[East German percentile values for newborn infants--1985 relation of age, parity, body weight and height of the mother and the birth weight of newborn infants].

Based on an analysis of very comprehensive data material multidimensional investigations on connections between age, parity, weight and height of mothers and birth weight of infants have been made. Maternal height as well as maternal weight have a considerable influence on birth weight of infants. At the same maternal height infant birth weight will rise in connection with the rise of maternal weight. Maternal age, parity and weight are correlated variables. The older a woman, the higher on average is not only her parity but at the same her weight, too. The rise of maternal weight per year in the statistic mean amounts to 401 g. Moreover, at the same age like primiparae second and subsequent parae are of higher weights so that partly maternal weight contains indirectly not only maternal age but also maternal parity. The following range of the influencing factors maternal height, weight, parity and age on birth weight can be given: 1. Height and weight as relatively equivalent factors, 2. parity and 3. age.

[Use of the new East German percentiles in the classification of newborn infants].

Re-classifying the newborns treated in the NICU of the University of Greifswald between 1983 and 1987, according to the percentiles (birthweight and length) of Lubchenco and the new standards recommended for the GDR after a data sampling in 1985, identic results could be found for the number of LGA using the 90th percentile. Certainly until the 34th week of gestation (but probably also thereafter), using the 10th percentile of the GDR standard, a to high rate (greater than 20%) of SGA infants could be found, whereas the range of birthweights of our own patients fitted exactly the distribution found by Lubchenco. The reason might be an underrepresentation of lifeborn VLBW infants in the GDR sample due to differences of the definitions used for the assessment of lifeborns and stillborns/fetal loss. Therefore, for practical use of the new standards as criterion for classifying AGA/SGA newborns until the 34th week of gestation the 5th percentile is recommended. A reevaluation of the distribution of birthweight in the GDR population is proposed including even-dead newborns in the data sampling.

[New percentile values for birth weight, body length, head and breast circumference of newborn infants in East Germany].

Somatometric data of 51,570 newborns (singles) delivered between the 28th and 42th week of gestation from different, representative areas of the G.D.R., concerning birth weight, body length, head and breast circumference are presented as tables and percentile curves separately for either sex. The values are recommended as reference values for the assessment of fetal development related to gestational age in the G.D.R.

[The effect of acetylsalicylic acid on gastric transmural potential difference in healthy children and in children with inflammatory diseases].

The transmural potential difference (tpd) of the gastric mucosa was investigated in relation to age and sex of the patients. The administration of acetylsalicylic acid in healthy children causes no significant decrease in gastric potential difference. In children with acute of chronic inflammatory diseases (elevated blood sedimentation velocity) the tpd was found to be increased compared with normal controls. After local ASA application a temporary, significant decrease of the tpd to normal values could be observed.

[Neonatal percentile values for East Germany--1985. 3. On the correlation of body height and weight of the mother in a two-dimensional classification of newborn infants according to birth weight and gestational age].

There were published percentiles of birthweight in relation to bodyheight of the mother as well as in relation to bodyweight and parity, bodyheight and bodyweight of the mother for term range 37th to 41st week of pregnancy. They were calculated through median quantities. The dates again derive from the investigation of 51,570 single births in elected regions of GDR in 1984 and 1985. The length of pregnancy was stated in full weeks. It was determined on the base of three parameters (by Naegele method, by sonography, and by neonatal maturity judgment). Selected percentiles of birthweight regarding the above influencing factors, distributed into sex, were summarized for use in practice.

Bilirubin binding and acid-base equilibrium in newborn infants with low birthweight.

Acidosis is known as a risk factor for the development of bilirubin encephalopathy in neonatal jaundice. However, few attempts have been made to evaluate the influence of acid-base state on bilirubin-albumin binding state in blood of newborn infants. Therefore, in 171 appropriate and 83 small for gestational age newborns (birthweight less than 2,500 g) the acid-base state in blood and bilirubin (BR) binding state in serum was measured at the ages of 3, 4, 5, and 8 days. There is a weak but significant correlation between standard base deficit and the ratio BR/reserve albumin as well as the toxic potential of serum BR. The results suggest that the higher risk in acidosis is not only caused by increased tissue binding of BR but also--at least partially--attributable to decreased BR binding in serum.

[Transepithelial electric potential difference (tPD) in clinical medicine. Methodologic aspects of measuring potential differences in the gastrointestinal tract].

The transepithelial potential difference of the gastrointestinal mucosa as a diagnostic parameter obtains increasing importance in clinical medicine. Therefore it is necessary to consider methodical aspects of the tPD-measurement under in-vivo conditions. The current paper presents methodical considerations for instance electrode arrangement, positions, composition of the electrolyte electrodes, diffusion potentials, rate of perfusion and unstirred water layer and their influence on the tPD-measurement.

[Quality assessment in neonatology].

After discussing some general factors influencing the result of medical care at all (socio-economic conditions, biological factors), the difficulties of neonatal quality control compared with the assessment of quality in obstetric care are pointed out (greater variability of the data, evaluation of long-term effects). In addition to the obstetric system of data collection a similar evaluation of neonatal data is recommended to be introduced and definitions for general use are given. The recommendations are based on own investigations of the predictive value of some data gained within the perinatal period (history, physical examination, laboratory tests etc.). The high expenditures of personnel and equipment, at present, don't allow a nationwide introduction of such records. But in future perinatal centers should collect their data in an unique way according to this recommendations for comparing the efficiency of perinatal care in voluntary programs of quality control. The experiences collected by those analyses will be of great value concerning planning the design of a nationwide data recording system in perinatal medicine.

The transmural potential difference (tmpd) of the bronchial mucosa in children with chronic nonspecific respiratory diseases (CF- and non-CF-children).

In 49 children with chronic nonspecific respiratory diseases (CNSRD) of them 6 with Cystic Fibrosis (CF), 18 with extrinsic bronchial asthma and 25 children with relapsing or chronic bronchitis, the tmpd were measured in the tracheobronchial system (bronchoscopy under general anaesthesia). The tmpd differed statistically highly significant (p less than 0.001). In asthmatics with significant eosinophilia in the bronchial secretions of the main bronchus we found a tmpd of 26.2 (+/- 9.2) mV, in bronchitics of 18.7 (+/- 6.2) mV and in CF-children receiving routinely N-acetylcysteine 6.1 (+/- 1.8) mV. Since the local application of this drug produced an additional immediate decrease of the tmpd in CF-children this suggests that such drugs, the presence or absence of eosinophils in the secretions, the products of intermediate cell metabolism or the different pathogenic process could be responsible for the varying values of the tmpd in the respiratory tract.

Increased concentrations of tyrosine and phenylalanine in the cerebrospinal fluid as a possible reflection of related changes of the brain tissue in newborns with intra-uterine growth retardation.

Plasma and cerebrospinal fluid concentrations of tyrosine and phenylalanine were measured fluorometrically in human term neonates appropriate for gestational age and in neonates small for gestational age with severe intra-uterine growth retardation of Type II. The intra-uterine retarded newborns showed significant differences of both plasma and cerebrospinal fluid tyrosine levels as compared with the non-retarded newborns: the mean plasma and cerebrospinal fluid tyrosine were determined to be 2.0 and 2.5 times, respectively, higher in the retarded group. The mean cerebrospinal fluid phenylalanine content was also increased in these newborns. The findings may reflect cellular disturbances of the tyrosine metabolization in the developing brain as it can also be found in experimental intra-uterine growth retarded animals.
