[Percutaneous drainage of the biliary tract by puncture with a fine needle].

Temporary pre-operative and post-operative as well as permanent palliative percutaneous biliary-tract drainage was performed in 12 patients with biliary tract obstruction; in three the drainage was internal and in nine external. Main indication of the method is permanent palliative decompression in cases of inoperable malignant tumour with obstructive jaundice. Pre-operatively, percutaneous biliary tract drainage serves as a temporary measure in order to perform an operation after decompression of the biliary tract and improving the patient's general condition. The only serious complication was biliary peritonitis after a drainage catheter had slipped out.

Biliary Tract Neoplasms↗

Intraductal proliferation in the pancreas and its relationship to human and experimental carcinogenesis.

In 21 patients who had undergone total pancreatectomy for pancreatic head carcinoma, the uninvolved pancreas was examined with regard to the type, incidence and regional distribution of duct epithelial proliferation. The results were compared with those in 37 operative specimens from patients with chronic pancreatitis, in 46 normal pancreases from autopsies and with findings in experimental pancreatic carcinogenesis. While the incidence of squamous metaplasia and non-papillary epithelial hypertrophy varied little in the different groups, papillary epithelial hyperplasia was found three times more often in cases of carcinoma, with associated mild duct obstruction. Atypical epithelial proliferation was only detected in the vicinity of carcinomas. Unequivocal transition from papillary hyperplasia to atypical proliferation was not observed. In hamsters treated with dihydroxy-di-n-propylnitrosamine (DHPN) for induction of pancreatic duct carcinomas, the early duct lesions closely resembled atypical epithelial proliferation of human pancreas. It is concluded that (1) papillary epithelial hyperplasia is probably only indicative of early duct obstruction and/or a general neoplastic stimulus, (2) intraductal epithelial proliferation with atypia is a true precursor of duct carcinoma, and (3) chronic pancreatitis lacks atypical duct lesions.

[Preoperative percutaneous transhepatic drainage of the biliary system (PTCD) in malignant obstructive jaundice (author's transl)].

Percutaneous transhepatic intubation and drainage of the biliary system were performed in 23 patients, 19 cases preoperatively because of malignant obstructive jaundice. With 12 days the bilirubin serum level went to normal values, and the patients general condition and liver function showed marked improvement. By preoperative PTCD the mortality of palliative procedures in pancreatic carcinoma decreased. Complications occurred in five cases and were easy to manage. Clinical aspects such as indication, optimal period of drainage, and value of other techniques in bile drainage are discussed.

[Progress in the localization of insulinomas].

Preoperative percutaneous transhepatic portal vein catheterization with selective blood sampling from the pancreatic veins and insulin radioimmunoassay is a very successful new procedure for localization of insulinomas. A careful peroperative inspection and bidigital palpation of the whole pancreas helps to avoid the problems of undetected insulinoma.

Adenoma, Islet Cell↗

[Surgery of tumors of the endocrine pancreas. Result of a survey made in the Federal Republic of Germany].

Within a ten-year scan (1967-1976) 207 insulinomas, 50 gastrinomas, 8 Verner-Morrison tumours, 5 glucagonomas and 12 endocrine pancreatic tumours with associated MEA syndrome (multiple endocrine adenomatosis) were treated surgically at various university hospitals (information obtained by questionnaire). Half of the insulinomas were treated by enucleation, one third by resection of the tail of the pancreas. Total gastrectomy was the procedure of choice in 80% of patients with gastrinoma, but sometimes pancreatic resection to remove the tumour was added. An new therapeutic concept of using histamine-H2 receptor antagonists for treating patients with Zollinger-Ellison syndrome is discussed. In the eight patients with a Verner-Morrison syndrome removal of the tumour or distal pancreatic resection was the procedure of choice.

Biliary Tract Diseases↗

[Experimental induced pancreatic carcinoma after choledochojejunostomy (author's transl)].

The pancreatic carcinogen DHPN (dihydroxypropylnitrosamin) was administered to Syrian golden hamsters. By intragastric administration more pancreatic carcinomas in the head of the gland were induced than by subcutaneous injections. After choledochojejunostomy the incidence of carcinomas was only 5--7% and the lesions were similar in multiplicity and distribution without relationship to the way of administration. The importance of biliary reflux in pancreatic tumor induction is discussed and the role of bile concerning the preferred localisation of the tumors in the head of the pancreas.

[Cancer of Vater's ampulla].

From 1964 to 1977 a total of 50 patients were operated on for carcinoma of the ampulla of Vater, the second most frequent tumor of the periampullar region. The cardinal symptom was jaundice. The diagnostic procedures of high value were roentgenographic examination of the upper gastrointestinal tract with the use of barium, also hypotonic duodenography, sonography, and percutaneous transhepatic cholangiography (ERCP, angiography). Curative treatment was possible by duodenopancreatectomy, with a resection rate of 74%. The 5-year survival rate was 27% with a postoperative death rate of 13.5%.

Acute Kidney Injury↗

[Retroperitoneal fibrosis and pancreatitis (author's transl)].

A case of retroperitoneal fibrosis causing pancreatitis is reported; another case is reported as well, where chronic pancreatitis caused secondary symptomatic retroperitoneal fibrosis. Differential diagnosis of the 2 diseases is discussed. Surgical therapy has to eliminate complications and reduce retroperitoneal fibrous tissue structures; this has to be followed by continuous drug therapy aimed at slowing proliferation of fibrous tissue.

[Total duodenopancreatectomy as a standard operation in pancreatic cancer].

Total pancreatectomy is recommended to improve the poor prognosis for patients with pancreatic cancer. Several factors support the use of this method as the best chance of cure: no cancer is left behind at the line of pancreatic resection, radical operation in case of multicentric or diffuse pancreatic carcinoma, more radical lymphadenectomy, no complications relative to the pancreaticojejunal anastomosis, and, primarily, longer survival rates. Operative treatment must be individual and depend on tumor stage. In case of localized operable pancreatic carcinoma, total pancreatectomy seems the best way of achieving more cures.

Diabetes Mellitus↗

[Chronic tendovaginitis in parasitic diseases].

A case of chronic tendovaginitis was found to be caused by parasites (filariae). The parasites had apparently been killed by the treatment of the disease. Histologic investigations showed that this pathologic process was a case of the granulomatous form of tendovaginitis. Differentiation from the other forms of tendovaginitis was clearly established by the morphologic picture.
