Clinical evaluation of human granulocyte colony-stimulating factor in chemotherapy for ovarian cancer.

The clinical evaluation of human granulocyte colony-stimulating factor (G-CSF) in 38 patients treated with chemotherapy for ovarian cancer stage III was investigated among 3 groups. G-CSF was not given in group A (19 courses), was administered from day 5 of chemotherapy in group B (53 courses), and was given when the WBC count decreased to below 2,000/mm3 in group C (29 courses). The time to nadir was significantly shorter in group B compared with groups A and C and revealed 10 days for the WBC count and 11 days for the neutrophil count (p <0.01), with mean nadir values of 2,896/mm3 and 982/mm3 respectively, and so the count of WBC and neutrophil have been kept during the course. The effect of G-CSF was not modified by age, body weight or the number of chemotherapy courses in groups B and C. These results demonstrate that early treatment with G-CSF may allow increased intensity of chemotherapy by using greater doses or by shortening of the interval between cycles.

A case of pseudo-Bartter's syndrome due to intestinal malrotation.

Intestinal malrotation presenting beyond the neonatal period is associated with a multiplicity of symptoms, which are often non-specific and, consequently, are associated with delays in diagnosis. Pseudo-Bartter's syndrome, which mimics the manifestations of Bartter's syndrome, can be caused by a severe chloride deficiency secondary to vomiting, diarrhea, perspiration, diuretic abuse and so on. We describe a 6 year old boy who had been admitted to hospital three times during the preceding year. The patient lapsed into a critical condition with profound hypochloremia and hypokalemic metabolic alkalosis induced by extremely massive vomiting. The attacks of vomiting were spasmodic and self-limited. During the episodes of vomiting he fulfilled the criteria of pseudo-Bartter's syndrome, including hyperreninemia, hyperaldosteronism and normal blood pressure, but in the intervals between attacks he was completely asymptomatic. At the third admission, examination supported an overall clinical picture of bowel obstruction, which was confirmed by radiographic examination. Laparotomy revealed a midgut volvulus with intestinal malrotation. After surgery he made a good recovery and was symptom-free. In this patient, the high degree of hypochloremia and hypovolemia activated the renin-angiotensin-aldosterone system, then aldosterone promoted intensive reabsorption of sodium and excretion of potassium into the urine. Consequently the diagnosis of pseudo-Bartter's syndrome was establish on the basis of an extreme decrease in urinary chloride and an increase in urinary potassium concentration. It is relatively rare for vomiting due to intestinal malrotation to induce pseudo-Bartter's syndrome. The importance of considering this rare diagnosis in such cases is discussed.

A positive correlation between T-cell-receptor mutant frequencies and dicentric chromosome frequencies in lymphocytes from radiotherapy patients.

Dose estimates for the assessment of future risks, following accidental exposure to radiation, for certain diseases such as cancer usually rely on both physical and biological quantitative analyses. A traditional biological method of choice is the measurement of chromosome aberration frequencies in peripheral-blood lymphocytes. However, thorough examination of large sample populations is time and labor intensive. Recently, it became possible to measure mutant frequencies in T lymphocytes; one method is a colony assay at the HPRT gene, and the other is a flow-cytometric assay at the T-cell-receptor (TCR) gene. To test for the possible use of these mutation assays, concurrent measurements were taken on blood samples from women who previously received a full course of radiation therapy for gynecological cancer. The results showed that the frequency of TCR mutants correlated reasonably well with that of dicentric chromosomes, whereas the frequency of HPRT mutants did not. Possible uses of the TCR mutation assay in combination with the conventional chromosome analysis or micronucleus assay after exposure of a relatively large population are discussed.

Cytological analysis for human papillomavirus DNAs in cervical intraepithelial neoplasia by in situ hybridization.

Human papillomavirus (HPV) type 16 and 18 DNAs are reported to be associated with uterine cervical cancer. In order to investigate the relationship between the presence of HPV DNA and cervical intraepithelial neoplasia (CIN), we attempted the cytological detection of HPV DNAs in uterine cervical smear samples. The samples included those of severe dysplasia and carcinoma in situ (CIS). They were analysed by DNA-DNA in situ hybridization using biotinylated HPV DNA probes. The results of in situ hybridization analysis revealed that HPV sequences were present in the nuclei of cells with koilocytotic atypia. When probed for HPV type 6, 11, 16 and 18, the nuclei of dysplastic cells and cancer cells were positive for HPV type 16 and 18 DNA. Out of 26 CIN cases, 17 contained HPV type 16 DNA and 5 contained HPV type 18 DNA. We suggest that cytological analysis for HPV sequences by the in situ hybridization technique might provide a molecular diagnosis for assessment in uterine cervical intraepithelial neoplasia.

