[Ectopic parotid tissue. An usual differential cervical cystic tumor diagnosis].

Cystic tumors of the neck are usually cysts, inflammatory masses, lymph nodes, benign and malignant tumors or metastases. Malformations are less common. Ectopic salivary tissue is a rather rare but clinically important malformation. Accessory and aberrant tissue can be identified histologically. There may be an association with lymphatic tissue. Possible complications include inflammation as well as the development of benign and in rare cases malignant salivary gland tumors. We report on two patients who underwent surgical treatment under the tentative diagnosis of epidermal cysts which histologically turned out to be ectopic parotid tissue.

Hair removal using a 5-msec long-pulsed ruby laser.

BACKGROUND: Unwanted hair is a widespread cosmetic problem. Many temporary methods of hair removal exist, with laser hair removal rapidly becoming the most widely used modality. OBJECTIVES: Our objective was to evaluate the efficacy and side effects of a long-pulsed ruby laser for epilation in patients with varying skin types and hair color 6 weeks and 6 months after treatment. METHODS: Seventy-four patients received one to four treatments with a long-pulsed ruby laser (694 nm, 5-msec pulse length, 6 mm spot size, 17.5-32 J/cm2) in different body areas. RESULTS: After a mean number of treatments of 1.98 (range 1-4) the mean clearance was 51-75% after 6 weeks and less than 25% after 6 months. A lower percentage of hair regrowth was observed in the facial region than on the trunk or legs and in dark hair compared to blond hair. CONCLUSION: Laser epilation provides a quick and relatively comfortable, but nonpermanent modality for hair removal with few side effects.

[A case of Carney complex].

HISTORY AND CLINICAL FINDINGS: For some months a 57-year-old woman had noted increasing shortness of breath, associated in the last few weeks with undirected vertigo and several brief periods of lost consciousness. She was finally admitted because of additional central facial paresis. On auscultation a high-frequency systolic murmur was heard over the apex and a discrete diastolic murmur over Erb's point. There were numerous facial freckles and three cutaneous myxomas. INVESTIGATIONS: Echocardiography revealed irregular tumours throughout the left atrium and a large broad-based one prolapsing through the mitrale valve in diastole. Computed tomography demonstrated a 6 x 6 cm tumour in the left lower abdomen, probably arising from the left ovary, and a second 3 x 3 cm presacral tumour. TREATMENT AND COURSE: At cardiac surgery four tumours were found in the left atrium and resected: histologically they were benign myxomas. Removal required extensive resection in the area of the interatrial septum and the atrial wall, resulting in 2 degrees AV block for which a VDD pacemaker was implanted. CONCLUSION: Atrial myxomas may be the cardinal sign of the Carney Complex, an autosomal dominant syndrome with cutaneous myxomas, myxoid abdominal tumours, hormone-producing tumours in the testicles, adrenal cortex or hypophysis, schwannoma as well as lentigines. For this reason, further tumours should be looked for if freckles and/or cutaneous tumours are found in association with an atrial myxoma. The patient and family should be informed about the genetic aspects.

A pulsar, the heliosphere, and pioneer 10: probable mimicking of a planet of PSR B1257+12 by solar rotation

Doppler data generated with the Pioneer 10 spacecraft's radio carrier wave between 1987 and 1995 show a 25.3-day periodicity which is related to the solar rotation. The timing data of the pulsar PSR B1257+12 also show a periodicity of 25.34 days, which has been explained as a signature of the pulsar's barycentric motion in response to the existence of a small moon-like object. However, because PSR B1257+12 is located close to the ecliptic and because the timing variations are in the range of microseconds, it is likely that the pulsar signal is affected by the same mechanism acting on the Pioneer 10 Doppler data. Hence, the hypothesized inner planet around PSR B1257+12 is probably an artifact of the heliosphere.

Bihippocampal damage with emotional dysfunction: impaired auditory recognition of fear.

A right-handed man developed a sudden transient, amnestic syndrome associated with bilateral hemorrhage of the hippocampi, probably due to Urbach-Wiethe disease. In the 3rd month, despite significant hippocampal structural damage on imaging, only a milder degree of retrograde and anterograde amnesia persisted on detailed neuropsychological examination. On systematic testing of recognition of facial and vocal expression of emotion, we found an impairment of the vocal perception of fear, but not that of other emotions, such as joy, sadness and anger. Such selective impairment of fear perception was not present in the recognition of facial expression of emotion. Thus emotional perception varies according to the different aspects of emotions and the different modality of presentation (faces versus voices). This is consistent with the idea that there may be multiple emotion systems. The study of emotional perception in this unique case of bilateral involvement of hippocampus suggests that this structure may play a critical role in the recognition of fear in vocal expression, possibly dissociated from that of other emotions and from that of fear in facial expression. In regard of recent data suggesting that the amygdala is playing a role in the recognition of fear in the auditory as well as in the visual modality this could suggest that the hippocampus may be part of the auditory pathway of fear recognition.

