A recent trend of drug dependence in Kurume district (southern Japan).

With the intention of grasping the trend of drug dependence in recent years, a comparative study was made of the relative prevalence of three different types of drug dependence, i.e., the sociopathic, neurotic and iatrogenic types, in each of the three postwar decades among patients seen at the neuropsychiatric clinic of Kurume University. As a result it was found that the recent trend of drug dependence has the following features: (1) multiplicity of drugs involved, (2) a continuing high incidence of the iatrogenic types, (3) resurgence of the sociopathic type, making the recent pattern of drug dependence similar to that prevailing in postwar years, and (4) prevalence of dependence or abuse among the younger generation. Consideration was also given to the sociopathologic significance of these recent features of drug dependence.

The effects of a new benzodiazepine derivative, ID-540, on the averaged photopalpebral reflex in man.

The effects of 7-chloro-5-(2-fluorophenyl)-1-methyl-1 H, 1.4-benzodiazepin-2(3H)-one (ID-540), a recently introduced benzodiazepine derivative, on the averaged photopalpebral reflex (PPR), subjective symptoms, and serum levels of ID-540 and its principal metabolite, N-desmethyl-ID-540, following an oral dose of 0.5 mg or placebo were investigated in six male Japanese students in a double-blind, crossover design. The peak latencies of PPR showed a statistically significant prolongation, with maximum level at 3 h after administration, which recovered to the initial level within 4 h. The amplitude of PPR failed to show a definite response to the drug. The serum concentration of ID-540 reached a peak level 2-3 h after administration, and then decreased at 4 h. N-Desmethyl-ID-540 exhibited a slow- gradual rise in serum. The latencies of PPR were positively correlated with the serum level of ID-540 but not with the N-desmethyl-ID-540. It is concluded that the PPR test may be a useful method for predicting the clinical effects of psychotropic drugs.

Behavioral effects of protirelin in schizophrenia.

The effects of protirelin were compared with those of placebo in a multi-institutional, double-blind, controlled study of 143 chronic schizophrenic patients. Treatment with neuroleptics was continued during the study and 4 mg protirelin per day was orally administered for 14 days. Protirelin appeared to be significantly superior to placebo on the physicians' global ratings of efficacy. Increased motivation for work was isolated as a target effect but such symptoms as facial expression, emotional rapport, and psychomotor activity also appeared to be improved by addition of protirelin to the neuroleptics. Onset of the therapeutic effect of protirelin occurred within a week of medication in most of the patients who showed improvement. No adverse reactions were encountered.

Clinical experience with Org GB94 (mianserin hydrochloride), a new tetracyclic antidepressant.

(1) Org GB94 (Mianserin hydrochloride), a new tetracyclic antidepressant, was administered to 21 patients with depression or with depressive states. (2) Remarkable antidepressive response was obtained with Org GB94 in moderate and mild forms of endogenous, involutional and senile depression. (3) The onset of action of Org GB94 was rapid. (4) Anticholinergic side effects were extremely rare and other untoward effects were mild in nature.

Epilepsy and folic acid.

With 15 cases of epilepsy as the subject, studies were made of the therapeutic effects of folic acid on psychiatric symptoms and clinical seizures. Folic acid proved effective for a part of chronic and prolonged psychiatric symptoms shown by epileptic patients and brought on slight improvement in hyperexcitability, aggressiveness, bad humor and slowness of psychomotor activities. Improvement in mood was also observed. However, incidence of clinical fits increased in three cases. Electroencephalograms showed the desynchronizing phenomenon following oral administration of folic acid. Appearance or increase of spikes was noted in six cases. The epileptic group showed significantly low folic acid level in both serum and spinal fluid compared with the non-epileptic group. When folic acid was administered orally to the epileptic group, the serum folic acid level increased easily but the folic acid level in spinal fluid hardly increased.

Effects of ID-540 on averaged photopalpebral reflex in man.

Effects of ID-540, a recently introduced benzodiazepine derivative, on the averaged photopalpebral reflex (PPR), subjective symptoms and serum levels of ID-540 and its principal metabolite, N-desmethyl-ID-540 following an oral dose of 0.5 mg were investigated in 4 healthy male Japanese students in their early twenties. Both the latencies of PPR, P1 and P2 latency, showed a prolongation and maximum level at 2-2.5 hours after administration and tended to decline thereafter to control levels. The serum concentration of ID-540 showed a peak level at 2 hours after dosing, then showed a decline at 4 hours. The N-desmethyl-ID-540 exhibited a slow, gradual rise in the serum over the first 4 hours and there was a tendency toward a continued rise even at 24 hours. These parameters were of striking resemblance in the time-course of changes after oral dosing. Thus the PPR test may be a useful means of determining the clinical effects of anxiolytic agents. ID-540 appears to be an agent with remarkable anxiolytic effects.

Clinical features and laboratory findings of vibration disease: a review of 300 cases.

Clinical features and laboratory findings of 300 inpatients with vibration disease before and after treatments were reviewed. Having been using chain saws or pneumatic hammers for a long period, the patients were afflicted with Raynaud's phenomenon, numbness, pain or stiffness of fingers, pain of elbows and neck, stiffness of shoulders and lumbago. They had high incidences of complaints due to the disorder of the central nervous system, especially of the higher center of the autonomic nervous system; i.e. headache (52.0%), palmar hyperhidrosis (70.0%), forgetfulness (78.2%), fatiguability (61.3%), tinnitus (41.8%), impotence (55.1%), etc. Laboratory findings of the autonomic nerve activity tests, electroencephalograms and audiograms also suggested the disorder of the central nervous system. Treatments during three months had improved significantly the subjective symptoms and the objective findings (p less than 0.05 to 0.001). Thus, vibration disease should be considered as a systemic disease, including disorders of the central nervous system, especially of the higher center of the autonomic nervous system, and disturbances of the peripheral functions.

Behavioral effects of L-dopa and thyrotropin-releasing hormone in schizophrenic patients: a preliminary report.

L-Dopa (in a dose of 300-400 mg daily) was used concomitantly with conventional antipsychotic drugs in four patients with long-standing schizophrenia (of more than 10 years' duration). In two of these four patients marked symptomatic improvement was observed, while in the other two symptoms remained unchanged. Symptoms that were found most responsive included disturbance of contact with others and emotional poverty.