Precise diagnosis by gene analysis and successful management of delivery in three patients with type IIB von Willebrand disease.

Type IIB is a rare variant of von Willebrand disease (vWD). An affected individual's bleeding tendency and thrombocytopenia are exacerbated with pregnancy, and the proper management of these patients during delivery has not been well established. Since it is important to distinguish this disease from the platelet type vWD in order to administer the appropriate therapy, gene analysis is necessary to make the precise diagnosis. We now report the successful management of delivery in three patients, who were diagnosed as having type IIB vWD by the detection of missense mutations in the von Willebrand factor (vWF) gene (C3916-->T and C3922-->T). These changes cause Arg543-->Trp and Arg545-->Cys substitutions in the A1 domain of vWF. We were able to manage the bleeding tendency of these patients at delivery mainly with vWF concentrates to supply normal vWF.

[Photodensitometry: microdensitometry (MD): digital image processing method (DIP)].

The principles of microdensitometry (MD) and digital image processing method (DIP), as well as the application of these methods to measure bone mineral density in clinical practice, were mentioned in the report. MD and DIP assess bone mineral content and bone mineral density by analyzing relative contrast of the metacarpus II on X-ray image. However, the parameters obtained by these methods have been reported to be closely related to lumber vertebral bone mineral density and whole-body bone mineral content as measured by dual energy X-ray absorptiometry (DXA). Being easy to use, MD and DIP are adequate for the screening of osteoporosis. Once any reduction in bone mineral content or bone mineral density is shown by MD or DIP, it is recommendable to measure bone mineral density of vertebrae and femoral neck by DXA.

[Distribution of platinum in the female genital tract after intraarterial carboplatin infusion during the operation of uterine cervical cancer].

We investigated the platinum concentration in the female genital tract, the regional lymph nodes and serum after intraarterial carboplatin 150mg infusion during the operation of uterine cervical cancer. The following results were obtained. 1. The mean platinum concentration in the region of cervical cancer was highest (1.77 micrograms/g wet tissue of the female genital tract). However the other genital tract including the endometrium, ovary and fallopian tube showed the same platinum concentration. 2. The level of the serum total platinum concentration became maximum at 30 min and gradually decreased thereafter. From these results, intraarterial CBDCA infusion seemed to show the therapeutic effect against the primary region of uterine cancer and the other genital tract.

[Study of proliferating cell nuclear antigen (PCNA), nucleolar organizer regions (NORs) and mitotic activity in epithelial ovarian tumors].

We investigated the proliferating cell activity of 121 epithelial ovarian tumors with formalin-fixed, paraffin-embedded tissue sections by means of the proliferating cell nuclear antigen (PCNA) labeling index, the mean number of nucleolar organizer regions (NORs) and mitotic activity. In the order of the malignant group (82 cases), the borderline-malignant group (19 cases) and the benign group (20 cases), the mean PCNA labeling index was 32.9, 12.8, 3.4(%), the mean number of NORs was 11.1, 5.8, 2.9 (/nucleus) and the mitotic index was 30.8, 8.0, 0.5 (/10HPF) and there was a significant difference (p < 0.001) between groups. There was a positive correlation (p < 0.001) among the PCNA labeling index, the mean number of NORs and the mitotic index. In 11 cases in the malignant group, the PCNA labeling index, the mean number of NORs and mitotic index in postchemotherapy were significantly lower than those in the prechemotherapy. It was suggested that the evaluation of proliferating cell activity in malignant cases was useful in investigating the malignant potential as well as the effect of chemotherapy.

[Comparative study of HPV infection and proliferating cell nuclear antigen of uterine cervix].

