The transformation in vitro of peripheral lymphocytes of malnourished children.

Children with protein energy malnutrition showed high deranged cellular immunity as evidenced by impairment of lymphocyte transformation after stimulation by phytohaemagglutination (PHA). The proliferative response (PR) to PHA measured by estimating incorporation of tritiated thymidine into newly synthesized DNA. In-vitro proliferative response to PHA was used as a marker for studying the functional characteristics of T lymphocytes of children with different categories of malnutrition. PHA response of peripheral blood lymphocytes obtained from different categories of severely malnourished children were significantly low compared to healthy control children (P < 0.01). The results indicate that cell mediated immunity was grossly depressed in severe malnutrition.

Incidence of Aeromonas isolated from diarrhoeal children and study of some virulence factors in the isolates.

Stool samples from 305 children with diarrhoea and equal number of age and sex matched non-diarrhoeal control children, less than 5 years of age, were examined during the period from Sept 1988 to April 1989. Aeromonas spp. were isolated from 37 (12.1%) diarrhoeal and 05 (1.6%) control cases. Out of 37 diarrhoeal isolates 13 (35.1%) were A. hydrophila, 19 (51.1%) A. sobria and 05 (13.5%) A. caviae. All the isolated strains were tested for haem agglutination property and haemolysin production. Seventeen diarrhoeal and 05 control isolates were tested for cytotoxin production in He La cell line and enterotoxin production in rat ileal loop model and suckling mouse model. Chinese hamster ovary cell (CHO) assay and Gm-1 ELISA methods were also employed. Cytotoxin production was found in 82.5% of diarrhoeal and 40% of control isolates. Haemagglutination was found in 62.1% of Aeromonas isolated from diarrhoeal children and 20% from control children. Enterotoxin production was detected in 58.8% diarrhoeal and none of the control isolates by either of the methods. Of the virulence factors enterotoxin production was found to correlate well with enteropathogenicity but haemolysin, cytotoxin and haemagglutinin did not.

Indirect fluorescent antibody test in the serodiagnosis of visceral leishmaniasis in Bangladesh.

Indirect fluorescent antibody test (IFAT) was found to be 100% sensitive and 100% specific when tested with sera from 125 parasitologically proven kala-azar patients, 100 healthy controls and 50 sick control cases. Promastigotes of both L. enriettii and L. donovani as well as amastigotes of L. enriettii were found equally satisfactory as antigen for IFAT. From the results of the present study, it is concluded that IFAT is a highly sensitive as well as specific serological test for the diagnosis of kala-azar.

Studies on virulence determinants of Plesiomonas shigelloides.

Virulence determinants of nineteen strains of Plesiomonas shigelloides isolated from stool samples of diarrhoeal children were studied. Heat-labile toxin was detected in seven strains using rat ileal loop model and in none of the strains using Chinese hamster ovary cell assay system and by Enzyme-linked immunosorbent assay. Rat ileal loop model was used for the first time to assay toxin in P. shigelloides and is suggested to be a cheap and effective method of detecting labile toxin in the organism. Heat stable toxin, cytotoxin, hemolysin and hemagglutinin were not detected in the strains tested. Invasiveness could not be established by using HEp-2 cell assay system. The results of this study provide some experimental support for an etiological role for P. shigelloides in the production of diarrhoea.

Campylobacter jejuni as a cause of acute diarrhoea in children: a study at an urban hospital in Bangladesh.

The importance of C. jejuni as an aetiological agent of childhood diarrhoea was investigated at an urban children's hospital in Dhaka over a period of 1 year. C. jejuni was isolated from 25.5% of 102 diarrhoeal patients compared to 8.6% of 93 age and sex-matched healthy control children studied (P less than 0.002). The organism was isolated as a single pathogen in 17.6% of diarrhoeal patients. No C. coli was detected. The infection rate was significantly higher (P less than 0.05) amongst children up to 1 year of age (32.8%) compared to those aged over 1 year (15.9%). The clinical features of the majority of Campylobacter-positive cases resembled toxin-mediated secretory type diarrhoea. A fourfold rise of antibody titre against autologous Campylobacter strains was observed in the convalescent sera of Campylobacter-positive cases. The findings strongly suggest that C. jejuni is an important aetiological agent of childhood diarrhoea amongst Bengali children and therefore should be looked for in diarrhoeal illness.

Special properties associated with virulence of urinary Escherichia coli.

Escherichia coli (E. coli) isolated from 53 urine and 26 stool samples of patients with urinary tract infection (UTI) and 50 stool samples of normal individuals were studied to see their hemolytic, hemagglutinating and 'O' antigenic properties which might be related to virulence of the organism. Significantly higher number of E. coli isolated from urine of UTI patients were found to possess hemolytic, hemagglutinating properties and more frequently belonged to certain "O" antigenic groups either singly or in combination as compared to those isolated from stool of both UTI patients and normal individuals. Hemagglutinating property appeared to be most frequently associated with UTI.

Evaluation of co-agglutination (COA), counter immunoelectrophoresis (CIE), culture and direct microscopic (Dm) examination of cere-brospinal fluid (CSF) for detection of meningitis caused by common bacterial pathogens.

