A 19-year-old male with papillary adenocarcinoma of the seminal vesicle with cyst is reported. Cytological and histopathological studies were carried out and the relevant literature was perused. There are only 6 cases of carcinoma and 40 cases of cyst of the seminal vesicle in the Japanese literature. Moreover, ours is the first reported case of the co-existence of carcinoma and cyst in the seminal vesicle. There may be a pathogenetic relationship between the carcinoma and cyst in our case.
Changes of secretory activity and structure of the adrenal cortex were observed in dogs subjected to hemorrhagic shock. Blood cortisol and aldosterone levels were rapidly increased after hemorrhage. The cortisol level reached a maximum value about 5 times that of normal 1 h after hemorrhage, while aldosterone levels attained maximum levels showing a 10-fold increase over normal, considerably later (at the end of the impending stage) than the cortisol. Cortisol secretion was quickly depleted at the end of critical stage and terminal stage of normovolemic shock, while the aldosterone secretion remained relatively high. Morphologically, sticking of leukocytes to the sinusoidal wall and accumulation of SER around lipid droplets in the fasciculata and reticularis cells became conspicuous 1 h after hemorrhage. At the end of the impending stage, infiltration of leukocytes occurred in the zona fasciculata and reticularis. Irregular masses of fibrin strands appeared in the sinusoidal space and at large gaps of the sinusoidal wall. Degenerative changes such as aggregation of SER, dilatation of endoplasmic reticulum and edema were observed in the cells of zona fasciculata and reticularis, especially of inner half of the former. At the end of the critical stage, small necrotic foci and hemorrhage with inflammatory infiltration were scattered in the inner half of the zona fasciculata. Degeneration of the cells of zona fasciculata and reticularis became more severe in this period, while the cells of zona glomerulosa remained unchanged. At the terminal stage of normovolemic shock after reinfusion, degeneration and necrosis were remarkable in the cells of zona fasciculata and reticularis, especially of the inner half of the zona fasciculata. Moreover, destruction of cordal structure, hemorrhage and inflammatory infiltration were distinct in the zona fasciculata and reticularis, while the zona glomerulosa was relatively well preserved.

Effects of morphine hydrochloride on the cerebellum were examined in cats. Morphine (2.0 mg/kg, i.v.) decreased the potentials by superficial radial nerve (SR), nucleus reticularis lateralis (LRN), or nucleus olivaris inferior (ION) stimulation on the cerebellar cortices. These effects were antagonized by naloxone hydrochloride (0.4 mg/kg, i.v.). In the LRN or ION, morphine decreased the potentials evoked by SR stimulation. In the LRN- or ION-lesioned cat, morphine decreased the potentials evoked by SR stimulation on the cerebellar cortices. This opiate decreased the potentials evoked by tooth pulp stimulation on the vermis, and this effect was also antagonized by naloxone. Morphine slightly decreased the potentials evoked by SSA-1 stimulation on the cerebellar cortices. Furthermore, the potentials evoked by SR stimulation on the SAA-1 were slightly decreased by morphine. These results may suggest that the cerebellum plays an important role in not only the motor system, but also the sensation of pain.
In intact cats, morphine hydrochloride (2.0 mg/kg, i.v.) depressed spontaneous discharge of the Purkinje cell in 8 of the 15 neurons tested. These effects of morphine were antagonized by naloxone hydrochloride (0.4 mg/kg, i.v.). This opiate significantly decreased the evoked potentials produced by superficial radial nerve (SR) stimulation in the molecular layer of the cerebellar cortex. Although morphine decreased the potentials evoked by SR or tooth pulp stimulation in the nucleus fastigii and nucleus interpositus, this drug hardly influenced them in the nucleus dentatus. In the decerebrated cats, morphine significantly increased the potentials evoked in the cerebellar cortices by SR stimulation. In the somatosensory area I (SSA-I)- or motor area (MA)-lesioned cats, morphine decreased the cerebellar potentials evoked by SR stimulation, but increased them in the SSA-I- and MA-lesioned cats. Naloxone antagonized these effects of morphine. From our results, it appears that the decreasing effects of morphine on the cerebellar potentials evoked by SR stimulation may be affected by the functions of the SSA-I and MA of the cerebral cortex and that morphine may influence the cerebellum.

Following results were obtained from intravenous administration of T-1982 (cefbuperazone) 1 g by measuring its concentrations in uterine arterial serum, cubital venous serum, oviduct, ovary and several sites in uterine tissue. Endometrium showed the highest concentration among various uterine tissues by any administration (bolus injection, dripping infusion for 1 or 2 hours). Transfer concentrations about 1 hour after the end of 1 hour drip infusion proved to be almost the same as 2 hours drip infusion. In the field of obstetrics and gynecology, it was considered that T-1982 has good efficacy in infections especially caused by E. coli, Klebsiella and Proteus.

