Enzyme immunoassay for detection of total and IgM-specific antibodies to hepatitis A virus and its clinical application.

Antibody to hepatitis A virus (anti-HAV) and IgM class antibody to HAV (IgM anti-HAV) in sera from 73 patients with hepatitis A and from 550 normal subjects were measured by enzyme immunoassay (EIA) and the results were compared with those of radioimmunoassay (RIA). Since RIA has the disadvantage of requiring radioisotopes and special equipment, the clinical applicability of EIA and possible methodological problems were evaluated. The EIA for anti-HAV showed an excellent correlation with RIA, indicating its usefulness for the demonstration of the immune status in these subjects. Positive results of anti-HAV were obtained in the early stage after the onset of hepatitis A. However, pretreatment for inactivation of samples was required. False-positive reactions were found in sera to which sodium azide was added as preservative. In the measurement of IgM anti-HAV, a fundamental study revealed quite satisfactory results, correlation with the results of RIA was excellent. In patients with hepatitis A, the titers reached a peak in the second to third week, followed by a gradual decline. Changes to a negative reaction were never encountered within three months. We concluded that the EIA is an useful tool in the diagnosis of hepatitis A and can replace RIA.

An unusual case of gastric carcinoma initially presenting bone metastasis and later transverse myelopathy.

A 44-year-old man with Borrmann type III gastric carcinoma initially presenting bone metastasis and later transverse myelopathy is reported. Chest and skull roentgenograms showed multiple punched-out lesions. A bone scintigram showed multiple abnormal uptake. Histological examinations of biopsy and autopsy materials revealed signet-ring cell carcinoma of the stomach. Disseminated carcinomatosis of the bone marrow was ruled out histologically and neither microangiopathic hemolytic anemia nor disseminated intravascular coagulopathy was observed. Because this patient with bone metastasis did not have metastasis to the liver and lungs we propose that a nonportal route through the vertebral venous plexus might be an alternative to the portal route of bone metastasis from gastric carcinoma. During the last 20 years, only 10 such cases including this one have been reported in Japan. Their prognosis was quite poor and in most of them the malignancy was either undifferentiated adenocarcinoma or signet-ring cell carcinoma. If a bone metastasis is found as the initial sign, physicians should keep in mind that the primary lesion might be in the stomach, or in the thyroid, kidney, lung or prostate as another point of search.

Comparison of therapeutic usefulness of vitamin D3 analogs on osteoporosis caused by ovariectomy in rats.

Ovariectomy induced a remarkable reduction in the ash content of the femur and histologically a disappearance of the trabecular bone in tibial metaphysis accompanying the decrease of serum calcium level and urinary calcium excretion in young immature rats. Treatment with 1 alpha-hydroxyvitamin D3, 1,25-dihydroxyvitamin D3 and 1,24R-dihydroxyvitamin D3 for 3 months from 7 to 9 months after the ovariectomy dose-dependently increased serum and urinary calcium, bone ash, and histologically partly improved a loss of trabeculae. The 24R,25-dihydroxyvitamin D3 doses, on the other hand, produced no therapeutic effect on the bone disorders in ovariectomized rats, except for the slight effect on the increasing activity of serum calcium.

Hepatic granuloma. Report of a case successfully treated by hepatectomy and antituberculous therapy.

A 62 year-old male presented with high fever, weight loss and painful hepatomegaly. The clinical presentation, results of laboratory studies, angiography and laparotomy suggested hepatic abscess or cholangiocarcinoma. He underwent left lobe hepatectomy, and pathological examinations revealed granuloma. Acid-fast bacilli, fungi and parasites were not identified by special stainings. Hepatic tuberculosis was considered most likely because, after operation, an intermediate-strength purified protein derivative (PPD) skin test gave strongly positive results and his subsequent response to antituberculous treatment was excellent. His complex clinical features and the diagnostic difficulties were outlined.

Effect of TEI-4120 (3-benzoyl-N-beta-ethoxyisopropyl-2-methlindole) on carrageenan induced disseminated intravascular coagulation (DIC) in rats.

In order to evaluate TEI-4120 (3-benzoyl-N-beta-ethoxyisopropyl-2-methylindole), as a possible therapheutic compound to disseminated intravascular coagulation (DIC), DIC model in rats was induced by the treatment of lambda-carrageenan according to the method of Sugiyama et al. with minor modification. Two hours after intraperitoneal injection of lambda-carrageenan, plasma fibrinogen decreased and platelet count, prothrombin time, serial thrombin time and plasma TXB2 prolonged or increased. By use of this model, the activities of TEI-4120, new antithrombotic compound, was evaluated compared with three other compounds, aspirin, warfarin and heparin. As a result, TEI-4120 is effective in this model. Combination therapy of TEI-4120 and warfarin is more effective than single administration of TEI-4120. The effect of combination therapy of TEI-4120 and warfarin is consistent with that of heparin. The above result suggests that TEI-4120 seems to be a promising compound to the therapy of DIC.

