Even nowadays, early gastric carcinoma of the cardiac portion still presents numerous diagnostic and pathological problems. Fifteen cases of early cancer of the cardiac portion were resected in the past 16 years. In order to clarify their characteristics, we carried out a comparison between the cardiac portion and the upper 3rd portion. The results were as follows: 1) There were cases of IIc type on the gross appearance of the cardiac portion. 2) As for the histological type, here were 14 cases of the differentiated type. on the other hand, there were 40% of the undifferentiated type of the upper 3rd portion. 3) Ten cases of early carcinoma of the cardiac portion had the lesion in the cardiac zone, while most of the early cancer of the upper 3rd portion had a strong connection with the fundic cone.
Nineteen patients with solitary minute carcinoma (single cases) and 16 with foci of minute cancer, found as a component of multiple early gastric carcinoma (multiple cases) were studied. The results were as follows; 1) The histological type of the single cases was the differentiated type in 95%. 2) All lesions of the multiple cases were mucosal invasive carcinoma, while 16% of the single cases (3/19) had submucosal invasive carcinoma. 3) As for the distribution of the single and multiple cases according to the surrounding mucosal atrophy, the single eases were in the pyloric gland zone, the multiple cases in the intermediate zone.

Evidence was obtained for countercurrent diffusion of 14C-n-butanol in the rabbit renal cortex. After injection into the renal artery of perfused, isolated rabbit kidneys, concentrations of both 14C-butanol and tritiated water in the initial samples collected from the renal venous outflow exceeded those of 125I-albumin when the rate of perfusion was low. When flow was increased, the 125I-albumin curve was shifted to earlier times relative to the other labels. These observations confirm the existence of arteriovenous diffusion of 14C-butanol and tritiated water in the kidney. In a second set of experiments, 14C-butanol and tritiated water were infused for 10 or 60 seconds into the renal arteries of anesthetized rabbits, and the kidneys were then removed and frozen in liquid nitrogen. Cores were cut from the kidney surface and sectioned in a cryostat, and the ratio of 14C-butanol to tritiated water was calculated at increasing depths in the cortex. This ratio rose from 0.54 +/- 0.03 (SEM) at the surface to 0.98 +/- 0.07 at 3 mm beneath the surface in kidneys perfused for 10 seconds. This gradient was less steep after 60 seconds of perfusion. The early appearance of 14C-butanol relative to tritiated water in the renal venous outflow and delayed equilibration of 14C-butanol in the outer renal cortical tissue are consistent with counter-current diffusion. It is suggested that this exchange may occur between adjoining interlobular arteries and veins in the cortex and may contribute to high carbon dioxide tensions found near the renal surface.

Our new method for the graphical analysis of drug effects in the dog heart-lung preparation was extended to pulmonary circulation. The equilibrium points, at which the cardiac output (CO) curve and venous return (VR) curve cross each other in the right atrial pressure (RAP)-CO and left atrial pressure (LAP)-CO relations, were directly recorded on two X-Y recorders. The VR curves were obtained by inducing cardiac fibrillation and simultaneously occluding the pulmonary arterial trunk. The competence test, which was utilized previously for recording the CO curve in the RAP-CO relation (Ishikawa et al., 1978), was confirmed to be a good procedure for the CO curve in the LAP-CO relation. During the competence test, the mean pulmonary pressure (Pmp) which is the intercept of the pulmonary VR curve on the LAP axis in the LAP-CO relation was changed as much as the change in the systemic reservoir blood level, with little change in the slope of the pulmonary VR curve. When the reservoir blood level was 100 mm above the superior vena cava and the aortic pressure was 70 mmHg (control), the Pmp value was 136 +/- 7 mmH2O (n = 24). The slope of the pulmonary VR curve was not so different from that of the systemic VR curve in RAP-CO relation. Raising aortic pressure to 100 mmHg caused a shift of the equilibrium point to the right and slightly downwards, increased the Pmp value by 42.9 +/- 9.9 mmH2O (delta Pmp) and decreased the reservoir blood volume by 26.2 +/- 6.4 ml (delta V). The ratio delta V/delta Pmp was 0.66 +/- 0.11 ml/mmH2O. The continuous infusion of norepinephrine 4 micrograms/min caused a shift of the equilibrium point to the left and upwards, decreased the Pmp value by 67.1 +/- 19.1 mmH2O and increased the reservoir blood volume by 35.3 +/- 7.2 ml. The ratio delta V/delta Pmp was 0.56 +/- 0.16 ml/mmH2O. The continuous infusion of 5-hydroxytryptamine at the rate of 60 micrograms/min caused a shift of the equilibrium point almost vertically and downwards, increased Pmp value by 34.8 +/- 3.2 mmH2O and decreased the reservoir blood volume by 45.8 +/- 10.1 ml. The ratio delta V/delta Pmp was 0.88 +/- 0.10 ml/mmH2O.

Changes in RCA activity are known to affect the acid-base balance, but a clear relationship between RCA activity and acid-base balance has not yet been established. The relationship between RCA activity and acid-base balance was studied in tonometry with 4 mixed gases for 6-9 hr, in healthy adults at rest, in halothane anesthesia and in hypothermia. RCA activity increases when pH and HCO3-/PCO2 ratio increase and PCO2 decreases in vitro. It also increases when pH and HCO3-/PCO2 ratio decrease and the range of PCO2 broadens in vivo. The relationship between RCA activity and the HCO3-/PCO2 ratio is a fan shape, because a change of the HCO3-/PCO2 ratio precedes the change of RCA activity. Some factor which increases the RCA activity may be produced secondarily in the body under the influence of high PCO2. Probably the acid-base balance is corrected by reduction of the RCA activity in the case of alkalosis, and the acid-base balance may be corrected by an increase in RCA activity in the case of acidosis.

The effect of oral administration of bacteriolytic enzymes and enzymatically digested bacterial cell walls on immunostimulation in guinea pigs was studied. Guinea pigs were given lysozyme or pronase or both orally for a period of 8 days, and on day 7 they were primed with hepatitis B surface antigen. Circulating antibody titers to the antigen in the enzyme-treated group were significantly higher (four to six times, P less than 0.05) than those in nontreated control groups on day 16 after immunization. Stimulation of cellular immunity in the group receiving both lysozyme and pronase simultaneously was significantly increased compared with the group receiving only one of them. The humoral immune response was enhanced by oral administration of enzymatically digested cell walls isolated from Bifidobacterium longum. The result suggested that intestinal bacteria might be solubilized by oral administration of bacteriolytic enzymes and that the absorbable fragment of peptidoglycan released from the bacterial cell walls might be responsible for the enhanced host immune responses.