Cloning of the VASP (vasodilator-stimulated phosphoprotein) genes in human and mouse: structure, sequence, and chromosomal localization.

The genes encoding the vasodilator-stimulated phosphoprotein (VASP) in human and mouse were isolated, and major parts were sequenced. In both species the gene is composed of 13 exons with conserved exon-intron positions. The mouse VASP cDNA sequence was deduced from the genomic sequence. The predicted amino acid sequence is 89% identical to the human protein. The high nucleotide sequence homology extends not only over the coding regions but also into the 3'-UTRs, indicating a possible function in mRNA targeting or regulation of translation. Prominent 5' CpG islands including multiple SP1 sites indicate a housekeeping function of VASP. Using cosmid DNA as a probe for fluorescence in situ hybridization, the human VASP gene was assigned to chromosome 19q13.2-q13.3, an extended region with homology to mouse chromosome 7. A sequence overlap of the VASP 5'-region with the telomeric end of a cosmid contig physically links the VASP gene with ERCC1. VASP is located about 92 kb distal to ERCC1 and about 300 kb proximal to the myotonic dystrophy protein kinase gene.

[Alcohol in Munich secondary school students--a study for dissemination of experiences, knowledge, attitude and behavior].

The Munich Community Health Service (Medical School-Service Department) conducted an alcohol prevention project in ten public secondary schools. The 423 pupils (11-17 years of age) filled in a questionnaire on alcohol and discussed it afterwards under the guidance of a school physician and a teacher using guideline information. The results of the survey are presented in this paper. Additionally, suggestions for transferring the results to conceptualize and carry out preventive measures are made. The results indicate that the first experience with alcohol occurred early in life. One-fifth of the sample had already tried alcohol at the age of six. Immediate effects of alcohol (e.g. state of drunkenness) were experienced by 35.7 percent of the sample group. Kiosks, shops, and beverage stores were named most frequently as sources for alcohol. The sources of social support varied with respect to sex and age. With increasing age of the children, parents were contacted less frequently whereas the role of peers increased. The present results supply possible paths of action for the development of successful prevention strategies, e.g. by pointing out the early age of onset as well as specific sources of alcohol acquisition.

[The MRT of osteophytosis in experimental gonarthrosis].

PURPOSE: The value of MRI for the detection of knee osteophytosis was determined in an animal osteoarthritis model. METHODS: 10 dogs with experimentally induced unilateral osteoarthritis of the knee were investigated with MRI including 2-D-spin echo (SE) and 3-D-gradient echo (GE) imaging. The results were correlated with gross and histopathologic findings and with radiography. RESULTS: Osteophyte formation appeared early in the osteoarthritic process. Pathological analysis yielded 65 osteophytes. With 3-D-GE imaging, 91% of the osteophytes were detected, while 68% were visible on 2-D-SE images. With two-level radiography, 43% of the osteophytes were diagnosed. Additional use of tunnel view and conventional tomography increased the detection rate to 65%. Independent of the imaging technique, MRI revealed three different signal patterns of the osteophyte bone structure. Compared with the histologically evident fat marrow content and the grade of bone sclerosis, best agreement was achieved with 2-D-SE sequences. However, a discrepancy remained in 22%. A fibrocartilaginous layer at the osteophyte surface could be delineated especially on 3-D-GE images. CONCLUSIONS: MRI provides the highest standard in non-invasive diagnosis of osteophytes in the knee. 3-D-GE imaging in particular can demonstrate early and small osteophyte formation.

Dynamic contrast-enhanced MR imaging of the upper abdomen: enhancement properties of gadobutrol, gadolinium-DTPA-polylysine, and gadolinium-DTPA-cascade-polymer.

