CSF proteins in neurological disorders analyzed by immobilized PH gradient isoelectric focusing using narrow PH gradients.

We analyzed 352 paired CSF and serum samples with isoelectric focusing and immobilized pH gradients (IPG-IEF) using three different gradients. All MS patients (n = 35) had several (from 5 to 30) oligoclonal bands (OBs) in the CSF and most of them had few (from 1-8) OBs in serum. 54.5% patients with CNS infections (n = 12) had oligoclocal bands. OBs in these patients were fewer in number and located at the more acidic pH region compared with the MS samples. Also, corresponding serum bands were more often recognized. We suggest that IPG-IEF improves the diagnostic evaluation of the oligoclonal band pattern. The possibility of the use of extremely narrow gradients makes it a valuable tool for the analysis of selective proteins in various disorders.

Horizontal two-dimensional electrophoresis of cerebrospinal fluid proteins with immobilized pH gradients in the first dimension.

A horizontal two-dimensional electrophoresis method with immobilized pH gradient isoelectric focusing supplemented with carrier ampholytes in the first dimension was applied to cerebrospinal fluid (CSF) proteins. About 300 protein spots could be detected on the silver-stained two-dimensional maps of CSF samples. This high-resolution method is a tool worthy of consideration for the research of CSF proteins and disease-specific changes in different neurological disorders.

Circulating immune complexes containing chlamydial lipopolysaccharide in acute myocardial infarction.

The presence of circulating immune complexes (IC) was studied using two detection methods specific for chlamydial lipopolysaccharide (LPS) in paired serum samples of 44 patients (30 men and 14 women) with acute myocardial infarction (AMI). Forty-four random controls were individually matched for locality, age and sex with the AMI patients. As specificity controls for the IC assays single serum samples from 29 patients with diseases characterized by the presence of circulating IC were used. Fifty-seven per cent of AMI patients, 12% of their random controls and 10% of the patient controls were shown to have chlamydial LPS-specific immune complexes in their sera (P less than 0.0001, AMI versus random and patient controls). This finding provides further evidence of the possible association of chronic chlamydial infection with AMI.

First-trimester prenatal diagnosis of aspartylglucosaminuria.

Fetal aspartylglucosaminuria (AGU) was studied during the first trimester of pregnancy in six at-risk pregnancies using chorionic villus samples. The activity of aspartylglucosaminidase (AGA) was high in five cases, indicating an unaffected fetus. This was confirmed through delivery of healthy newborns with a normal pattern of urinary oligosaccharides. Low enzyme activity in an uncultured biopsy specimen and in cultured amniotic fluid cells in one case demonstrated that the fetus was affected. The pregnancy was terminated and the prenatal diagnosis was confirmed by showing reduced AGA activity in cultured fibroblasts of the fetus.

Intramembrane particles and filipin labelling on the membranes of autophagic vacuoles and lysosomes in mouse liver.

Morphologically detectable protein (intramembrane particles) and cholesterol (filipin labelling) in the membranes of autophagic vacuoles and lysosomes were studied in mouse hepatocytes using thin-section and freeze-fracture electron microscopy. Both isolated autophagic vacuoles and lysosomes, and intact tissue blocks were used due to the facts (i) that lysosomes are difficult to recognize in freeze-fracture replicas of intact hepatocytes, and (i) that filipin penetration into the tissue blocks is unsatisfactory. Intramembrane particle density was low in the membranes of early autophagic vacuoles (defined as round-shaped vacuoles in which an inner membrane parallel with the outer limiting membrane was clearly visible). The lysosomal membranes contained considerably more intramembrane particles. Particle-rich lysosomes or other vesicles were observed to fuse with the early autophagic vacuoles. The membranes of nascent autophagic vacuoles with morphologically intact contents were usually not labelled by filipin, whereas the membranes of all other autophagic vacuoles and lysosomes were heavily labelled. The increased cholesterol in the membranes of slightly older autophagic vacuoles is presumably derived from cholesterol-rich lysosomes or other vesicles fusing with the vacuoles and from the degrading organelles inside the autophagic vacuoles.

Serologic response against cardiolipin and enterobacterial common antigen in young patients with acute myocardial infarction.

