Effects of parathyroid hormone on urinary excretion of N-acetyl-beta-D-glucosaminidase in idiopathic hypoparathyroidism and pseudohypoparathyroidism.

N-acetyl-beta-D-glucosaminidase (NAG) is a lysosomal enzyme predominantly located in renal proximal tubules. In idiopathic hypoparathyroidism (IHP), 100 Units of human PTH (1-34) increased urinary excretion of NAG from 0.029 +/- 0.027 to 0.173 +/- 0.035 U/lGF (p less than 0.05) in two patients before treatment and from 0.025 +/- 0.004 to 0.189 +/- 0.092U/lGF (p less than 0.02) in four patients during treatment with active vitamin D3 (1,25(OH)2D3 or 1 alpha OHD3). In pseudohypoparathyroidism (PHP), PTH did not significantly increase the urinary excretion of NAG in one patient with before treatment (0.048 to 0.025 U/lGF) and four patients during treatment with active vitamin D3 (0.018 +/- 0.008 to 0.036 +/- 0.015 U/lGF). Increase in urinary excretion of NAG after injection of PTH may be a new indicator of renal effect of PTH.

Effects of microiontophoretically-applied morphine on the Purkinje cell in the cerebellum of the cat.

The effects of microiontophoretically-applied and pneumatically-applied morphine on the spontaneous discharge of Purkinje cells in the cerebellum of the anesthetized cat were examined. Microiontophoretic application of morphine produced both inhibitory and excitatory responses of the Purkinje cells. Pneumatic application of morphine produced similar effects to those of microiontophoresis. Both types of application of morphine induced dose-dependent responses. Excitatory responses were antagonized by naloxone (opiate antagonist), but inhibitory responses were not affected by naloxone, propranolol (beta-receptor antagonist) or methysergide (serotonin antagonist). Bicuculline and picrotoxin, GABA antagonists, abolished completely the morphine-induced inhibitory response. These results suggest that morphine-induced excitation is connected with the opiate system and that inhibition is related to the GABAergic system, in the cerebellum of the cat.

The deviation of the body's sway center with galvanic stimulation.

We investigated galvanic body sway of ten healthy adults and eight patients by using the averaging program for measuring T1 and T2, which were latencies at the onset and at the cutoff of galvanic stimulation, respectively, and the plotting program, which adopted 1.73 sec as T1 and 0.39 sec as T2, for calculating the coordinates of the body's sway center. We estimated galvanic body sway with the difference between the coordinate of the body's sway center during stimulation and that during no stimulation. The deviation in lateral direction on the healthy subjects ranged from 0.7 or 0.9 to 2.1 cm at right- or left-side stimulation. One of the patients with peripheral disorders revealed differences between right- and left-side stimulation. The other patients did not reveal any differences. In antero-posterior direction six of the healthy subjects deviated backward at right-side stimulation and nine subjects deviated backward at left-side stimulation. The patients with peripheral disorders did not reveal any differences. The other patients revealed some differences. Waves computed by the averaging program do not always show the quantity of galvanic body sway. The deviation of the body's sway center shows the quantity of body sway consistently. The body's sway center is considered to be a suitable parameter in the galvanic body sway test for statistical analysis.

[Preventive effect of KZ-1026 on acute liver injury induced by CCl4 in rats: histochemical, enzyme-histochemical and ultrastructural studies].

The preventive effect of KZ-1026 on acute liver injury induced by CCl4 in rats was evaluated histochemically, enzyme-histochemically and ultrastructurally. Rats that received 50% CCl4 (2 ml/kg) intraperitoneally were sacrificed at 3, 7 and 24 hr. Remarkable reductions in hepatic glycogen, RNA and G-6-Pase activity were observed in the centrilobular area at 3 hr, and ballooned cells appeared in the mid-zone at 7 hr. At 24 hr, the above histochemical parameters in the hepatocytes of the centrilobular area and mid-zone were extensively reduced, while the number of ballooned cells in the mid-zone was increased. KZ-1026 (200 mg/kg) was given orally at 24 and 4 hr before, simultaneously with or 3 hr after CCl4 treatment, and each rat was sacrificed at 24 hr after CCl4 administration. Pretreatment with KZ-1026 24 hr before CCl4 administration prevented reduction of RNA, glycogen and G-6-Phase activity, as well as disruption of rER and proliferation of sER due to CCL4 toxicity. This preventive effect of KZ-1026 was reduced by posttreatment; however, only the decrease in cytoplasmic RNA was well prevented. These results suggested that KZ-1026 is protective against CCl4-induced acute liver injury.

