In vivo voltammetric studies of the effect of morphine on the serotonergic system in the cat spinal cord.

The purpose of the present paper is to investigate the effect of morphine on the serotonergic system through an analysis of electrochemical signals in the spinal cord of the anesthetized cat. Electrochemical signals showed a peak of 5-hydroxyindole at 280-300 mV, and were highest at a depth of 1500-2500 microns in the dorsal horn of the spinal cord. Electrical stimulation of the nucleus raphe magnus (NRM) produced an increase in the electrochemical signal. Five-hydroxytryptophan (25 mg/kg, i.v.) also enhanced the signal remarkably. Both systemic administration of morphine (1-2 mg/kg, i.v.) and microinjection (5 and 10 micrograms/microliter) of morphine into the NRM increased the signal. In conclusion, it is suggested that morphine-induced enhancement of the peak of 5-hydroxyindole contributes to the activity of the serotonergic system in the cat spinal cord.

A case of cerebellar hamartoma suggesting abnormal cell migration.

A rare case of hamartoma of the left cerebellar hemisphere was recognized in an 11-month-old male infant whose mother had a history of unspecified medication in the early gestational period and had a difficult delivery. A notably large head and marked developmental disorders, like hypotonic cerebral palsy, were observed soon after birth. A computed tomogram revealed an iso-minimally enhanced large mass in the left cerebellar hemisphere, which deformed the fourth ventricle and compressed the right cerebellum, as well as moderate cerebral atrophy. Histologically, the border between the cerebellar cortex and this tumor was not apparent. The main tumor, located in the cerebellar white matter, was composed of numerous scattered Purkinje cell-like neurons and glial cells surrounded by abundant GFAP-positive matrix. The small part of the tumor, located near the choroid plexus, was composed of intensely proliferated capillaries such as in capillary hemangioma, and numerous fibrocytes, which were intermingled with several large Purkinje cell-like neurons and some with several large Purkinje cell-like neurons and some GFAP-positive glial cells. The cerebellar cortex showed a thin molecular layer with some residual external granular cells, a marked decrease of Purkinje cells and a moderate decrease in the internal granular layer, in which large Purkinje cell-like neurons were scattered. Purkinje cells and large Purkinje cell-like neurons scattered in the internal granular layer, cerebellar white matter and choroid plexus showed positive immunoreactivity for anti-Leu-4 monoclonal antibody, which is known to be a marker for Purkinje cells.(ABSTRACT TRUNCATED AT 250 WORDS)

Titanium dioxide deposition and adenocarcinoma of the lung.

A case of titanium dioxide pneumoconiosis accompanied by lung cancer is reported. The patient was a fifty-three-year-old male, who was engaged in packing titanium dioxide for about thirteen years. At autopsy, a papillary adenocarcinoma was located in the right lung. Titanium was diffusely deposited in the lung and was engulfed by macrophages in the interstitium and alveolar spaces. Slight fibrosis of the interstitium around bronchioles and vessels was noticed as an effect of titanium deposition.

Cimetidine prophylaxis for gastrointestinal bleeding in an intensive care unit.

The efficacy of cimetidine in the prevention of gastrointestinal haemorrhage in a general intensive care unit was evaluated in 221 patients by a placebo controlled double blind randomised study. Criteria for bleeding were (i) haematemesis or gastric aspirate greater than 50 ml fresh blood, (ii) melaena or fresh blood per rectum with an upper source verified by endoscopy if the gastric aspirate was clear, (iii) a fall in haemoglobin level greater than 2 g/dl in a 24 hour period associated with either 4+ occult blood in stools, or coffee ground gastric drainage of at least 100 ml. The drug and placebo groups were similar for age, sex, duration of study and risk factors. One hundred and fourteen received cimetidine and 107 placebo. Only 8% of the patients bled with no significant difference between the two groups (6/114 cimetidine, 11/107 placebo; p = 0.16). There was no correlation between the frequency of bleeding and either the number of risk factors per patient or the duration of intensive care unit stay. Thirteen patients died in each study group, resulting in overall mortality of 12%. The low incidence of haemorrhage, the lack of statistical benefit from cimetidine and the similar mortality all argue against the routine use of this drug in intensive care unit patients.

[Fundamental study of imipenem/cilastatin sodium in obstetrics and gynecology].