Elevation of anticardiolipin antibodies has been observed in myocardial infarction and in many infections. To elucidate this topic, paired serum specimens from 40 patients with acute myocardial infarction were tested for anticardiolipin antibodies (solid-phase enzyme immunoassay) and enterobacterial common antigen antibodies (indirect hemagglutination test). Forty-one randomly selected individuals and 30 patients with chronic coronary heart disease served as controls. All individuals were males whose maximum age was 50 years. In patients with acute myocardial infarction the levels of anticardiolipin antibodies in paired sera rose significantly in all immunoglobulin classes (18% in the IgG class, 26% in the IgA class, and 43% in the IgM class), whereas no elevations occurred in the other groups. Fifteen of the acute myocardial infarction patients also showed at least a fourfold increase in enterobacterial common antigen antibodies, as compared with only one such increase in the two other groups combined. The increases were less marked than those seen in pronounced enterobacterial infections. These two reactions were closely associated.

A radiological study of degenerative vascular changes in the external carotid region and carotid bifurcation.

In a series of 79 autopsies performed on subjects aged from 18 to 95 years occlusions in the external carotid, maxillary and inferior alveolar arteries were rare: only two cases were found in the maxillary and one in the inferior alveolar arteries. Tortuosity of the arteries increased with age, being most extensive in the external carotid and least in the inferior alveolar artery. Variations in the diameter of the maxillary artery occurred in 11 persons (14%), the incidence increasing with age. The youngest person showing such changes was aged 55 years. The average diameter of the arterial lumen of the inferior alveolar artery decreased with advancing age. This narrowing of the lumen also correlated with the loss of teeth. Calcified atherosclerotic plaques in the carotid bifurcation were present in 59% of the cases studied. In persons aged over 60 years a correlation was found between the carotid calcifications and coronary artery disease.

Serological evidence of an association of a novel Chlamydia, TWAR, with chronic coronary heart disease and acute myocardial infarction.

Paired sera from 40 male patients with acute myocardial infarction (AMI), 30 male patients with chronic coronary heart disease (CCHD), and 41 controls, matched for sex, age, time, and locality were investigated for antibodies to a novel type of Chlamydia sp, TWAR, and to chlamydial lipopolysaccharide (LPS) group antigen. 27 patients with AMI (68%), and 15 (50%) patients with CCHD had raised IgG (greater than or equal to 128) and/or IgA (greater than or equal to 32) titres in the microimmunofluorescence test with chlamydia TWAR. Both frequencies were significantly higher than in the controls (7, 17%). 26 (68%) of 38 patients with AMI also showed a significant seroconversion in enzyme immunoassay with LPS antigen; this response was absent in all patients with CCHD and all but 1 of the controls. Chronic chlamydial infection could be a factor in the pathogenesis of cardiovascular diseases.

Blood pressure and five year survival in the very old.

During 1977-8 we measured blood pressure in 561 old people (83% of those aged 85 or more living in Tampere) and analysed their five year survival according to their blood pressure group. The subjects were divided into six groups on the basis of their blood pressures (from less than 120 to greater than 200 mm Hg systolic, from less than 70 to greater than 110 diastolic). The greatest mortality was observed in those in the lowest systolic and lowest diastolic groups. Mortality was least in subjects with systolic pressures of 160 mm Hg or more and diastolic pressures of 90 mm Hg or more. The most essential finding in this series of the very old was an increased mortality in the lowest blood pressure groups.

Immobilized pH gradient isoelectric focusing of cerebrospinal fluid proteins.

Isoelectric focusing in immobilized pH gradients, supplemented with 0.5% w/v carrier ampholytes was applied for studies of native proteins, especially immunoglobulin G, in cerebrospinal fluid and serum. All 72 paired samples were run on pH 4-10 gels; 25 of them were also examined in pH 7-10 gels. Silver staining and nitrocellulose blotting with amplified immunoperoxidase detection of immunoglobulin G were used for protein visualization. Intrathecally produced immunoglobulin G was resolved into sharply focused, straight and easily identifiable fractions. The pH gradients were stable and the inter-gel reproducibilities of individual immunoglobulin G patterns were good.

Electrocardiographic findings and 5-year cardiovascular mortality in very old people.

The electrocardiograms (ECGs) of 558 individuals, 83% of the total population aged 85 years or over in the city of Tampere in 1977, were assessed according to the Minnesota code in a health survey. The underlying causes of death were identified from the death certificates during the following five years. The total mortality rate was 70%, while the cardiovascular mortality was 51%. The lowest 5-year mortality from cardiovascular and cardiac diseases was observed in subjects with no codable or only minor ECG abnormality in the initial examination. The highest cardiovascular mortality rate was found in individuals with atrial fibrillation, atrioventricular blocks and left or right bundle branch block. The cardiac mortality was related to left or right bundle branch block and atrial fibrillation. Mortality from stroke showed the highest association with clockwise rotation and left high R waves.
