A congenital dermal sinus at the midline of the parietal region, which extends into the falx, is relatively rare. The following case is illustrative. A 3-year-old girl was noticed by her parents to have tumor at the midline of the parietal region without other disturbances and complaints. This girl had the normal mental and motor development, and was admitted to our clinic for cosmetic surgery. A round, elastic soft tumor, roughly 1x1x1 cm in diameter, was located at the midline of the parietal region. This tumor had a port wine cutaneous discoloration and an absence of normal hair growth in this area. Abnormal hair did not protrude from the sinus. This tumor had no dimple nor sinus opening. Skull XP showed a round lucent bone defect at the midline of the parietal region. This bone defect with sclerotic margin, was about 1 cm in diameter. Skull XP and tomography, with contrast medium injection into the tumor, showed that this tumor extended into the falx through bone defect. Angiography showed that there were no relations between the tumor and the superior sagittal sinus. PEG showed that there were no mass lesion nor dilatation of ventricles. Both spinal tap and EEG were normal. Operation for the congenital dermal sinus was performed on Nov. 4, 1977. This congenital dermal sinus extended into the falx and congenital cell rest tumor was not elicited.

Three cases of brain abscess following an occlusion of the internal carotid artery were reported. Case 1: A 6-year-old girl with congenital heart disease was admitted with headache, disturbance of consciousness and left hemiparesis. Right carotid angiography revealed an occlusion of the right internal carotid artery. After 6 months, she was readmitted with high fever. CT scan revealed a low density area and a ring-like shadow at the same site of cerebral infarction. Case 2: A 69-year-old man was admitted in semicoma and with right hemiplegia. Left angiography revealed an occlusion of the left internal carotid artery. After 2 months, a brain abscess was noted in the infarcted area. Case 3: A 20-year-old man with congenital heart disease, was admitted due to headache, vomiting and high fever. CT scan revealed a brain abscess in the right frontal lobe. Carotid angiography showed bilateral internal carotid artery occlusion. We concluded that diminution of cerebral oxygen and encephalomalacia are predisposing factors to the evolution of brain abscess.

The present study was undertaken to compare the effects of maximal treadmill and bicycle exercise on maximum oxygen uptake and blood flow in the lower extremity. Mean maximum oxygen uptake in maximal treadmill exercise was higher than that in bicycle exercise (p less than 0.001). Mean values and standard errors of blood flow measured immediately after maximal treadmill and bicycle exercise in the thigh were 39.1 +/- 4.0 and 44.2 +/- 2.8 ml/100 ml . min, the difference not being significant. However, a significant difference in blood flow in the calf measured immediately after both types of exercise was observed (p less than 0.001). Blood flow in the thigh immediately after bicycle exercise was significantly higher than that in the calf (p less than 0.001), whereas the difference between thigh and calf in treadmill exercise was small and statistically not significant. Leg blood flow, the average value of blood flow of the thigh and calf added together, was used as an index of blood flow in the lower extremity. It was found that the leg blood flow was significantly higher on the treadmill than with bicycle exercise (p less than 0.05). From these results, it is suggested that the lower maximum oxygen uptake observed during bicycle exercise as compared with treadmill exercise seems to be due to a lower blood flow in the lower limb.