The enhancement properties of gadobutrol (40 and 80 mumol/kg body weight, 550 daltons), gadolinium-DTPA-polylysine (20 mumol/kg body weight, 53,000 daltons) and gadolinium-DTPA-cascade-polymer (20 mumol/kg body weight, < 30,000 Daltons) were investigated in abdominal MR imaging using a pig model (n = 24). Signal intensities before and after contrast media application were assessed using a fast single slice FLASH sequence. Measurements were made every 4 s within the first 116 s, every minute between 4 and 10 min and after 15, 20, 30, 40, 50, 60, 90, and 120 min after contrast media injection. Injection of gadobutrol resulted in typical signal intensity curves characterizing it as an extracellular agent similar to gadopentetate dimeglumine. Significant enhancement was found in all tissues except the trunk muscles when the lower dose was administered. Gadolinium-DTPA-polylysine injection resulted also in significant enhancement of the liver, the pancreas, and the renal cortex, but not of the trunk muscle, reflecting its blood pool properties known also from other macromolecular contrast agents. The signal intensity curves obtained after gadolinium-DTPA-cascade-polymer injection were similar to those obtained after polylysine injection, stressing the blood pool character of this new type of blood pool agent.

Using administrative health data to monitor potential adverse health effects in environmental studies.

Following episodes of environmental contamination, health professionals are limited in their ability to detect adverse health effects in surrounding communities due to lack of relevant baseline health data, resources, and appropriate control populations. The objective of this study was to ascertain the feasibility of using administrative health data for these purposes. The Manitoba Health Services Commission's (MHSC) database is comprehensive since universal health care is free in Canada. As part of an evaluation of two proposed hazardous waste treatment sites, the feasibility of using MHSC's data was tested by (a) defining the two study and control sites through use of MHSC's population registry and (b) determining baseline morbidity rates through analysis of MHSC's physician visit payment files; diagnoses were coded using ICD-9-CM. The results indicated that there were some differences between the groups studied in the age- and sex-standardized morbidity rates of diagnoses potentially influenced by exposures to chemicals. Use of administrative data provided by a national health service is an inexpensive and efficient way to create and follow potentially exposed cohorts residing in defined communities. Despite limitations related to small populations in exposed communities and lack of standardized diagnostic criteria by physicians, this method should be explored further in environmental studies.

Particle trajectories in seeds of Lactuca sativa and chromosome aberrations after exposure to cosmic heavy ions on Cosmos Biosatellites 8 and 9.

The potentially specific importance of the heavy ions of the galactic cosmic radiation for radiation protection in manned spaceflight continues to stimulate in situ, i.e., spaceflight experiments to investigate their radiobiological properties. Chromosome aberrations as an expression of a direct assault on the genome are of particular interest in view of cancerogenesis being the primary radiation risk for man in space. In such investigations the establishment of the geometrical correlation between heavy ions' trajectories and the location of radiation sensitive biological substructures is an essential task. The overall qualitative and quantitative precision achieved for the identification of particle trajectories in the order of approximately 10 micrometers as well as the contributing sources of uncertainties are discussed. We describe how this was achieved for seeds of Lactuca sativa as biological test organisms, whose location and orientation had to be derived from contact photographies displaying their outlines and those of the holder plates only. The incidence of chromosome aberrations in cells exposed during the COSMOS 1887 (Biosatellite 8) and the COSMOS 2044 (Biosatellite 9) mission was determined for seeds hit by cosmic heavy ions. In those seeds the incidence of both single and multiple chromosome aberrations was enhanced. The results of the Biosatellite 9 experiment, however, are confounded by spaceflight effects unrelated to the passage of heavy ions.

[MRI of the hyaline knee joint cartilages. Animal and clinical studies].

The value of MR imaging for the detection of hyaline cartilage lesions using 2-D spin-echo and 3-D gradient-echo imaging was evaluated in an animal experiment in 10 dogs and in a clinical study in 30 patients. MR imaging findings were compared with histopathological and arthroscopy findings, respectively. Using MRI neither grade I nor grade II hyaline cartilage lesions were detectable. In the animal experiments 77% of grade III lesions and all the grade IV lesions were seen. However, in the clinical study only about the half of grade III and IV lesions were detected. 3-D gradient-echo MR imaging was superior to 2-D spin-echo imaging (p < 0.001), while 3-D FLASH and 3-D FISP did not differ significantly in the detection rate (p < 0.34). 3-D gradient-echo MR imaging seems to be the best method for the delineation of high grade cartilage lesions. However, early stages of cartilage degeneration are invisible even with this imaging modality.

Biochemical variation of blood proteins in race hounds.

