[Perinatal management of congenital anomalies of the urinary tract detected in utero].

The routine use of maternal ultrasonography has led to frequent discovery of fetal anatomical abnormalities of the urinary tract. Herein we report on 35 cases with congenital urogenital anomalies which were found during the last 5 years by the prenatal ultrasonography and referred to our clinic either for treatment or consultation. 1. Findings of prenatal ultrasonography There were 12 cases of hydronephrosis, 7 of multicystic dysplastic kidney, 4 of megaureter, 3 of posterior urethral valve, 2 of ureterocele, 2 of vesicoureteral reflux and 5 other cases. Fetal anomalies were diagnosed during routine maternal examination, as intrauterine growth retardation, oligohydramnios, or the recurrent risk secondary to genetic risk factor or previous abnormal pregnancies. Prenatal diagnosis was made as early as 20 weeks of gestation in a case of hypoplastic urethra with large bladder diverticula. In about 30% of cases, abnormalities were found before 30 weeks of gestation. Oligohydramnios was evident in 5 cases. Prenatal diagnosis was correct in about 60% of cases. In the remainder the diagnosis was either incomplete or incorrect. Errors in the diagnosis resulted from difficulties in the differentiation of dilated ureter, intestinal dilation or intraperitoneal cystic masses. 2. Management in newborn periods Of the patients, 12 were treated urologically in newborn periods. Although patients with distended bladder and dilatation of upper tracts first underwent therapeutic placement of the urethral catheter, upper tract diversion with nephrostomy or tubeless ureterocutaneostomy was required in 2 because of continued elevation of serum creatinine levels. In 2 neonates nephrectomy was indicated for unilateral multicystic kidney, because the large cystic mass might compress the intestine.(ABSTRACT TRUNCATED AT 250 WORDS)

[Estimation of glomerular filtration rate from the urinary excreted contrast medium concentration].

In the animal experiments, glomerular filtration rate (GFR) values were estimated from the urinary response to the intravenously administrated iopamidol. By introducing some reasonable assumptions, mass balance equations about the contrast medium were transformed and GFR was defined as the ratio of logarithm of urinary concentration by time. Urinary concentrations were determined by measuring the photodensity on the X-ray film of urine specimens. Obtained values of GFR were well fitted to those obtained by the analysis of the plasma disappearance curve. The advantage of this method is that measurement of urinary volume is not necessary.

[Spontaneous resolution of reflux in children with primary VUR].

Although the adverse effects of vesicoureteral reflux on the renal function and renal growth have been well documented, there is still controversy regarding the factors for spontaneous cure of reflux and the time required for the resolution. We performed a retrospective analysis of 829 ureters in 541 children who have been followed for more than 3 years since 1973. Reflux was judged as spontaneously cured only after 2 micturition cystourethrograms (MCU) had shown no reflux. In total reflux resolved in 220 ureters (26.5%) of 182 children (33.6%). Resolution rate was 39.5% in boys and 28.0% in girls, and there was statistically significant difference between both sexes. In children who had underwent urethrotomy on congenital urethral stenosis, e.g. bulbous urethral stenosis in the boy and distal urethral stenosis in the girl, resolution rate was 43% in boys and 23% in girls. Resolution rate according to VUR-grade was 85.5% in grade I and II reflux, 45.8% in grade III, and 6.7% in grade IV and V. Of 92 children who underwent urodynamic evaluations, 37% showed findings of unstable bladder. Renal scarring was observed in 7.3% of renal units which was significantly less frequent than the incidence among total renal units with reflux. Reflux was resolved in 30% of renal units without scarring and 10% of renal units with scarring. The incidence of small kidney in the spontaneously resolved group was 3.6%, which was significantly lower compared to the total renal units with reflux. The mean age at resolution of reflux was 5.7 years in boys and 7.5 years in girls.(ABSTRACT TRUNCATED AT 250 WORDS)

General pharmacological profiles of the new beta-adrenoceptor antagonist carvedilol.