Preventive effect of MK-733 (simvastatin), an inhibitor of HMG-CoA reductase, on hypercholesterolemia and atherosclerosis induced by cholesterol feeding in rabbits.

MK-733 was found to prevent an increase of serum cholesterol levels in cholesterol-fed rabbits, and lovastatin also markedly inhibited their increase. MK-733 and lovastatin inhibited the increase of very low density lipoprotein (VLDL) and low density lipoprotein (LDL) cholesterol, and it slightly affected the high density lipoprotein (HDL) cholesterol levels. MK-733 and lovastatin suppressed the increase of serum phospholipid levels and slightly affected the triglyceride levels. MK-733 suppressed the development of atherosclerosis in coronary arteries and aorta, and lovastatin also diminished their development.

[Partial remission of pulmonary metastasis of malignant gonadal stromal tumor by salvage chemotherapy].

Fourteen malignant cases of gonadal stromal tumor were found among the cases reported in literature, but the roles of chemotherapy and radiotherapy have not been defined. We treated a malignant case and achieved 98.9 per-cent remission of lung metastases by salvage chemotherapy (cis-platinum + VP-16). A 34 year-old desk-worker, underwent left orchiectomy for testicular tumor on March 20, 1984. Histologic examination showed a malignant gonadal stromal tumor invading into the epididymis. Three course of adjuvant chemotherapy with cis-platinum, vinblastine, and peplomycin (PVP) were performed. However, chest X-ray films 13 months later revealed multiple lung metastases. Although 3 courses of combination chemotherapy with cyclophosphamide, adriamycin, vincristine, and prednisolone (CHOP) were given, no remission was obtained. However following 4 courses of combination chemotherapy with cis-platinum and VP-16, 98.9 per-cent remission of lung metastases was achieved. Subsequent bilateral residual lung tumors were resected. However, lung metastases recurred 3 months later, and the patient died on February 29, 1988. The autopsy revealed extensive lung, liver and pancreas metastases, but no lymph node metastases.

Localization of type III procollagen mRNA in areas of liver fibrosis by in situ hybridization.

Localization of type III procollagen mRNA in human liver was studied by in situ hybridization using human alpha 1(III) procollagen cDNA. Frozen and paraformaldehyde-fixed sections of biopsied human liver from patients with chronic hepatitis and cirrhosis were examined using the digoxigenin-labeled cDNA probe. Localization of the type III procollagen mRNA was demonstrated not only in the cytoplasm of mesenchymal cells but also in a large number of hepatocytes, in proportion to the extent of fibrosis. These results suggest that hepatocytes play an important role in fibrogenesis in the liver.

[A case of adenocarcinoma of the thymus].

A case of adenocarcinoma of the thymus is reported in this paper. To the best of our knowledge, this is the first case of thymic adenocarcinoma in the world literature. A 51-year-old female was admitted to our hospital because of hyperthyroidism and anterior mediastinal tumor. After control of hyperthyroidism, thoracotomy was performed and the tumor was located in the right lobe of the thymus macroscopically. The tumor was extirpated and the specimen was revealed to be multiple thymic cyst with papillo-tubular adenocarcinoma proliferating in one of the cysts. The variability of the lining cells i.e. squamous, cuboidal, columnar, and transitional cell, suggest that the thymic cyst is of congenital origin and the adenacarcinoma is originated from multipotential lining cells of the cyst. She has no evidence of recurrence for 14 years postoperatively, therefore there is almost no possibility of metastasis from other organ to the anterior mediastinum.

Neovascularization of pericellular fibrosis in alcoholic liver disease.

