comparison with precontrast MR images, and in seven of 13 cases (54%) in comparison with the overall evaluation of precontrast CT, postcontrast CT, and precontrast MR images. Tumor demarcation was unchanged in the remaining cases; no deterioration was observed with Gd-DTPA administration. MR imaging with Gd-DTPA should be performed to better evaluate the extent of intracranial mass lesions and to provide additional information.

Angiographic observations in idiopathic trigeminal neuralgia.

Twenty-four patients with idiopathic trigeminal neuralgia were treated by neurovascular decompression surgery. All patients underwent preoperative vertebral angiography. After surgery, the vertebral angiograms of these patients were retrospectively reviewed. In 23 of the 24 patients, the superior cerebellar artery (SCA) and/or the anterior inferior cerebellar artery (AICA) compressed the trigeminal nerve. In the remaining case, no such compression by vessels was observed. On review of the angiograms, most of the compressing arteries, and the sites and numbers of compression points could be identified. Most of the caudal points of the SCAs which did not compress the trigeminal nerve were located higher than the compression points of the SCAs. It was therefore concluded that vertebral angiography is useful in the preoperative evaluation of compressing arteries. Since superimposition of arteries could be avoided, the straight AP projection was the most valuable of the three projections employed.

MR imaging compared with CT, angiography, and myelography supplemented with CT in the diagnosis of spinal tumors.

To clarify the significance of MR imaging and the present status of CT, angiography, and myelography supplemented by CT (M-CTM), the radiological findings of 50 spinal tumors were reviewed and analyzed. MR imaging was most effective for visualizing morphological features such as the margins and/or inner structures of the tumors. CT was also effective for imaging "dumbbell" neurinomas and extradural tumors. Angiography was necessary in one hemangioblastoma and in cervical extradural tumors. M-CTM visualized the morphology of intradural extramedullary tumors and extradural tumors, but provided no new information in most of these cases. It was concluded that when using MR the indications for CT, angiography, and M-CTM are limited and that CT or angiography should be performed only in selected cases. M-CTM appeared to be unnecessary.

Solitary choroid plexus lipomas: CT and MR appearance.

Using MR imaging, the authors detected 23 cases of intracranial lipomas, among which were three cases of solitary choroid plexus lipomas. All the lipomas were located in the choroid plexus at the trigone. One of the three solitary choroid plexus lipomas was very small and could not be detected using CT. The authors believe that solitary choroid plexus lipomas are not as rare as was previously thought.

Postcontrast 3D-TOF-MR angiography of intracranial venous angiomas.

The clinical value of three-dimensional time-of-flight magnetic resonance angiography (3D-TOF-MRA) was assessed in 15 patients with intracranial venous angiomas (VAs), by comparing results thereof with those of postcontrast T1-weighted MR images (MRIs). All 15 VAs were identified both on the postcontrast MRIs and MRAs. The MRAs surpassed the MRIs in imaging entire structures, including the draining veins of the supratentorial VAs. For the infratentorial VAs, the MRAs and the MRIs were of equal value. Although the addition of MRA to the MRI procedure incurs more time and expense, the complete structure of the VA can thereby be identified three-dimensionally on MRA. Conventional angiography is apparently not required for the confirmation of a VA. Thus, it is concluded that MRA is very useful for diagnosing VAs.

Intracranial metastasis of renal cell carcinoma: MR imaging.

The purpose of this study was to evaluate the magnetic resonance (MR) findings of intracranial metastatic tumors of renal cell carcinomas (RCCs). MR images of seven patients with intracranial metastases of RCCs were retrospectively reviewed. Each of the seven patients had a single lesion, including five supratentorial intraparenchymal lesions, one 4th ventricular lesion, and one pituitary lesion. Intra- and/or peritumoral flow voids were observed in five of the seven lesions. The signal intensities of the tumors were not specific. All lesions showed gadolinium enhancement, including solid (3), nearly solid (2), and ring enhancement (2). We concluded that intra- and/or peritumoral flow void is a relatively characteristic MR finding of intracranial metastases of RCCs.

Intra-arterial digital subtraction angiography with extra-large fields using a computed radiography system in evaluating peripheral vascular disease.

The authors evaluated the usefulness of intra-arterial digital subtraction angiography (DSA) with extra-large field sizes using a computed radiography (CR) system in evaluating peripheral vascular disease (PVD). Intra-arterial DSA using CR was performed in 55 patients with suspected PVD. A 4 F catheter was advanced into the abdominal aorta via the transbrachial approach and 90 ml of contrast medium was injected during six exposures using a long leg changer mounting a total of 18 imaging plates on six surfaces. Visualization of the superficial femoral and popliteal arteries was judged as diagnostic in all cases. The abdominal aorta, iliac arteries, and subtrifurcation were also visualized in most cases, but visualization was suboptimal in some cases. As a complication, median nerve palsy occurred in one case. This technique is thought to be a useful method for evaluating PVD because of the advantages of a large field of view and wide exposure latitude, in spite of its relatively long processing time.

Dural non-cavernous sinus arteriovenous fistulas symptomatically simulating spontaneous carotid-cavernous fistulas: an analysis of angiographic findings.

The sites of fistulas and patterns of venous drainage in 34 consecutive cases of dural AVFs manifesting symptoms suggesting CCFs were retrospectively analyzed to determine the frequency of dural AVFs in sites other than the cavernous sinus and to ascertain their causative mechanisms. In five cases (15%), dural AVFs were demonstrated in sites other than the cavernous sinus. Among four, retrograde venous flow due to occlusion of the outflow tract or a rapid-flow shunt resulted in filling of the superior ophthalmic vein. A rapid-flow dural AVF in the anterior cranial fossa showed no occlusive changes in the sinuses, and increased pressure in the cavernous sinus was thought to be responsible for the symptoms. Embolization was effective for relief of the symptoms. Symptoms mimicking CCFs can be seen in dural AVFs in sites other than the cavernous sinus with retrograde venous drainage or with a rapid-flow shunt, conditions which are not as rare as previously believed.

Intracranial penetrating injuries via the optic canal.

Two cases of intracranial penetration of a plastic or wooden chopstick via the optic canal are described. CT scans showed the chopsticks as linear hypodense structures in the suprasellar cistern contiguous with the optic canal. In one case, MR imaging was performed, which clearly depicted the foreign body and adjacent brain structures. Although they are extremely rare, transorbital intracranial penetrating injuries via the optic canal require physicians' awareness.

Carcinosarcoma of the esophagus: three cases with immunohistological examination.

Carcinosarcoma of the esophagus is a rare malignant tumor with both carcinomatous and sarcomatous components. We present 3 cases of carcinosarcoma of the esophagus and discuss the histogenesis of the tumors. We performed immunohistochemical studies using various antibodies: anti-cytokeratin, anti-vimentin, anti-smooth muscle actin, anti-p53, and MIB 1 reacting with Ki-67 nuclear antigen.