General pharmacological properties of carvedilol (BM 14.190) were investigated in comparison with propranolol. 1. Central nervous system: Carvedilol caused reduction of awareness, motor activity and muscle tone, and staggering gait in Irwin's test (mice). It decreased spontaneous motor activity and potentiated hexobarbital anesthesia (mice). It lacked anticonvulsant activity (mice) and did not have any effect on body temperature (rabbits). Various changes were produced in mono- and polysynaptic spinal reflex (cats). In EEG, a slight arousal pattern was observed (cats). These effects of carvedilol were weaker than those observed after propranolol administration in general. Carvedilol, however, caused potentiation of hexobarbital anesthesia at lower doses than propranolol. Carvedilol inhibited acetic acid-induced writhing syndrome, whereas it failed to show analgesic activity in the tail-pinch test (mice). Propranolol inhibited both pain reactions. 2. Respiratory and cardiovascular system (dogs): Carvedilol increased respiratory rate and decreased expiratory velocity. It produced hypotension and bradycardia. Cardiac contractility was reduced and femoral blood flow was transiently increased after carvedilol administration. Propranolol induced weaker hypotension and greater bradycardia in comparison with carvedilol. It decreased femoral blood flow. 3. Autonomic nervous system: Carvedilol had little or no effects on pupil size, whereas propranolol produced mydriasis (rabbits). Carvedilol inhibited pressor response to norepinephrine (rats), and it also reduced the nictitating membrane contraction induced by pre- and postganglionic sympathetic nerve stimulation (cats). Propranolol did not show any inhibitory effect on pressor response to norepinephrine and on the contractile response induced by preganglionic sympathetic nerve stimulation. 4. Smooth muscle: Carvedilol produced inhibitory effects on spontaneous motility, and contractile responses to acetylcholine, histamine, nicotine, serotonin and BaCl2 in isolated ileum (guinea pigs). It also inhibited contractile responses to acetylcholine and histamine in isolated trachea (guinea pigs), and spontaneous motility in isolated uterus (rats). Carvedilol reduced norepinephrine-induced contraction of isolated vas deferens at lower concentration (guinea pigs). 5. Digestive system: Decrease in intestinal motility was observed after intravenous administration of carvedilol and propranolol (rabbits). However, carvedilol failed to influence on gastric motility and tonus, whereas propranolol increased them (rabbits). Carvedilol, like propranolol, induced little or no effects on gastro-intestinal transit (mice) and gastric emptying rate (rats). Both drugs decreased gastric secretion at similar dose (rats). Carvedilol at higher doses produced lesion of gastric mucosa, whereas propranolol did not show these effects (rats). 6. Skeletal muscle: In in vitro experiment, carvedilol, like propranolol, reduced the contractile response of diaphragm to nerve and muscle stimulation (rats)...

Adrenergic beta-Antagonists

[Evaluation of pluronic F127 as a base for gradual release of anticancer drug].

The property of pluronic F127 (PLF) as a base for an anticancer agent, mitomycin C (MMC) was evaluated. A weighed amount of PLF and MMC were slowly added to cold water and mixed completely. The solution was warmed to 37 degrees C and MMC containing PLF gel was formed. The cumulative amount of MMC which was released from the gel during a 3-hour period was calculated. 5.5 percent of MMC was released from PLF-MMC gel which contained 20 or 25% PLF and 3.8% MMC from PLF-MMC gel containing 30% PLF. Intraperitoneal (i.p.) administration of large amount of PLF gel alone in rabbits or mouse revealed severe hepatorenal toxicity, and acute i.p. administered LD50 of PLF was supposed to be between 1.7 g and 5.0 g/kg body weight in mouse. Further evaluation of gradual releasing property of PLF is needed.

Preliminary studies on the acid soluble glycoproteins in serum and their diagnostic value for acute inflammatory diseases in cattle.

The concentration of acid soluble glycoproteins (ASG) in serum was determined by the Coomassie brilliant blue G-250 method as a means of assessing the amount of the acute phase protein in bovine serum. The mean (+/- SD) ASG concentration in sera from 30 normal dairy cows was 98.0 (+/- 31.5) mg/dl (range 42.2-161.0 mg/dl). The mean serum ASG concentrations in cattle suffering from enzootic bovine leukosis and enzootic bovine leukosis associated with glomerulonephritis were significantly (p less than 0.001) higher than those of normal cows. Elevated ASG concentrations were observed in cows with acute clinical mastitis. Higher (1.1-2.9 fold) ASG concentrations were observed in sera from cattle on the fifth day after surgery than in those taken at the preoperative stage. The measurement of ASG in serum could be used routinely in bovine clinical medicine as a useful indicator of inflammatory processes.

Effects of parathyroid hormone on urinary excretion of N-acetyl-beta-D-glucosaminidase in idiopathic hypoparathyroidism and pseudohypoparathyroidism.

