[Neurinoma arising from the first branch of the trigeminal nerve: a case report].

A case of a neurinoma arising from the first branch of the trigeminal nerve in a 40-year-old female is reported. The patient was admitted with the chief complaint of loss of Lt. visual acuity and mild hypoesthesia in the area of the first branch of the trigeminal nerve. A CT scan and MRI revealed a tumor extending through the superior orbital fissure into the orbita. Subtotal resection of the tumor was performed by a fronto-orbito-zigomatic approach and a histological diagnosis of neurinoma was made. Although the hypoesthesia remained, the visual acuity was markedly improved postoperatively. A neurinoma arising from the first branch of the trigeminal nerve is very rare. To our knowledge, including our case, only five cases which were described for clinical and diagnostic features and surgical management have been reported. There were three males and two females, and the age ranged from 1 to 57 years. Neurologically, all cases presented hypoesthesia in the area of the first branch of the trigeminal nerve and exophthalmus on admission. Visual disturbance was found in three cases. Radiologically, the enlargement of the superior orbital fissure was revealed in two cases. Angiography performed in three cases demonstrated the avascular mass. Three patients received CT scan and only the present case used MR imaging. Surgical resection was performed in all cases through various approaches. The fronto-orbito-zygomatic approach which was chosen in our case was useful for obtaining a sufficient operative view. As in our case, excellent outcome was achieved in three other cases due to successful tumor resection.

Self-assembling automata: a model of conformational self-assembly.

An abstract model of self-assembling systems is presented where assembly instructions are written as conformational switches-local rules that specify conformational changes of a component. The model, the self-assembling automaton, is defined as a sequential rule-based machine that operates on one-dimensional strings of symbols. Classes of self-assembling automata are defined based on classes of subassembly sequences in which the components self-assemble. The minimum number of conformations is provided which are necessary to encode subassembly sequences in the each class. It is shown that three conformations for each component are enough to encode any subassembly sequences of a string with arbitrary length.

[Pulmonary veno-occlusive disease developed in a man given long-term treatment with anticancerous agents].

A 67-old man was referred to our hospital because of dyspnea on exertion and severe hypoxia. He had been given, tegafur and OK 432 for seven years following an operation for gastric cancer. Pulmonary hypertension was noted by right heart catheterization. The findings of a transbronchial lung biopsy resulted in a diagnosis of pulmonary veno-occlusive disease. Pulmonary hemodynamic studies were performed for five different agents: nifedipine, beroprast sodium (PGI2), nitroglycerin, theophylline, and isosorbide dinitrate. However, none of these agents showed significant effects on pulmonary arterial pressure or pulmonary vascular resistance. Treatment with glucocorticoid relieved the patient's symptoms without any apparent effect on pulmonary hemodynamics. The long-term administration of anticancerous agents (tegafur) were thought to have caused pulmonary veno-occlusive disease to develop in this patient.

[Movement-related cortical potentials upon forward-stepping in patients with Parkinson disease].

We investigated movement-related cortical potentials(MRCPs) in 20 patients with Parkinson disease(PD group) and 6 normal age-matched controls when they were stepping forward volitionally from standing position. PD group was divided in three groups according to the clinical courses and motor function scores in Langston's criteria: Ia; within 6 years and higher than 20 points, Ib; within 6 years and within 20 points, II; over 6 years. In Ia group their negative slope(NS') was larger than those in the other three groups, while their Bereitschaftspotential showed no statistical difference. The results of our investigation shows that, in these cases, the function of cerebral cortex and/or cerebellum can be enhanced compensating the dysfunction of basal ganglia.

[Psychological investigation on spa therapy in patients with bronchial asthma].