Following results were obtained from drip intravenous administration of imipenem/cilastatin sodium (MK-0787/MK-0791) (500 mg/500 mg) by measuring concentration of MK-0787 in uterine arterial plasma, cubital venous plasma, oviduct, ovary and several sites in uterine tissue in cases of simple hysterectomy, and pelvic cavity fluid in cases of radical operation. Cervix uteri, portio vaginalis, myometrium showed higher concentration among various uterine tissues in any time after the end of administration. In cases of radical operation, the pelvic cavity fluid showed 6.6 approximately 7.8 micrograms/ml at 5 hours after the end of administration. In the field of obstetrics and gynecology, it was considered that MK-0787/MK-0791 has good efficacy in infections especially caused by Gram-positive aerobic bacteria.

Esophageal obstruction secondary to a mediastinal pancreatic pseudocyst.

The authors report a rare complication of pancreatic pseudocyst. A 47-year-old man suffered rapidly progressing dysphagia and complete esophageal obstruction secondary to extrinsic compression by mediastinal extension of a pancreatic pseudocyst. Ultrasonography and especially computerized tomography were instrumental in diagnosis and in the planning of the operation. A posterior cystogastrostomy successfully drained the pseudocyst and relieved the esophageal obstruction.

[A case of combined anomaly of horseshoe kidney, retrocaval ureter and pelviureteric stenosis].

A case of combined anomalies of the urinary tract, horseshoe kidney, retrocaval ureter and pelviureteric stenosis, is presented. Routine urological examination of a three-year-old girl who complained of recurrent cystitis, showed horseshoe kidney and right hydronephrosis. Surgical exploration revealed the retrocaval ureter, and the isthmus of the horseshoe kidney lying between the aorta and the inferior vena cava. The pelviureteric junction itself was also stenotic and compressed from behind by the aberrant vessels. In addition to the dismembered pyeloplasty in front of the vena cava, isthmotomy was performed. The literatures on such combined anomalies were reviewed.

Surgical principle in total artificial heart--an experimental review.

Based on implantation experiments in calves resulted some experiences for the implantation technic of artificial hearts. After resection of the natural ventricles and of the valvular parts of the aorta and pulmonary artery the connectors should be adapted and implanted. The first connected left ventricle get a caudal-lateral traction especially under supervision of the pulmonary veins and optimal development of the aorta. During connection of the right ventricle it is necessary to check up the inflow - and outflow tract situation. May be, the left ventricle get with this maneuver an unfavourable caudal kinking. Careful preparation and splitting of the pericardium is necessary to avoid obstructions for the venous flow. The closing of the chest and initial changes in position of the animal may alternate good positions of the heart, if the resection as well as preparation, adaptation and bedding don't remarked the discussed principles. One of the most relevant aspects in achieving the consistent survival of an animal undergoing the experimental implantation of a total artificial heart is obviously related to the correct anatomic placement of prosthetic ventricles inside the chest. In this case, the skillful surgical procedures based on an established surgical principle are considered to be attributable in obtaining the optimal hemodynamic situation. Description of the surgical procedures has been made elsewhere. However, it has been difficult to find a well defined surgical principle although particular milestone has already been made by a clinical application of the total artificial heart in 1982. Although the presently available prosthetic ventricles generally fit better in the chest, the problems in design and size of the prosthesis still exist.(ABSTRACT TRUNCATED AT 250 WORDS)

Time dependent effects of selenium on cadmium-induced acute mouse testicular damage.

Twenty-four hr after a single subcutaneous injection of Cd (0.024 mmol/kg), the testis showed severe and widespread degeneration. The damage was accompanied by an increase of lipoperoxide and a decrease of glutathione in the testis. Se (0.048 mmol/kg) injections 2 hr before, at the same time, and 1 hr after the Cd injection, decreased the damage. In these groups, the level of glutathione and lipoperoxides was restored to the control level. With a Se injection 6 hr after the Cd injection, the preventive effect of Se was no longer found. In the testis in which Se injection alleviated the damage caused by Cd, the changes in Ca, Mg and Zn were also restored to the control level. Uptake of Cd into the testis was stimulated by the Se injection. However, the uptake still remained at a low level in the testis in which the Se preventive effect was not found. These results suggest that the preventive effect of Se against testicular Cd toxicity is dependent on the interval between Cd and Se injections.

Relationship between movements of the head and the center of gravity of the body in Menière's disease.

The body sway of the patients with unilateral and bilateral Ménière's disease was recorded by using a cephalograph and a stabilometer, and analyzed with a microcomputer. The results show that the locus type, locus area and locus length traced by the head's center and the body's center of gravity are useful parameters of the body sway, representing the characteristics of peripheral vestibular disorders. The noteworthy characteristics are shown in the right-and-left type and forward-and-backward type in statokinesigram, in the significant differences of locus area and/or locus length between the head movement and the movement of the center of gravity, and the change of the averaged divisional frequencies of the body sway with eye-closing.