In the present study biochemical polymorphisms were investigated in blood of different race hounds and compared to those of other dog breeds. Twenty-three proteins were separated electrophoretically in starch gel and polyacrylamide gel respectively. For 9 proteins genetic variants could be assured: postalbumin-1 (PA-1), postalbumin-3 (PA-3), transferrin (TRF), plasma esterase (ES), leucine arylaminopeptidase (LAP), haptoglobin (HP), glucose phosphate isomerase (GPI), superoxide dismutase (SOD), albumin (ALB). The already known HP polymorphism was analyzed and described in more detail. In addition, a sex independent variation was found for the GPD.

Randomized placebo-controlled trials of antibiotics for acute bronchitis: a critical review of the literature.

BACKGROUND: Acute bronchitis is a common clinical problem that causes considerable morbidity and presents both diagnostic and treatment dilemmas for the physician. An evaluation of all published randomized controlled trials of antibiotics in the treatment of acute bronchitis was conducted to (1) quantitatively assess methodologic rigor, (2) determine if effectiveness of antimicrobial therapy is known, and (3) analyze strengths and weaknesses of randomized controlled trials in family practice settings. METHODS: A scoring system for the evaluation of randomized controlled trials was adapted for this study. Four raters, who were blinded to which journals published the studies and the type of antibiotic used in each study, assessed the six-randomized clinical trials for treatment of bronchitis identified through a literature search. The trials were rated according to criteria that measured internal validity. RESULTS: Scores for internal validity ranged from 65.5 to 102.5 points with a maximum possible score of 120 points (54.6% to 85.4%). The two trials with the highest scores assessed doxycycline and showed no benefit from use of this antibiotic. Single trials that studied erythromycin and trimethoprim-sulfamethoxazole showed improvement in outcome from use of these drugs; however, of the six trials, these two studies ranked fourth and fifth for internal validity. Low scores resulted from small sample size, possible contamination with other treatment measures, and poor assessment of subjects' compliance with antibiotic regimen. CONCLUSIONS: An evaluation of the current literature does not support antibiotic treatment for acute bronchitis. Further studies of this common illness are indicated. It is hoped that this critical review of randomized control trials will prove useful in the planning of future studies, in placing greater emphasis on methodologic rigor, and in giving greater consideration to the practical constraints of research in the family practice setting.

Computed tomographic enhancement of the liver, liver abscesses, spleen, and major vessels with perfluorooctylbromide emulsion. Influence of dosage and injection velocity in an animal model.

RATIONALE AND OBJECTIVES: Computed tomographic (CT) enhancement of the liver, liver abscess, spleen, and major vessels was investigated between 2 and 48 hours after intravenous administration of perfluorooctylbromide (PFOB emulsion) in an animal model of 63 rabbits. METHODS: Twenty-one animals received 3 g/kg PFOB as a fast bolus injection. Using a slow infusion rate, the same number of animals received either the same dose (3 g/kg) or half the dose (1.5 g/kg). RESULTS: Vascular enhancement was best after bolus injection of 3 g/kg emulsion. The density peak occurred after 2 hours. A continuous enhancement of approximately 100 Hounsfield units (HU) was observed up to 24 hours in the animals receiving 3 g/kg, independent of the injection velocity. A density peak of 70 HU was found 2 hours after the infusion of 1.5 g/kg. The density peak of the liver, the spleen, and the abscess wall was observed 48 hours after emulsion administration in all groups receiving 3 g/kg. The peak was approximately 150 HU for the liver, 400 HU for the spleen, and 150 HU for the abscess wall. In animals receiving only 1.5 g/kg perflubron, the peak density of the abscess wall was 132 HU after 12 hours, approximately 80 HU for the liver between 2 and 48 hours, and approximately 280 HU after 48 hours for the spleen. CONCLUSIONS: PFOB emulsion produces the highest vascular enhancement within the first 2 hours after the bolus injection of 3 g/kg. For spleen and abscess wall imaging, even the relatively low dose of 1.5 g/kg produced a satisfactory enhancement level for a significant length of time, whereas liver enhancement was best after administration of the higher dose.

[Early and late results of PTA in relation to the patient's age].

506 cases of percutaneous transluminal angioplasty performed between 1983 and 1988 in the iliac, femoropopliteal and crural segments were analyzed with special consideration of the patients' age. The early results were significantly worse only in the age group over 70. The late results were evaluated by the life table method. Relapses within the first 6 months occurred in 13.8% of the patients over 60 and in 8.2% of the younger age group (under 60). After 12 months, however, no significant difference was observed between the two age groups. The rate of complications (overall value 7.9%) was twice as high in the geriatric age group than among the younger patients. Type and extent of the occlusion, clinical stage and run-off conditions determine the early and late results.