To evaluate the effects of complex spa therapy (swimming training in a hot spring pool + fango therapy + inhalation of iodine salt solution) on psychological factors, three psychological examinations; CMI (Cornell Medical Index), SDS (Self-Rating Depression Scale) and CAI (Comprehensive Asthma Inventory); were given 25 patients with bronchial asthma (10 male, 15 female, mean age 60 years), and the results of these examinations were compared before and after spa therapy. 1. Physical symptoms, respiratory system and CIJ symptoms by CMI were significantly improved after spa therapy, however the improvement of psychical symptoms was not significant. 2. In SDS, 11 of 25 patients showed more than 40 points, indicating depressive state. The number of such patients decreased from 11 to 2 and the mean SDS score significantly improved from 38.7 to 34.2 after spa therapy. 3. Conditioning, suggestion, fear of expectation, frustration, flight into illness, negative attitudes towards prognosis, and decreased motivation towards therapy by CAI were significantly reduced after spa therapy, with the reduction in negative attitudes towards prognosis and decreased motivation towards therapy being relatively large. Furthermore, CAI score, which is the average of the categories in CAI, was also significantly decreased from 37.9 to 28.4 after spa therapy. These results suggested that complex spa therapy improves psychological factors in patients with bronchial asthma.

Slow negative cortical potential preceding the onset of postural adjustment.

We investigated whether a cortical potential exists, that is similar to the Bereitschaftspotential, preceding postural adjustment followed by voluntary ballistic rising on tiptoe in 10 healthy subjects. On the basis of the electromyogram (EMG) activities of the soleus muscle, the onsets of the premotion silent period (PMSP) and EMG discharge were determined. The negative potentials associated with a voluntary rise-on-tiptoe movement with respect to EMG onset were similar to the readiness potential associated with voluntary foot movement. The slopes of the slow negative potential associated with the PMSP onset were significantly more negative than those of the potential associated with rise-on-tiptoe movement, particularly over the frontal electrode positions. The results suggest that a cortical potential precedes postural adjustment that is followed by voluntary rising on tiptoe.

[Long-term maintenance therapy of multifocal motor neuropathy by weekly administration of human immunoglobulin].

High-dose intravenous immunoglobulin therapy for multifocal motor neuropathy (MMN) exhibits a beneficial, but temporary effect in most cases. We experienced a patient with MMN having a high titer of anti-GM1 ganglioside antibody. He was weekly administered human immunoglobulin, and his muscle strength was successfully improved and maintained at the improved level after high-dose human immunoglobulin infusion. The pattern of improvement and maintenance by weekly administration of human immunoglobulin was variable among the muscles; e.g. only a minimal improvement was observed in the most severely involved brachioradial muscles. Successive measurements with a hand-held dynamometer revealed in detail the changes in the strength of each muscle during treatment. Anti-GM1 ganglioside antibody titers did not significantly change during the treatment, but a slight improvement in motor conduction amplitudes and velocities was observed in mildly affected nerves. Our findings suggest that weekly administration of human immunoglobulin is a safe and effective therapy for MMN and that the anti-GM1 ganglioside antibody titer may not be a good indicator for this therapy.

Improvement of lesions shown on MRI and CT scan by administration of dichloroacetate in patients with Leigh syndrome.

Brain lesions exhibited on MRI and CT scan in 2 patients with mitochondrial encephalomyelopathy representing Leigh syndrome were improved by administration of dichloroacetate (DCA). One patient had pyruvic acid dehydrogenase complex (PDHC) deficiency, the other had complex I deficiency. The efficacy of DCA was transient in the patient with the PDHC deficiency, lasting for about 2.5 months. The patient died at the age of 6, about 2 years after the initiation of DCA treatment. DCA administration was started in the patient with complex I deficiency when he was 15 months old and it is still effective at his present age of 24 months. His motor ability is developing, and he could walk without support at the age of 19 months. DCA administration should be tried in patients with mitochondrial diseases.

Subassembly generation via mechanical conformational switches.

A question is posed on how a particular subassembly sequence is generated in randomized assembly. An extended design of mechanical conformational switches [16] is proposed that can encode several subassembly sequences. A particular subassembly sequence is generated due to conformational changes of parts during one-dimensional randomized assembly. The optimal subassembly sequence that maximizes the yield of a desired assembly can be found via genetic search over a space of parameterized conformational switch designs, rather than a space of subassembly sequences. The resulting switch design encodes the optimal subassembly sequence so that the desired assemblies are put together only in the optimal sequence. The results of genetic search and rate equation analyses reveal that the optimal subassembly sequence depends on the initial concentration of parts and the defect probabilities during randomized assembly. The results indicate that abundant parts and parts with high defect probabilities should be assembled earlier rather than later.