On fluctuation of hearing in otitis media with effusion after myringotomy.

Otitis media with effusion (OME) is known to occur in school or preschool children. Its chief symptom is hearing disturbance without otalgia. One of its effective therapies is myringotomy and suction of secretion out of its morbid tympanic cavities. This improvement differs with frequencies. The purpose of this paper is to present the mechanism of hearing recovery after myringotomy. The mean audiogram of OME showed a flat shape with 0 dB of bone conduction and 37.5 dB of air conduction. Hearing level after myringotomy recovered at high tones, but not recovered at low ones. One week later, the mean audiogram without perforations of eardrums showed hearing improvement at low tones. These findings suggested that slower recovery at low tones might be due to incision of an eardrum and closure of its perforation. To confirm this hypothesis we performed an experiment, by which the vibratory mechanism of a thin membrane was disclosed. The result showed that the vibratory modality of a membrane was influenced by incision of it. In conclusion, it is considered that though myringotomy of OME gets hearing improvement, it could result in hearing impairment, probably due to the vibratory modality of an eardrum.

Elastofibroma of the greater omentum.

A rare case of elastofibroma developing in the greater omentum of a 76 years old woman was reported. The greater omentum was thickened in a plate-like shape to a size of 24 X 15 X 2 cm, and a firm mass as large as 5 X 4 X 3 cm was formed in a portion of the omentum. Granulomatous inflammation associated with the calcification was present in the center region of the mass. Collagen fibers and elastofibroma fibers were markedly proliferated around the granuloma. Fibrosis accompanied with the formation of elastofibroma fibers was observed not only around the granulomatous inflammatory focus but also throughout the thickened omentum. The elastofibroma fiber in this case lost its stainability to resorcin-fuchsin and orcein after 4 hours treatment with elastase. It may be inferred that not only mechanical stresses but also inflammatory stimuli contribute to the development of elastofibroma.

[Studies on coccidiosis in guinea pigs. 2. Epizootiological survey].

An epizootiological survey has been carried out on naturally occurring coccidiosis in Hartley guinea pigs (weight, 250g) purchased by the National Institute of Health, Tokyo during the period 1964 to 1982. Coccidial infections in breeding colonies of guinea pigs were observed very frequently in weaned animals but scarcely in adult and suckling animals. Oocysts of Eimeria caviae were detected in 53.8% of the 7,162 fecal samples collected from transportation boxes and coccidiosis occurred in 39% of the 1,461 dead or culled animals obtained during the routine one week quarantine period. In the period 1964 to 1971, particularly high rates of prevalence of oocysts, between 55-86%, and incidence of coccidiosis, between 55-76%, were observed. These rates were clearly reduced in the period 1972 to 1982, with a lower rate of isolation of oocysts ranging from 14-48% and les than 20% incidence of coccidiosis (except in 1981 and 1982). The monthly fluctuation of occurrence rates of oocysts and clinical coccidiosis differed over the period of study. From 1964 to 1971, the high prevalence of oocysts was consistently observed accompanied by a bimodal pattern of incidence of coccidiosis in April (85%) and October (78%). In the period 1972 to 1982, both parameters showed a single peak, for prevalence of oocysts in June (60.7%) and for incidence of coccidiosis in May (45%). Oocysts in feces disappeared in February and March and coccidiosis occurred irregularly in 1981 and 1982.(ABSTRACT TRUNCATED AT 250 WORDS)

[Occurrence of sick animals in conventional guinea pigs obtained from commercial breeders during 1964 to 1982].

A quarantine was performed on conventional Hartley guinea pigs free from Streptococcus zooepidemicus, Bordetella bronchiseptica and Salmonella spp., but infected with Eimeria caviae, which were purchased from twenty-five commercial breeders by the National Institute of Health. Physical examinations revealed an incidence of 0.88 per cent or 1,461 sick animals in 166, 050 guinea pigs quarantined during the period 1964 to 1982. The result obtained showed the following significant differences between the periods 1964 to 1971 and 1972 to 1982: Annual incidence of sick animals in the period 1972 to 1982 increased twice as many as 0.56 to 0.81 per cent during the 1964 to 1971, and monthly incidence showed bimodal occurrence at April and October in the former period but continual occurrence from November to April in the latter period. Major clinical signs in the former period were death and diarrhea, which occurred usually within a week after arrival of the guinea pigs at our institute and caused significant decrease of body weight, but those in the latter period were retarded growth and weakness which became detectable during 1-3 weeks after arrival of the animals. Discussions were made on possible reasons concerning the differences in incidence of sick animals during the two periods.