N-acetyl-beta-D-glucosaminidase (NAG) is a lysosomal enzyme predominantly located in renal proximal tubules. In idiopathic hypoparathyroidism (IHP), 100 Units of human PTH (1-34) increased urinary excretion of NAG from 0.029 +/- 0.027 to 0.173 +/- 0.035 U/lGF (p less than 0.05) in two patients before treatment and from 0.025 +/- 0.004 to 0.189 +/- 0.092U/lGF (p less than 0.02) in four patients during treatment with active vitamin D3 (1,25(OH)2D3 or 1 alpha OHD3). In pseudohypoparathyroidism (PHP), PTH did not significantly increase the urinary excretion of NAG in one patient with before treatment (0.048 to 0.025 U/lGF) and four patients during treatment with active vitamin D3 (0.018 +/- 0.008 to 0.036 +/- 0.015 U/lGF). Increase in urinary excretion of NAG after injection of PTH may be a new indicator of renal effect of PTH.

Effects of microiontophoretically-applied morphine on the Purkinje cell in the cerebellum of the cat.

The effects of microiontophoretically-applied and pneumatically-applied morphine on the spontaneous discharge of Purkinje cells in the cerebellum of the anesthetized cat were examined. Microiontophoretic application of morphine produced both inhibitory and excitatory responses of the Purkinje cells. Pneumatic application of morphine produced similar effects to those of microiontophoresis. Both types of application of morphine induced dose-dependent responses. Excitatory responses were antagonized by naloxone (opiate antagonist), but inhibitory responses were not affected by naloxone, propranolol (beta-receptor antagonist) or methysergide (serotonin antagonist). Bicuculline and picrotoxin, GABA antagonists, abolished completely the morphine-induced inhibitory response. These results suggest that morphine-induced excitation is connected with the opiate system and that inhibition is related to the GABAergic system, in the cerebellum of the cat.

The deviation of the body's sway center with galvanic stimulation.

We investigated galvanic body sway of ten healthy adults and eight patients by using the averaging program for measuring T1 and T2, which were latencies at the onset and at the cutoff of galvanic stimulation, respectively, and the plotting program, which adopted 1.73 sec as T1 and 0.39 sec as T2, for calculating the coordinates of the body's sway center. We estimated galvanic body sway with the difference between the coordinate of the body's sway center during stimulation and that during no stimulation. The deviation in lateral direction on the healthy subjects ranged from 0.7 or 0.9 to 2.1 cm at right- or left-side stimulation. One of the patients with peripheral disorders revealed differences between right- and left-side stimulation. The other patients did not reveal any differences. In antero-posterior direction six of the healthy subjects deviated backward at right-side stimulation and nine subjects deviated backward at left-side stimulation. The patients with peripheral disorders did not reveal any differences. The other patients revealed some differences. Waves computed by the averaging program do not always show the quantity of galvanic body sway. The deviation of the body's sway center shows the quantity of body sway consistently. The body's sway center is considered to be a suitable parameter in the galvanic body sway test for statistical analysis.

[Preventive effect of KZ-1026 on acute liver injury induced by CCl4 in rats: histochemical, enzyme-histochemical and ultrastructural studies].

The preventive effect of KZ-1026 on acute liver injury induced by CCl4 in rats was evaluated histochemically, enzyme-histochemically and ultrastructurally. Rats that received 50% CCl4 (2 ml/kg) intraperitoneally were sacrificed at 3, 7 and 24 hr. Remarkable reductions in hepatic glycogen, RNA and G-6-Pase activity were observed in the centrilobular area at 3 hr, and ballooned cells appeared in the mid-zone at 7 hr. At 24 hr, the above histochemical parameters in the hepatocytes of the centrilobular area and mid-zone were extensively reduced, while the number of ballooned cells in the mid-zone was increased. KZ-1026 (200 mg/kg) was given orally at 24 and 4 hr before, simultaneously with or 3 hr after CCl4 treatment, and each rat was sacrificed at 24 hr after CCl4 administration. Pretreatment with KZ-1026 24 hr before CCl4 administration prevented reduction of RNA, glycogen and G-6-Phase activity, as well as disruption of rER and proliferation of sER due to CCL4 toxicity. This preventive effect of KZ-1026 was reduced by posttreatment; however, only the decrease in cytoplasmic RNA was well prevented. These results suggested that KZ-1026 is protective against CCl4-induced acute liver injury.

Preventive effect of MK-733 (simvastatin), an inhibitor of HMG-CoA reductase, on hypercholesterolemia and atherosclerosis induced by cholesterol feeding in rabbits.

MK-733 was found to prevent an increase of serum cholesterol levels in cholesterol-fed rabbits, and lovastatin also markedly inhibited their increase. MK-733 and lovastatin inhibited the increase of very low density lipoprotein (VLDL) and low density lipoprotein (LDL) cholesterol, and it slightly affected the high density lipoprotein (HDL) cholesterol levels. MK-733 and lovastatin suppressed the increase of serum phospholipid levels and slightly affected the triglyceride levels. MK-733 suppressed the development of atherosclerosis in coronary arteries and aorta, and lovastatin also diminished their development.