The relationship between sinusoidal capillarization and pericellular fibrosis was studied in 29 specimens of human liver from patients with alcoholic liver disease. Immunohistochemically, factor VIII-related antigen was not observed in the normal sinusoidal lining cells, but was localized in the capillaries which proliferated in the pericellular fibrotic region. Fibronectin was localized in the proliferated endoplasmic reticulum of newly formed vascular endothelial cells and hepatocytes with the development of pericellular fibrosis, laminin became apparent in the plasmalemmal vesicles of capillary endothelial cells and pericytes and in the endoplasmic reticulum of mesenchymal cells in Disse's space. From these results, it was revealed that pericellular fibrosis was closely related to sinusoidal capillarization. Furthermore, it is suggested that this sinusoidal capillarization may be caused by neovascularization, and that the extracellular matrix produced by endothelial cells, hepatocytes and mesenchymal cells may promote this process.

Elastosis in meningioma. A case report and electron microscopic study.

A 55-year-old man underwent left frontoparietal craniotomy for total removal of a parasagittal meningioma. The tumor showed a predominant pattern of meningotheliomatous meningioma and a partial pattern of angiomatous, transitional and fibrous meningioma. A striking feature was the appearance of unusually numerous elastinophilic material in the intercellular stroma. We observed a process in which the progress of elastosis changed the appearance of meningotheliomatous meningioma to a hypocellular, and subsequently acellular fibroelastotic node. Three years after the first craniotomy, meningioma recurred at the same site. The recurrent tumor also showed the histological pattern of meningotheliomatous meningioma with stromal elastosis, as previously observed. This is the first report of a case of meningioma with stromal elastosis. The present case represents the first unequivocal demonstration of elastogenesis in meningioma.

[Effects of sera of gynecological cancer and pregnancy on cellular immunity].

The effects of sera from untreated gynecological cancer patients in 1st, 2nd, 3rd trimesters of normal pregnancy on PHA and MLR induced lymphocytes blastformation, natural killer cell activity and killer T cell induction were analysed. 1. The 3H-thymidine uptake of lymphocytes by PHA and MLR significantly decreased in cancerous and 3rd trimester pregnant sera. It is thought that the existence of non specific immunosuppressive factors in these conditions, and this phenomenon might be caused by coating and/or masking of the surface of the effector cell. The factors might have an effect on the proliferation phase of lymphocyte blastoformation. 2. Normal human lymphocytes pretreated with cancerous sera inhibited natural killer cell activity. This phenomenon could not be demonstrated with pregnant sera. 3. Cancerous and 3rd trimester pregnant sera suppressed the process of killer T cell induction caused by alloantigen.

[Flow imaging of the cardiovascular system using magnetic resonance imaging].

Blood flow images by magnetic resonance imaging (MRI) using a 0.25 T unit were evaluated for nine normal volunteers and 108 subjects with a variety of cardiovascular abnormalities. Using the non-gated short-spin echo (SE) technique, blood flow in the cardiovascular systems was not imaged in the normal volunteers. Using end-systolic and end-diastolic SE techniques for the normal subjects, blood flow in the cardiac chambers was not clearly imaged. Blood flow in the ascending aorta and aotric arch often did not appear in the gated SE images of the normal subjects. However, blood flow in the descending aorta was often observed in the gated SE images. Blood flow imaging was demonstrated by both non-gated and gated SE techniques in regions where blood flow was relatively slow; for example, in the left atrium of mitral stenosis, in an aortic aneurysm, in a false lumen of an aortic dissection, and in the left ventricle having old myocardial infarction. Using the non-gated inversion recovery (IR) technique, no blood flow was imaged in the cardiovascular system except in the left atrium of one case with mitral stenosis. Using the non-gated short SE technique, there was good correlation between the thrombus formation and the presence of blood flow images in the left atria of 17 patients with mitral stenosis, and in the aneurysmal portions of the aorta or in the false lumens of aortic dissection of 18 patients. It was suggested that mural thrombi in such diseases were related to the relatively slow blood flow. Blood flow imaging easily distinguished stagnant blood flow from mural thrombi using non-gated short SE, end-systolic SE, and IR techniques. Thus, blood flow imaging using MRI should become an important means of evaluating the cardiovascular system.