[A case of advanced gastric cancer associated with smouldering adult T-cell leukemia (ATL)].

A 43-year-old man was admitted to our clinic because of a 2-month history of anorexia, general fatigue and an upper abdominal mass. Upper GI series and endoscopic examination revealed Borrmann IV-type gastric cancer. The histologic diagnosis was undifferentiated adenocarcinoma. His peripheral blood contained approximately 3% atypical lymphocytes. These lymphocytes were identified to be ATL-cell based on E-rosette formation assays and anti-ATL associated antigen-antibody tests. Although the patient had no typical ATL-symptoms, we made a diagnosis of double cancer, i.e. gastric cancer and smouldering ATL. This is the first reported Japanese case of gastric cancer associated with smouldering ATL relationship between gastric carcinoma and other malignant diseases, especially ATL, is discussed.

Serum gastrin levels in patients with thyroid dysfunction.

Fasting serum gastrin levels measured by radioimmunoassay were found to be elevated in patients with hyperthyroidism and low in patients with hypothyroidism. The oral administration of beef extracts resulted in more increase of serum gastrin in hyperthyroid patients than in normal subjects. After restoration of the euthyroid state by treatment, no more increase in serum gastrin levels was observed. Slight correlation between gastrin levels and serum T3 levels was observed in pretreated hyperthyroid patients (r = 0.40), but significant correlation between them was found after restoration of the euthyroid state by treatment (r = 0.50). However, it seemed to be able to divide into two groups in the pretreated patients. One was a patient group whose gastrin levels correlated closely to serum T3 levels (r = 0.83, p less than 0.01). The other was a group whose serum gastrin levels remained in low even in high T3 levels (r = 0.81, p less than 0.01). Different sensitivity to thyroid hormone in the G-cells of gastrointestinal tract may exist in these two groups, because patients age and duration of their illness were not different between them.

Hypercholinesterasemia in patients with hepatocellular carcinoma: a new paraneoplastic syndrome.

Two cases of hepatocellular carcinoma (HCC) were described, in whom hypercholinesterasemia was found. Histochemical examinations revealed that there was a significantly increase in enzyme activity of cholinesterase in liver tissue slice obtained from the part of carcinoma in case 1. It was found that cholinesterase activity in homogenized liver tissue in part of carcinoma was much higher than that of non-carcinoma, and even in other HCC cases, hepatic cirrhosis and control liver tissues. These results suggested that HCC cells were capable of producing cholinesterase and, therefore, that hypercholinesterasemia was an additional paraneoplastic syndrome in HCC.

A case of bleeding duodenal varices located in the third portion.

We described a patient with isolated duodenal varices of the third portion, preoperatively diagnosed by hypotonic duodenography and endoscopy. These varices seemed to be caused by portal hypertension due to liver cirrhosis. The third to the fourth portion of duodenum should be examined carefully in patients with liver cirrhosis, especially with gastrointestinal bleedings of unknown origin.

Cronkhite-canada syndrome associated with gastric cancer: report of a case.

A 71-year-old man with characteristic clinical and pathological pictures of Cronkhite-Canada syndrome associated with gastric cancer is presented. Histological examinations of gastric and colonic polyps which resembled juvenile type revealed no malignant cells. The patient slowly recovered after subtotal gastrectomy and elemental diet supplement with antiplasmin agents. As for as is known to the authors, a total of 69 cases with this syndrome were reported in Japan including our patient. Of these cases 8 had cancer of the gastrointestinal tract. Two of them, including our case, had gastric cancer. Another one was associated with gastric cancer but not intestinal polyposis. The remaining 5 cases were associated with colonic cancer. Caution must be paid to the fact that cachexia or malabsorption in some patients with this syndrome results from gastrointestinal malignancy. The prognosis of this syndrome consisted of 20 deaths including 2 postoperative deaths, 33 of improvement including 4 of natural remission and 4 of remission after gastrectomy or colectomy, 7 unchanged and the remainder not described.

Clinical usefulness of serum cholylglycine determination in various liver diseases.

In order to study the clinical significance of cholylglycine (CG), CG in both fasting levels and after oral loading of 300 mg ursodeoxycholic acid (UDCA) were determined by radioimmunoassay in 313 patients with various liver diseases. Elevated fasting CG levels were found in 66.6% of patients with acute hepatitis (AH), 34.8% chronic inactive hepatitis (CIH), 79.3% chronic active hepatitis (CAH), 97.1% liver cirrhosis (LC), 94.7% hepatocellular carcinoma (HCC), and 80% of gall stones (GS). Changes of fasting CG during the course of AH were found to be parallel with values of transaminases. On oral loading of UDCA, different CG responses were found in CIH and CAH, and in compensated and decompensated LC. From these results we concluded that the determinations of serum CG gave additional informations of various liver diseases.