Cerebrospinal fluid from 260 children clinically diagnosed as meningitis were examined by Dm, culture, COA and CIE test. Dm revealed the presence of bacteria in 41 (15.8%) whereas culture showed growth of organism in 52 (20%) cases. COA and CIE test were done for the detection of antigen of H. influenzae, S. pneumoniae and N. meningitidis. Among the 3 methods viz. culture, COA and CIE test which were used for the detection of the above three organisms COA detected the maximum numbers (23.5%). COA test could detect antigen in both culture positive and culture negative CSF samples. COA test detected 100% of pneumococcal, 88.5% of H. influenzae and 66.7% of N. meningitidis antigens from CSF. Diagnosis by CIE in detecting H. influenzae and N. meningitidis antigens is inferior to culture and COA, whereas in detecting pneumococcal antigens CIE is superior to culture. So COA is a valuable, cheap, rapid and sensitive method for the diagnosis of meningitis caused by the above three organisms and when used along with culture 100% of cases can be diagnosed.

Penicillinase-producing Neisseria gonorrhoeae in Dhaka, Bangladesh.

Among the 52 strains of Neisseria gonorrhoeae isolated from cases of acute urethritis attending the dermatovenerology department of different hospitals of Dhaka city, 5(9.6%) strains were identified as penicillinase-producing (PPNG) and had minimum inhibitory concentration (MIC) of penicillin 4.0 micrograms/ml or more. Of the rest 47 (90.4%) non-penicillinase-producing strains (non-PPNG), 18(38.3%) were fully sensitive to penicillin (MIC = 0.06 micrograms/ml or less), 25(53.2%) had diminished sensitivity (MIC = 0.125 to 0.5 micrograms/ml) and 4(8.5%) were resistant to penicillin (3 had MIC 1 micrograms/ml and 1 had MIC 2 micrograms/ml).

Purification of residualizing glycoconjugate labels for protein by reversed-phase high-pressure liquid chromatography.

Residualizing labels are radioactive or fluorescent tracers used for identifying the tissue and cellular sites in which circulating proteins are catabolized in the body. When attached to protein the labels do not affect normal mechanisms of protein catabolism, but remain at the cellular site of protein uptake, after the carrier protein itself is degraded to diffusible catabolites. Until recently these labels consisted of biologically indigestible carbohydrates attached to a radioactive reporter molecule. In this report we describe the synthesis and purification of a new fluorescent residualizing label, N,N-dilactitol-N'-fluoresceinyl-ethylenediamine. The label is prepared by first derivatizing ethylenediamine 1:1 with fluorescein isothiocyanate and then coupling lactose to the remaining primary amino group by reductive amination. A rapid one step purification of this and other glycoconjugate labels by reversed-phase high-pressure liquid chromatography is described.

The aetiology of diarrhoea in children at an urban hospital in Bangladesh.

Enteric pathogens were studied in 104 cases with acute diarrhoea and in 74 age and sex matched concurrent controls. One or more pathogens were isolated from 59.1% of cases compared with 20.4% of controls (P less than 0.001). Single enteropathogens were detected in 33.7% and multiple enteropathogens in 25.4% of the cases. Enteropathogenic Escherichia coli (EPEC), Campylobacter jejuni, rotavirus, Vibrio cholerae non-O1 and enterotoxigenic Escherichia coli were the major pathogens detected. The high rate of isolation of EPEC from diarrhoea cases (23.1%) indicated a definite role for this pathogen in causing endemic diarrhoea in Bangladesh.

Enteropathogenic Escherichia coli diarrhea in hospitalized children in Bangladesh.

The role of enteropathogenic Escherichia coli (EPEC) was evaluated in a group of children with endemic diarrhea admitted to Dhaka Shishu Hospital in Dacca, Bangladesh. EPEC was detected in fecal samples of 23% of 104 cases and 8% of 74 concurrent control children. The most commonly isolated EPEC strains were serogroups O20a, O20c:K61; O20a, O20b:K84; O26:K60; and O18a, O18c:K77. Except for O26:K60, these groups had not been reported from Bangladesh. On testing for enterotoxin production, only two strains (serogroups O26:K60, O18a, and O18c:K77) were enterotoxigenic. None was enteroinvasive as tested in the guinea pig conjunctivitis model. Our study supports the concept that EPEC may be an important cause of endemic diarrhea in Bangladesh.

Diarrhoeagenic Escherichia coli in hospitalized children in Dhaka.

One hundred and four cases of acute diarrhoea upto the age of eight years and seventy four age and sex matched controls were studied to isolate enteropathogenic (EPEC), enterotoxigenic (ETEC), and enteroinvasive (EIEC) Escherichia coli. About 23% EPEC and 9% ETEC were isolated from diarrhoeal patients in contrast to 8% and 1% from control respectively. Among the twelve different serotypes of EPEC isolated, O2Oa O2Oc: K61 (33.3%) was the most predominant. No EIEC was isolated by sereny test. In 8 diarrhoeal cases, no other enteropathogen except EPEC was isolated. The study has highlighted the important role that enteropathogenic Esch. coli could play in causing endemic diarrhoea in Bangladesh.

General yeast infection in Bangladeshi women using contraceptives.

A study on the vaginitis due to candida infection among Bengali women using different contraceptives was carried out. A total of 368 patients using three different contraceptives (Pills, Injectables, IUD) and 316 age and parity matched women as control were studied. The incidence of vaginal yeast infection was highest (17.6%) among pill user. In control group, the incidence is lower (6.7%). The difference is statistically significant (P less than 0.05). No significant difference in vaginal yeast infection in women using other two types of contraceptives (Injection, IUD) in contrast to control group was found (P greater than 0.5). Candida albicans was the most common type of yeast infection (91.8%). No Torulopsis glabrata was isolated in this study. we did not find any relationship between duration of contraceptive usages and higher incidence of vaginal candidiasis (P greater than 0.5). This study also indicated that culture examination is much more sensitive than microscopic examination of vaginal swab in detecting yeast infection.