We investigated the proliferating activity of human papillomavirus (HPV) infection in cervical neoplasia obtained from 37 mild dysplasia, 26 moderate dysplasia, 34 severe dysplasia and 26 carcinoma in situ (CIS) and 10 normal cervical epithelium by in situ hybridization (ISH) with biotinylated HPV 6, 11, 16 and 18 DNA probes and by immunohistochemical staining with monoclonal antibody (PC10) to proliferating cell nuclear antigen (PCNA). The results were as follows: 1) The immunohistochemical study of PCNA was useful as a marker to detect proliferating activity in formalin-fixed, paraffin-embedded tissue sections. The PCNA labelling index was significantly increased as the grade of cervical neoplasia become higher (normal cervical epithelium: 3.5%, mild dysplasia: 10.3%, moderate dysplasia: 16.9%, severe dysplasia: 23.7%, CIS: 36.3%) (p < 0.01). 2) The PCNA labelling index of the HPV type 6, 11, 16, 18 positive group was not significantly different from that of the no HPV detected group in each grade of cervical lesions. 3) The PCNA labelling index group positive for HPV 16, 18 DNA was significantly higher than the HPV 6, 11 positive group and the no HPV detected group at every grade of dysplasia (p < 0.05). The precancerous lesions positive for HPV type 16 or 18 were thought to be a high risk group judging from the proliferating activity. These results suggest that PCNA can be used as one of the growth factors in cervical neoplasia and that HPV type 16 and 18 infection had a significant proliferating cell activity in every grade of cervical dysplasia.

[Molecular biological study on multifocal infection of HPV in uterine cervix, vagina and vulva].

Vaginal and vulvar scrapes from 30 patients which have HPV types 16 or 18 positive cervical neoplasia (13 of mild dysplasia, 5 of moderate dysplasia, 4 of severe dysplasia, 3 of carcinoma in situ, and 5 of invasive carcinoma) were examined by the polymerase chain reaction (PCR), to detect amplified E7 gene of HPV types 16 and 18 DNA sequences. In 24 vaginal and 15 vulvar scrapes, the same type of HPV as in cervical scrapes was detected. The frequencies of HPV DNA in vaginal and vulvar scrapes for histological diagnosis of cervical neoplasia were 84.6% (11/13) and 53.8% (7/13) in mild dysplasias, both were 100% (5/5) in moderate dysplasias, 75% (3/4) and 25% (1/4) in severe dysplasias, 66.7% (2/3) and 33.3% (1/3) in CISs, and 60% (3/5) and 20% (1/5) in invasive carcinomas, respectively. The results of vulvar scrapes showed that the frequency of HPV DNA in mild and moderate dysplasias was significantly higher than that in severe dysplasias, CISs and invasive carcinomas (p < 0.05). We suggest that, in addition to the investigation for the natural history of HPV infection, cell dynamics, local immunological status and any other factors in the HPV-infected lesion must be investigated to clarify the carcinogenesis of HPV in the uterine cervix. Furthermore, HPVs detected in the vagina and vulva were thought to have disappeared without reaching an integrated form during the follow-up of cervical neoplasia with HPV infection over a long period.

[Molecular biological study on the overexpression of c-myc gene in uterine cervical carcinomas].

The expression of mRNA for c-myc gene was investigated in 30 cases of human uterine cervical carcinoma by Northern blot hybridization, and c-myc gene amplification was also examined by Slot blot hybridization. Overexpression of c-myc gene was detected in one (7.7%) of 13 cases of carcinoma in situ (stage 0), one (10.0%) of 10 stage I and 4 (57.1%) of 7 stage II uterine carcinoma, and c-myc gene amplification was detected in 2 cases (6.7%) in which c-myc gene overexpression was also found. The patient whose c-myc mRNA level was more than 10 times as high as in normal uterine cervical tissue relapsed and died within 2 years after the first treatment. Our results suggest that the overexpression of c-myc gene occurred more frequently in advanced than in early uterine cervical carcinoma and that it might be useful in prognosing cervical cancer.

Expression of basic fibroblast growth factor in human gastric carcinomas.

The expression of mRNA for the basic fibroblast growth factor (FGF) gene was examined in seven human gastric carcinoma cell lines and in tissue from 29 gastric carcinomas together with the adjacent normal mucosa. Among the seven gastric carcinoma cell lines, the MKN45 cell line expressed mRNA for the basic FGF gene. Basic FGF protein production was confirmed by flow cytometric analysis and immunohistochemistry. Among the surgical specimens, 16 (55%) of 29 gastric carcinomas showed higher levels of basic FGF mRNA than the normal mucosa. Interestingly, in scirrhous gastric carcinomas characterized by their fibrous stroma and rapid growth, 9 (69%) of 13, samples examined revealed higher levels of basic FGF mRNA than normal mucosa, whereas only 3 (33%) of the 9 well differentiated adenocarcinomas studied produced similar results. Immunohistochemically, basic FGF protein was localized in tumor cells. These results suggest that basic FGF produced by tumor cells may play an important role in producing fibrosis and angiogenesis in gastric carcinomas.
