[Morphologic studies of the toxicity of ethacrynic acid in the spiral prominence].

Ultrastructural changes in the guinea pig spiral prominence were studied at various times after a single intravenous injection of ethacrynic acid (40 mg/kg body-weight). Initial swelling of endolymph-facing epithelial cells was followed by dilatation of the intercellular spaces and marked shrinkage of the stroma cells surrounding the spiral prominence vessel. While the changes in the stria vascularis and the spiral prominence progressed at about the same pace, the regression to normal ultrastructure set in earlier in the spiral prominence.

Macromolecular transport in the spiral prominence.

Transport of macromolecules in the spiral prominence of the guinea pig was studied after perilymphatic and intravenous horseradish peroxidase (HRP) injection. HRP was taken up by the spiral prominence epithelium from the perilymphatic space and in a much higher degree from the endolymphatic space. A highly endocytic activity was shown by the stroma cells surrounding the spiral prominence vessel. HRP particles were stored in the cytoplasm within vesicles and vacuoles of different sizes and different contrasts. The mode of tracer storage in the spiral prominence epithelium and in the stroma cells from the morphological point of view suggests lysosomal digestion. HRP was transported from the perilymphatic space into the spiral prominence vessel lumen by means of vesicular transport through the vessel wall, while transport of the tracer from the vessel lumen into the perilymphatic space after intravenous injection was not observed.

[Antibodies against viruses of the herpes group in cerebrospinal fluid in idiopathic facial paralysis. Enzyme immuno-serologic studies].

In a prospective study, paired CSF samples and sera of 12 patients suffering from Bell's palsy were examined for IgG and IgM antibodies against Herpes simplex virus (HSV), Varicella-zoster virus (VZV) and Cytomegalovirus (CMV). For determination of antibodies an enzyme-linked immunosorbent assay was carried out. CSF- and serum samples were taken within one week after Bell's palsy had been diagnosed and were compared with further CSF- and serum samples taken 2 weeks later. The results are discussed.

[Acute neurological complications following intrathecal fluorescein injection (author's transl)].

This report describes two cases of grand mal seizures following intrathecal injection of fluorescein to delineate CSF-leakage. Recurrent phases of apnea necessitated artificial ventilation in one case. The results of CSF and serum investigations are discussed as well as EEG and EMG findings. As fluorescein has proved to be a potentially epileptogenic drug, its use for patients with a past history of seizures should be very carefully considered. All patients having received intrathecal fluorescein should be kept under observation for at least 4 to 6 hours.

[On the viral etiology of Bell's palsy (An enzyme-linked immunosorbent assay study) (author's transl)].

In a prospective study paired sera of 14 patients suffering from Bell's palsy were examined for antibodies against Varicella-zoster Virus (VZV), Herpes-simplex Virus (HSV), Cytomegalovirus (CMV) and Epstein-Barr Virus (EBV). For determination of antibodies against VZV, HSV and CMV an enzyme-linked immunosorbent assay was carried out. Indirect immunofluorescence was carried out to determine antibodies against EBV. Serum samples were taken within 5 days of Bell's palsy having been diagnosed and were compared with further serum samples taken 4 weeks later. No evidence of significant differences between the antibody titers of the paired sera was found. The viral etiology of Bell's palsy due to an exogenous infection or by activation of a latent infection seems unlikely.

[Basal cell carcinoma developing in a preauricular fistula (author's transl)].

Malignant neoplasms in lateral cervical cysts and fistulae have been frequently described in the literature. In the following case, a tumor is reported which was found in the unusual localization of a preauricular fistula. The histologic diagnosis was basal cell carcinoma. In this 41-year-old male patient the tumor led to local recurrent inflammatory symptoms. Therapy consisted first of excision of the fistula. After final diagnosis was made, total parotidectomy and an additional excision of the skin were carried out.

[Recurrent periarteritis nodosa with predominantly cerebral signs in children (author's transl)].

Periarteritis nodosa occurs also in children and has the same wide spectrum of signs and symptoms as in adults. In a 4 1/2 year-old girl the disease at first was characterised by cerebral, encephalitis-like symptoms, arterial hypertension, intestinal wall necrosis and, finally, pericardial tamponade as a result of rupture of an arteritic coronary artery aneurysm. An earlier attack had involved liver and skeletal muscle, with necrosis in the liver. Peculiar morphological features within the "classical" course of periarteritis nodosa are endarteritic changes of single retroperitoneal and coronary arteries.

MR imaging of the oropharynx and tongue: comparison of plain and Gd-DTPA studies.

The effect of intravenously administered gadolinium-diethylene-triamine pentaacetic acid (Gd-DTPA) on signal intensity in comparison to plain noncontrast imaging was analyzed. Sixty-one patients with diseases of the oropharynx and tongue base were examined using multiplanar magnetic resonance (MR) imaging. The contrast medium Gd-DTPA was administered in 26 cases. Thirty-eight patients also underwent CT. Forty-one patients had surgery and pathological verification. Plain MR imaging was equal or superior to CT in all except one patient. Marked contrast enhancement produced by Gd-DTPA was observed in carcinomas, sarcomas, inflamed salivary glands, and in normal pharyngeal mucosa. The usefulness of Gd-DTPA may be increased by measuring the enhancement in signal intensity versus time to allow a better differentiation of histological features.

Demonstration of statistically significant correlations between 8 and 12 kHz atmospherics and sudden deafness.

In the last few years we have shown that atmospherics of different frequencies correlate with the onset of epileptic fits, heart infarcts and with the intensity of an inflammation reaction in rats. Now we show that sudden deafnesses are related (multiple R2 - 0.08) with the onset of 12 kHz in the first part of the day before the sudden deafness and the absence of 8 kHz in the first part of the day of this event.

Ultrastructural localization of K+-dependent, ouabain-sensitive NPPase (Na-K-ATPase) in the guinea pig inner ear.

Ultrastructural localization of the phosphatase component of Na-K-ATPase in the inner ear of the guinea pig was studied utilizing p-nitrophenyl phosphate (NPP) as substrate. In the stria vascularis NPPase activity was restricted to the cytoplasmatic side of the basal plasma membranes of the marginal cells. In the spiral prominence enzyme activity was resolved only occasionally on the basolateral membranes of the spiral prominence epithelium, especially after longer incubation periods. The remaining inner ear tissue was unreactive. Possible transport mechanisms of potassium across the apical marginal cell membrane are discussed.