[Severe bradycardia and hypotension during epidural anesthesia in a patient undergoing hemorrhoidectomy].

Severe bradycardia and hypotension developed suddenly in two patients undergoing hemorrhoidectomy under lumbar epidural anesthesia in the jackknife position, about 5 to 10 min after epidural administration of buprenorphine. Severe vago-vagal reflex was supposed to have been induced with buprenorphine in a situation where venous return was decreased by epidural anesthesia and the jackknife position.

[Movement-related cortical potentials (MRCPs) and voluntary movement--clinical usefulness of MRCPs].

The gradient of negative slope (Ns') changed in parallel with the velocity of step movement in normal individuals. We can not evaluate the MRCPs without considering this factor among patients and/or subjects. We recorded the MRCPs of thirty nine patients with cerebellar ataxias and sixteen patients with Parkinson disease (PD). Goniometer was attached on the patient's wrist to measure the velocity of the wrist movement. In the patients with spinocerebellar degeneration with denate nucleus lesion (Machado-Joseph disease, DRPLA, MERRF, dyssynergia cerebellaris myoclonica, galactosialidosis), the gradients of Ns' were reduced, although the movements themselves were as fast as normal control. In the patients with spinocerebellar degeneration without this lesion, the MRCPs were able to record as normal control, and their gradients of Ns' became steeper according to the increase of the movement angular velocity. On the contrary, in the patients with Parkinson disease, the gradients of Ns' were steeper in the patients with slow movement than in those with fast movement. The MRCP helps us to investigate the pathophysiology of the movement disorders. It is desirable that physiological data on the MRCP are extensively accumulated.

[Home chemotherapy for peritoneal carcinomatosis].

Nineteen patients diagnosed with peritoneal carcinomatosis were treated by in-home chemotherapy over a period of four years from August, 1990 to July, 1994. Primary diagnoses of the four male and 15 female patients included 12 cases gastric cancer (four males, eight females), five cases of ovarian cancer, one case (female) of appendicular cancer, and one case (female) of breast cancer. In addition to oral administration of UFT-E and 5'-DFUR, chemotherapy included weekly intravenous injection of a massive dose of 5-FU (1,000 mg/m2), subselective intraaortic infusion and intraperitoneal infusion using a reservoir. These methods were used individually and in combination. The drugs used included 5-FU, CBDCA, CPA, THP, and EPIR. Subselective intraaortic infusion was performed by low dose continuous infusion using the Baxter infusor multiday type. Six gastric cancer patients lived normally for over one year, while four died in less than a year. All ovarian and appendicular cancer patients were CR, the breast cancer patient was PR. Ten patients continued working at their jobs while receiving at home chemotherapy treatments. Diuretics were used to alleviats ascites. Although there were no side effects on digestive organs, 5-FU and CBDCA were mixed with 100 mg hydrocortisone in the infusor to improve cachexia, and promote appetite and activity. Bone marrow suppression was very slight at these dosages, and weekly checkups were adequate. The at-home rate (number of days at home/entire period since onset) of all patients was 78%.

[Initial myocardial uptake and myocardial clearance of 123I-metaiodobenzylguanidine in patients with ischemic heart disease of left ventricular dysfunction and dilated cardiomyopathy].

We studied initial myocardial uptake and myocardial clearance of 123I-metaiodobenzylguanidine (MIBG) in patients with ischemic heart disease of left ventricular dysfunction and dilated cardiomyopathy. Eleven patients with ischemic heart disease of left ventricular dysfunction (IHD group), 6 patients with dilated cardiomyopathy (DCM group) and 7 control cases (control group) were studied. Heart to mediastinum activity ratio (H/M) of early and delayed image was significantly lower in the IHD and DCM groups than in the control group. Although initial myocardial uptake of MIBG showed no significant difference among three groups, myocardial clearance of MIBG was significantly higher in the IHD (35 +/- 11%) and DCM (48 +/- 13%) groups than that in the control group (19 +/- 10%). H/M of delayed image was related to the left ventricular size, initial MIBG uptake and MIBG myocardial clearance. Negative correlation was observed between MIBG myocardial clearance and left ventricular ejection fraction in all cases. In conclusion, initial myocardial uptake of MIBG were not decreased in patients with IHD and DCM. Enhanced myocardial clearance of MIBG was observed not only in patients with DCM but also in patients with IHD.

