Morphological characteristics of incrustates of intrauterine contraceptive devices.

The morphology of incrustates formed on intrauterine contraceptive devices (IUD) is described. It is demonstrated that due to manufacturing technology some IUDs have long, parallel excavations serving as sites for incrustate formation. A regional difference of incrustation on the different parts of IUD has also been observed. Electron microscopy was carried out in tissue fragments attached to removed IUDs. Alterations correspond to acute or chronic inflammation. Incrustate on the surface of IUD appears to have a primary role in induction of the process.

[Incrustation of marker threads of intrauterine devices in the uterine cavity and cervix uteri].

If the indicating thread of intrauterine contraceptive devices is situated inside the uterine cavity, it is incrustated in the same way with calcium carbonate like the device itself. In 20 per cent the intracervical part of the thread is incrustated, too. The danger of an ascending inflammatory disease is increasing with the precipitation of substances, probably, denaturated fibrine or mucine according to the infrared spectral analysis.

Correlation between the incrustation of intrauterine devices and the duration of their use.

The usefulness and side-effects of intrauterine contraceptive devices (IUDs) are evaluated on the basis of various aspects (e. g. effect of postabortal and post-partal insertion, expulsion of device, frequency of pregnancies, bleeding and inflammation, etc.) [1, 2, 3, 4]. However, the incrustation of the device has been ignored although the coating on the major part of the surface is easy to study. The rough surface coating is very likely to be associated with some side-effects [5]. In our report we have tried to find some links between the degree of incrustation and the duration of using the device.

[Calcium-containing incrustations on IUD's--detection using electron scanning microscopy].

On the surfaces of intrauterine contraceptive devices the occurrence of incrustations has been noticed, which presented blade or needle crystal structure on electron microscopic scanning exposures. In conformity with earlier findings, these products showed the character of calcium compounds. Attention is called to the significant part that such an incrustation is likely to play in the rise of complications and harmful side effects.

[Electron microscopic changes in the endometrium caused by encrusted intrauterine contraceptives].

Alterations of the endometrium caused by incrustated intrauterine contraceptive devices were subjected to electron microscopic examination. The changes were of non-specific character and appeared in the epithelial cells. Dilatation of the endoplasmatic reticula, degranulation and augmentation of microfilaments were conspicuous. The lesions are likely to have played some part in the later developments.

[Incrustation of intrauterine devices].

Incrustation was found to have occurred to intra-uterine devices (Szontágh, Lippes, Cooper T 200), between one and three years from the dates of insertion. Calcium carbonate accounted for most of that incrustation and increased quantitatively, in the course of time. To avoid deterioration of complications, the recommendation is made to respond even to the slightest complaint by removal or exchange of an IUD when more than one year has passed from insertion.

Early detection of endometrial cancer by combined use of vaginal ultrasound and endometrial vacuum sampling.

With increasing lifespan and decreased incidence of uterine cervical cancer, the importance of the proper and early diagnosis of endometrial cancer has become a demand to gynecology. The objective of the present study is to evaluate the effectiveness of gynecologic diagnostic tools in detection of endometrial cancer in early stage. The patients (72) involved in the study, after giving their informed consent, were investigated by transvaginal ultrasound and subsequent vacuum endometrial sampling in office settings. Whenever the histology examination of endometrial vacuum sampling showed hyperplasia or carcinoma, it was reassured by sampling with dilatation and curettage. The analysis of sonography and histology results showed that in 4 cases (5.6%) endometrial hyperplasia and in two cases (2.8%) endometrial adenocarcinoma were present. Also analyzed were sensitivity, specificity, positive and negative predictive values for histology results and for sonographic findings. These results show that transvaginal ultrasound is a reliable method for screening for endometrial carcinoma. Moreover, in the case of pathologic endometrial change suspected by ultrasound, the combination of vacuum endometrial sampling with ultrasound examination can yield firm diagnosis in office settings, saving cost and time in early diagnosis of endometrial cancer.

Serum levels of trace elements (selenium, copper, zinc) in gynaecological diseases.

The serum levels of selenium, copper and zinc were examined in benign and malignant gynaecological tumours and in metropathia haemorrhagica. According to the authors, the examination of several trace elements is necessary for the determination of the nature of the process. The levels of significance of the changes are dealt with.

Pregnancy and delivery after operation for a pituitary microadenoma.

The authors report on the successful pregnancy and permanent symptom-free condition of a young woman who underwent operation for pituitary microadenoma. At the time of submitting the publication mother and child are healthy. According to the available literature this is the first such case reported in Hungary.