Experiments were conducted implementing a special technique for shifting instantaneously from continuous flow to pulsatile flow assistance by using the assistant heart and obtaining the following results: Within only a few minutes, the shift could easily be made from one type of flow assistance to another on calves with normal hearts. In the 10 calves in which ventricular failure was produced, this instantaneous shift from continuous to pulsatile flow assistance brought about remarkable improvements in their hemodynamics. At the occurrence ventricular fibrillation in 6 of the calves, the return to sinus rhythm was possible with aid of this shift mechanism.
Statistical studies on 2,040 new outpatients, 575 inpatients and 570 operative procedures at our department in 1982 revealed the following. The most frequent diseases among the outpatients were urogenital infections followed by anomalies and tumors. Over half of the inpatients were pediatric patients and the major diseases among inpatients were hypospadias, vesicoureteral reflux, urolithiasis, congenital urethral stenosis and undescended testis. A total of 570 operations were performed on 531 patients, and the major five operations were hypospadias repair (79), optic internal urethrotomy (71), TUR-P (49), ureterocystoneostomy (33) and orchidopexy (32).
A 56-year-old-married woman, gravida 3, para 2, was admitted with chief complaints of atypical genital bleeding, large abdominal and genital tumors, dysuria and abdominal pain. All laboratory examinations were noncontributory. Abdominal total hysterectomy with bilateral salpingo-oophorectomy was performed. The tumor measured 30 X 11 X 8 cm and weighed 1,600 g. It showed polypoid growth and most of the tumor was necrotic. The tumor cells resembled endometrial cells and 20 mitotic figures per 10 high-power fields were noted. Ten months postoperatively, the patient was in good condition with use of PSK.

We treated a 7-year-old girl in whom the clinical evidence of Ebstein disease was manifest after a surgical closure of an atrial septal defect. This manifestation, which was not evident either in the preoperative catheterization studies or during operative investigation of closure of the atrial septal defect, required further hemodynamic and angiographic evaluation. Tricuspid valve replacement with Hall-Kaster prostheses was carried out.
The effects of drugs on the potentials evoked by electrical stimulation on the sensorimotor area (SMA), nucleus reticularis tegmenti pontis (PTRN), nucleus reticularis lateralis (LRN), nucleus olivaris inferior (ION), or superficial radial nerve (SR) were investigated in cat cerebellar cortices. Pentobarbital-Na decreased the amplitude of the potentials evoked by all stimulations at every recording site. Pentobarbital-Na remarkably decreased the SMA- or PTRN-stimulation induced evoked potentials in the anterior lobe. Chlorpromazine decreased the amplitude of the potentials evoked by SMA stimulation on the anterior lobe and ipsilateral crus l, and the drug increased it on the posterior lobe and contralateral crus l. With precerebellar nuclei stimulations, chlorpromazine decreased the amplitude of the evoked potentials in cerebellar cortices, whereas SR stimulation-evoked potentials were remarkably increased in amplitude. Meprobamate increased the amplitude of the potentials evoked by precerebellar nuclei or SR stimulation at an early stage and then decreased it. Caffeine, picrotoxin, and strychnine remarkably increased the amplitude of the potentials in cerebellar cortices evoked by all stimulations. Strychnine in particular significantly increased the amplitude of the potentials evoked by SR stimulation. These results strongly indicate that CNS depressants and stimulants affect the cerebellar afferent pathways, probably in an indirect manner.
Since serine protease in involved in histamine release from mast cells, we attempted to prepare new protease inhibitors, trans-4-(guanidinomethyl)cyclohexanecarboxylic acid (GmcHX-CO2H) esters, and examined their inhibitory effects on typical serine proteases and on histamine release induced by compound 48/80. We compared their effects with those of trans-4-(aminomethyl)cyclohexanecarboxylic acid (AmcHx-CO2H) esters. AmcHxCO2H and GmcHxCO2H esters inhibited the esterolytic activity of trypsin, but GmcHx-CO2H esters had little or no inhibitory effect on caseinolytic activity whereas AmcHxCO2H esters strongly inhibited the latter. AmcHCO2H esters strongly inhibited plasmin but had no effect on chymotrypsin. GmcHxCO2H esters strongly inhibited the esterolytic activity of chymotrypsin, but had no effect on chymotrypsin-induced caseinolysis. Both GmcHxCO2H an AmcHxCO2H esters inhibited urokinase. Of the esters of AmcHxCO2H and GmcHxCO2H tested, only GmcHxCO2H p-tert-butylphenyl ester (GmcHxCOOPhBut) at low concentration (27 microM) strongly inhibited histamine release from rat mast cells induced by compound 48/80. GmcHxCOOPhBut was effective in preventing active systemic anaphylaxis and passively sensitized guinea pigs. Its effectiveness in preventing anaphylactic phenomena might be due to its strong inhibitory effects on histamine release from mast cells.