[Movement-related cortical potential upon stepping forward].

We investigated movement-related cortical potentials (MRCPs) in 10 healthy subjects when they were stepping the right leg forward volitionally from the standing position, and compared with MRCPs associated with the foot dorsiflexion solely in sitting position. We recorded electroencephalogram (EEG), electrooculogram, surface electromyogram (EMG) on lower limb, and statokinesigram. We examined each EMG onset and several points on the statokinesigram as trigger points. MRCPs triggered by the right tibialis anterior, the left gastrocnemius, and the left soleus muscle had slow negative potentials containing slow phase and steeper phase. The potentials of both phases had the largest gradient at Cz. MRCPs triggered by the EMG onset of these three muscles resembled those associated with the foot dorsiflexion in their wave form, time course, and distribution pattern, but spread widely over the anterior recording sites. On the other hand, MRCPs triggered by the first deviation on the statokinesigram--by that first deviation assumed to take the first and foremost movement in this paradigm, were found to be delayed after the trigger point. The tibialis anterior muscle contributes to elevating a foot, and the contralateral gastrocnemius and soleus muscle to sustaining the weight. We were able to record apparent MRCPs when we adopted these EMG bursts, which play an important role in stepping forward, as trigger points. These records will be useful to analyze the mechanism of gait disturbance.

[Study on clinical effects of tosufloxacin (TFLX) and the long-term low dose therapy for the prophylaxis of recurrent urinary tract infection].

Forty five patients with complicated urinary tract infections were treated with tosufloxacin (TFLX) in the initial antibacterial treatment. Excellent and moderate responses were obtained in 39 patients (86.7%). No side effects were seen. Clinical efficacy and safety of long-term preventive administration of TFLX (150mg, a day) were also examined in 39 patients who showed an excellent or a moderate response in the initial treatment. The period of the administration ranged from 28 to 112 days (average; 55 days). The rate of preventing recurrence of urinary tract infection was 92.3% on the last day of treatment. These findings suggested that TFLX was useful not only in treating the patients with complicated urinary tract infections but also in preventing recurrence of urinary tract infection.

[A method of tracheal intubation tried in a patient with multicentric reticulohistiocytosis].

A 48-year-old woman had suffered from multiple destructive arthritis, skin nodules and skin ulcer of bilateral legs caused by multicentric reticulohistiocytosis for 16 years. Skin transplantation and total hip joint replacement were scheduled. Tracheal intubation was anticipated to be difficult because of destructive arthritis of the temporomandibular joint. We tried a new method to intubate the trachea through a laryngeal mask. After topical application of 8% lidocaine to the oropharynx, a size 4 laryngeal mask was inserted and its cuff was inflated by 20 ml of air. Anesthesia was induced with 4% sevoflurane in 50% nitrous oxide via laryngeal mask. A fiberscope through a tracheal tube was inserted through the laryngeal mask into the trachea. A tracheal tube with the internal diameter of 6.5 mm, was successfully intubated. This method gave little discomfort to the patient.

[Immunohistochemical detection of proliferating cell nuclear antigen (PCNA) in carcinoma at the confluence of the main hepatic ducts and its relationship to expression of CEA, CA19-9 and EGF receptor].

The Distribution pattern and proportion of the positive cells of proliferating cell nuclear antigen (PCNA) were examined immunohistochemically to study the proliferative activities of 33 cases of carcinoma found at the confluence of the main hepatic ducts. Expressions of CEA, CA19-9 and EGF receptor (R) were further examined with serial histological sections and comparatively analyzed with the result of PCNA staining and the clinico-pathological features of the carcinoma. PCNA positive cancer cells were observed in greatest abundance at the deeply infiltrated region of the carcinoma. CEA and CA19-9 were also expressed most strongly in this region and a stromal staining pattern was predominant. However, negative or apical staining patterns of CEA and CA19-9 were more frequent in the surface or the lateral spreading regions of the carcinoma, where the PCNA positive cancer cells were fewer in number. EGFR expression was rarely observed in the present study.
