Development of a Stafne mandibular bone cavity. Report of a case.

The present study reports a case in which a clinically asymptomatic radiolucent change surrounded by a sclerotic wall developed below the second molar in the mandible of a middle-aged man. The change was diagnosed as a typical case of Stafne bone cavity. In the earlier literature, development of a mandibular bone cavity has been described in only three patients. The appearance of bone cavities in a mandible that has previously been found normal indicates that these defects are not congenital but developmental processes.

A controlled randomized study on the effect of long-term physical exercise on the metabolic control in type 2 diabetic patients.

The effects of 4 months' physical exercise on the metabolic control in non-insulin-dependent diabetes were studied in 25 patients divided randomly into exercise (n = 13) and control (n = 12) groups. In the exercise group glycosylated hemoglobin A1 fell from 9.6-8.6% (p less than 0.01) and the 2 hour plasma glucose in oral glucose tolerance test decreased from 19.7-16.5 mmol/l (p less than 0.01). The mean fall in fasting plasma glucose from 11.8-10.5 mmol/l was not statistically significant. The responses of plasma insulin and C-peptide to oral glucose increased significantly during the exercise period. However, patients with initially poorest metabolic control were not able to improve their physical fitness and did not show significant improvement in metabolic control. In the control group no changes occurred in any parameter of glucose metabolism. The results suggest that physical exercise is beneficial for the glycemic control in patients with mild and moderate non-insulin-dependent diabetes. Increased insulin response is at least one main mechanism of the action of exercise.

Mortality in relation to urinary characteristics in the very aged.

The value of urinary characteristics observed in a health survey of people aged 85 years or more was studied in a 5-year follow-up. Of the 561 aged people participating in the initial survey, 167 (30%) were alive after 5 years. Hematuria, pyuria, and albuminuria were associated with high mortality; urinary acidity was associated with low mortality. Positive bacterial staining, glucosuria, and specific gravity of urine were not related to the survival rate. Contrary to earlier observations, bacteriuria found in the urinary bacterial culture did not have any prognostic significance in respect of survival during the follow-up period. Judging from the results, the medical treatment of asymptomatic bacteriuria, with the aim of reducing the mortality of the very aged, appears to be unwarranted.

ECG findings and survival in very old people.

The electrocardiograms of 559 persons, 82% of the total population 85 years of age or over in the city of Tampere in 1977-1978, were evaluated according to the classification of the modified Minnesota code. The relative five-year survival rates matched by sex and age were calculated for subjects with various ECG findings. Of the series, 391 persons (70%) died during the five-year follow-up period. The lowest mortality rate was observed in those with no codable change or with minor abnormality in the ECG. Electrocardiographic abnormalities suggestive of ischaemic heart disease were associated with significantly increased mortality. In respect of single ECG changes the greatest mortality rate was found in subjects with atrial fibrillation or first-degree atrioventricular block revealed in the ECG. No increased risk of death was associated with left and right bundle branch block, supraventricular premature beats and ventricular premature beats.

Central neurilemmomas of the mandible. Report of a case.

Central neurilemmomas involving the jaws are rare benign tumours probably originating from the neural sheath. We report 2 cases involving the posterior part of the mandible. The 1st case (male, age 20 years) had no symptoms of a tumour, which was discovered by accident in an orthopantomogram. The 2nd case (female, age 66 years) had experienced pain because of a tumour. The radiographic appearance of these tumours seemed to be non-specific. These 2 neurilemmomas have recurred, probably due to incomplete primary removal of the tumour.

A health survey of the very aged in Tampere, Finland.

The population aged 85 years or over (n = 674) living in Tampere, Finland, was surveyed in 1977-78. Altogether, 561 persons (83%)--99 men and 462 women--were examined. The study comprised questionnaire, medical examination, laboratory tests, ECG and chest X-ray examination. Of the subjects, 24% were hospitalized, 22% were in old people's homes and 54% lived at home. The most common symptoms were aches and pains (24%), vertigo (22%), defective vision (15%) and defective hearing (12%). Congestive heart failure (49%), dementia or confusional state (28%) and urinary tract infection (22%) were the most common diseases.

Prevalence of ECG findings in very old people.

The electrocardiograms of 559 persons, 83% of the total population 85 years of age or over in the city of Tampere, were evaluated according to the classification of the modified Minnesota code. Electrocardiograms showed no codable item in only 3.6% of the subjects. The prevalence of the most frequent electrocardiographic findings was: T wave inversion 53.3%, counter-clockwise rotation 42.9%, ST junction depression 42.8%, premature beats 33.8%, Q-QS pattern 21.3% and atrial fibrillation 17.4%. The prevalence of left axis deviation, left anterior hemiblock and right bundle branch block was statistically significantly higher in men than in women. Electrocardiographic abnormalities suggestive of ischaemic heart disease were present in 45.3% with no statistically significant relation to sex or age. The results are discussed and compared with those of electrocardiographic studies concerning age groups over 80 and 90 years.

Urinary characteristics and infection in the very aged.

In the course of a health survey covering 561 inhabitants of Tampere, Finland, aged 85 or over, morning urine specimens were studied. 82% of the participants were women. Significant bacteriuria was recorded in 27% of the subjects, 13% of the men and 30% of the women. Pyuria was recorded in 47% of the total material. Replying to the questionnaire, 5% of the subjects reported urinary symptoms and 7% reported to be suffering from urinary tract infection (UTI) diagnosed by a physician. The prevalence of drug-treated UTI was 9% of the subjects replying to the questionnaire. Although positive urinary 'abnormalities' are common in very old people, they do not cause significant distress and do not constitute a disease requiring drug treatment in asymptomatic old people.

Blood glucose and diabetes mellitus in subjects aged 85 years or more.

Blood and urine glucose values and the prevalence of diabetes mellitus were determined in the course of a health survey covering inhabitants of Tampere, Finland, aged 85 or over. Out of the total cohort, 561 (83%) subjects, of whom 82% were women, were examined. A high blood glucose level (greater than or equal to 7.0 mmol/l) was recorded in 10% of the males and 7% of the females. The prevalence of diabetes mellitus in the series was 17%. Five per cent of the total series were on drug treatment for their diabetes; 5% of the diabetics were on insulin therapy and 25% took oral antidiabetic drugs, while 70% were either on diet or had no treatment. Diabetes seems to be common among the very aged, but it is generally mild and easy to treat.

Radioimmunoassay of human prostate-specific acid phosphatase. A sensitive and specific assay for semen detection in forensic medicine.

Human prostate-specific acid phosphatase was measured by a rapid and sensitive radioimmunoassay in samples which originated from investigations of suspected sexual assaults and in which microscopic examination did not show sperm cells. Of the 12 samples, Ouchterlony double immunodiffusion showed the presence of spermatic fluid in three samples, whereas radioimmunoassay detected an additional seven spermatic fluid-positive ones. It is also shown that the procedure is sensitive enough for forensic medical purposes to detect semen in highly diluted postcoital vaginal fluid, and no cross-reactivity with nonseminal samples tested was observed.
