Polylactide-l test rods showed an initial stability of 130 N/mm2 in the three-point bending test; the stability decreased after implantation in the dorsal muscle of the rat within 5 weeks to about half the initial value. The copolymer materials showed less initial strength, and the stability decreased under the same conditions much earlier. All materials showed good tissue compatibility. After a transverse section of the beagle radius was stabilized with plates and screws out of polylactide-l, a delayed bony union with callus formation was observed.
Possible indications for the use of biodegradable implants are presented and the chemical compounds in question are briefly described. The results of a laboratory investigation to determine mechanical stability, tensile strength and molecular weight are reported, in order to characterize the polymers and co-polymers of the lactide under study.
Experiences gained in the CO2 laser treatment of patients with oral dysplastic precancerous lesions are presented. Besides 7 lichens planus, 50 leukoplakias of all grades of dysplasia, and carcinoma in situ and one lentigo maligna were removed superficially with a defocused laser. Within the average follow-up period of 37 months, 22% local recurrences were observed. In comparison with conservative drug therapy, conventional surgical procedures, and cryosurgical therapy, the CO2 laser treatment of multicentric premalignant diseases of the oral mucosa can be recommended as an alternative therapy.

The use of the champy miniplate facilitates stable osteoxynthesis in the mandible with exclusive intraoral access. Reliable fixation of lateral and centrolateral fracture of the mid-face in the region of the sutura frontozygomatica is possible with miniplate osteosynthesis. The principle, which differs totally that of compression osteosynthesis, was described, and the operative procedure presented. The results of 431 miniplate osteosyntheses were reported.
Facial and mandibular fractures in 139 children and adolescents were treated in the maxillofacial surgery department of the University Hospital in Cologne (RFA) between 1st January 1974 and 30th June 1979. A critical analysis of this group of patients demonstrates the manner in which therapeutic directives in the Cologne department are partially modified. Conservative treatment is the rule in children up to 12 years of age. In the older group, aged 13 and 17 years, preference is now given to osteosynthesis by miniaturized screw plates combined with conventional treatment.

A six-year-old girl is presented with an increasing mass involving the anterior vestibule and the floor of the mouth. Histologic and immunohistochemical examination revealed a poorly differentiated epitheloid leiomyosarcoma with destruction of the mandible. According to the CWS-96-study the patient underwent preoperative chemotherapy followed by complete resection of the mandibular body with the surrounding soft tissues. Recurrent tumor became evident only three months later on. The further treatment consisted of tumor resection, oral chemotherapy and irradiation. Nevertheless tumor control could not be achieved. The patient died of progressive disease 16 months after diagnosis. Leiomyosarcoma is extremely uncommon in childhood, especially with localisation in the oral cavity. Diagnosis is based on histologic examination and immunohistochemistry. In the presented case the lack of smooth muscle actin expression made diagnosis difficult. The preoperative chemotherapy could not achieve reduction of tumor size, so that extensive surgery became necessary. Estimation of the prognosis of the oral leiomyosarcomas in childhood is difficult. High grade tumours and involvement of bone seem to be associated with bad clinical outcome. The presented case is confirming that.
36 osteotomies performed between 1982 and 1988 involved opening the maxilla. Postoperative clinical, radiographic and endoscopic follow-up revealed that the majority of the patients had no or only minor pathological alterations of the maxillary sinuses. Sinuscopy showed only 2 maxillary sinuses to have more extensive mucosal alterations. Remarkably, it was particularly the osteosynthesis material protruding into the maxillary sinus that was surrounded by or overgrown with non-irritated mucosa.
An animal model utilizing 26 inbred rats was aimed at the question if the regeneration of bone tissue is enhanced by implantation of osteoblasts previously cultured in vitro. Bone tissue was harvested from inbred rats (Lewis) and a cell line of rat osteoblasts was established. The osteoblasts were cultured in vitro and multiplied in three passages. Characterization of the cells was by various methods. Monocortical defects were created in the distal portion of the rat femurs by drilling; and the cells, embedded in 25% bone gelatine, were reimplanted into these defects. After observation periods of varying lengths, the femurs were examined radiographically and bone growth in the defect area was evaluated histologically. The results of this study indicated that there was an initial effect of the reimplanted cultured osteoblasts on the bone growth pattern. At a later point in time this effect could no longer be demonstrated in the young, healthy rats.
