[Immunologic findings in the newborn infant. II. Studies of humoral immunity, reference values of immunologic studies in the newborn infant].

After dealing with cellular immune aspects in the first part of the paper the authors describe in the second part the state of humoral immunity in healthy full-term newborns aged one to three days. The number of complementreceptor lymphocytes was 17.0 +/- 8.9% or 0.75 +/- 0.51 (mean +/- S.D.) respectively, with the EAC-rosette method. The percentage value equalled that of adults but the absolute count is higher. Using the immunodiffusion method IgA was detectable in 4 out of 132 newborns only 8.86 +/- 2.52 g/l (mean +/- S.D.) was measured as an average IgG concentration. With regard to the IgM content the values often exceeded 0.30 g/l in newborns of mothers who had been suffering from pre-eclampsia. Their mean value of 0.35 +/- 0.21 g/l, too, was significantly higher than 0.13 +/- 0.08 in the resting infants. Raised IgM values frequently coincided with high spontaneous transformation activities in lymphocyte cultures. The possible causes are discussed. Complement C3-component was measured on average with 0.78 +/- 0.18 g/l and C4-component with 0.34 +/- 0.11 g/l. When calculating reference values for newborns the results of infants with maternal pre-eclampsia have been omitted.

[Differential acetonemic vomiting diagnosis--recurrent Addison crises as an early sign of adrenoleukodystrophy].

After 5 years of cyclic vomiting an 8 year old boy was presented with coma and hyponatremia. ACTH and renin plasma concentrations were elevated, cortisol concentrations did not rise after ACTH-stimulation. Behavioural abnormalities including secondary enuresis and dysarthria drew attention to the possibility of an association of adrenal insufficiency with leucodystrophy. NMR tomography of the brain showed symmetrical demyelinated areas in the parieto-occipital regions. Very long chain fatty acids were elevated. The mother showed discrete neurological symptoms and elevated long chain fatty acids. Cyclic vomiting might suggest adrenoleukodystrophy.

[Effect of short-term aerogenic hypoxia on the behavior of heart rate and blood pressure in newborn swine].

The adaption of the cardiovascular system to hypoxia has been examined in piglets aged 3 to 81 hour. The reaction of the piglets to a shorttime normobaric hypoxia was very different: In 50% there was an inadequate reaction with decrease of blood pressure, heart rate, pulse pressure and therefore of stroke volume, in 30% there was in compliance with the adult mechanism a compensatory increase of blood pressure, heart rate, pulse pressure and stroke volume. Only in 2 cases a compensatory increase of blood pressure could be observed in combination with a simultaneous decrease of heart rate. The results refer once more to the fact, that the reactions of the cardiovascular system of the neonate to hypoxia can be very variable and require an individual therapeutic regime provided that they are transferable to the human newborn and premature baby.

[Morbidity and mortality in 1984-1985 of premature infants in breech presentation with a birth weight equal to or less than 1,500 g. A prospective study on the question of vaginal or abdominal delivery].

The effect of the obstetrician's choice for delivery route on the early morbidity and mortality of very low birth weight infants in breech presentation (less than or equal to 1500 g) was studied prospectively in 22 children. Criteria of the morbidity were beside antenatal cardiotocogram, the Apgar-score at 1, 5 and 60 min., the cord venous pH-value and the duration of assisted ventilation after immediate delivery. From the first to 28th day of life we continuously analyzed the lowest oxygen pressure, the highest carbon dioxide pressure, therapy with NaHCO3 and the duration of assisted ventilation. But there were no significant differences in morbidity data and mortality between the newborn infants after vaginal and abdominal delivery. However the brief duration of assisted ventilation and brief ventilation time with a FiO2 greater than 0.3, after abdominal delivery were striking despite of a negative starting point of these children. For that reason we are of opinion that a well-timed abdominal delivery of very low birth weight infants in breech presentation shows a favourable influence on the normalization of metabolic and respirations data in the postnatal period and by this on the early morbidity.
