[The treatment of condylar fractures in children].

34 children with fractures of the condylar process were treated by intermaxillary wiring with subsequent functional therapy and/or functional appliances. 17 patients were treated by means of a functional appliance only. The results indicate the advantages of the treatment by means of functional appliances.

[Antibiotic support with oral penicillin in ambulant therapy for odontogenic infections].

In an open clinical trial 51 patients suffering from odontogenous infections were treated with 3 mega units of propicillin (potassium salt) daily for a period of 5 to 10 days. During the therapy all objective and subjective symptoms disappeared or were markedly improved. Only two patients reported slight side effects (gastric disorders, exanthema). Propicillin proved to be an effective and well tolerated supplement for the surgical therapy of odontogenous infections.

[Functional treatment after condylar fractures with and without an activator].

A follow-up study was carried out of 21 patients aged between three and 13 years who had sustained fractures of the condylar process. The patients were divided into three groups according to the type of treatment carried out--functional orthopaedics, intermaxillary fixation with functional orthopaedics and intermaxillary fixation with exercises. The clinical results showed in all cases good, even excellent restoration of the function. Radiological examination, however, revealed that restoration of anatomical form only occurred in 20% of the patients. Since several years after the trauma the orthodontically treated group showed even a remodelling of the fractured condyles functional treatment approach is recommended.

[Indications and limitations of surgical fracture treatment in children and adolescents].

The cases of 173 children (under 16 years of age) treated for facial injuries at the Department of Oral and Maxillofacial Surgery. Cologne, FRG, between 1981 and 1989 are reported. 63 of these patients had fractures of the body of the mandible. 32 patients could be treated with miniplate osteosynthesis (after Champy). Our results show that accurate diagnosis is imperative if complications and late sequelae are to be avoided.

Re-implantation of homologous, cultivated osteoblast-like cells for improvement of bone regeneration. An animal study.

The regeneration of osseous tissues after implantation of rat osteoblast-like cells cultivated in vitro was investigated. A cell line of rat osteoblast-like cells originating from explants of four inbred rats (Lewis) was established. The osteoblast-like cells were cultivated in vitro and re-implanted after 3 passages in cell culture into artificial femur defects of the same inbred Lewis rats. The cultivated osteoblast-like cells were embedded in bovine bone gelatine and implanted together with hydroxylapatite granulate (12 animals), pyrolized calf bone (12 animals) or without any additional material (12 animals). These bone replacement materials were used as a carrier for the osteoblast-like cells. After 10, 20, 40 and 80 days animals were killed and the treated femora were taken for histological examination and determination of bone growth rate. It was demonstrated that the cultivated osteoblast-like cells in this experiment had no measurable effect on quantitative bone growth, but caused a significant difference in the histological bone regeneration pattern.

[Is resection still a timely procedure in the treatment of keratocysts?].

Between 1977 and 1987 a total of 31 patients with keratocysts were treated at the maxillofacial surgery department of the University of Cologne. In two patient groups with 11 major mandibular cysts each, two different approaches were used: mandibular resection in one, and enucleation following previous fixation of the cyst epithelium with Carnoy's solution in the other group. The fact that only one recurrence was observed in the second group, gives us reason to recommend this safe and, for the patient, unstressful procedure.

[Dysesthesia and anesthesia of the mandibular nerve following dental treatment].

An inquiry to all practising dentists of Schleswig-Holstein and Cologne was performed to estimate the incidence of long time lingual nerve damage. 50 cases were reported and analyzed according to findings and causes. Further on the Cologne inquiry took into consideration 43 injuries of the n. alveolaris inferior. The results of the study allow a statement on the necessity to inform the patient before the operation.