[Partial remission of pulmonary metastasis of malignant gonadal stromal tumor by salvage chemotherapy].

Fourteen malignant cases of gonadal stromal tumor were found among the cases reported in literature, but the roles of chemotherapy and radiotherapy have not been defined. We treated a malignant case and achieved 98.9 per-cent remission of lung metastases by salvage chemotherapy (cis-platinum + VP-16). A 34 year-old desk-worker, underwent left orchiectomy for testicular tumor on March 20, 1984. Histologic examination showed a malignant gonadal stromal tumor invading into the epididymis. Three course of adjuvant chemotherapy with cis-platinum, vinblastine, and peplomycin (PVP) were performed. However, chest X-ray films 13 months later revealed multiple lung metastases. Although 3 courses of combination chemotherapy with cyclophosphamide, adriamycin, vincristine, and prednisolone (CHOP) were given, no remission was obtained. However following 4 courses of combination chemotherapy with cis-platinum and VP-16, 98.9 per-cent remission of lung metastases was achieved. Subsequent bilateral residual lung tumors were resected. However, lung metastases recurred 3 months later, and the patient died on February 29, 1988. The autopsy revealed extensive lung, liver and pancreas metastases, but no lymph node metastases.

Localization of type III procollagen mRNA in areas of liver fibrosis by in situ hybridization.

Localization of type III procollagen mRNA in human liver was studied by in situ hybridization using human alpha 1(III) procollagen cDNA. Frozen and paraformaldehyde-fixed sections of biopsied human liver from patients with chronic hepatitis and cirrhosis were examined using the digoxigenin-labeled cDNA probe. Localization of the type III procollagen mRNA was demonstrated not only in the cytoplasm of mesenchymal cells but also in a large number of hepatocytes, in proportion to the extent of fibrosis. These results suggest that hepatocytes play an important role in fibrogenesis in the liver.

[A case of adenocarcinoma of the thymus].

A case of adenocarcinoma of the thymus is reported in this paper. To the best of our knowledge, this is the first case of thymic adenocarcinoma in the world literature. A 51-year-old female was admitted to our hospital because of hyperthyroidism and anterior mediastinal tumor. After control of hyperthyroidism, thoracotomy was performed and the tumor was located in the right lobe of the thymus macroscopically. The tumor was extirpated and the specimen was revealed to be multiple thymic cyst with papillo-tubular adenocarcinoma proliferating in one of the cysts. The variability of the lining cells i.e. squamous, cuboidal, columnar, and transitional cell, suggest that the thymic cyst is of congenital origin and the adenacarcinoma is originated from multipotential lining cells of the cyst. She has no evidence of recurrence for 14 years postoperatively, therefore there is almost no possibility of metastasis from other organ to the anterior mediastinum.

Neovascularization of pericellular fibrosis in alcoholic liver disease.

The relationship between sinusoidal capillarization and pericellular fibrosis was studied in 29 specimens of human liver from patients with alcoholic liver disease. Immunohistochemically, factor VIII-related antigen was not observed in the normal sinusoidal lining cells, but was localized in the capillaries which proliferated in the pericellular fibrotic region. Fibronectin was localized in the proliferated endoplasmic reticulum of newly formed vascular endothelial cells and hepatocytes with the development of pericellular fibrosis, laminin became apparent in the plasmalemmal vesicles of capillary endothelial cells and pericytes and in the endoplasmic reticulum of mesenchymal cells in Disse's space. From these results, it was revealed that pericellular fibrosis was closely related to sinusoidal capillarization. Furthermore, it is suggested that this sinusoidal capillarization may be caused by neovascularization, and that the extracellular matrix produced by endothelial cells, hepatocytes and mesenchymal cells may promote this process.

Elastosis in meningioma. A case report and electron microscopic study.

A 55-year-old man underwent left frontoparietal craniotomy for total removal of a parasagittal meningioma. The tumor showed a predominant pattern of meningotheliomatous meningioma and a partial pattern of angiomatous, transitional and fibrous meningioma. A striking feature was the appearance of unusually numerous elastinophilic material in the intercellular stroma. We observed a process in which the progress of elastosis changed the appearance of meningotheliomatous meningioma to a hypocellular, and subsequently acellular fibroelastotic node. Three years after the first craniotomy, meningioma recurred at the same site. The recurrent tumor also showed the histological pattern of meningotheliomatous meningioma with stromal elastosis, as previously observed. This is the first report of a case of meningioma with stromal elastosis. The present case represents the first unequivocal demonstration of elastogenesis in meningioma.
