Structures and conformations of metabolites of antitumor cyclic hexapeptides, RA-VII and RA-X.

Metabolites of antitumor cyclic hexapeptides, RA-VII and -X which were isolated from Rubia cordifolia were studied by hepatic microsomal biotransformation in rats and in bile juice of rabbits to which these drugs were administered intravascularly. Their structures and conformations were elucidated by two-dimensional nuclear magnetic resonance techniques, temperature effect on NH protons and nuclear Overhauser effect experiments. Specific N-demethylation of Tyr-3, O-demethylation and hydroxylation at aromatic rings of Tyr-3 and -5 were observed. Compared with metabolites of RA-VII, most of RA-X was excreted unchanged in the bile juice. Relationship among their structures, conformations and antitumor activities is also discussed.

[Postoperative pain relief by continuous epidural infusion of bupivacaine and buprenorphine].

Continuous postoperative pain relief produced by epidural block with bupivacaine and buprenorphine was evaluated in 12 patients after thoracotomy, 19 patients after upper abdominal surgery, and 14 patients after lower abdominal surgery. Patients initially received 8 ml of 0.25% bupivacaine and 0.1 mg of buprenorphine at recovery room in operating theater and continuously received the mixture of 0.25% bupivacaine and 5 micrograms.ml-1 buprenorphine at a rate of 1 ml.h-1 using a portable pump. About fifty percent of the patients did not need additional narcotics during 48 postoperative hours. About ninety percent of the patients needed one additional narcotics during 48 postoperative hours. The authors conclude that epidural analgesia with the mixture of bupivacaine and buprenorphine produces satisfactory postoperative pain relief.

[Group specific component protein (Gc) in patients with acute hepatic necrosis].

We studied the clinical significance of serum Gc levels in the sera of patients with acute hepatic necrosis. Gc concentration were measured using an EIA in double sandwich technique with purified Gc as standard. Mean levels of serum Gc were 6.0 +/- 7.4 mg/dl in fulminant hepatitis, 20.8 +/- 13.6 mg/dl in acute hepatitis and 39.4 +/- 13.8 mg/dl in normal subjects respectively. There was slight correlation between Gc and PTT (P less than 0.05). In the observation of sequential study in patients with fulminant hepatitis, serum Gc levels of live cases elevated but those of dead cases slightly elevated, thereafter decreased. These results suggested that serum Gc levels was the useful marker as a grade of liver cell damage and a parameter of regeneration in liver cells.

[A child case of Down's syndrome with intestinal tuberculosis and tuberculous pleuritis].

A 13 year-old girl with Down's syndrome was admitted to our hospital with a very positive reaction to PPD 5TU, abnormal shadow on the chest X-ray films and diarrhea. She suffered from acute enterocolitis one year ago, and then has been complaining of abdominal pain, appetite loss, and weight loss for a year. After admission, she was diagnosed as tuberculous pleuritis and suspected intestinal tuberculosis by laboratory examination. She recovered without sequelae by the combination therapy of SM, INH, and RFP, and was discharged after 5 months. The diagnosis of intestinal tuberculosis was confirmed by Colon Fiberscopy showing ulceration at the ileocecal region and simultaneous biopsy showing granuloma. Surgical treatment was not reserved, because she had no complications namely perforation and fistulization. We estimated that the onset of intestinal tuberculosis coincided with the acute enterocolitis which she had about one year ago. We realized the importance of paying attention to intestinal tuberculosis in the differential diagnosis of enterocolitis, especially regional enteritis. Furthermore, in the therapy of the immunocompromised host including Down's syndrome, we must pay attention to extra-pulmonary tuberculosis. Efficiency of SM for intestinal tuberculosis with complications was confirmed.

[Abnormal findings on IVP study manifested by invasion of pancreatic carcinoma].

Pancreas carcinoma is sometimes notoriously difficult to diagnose. Although it is well known that pancreas carcinoma can invade contiguous organs, no attention to urinary tract involvement is paid. Although urinary tract is involved at a late stage, it may represent the first symptomatic evidence of the disease. In such cases, pancreas carcinoma can be presented initially as a urologic disease and patients' symptom may not be differentiated from urinary tract diseases. Computed tomographic (CT) images and medical records of 50 patients with histologically proved pancreas carcinoma were reviewed. There were 10 cases with urinary tract involvement. IVP study was performed in 8 of these cases. Of the 10 cases 8 were male and 2 were female. There was a high incidence of left urinary tract involvement (left:right:bilateral = 7:2:1). Nine cases were pancreas body and tail carcinoma and 1 case was pancreas head carcinoma. The stages were all in IV. All cases were with confirmed evidence of urinary tract involvement at autopsy within 4 months following CT studies. We compared those IVP, CT images and findings of autopsy. In these patients, 4 cases showed direct invasion of the kidney, 5 cases ureteral involvement and 1 case metastasis to the urinary bladder and ureters. IVP showed inferior displacement of the kidney and/or ureteral displacement on the affected side, as well as collecting system distortion, including hydronephrosis secondary to ureteral compression. CT scan showed an abnormal mass which is contiguous to the pancreas, showing compression of the kidney and/or retroperitoneal extension with ureteral involvement. Metastasis to the urinary bladder is also identified.(ABSTRACT TRUNCATED AT 250 WORDS)
