We investigated the effect of a new Ca-antagonist, TA-3090 on supraventricular tachycardia (SVT) and conduction system, comparing it with the effect of Diltiazem Hydrochloride, in 11 patients who had paroxysmal SVT attacks. Seven of the 11 patients presented atrioventricular reentrant tachycardia (AVRT) via retrograde concealed conduction through an accessory pathway, and the others presented AV nodal reentrant tachycardia (AVNRT). After SVT was induced by means of programmed electrical stimulation at high right atrium, TA-3090 (0.1 mg/kg body weight) or Diltiazem (0.2 mg/kg) was administered intravenously for 3 minutes. TA-3090 terminated nine of 11 SVTs, while Diltiazem terminated four of 4 SVTs. On termination of SVT, both drugs interrupted A-H conduction during AVRT and the slow pathway during AVNRT. After TA injection, five of 11 SVTs could not be induced by programmed electrical stimulation, while two of 4 SVTs could not be induced after Diltiazem. In AVRT, three patients in which TA-3090 prevented SVT induction had a longer AV node effective refractory period than that of the others in which TA-3090 could not prevent SVT (330 +/- 46 vs 210 +/- 24 msec, p less than 0.01). However, SVT was induced more easily than before in three of the 11 patients treated with TA-3090 administration, and in one of the 4 patients treated with Diltiazem administration.(ABSTRACT TRUNCATED AT 250 WORDS)
The possibility that pretreatment with LPS (lipopolysaccharide obtained from Escherichia coli), an immune system stimulant and interferon inducer, could prevent the hepatotoxic effects of acetaminophen (NAPAP) was investigated in mice. When mice were pretreated with LPS (4 mg/kg), intraperitoneally for 24 hr the mortality caused by NAPAP was considerably reduced. Histological examination of the livers and leakage of the enzymes into the blood demonstrated that NAPAP-induced necrosis was decreased in LPS-treated mice compared to that induced by NAPAP alone. Pretreatment with 400 mg/kg or 800 mg/kg of NAPAP decreased the amount of covalently-bound acetaminophen metabolites. Since the level of hepatic glutathione and microsomal cytochrome P-450 were depressed in these experiments, it is concluded that LPS depresses the cytochrome P-450 species responsible for the formation of the toxic metabolites and that less reactive species are available for binding to cell macromolecules.
To improve the disintegration test for vaginal tablets described in the British Pharmacopoeia (BP), a monitoring apparatus was added, and tested with seven effervescent and five non-effervescent tablets. A tablet was placed on the metal disc of the BP disintegration apparatus, and the guided plate was placed on the tablet. The guided plate moved downward smoothly in a cylinder as the tablet disintegrated. The movement was recorded by using a kymograph. The end-point and process of disintegration of the tablet were automatically recorded and the results obtained suggest that the modified test is a useful tool for the quality control of vaginal tablets.
Attempts were made to investigate the disintegration test for vaginal tablets. Disintegration tests were done for four different commercial vaginal tablets (three lots each) by the watch glass method and Japanese Pharmacopoeia (JP) disintegration method, and the resulting profiles were compared to those by the modified British Pharmacopoeia (BP) method on a point of lot-to-lot variation of the disintegration times. The disintegration time of every tablet by the modified BP method was longest, followed by the watch glass method, and finally by the JP disintegration method. The results for lot-to-lot differences in disintegration times by the modified BP method were similar to those by the watch glass method. However, such lot-to-lot differences as found by the modified BP method and watch glass method were not always observed by the JP disintegration method. It was concluded from these results that the modified BP method was most suitable for investigating lot-to-lot differences in the disintegration of vaginal tablets.
A preliminary trial was conducted to determine if endotoxin would prevent acetaminophen (APAP)-induced hepatic toxicity. When mice were pretreated with endotoxin (1, 2 and 4 mg/kg, i.p.), the clinical chemistry profile and pathological alterations caused by APAP were reduced with a dose dependent manner. The efficacy of endotoxin in preventing APAP-induced hepatic necrosis should be further studied.
Amounts of particulate matter present in 17 small volume antibiotic injections marketed in Japan were determined using light obscuration particle analyzer (HIAC). The vial volume range of each batch of product was 7-20 ml, and each vial contained 1 g as antibiotic potency. In 4 products, contents of particles between 2.5 and 10 microns in diameter were counted 2,000-7,000 per vial, and particles in other products were counted less than 2,000 per vial. Numbers of particles greater than or equal to 10 microns in diameter were much less than the USP XXI criteria for particulate matter in small volume injections. The product of the highest counts for particles between 10 and 25 microns in diameter showed counts amounted to 0.13% of the USP XXI criteria. In the 25-50 microns particulate diameter range, particulate matters were detected only in 2 products, and they were less than 0.2% of the USP XXI criteria. Particles over 50 microns in diameter were not detected in any products. These results showed that 17 small volume antibiotic injections marketed in Japan had good qualities regarding contents of particulate matter.
The aim of the present investigation was to determine the effects of a single dose and serial doses of nonanoyl vanillylamide (NVA) administered subcutaneously on responses induced by chemical stimuli in the mouse. The treatment with a single subcutaneous injection produced a significant decrease in the number of abdominal contraction responses produced by intraperitoneal injections of phenylbenzoquinone and in the responsive time of licking induced by plantar injection of formalin. The serial subcutaneous administration of NVA caused a dose related antinociceptive effect against a chemical nociceptive stimulus using the phenylbenzoquinone-induced writhing test, while NVA was ineffective against a chemical stimulus, formalin. These results suggest that serial doses of NVA administered peripherally cause different effects in two types of chemically-induced nociception.

Histopathological, immunocytological and ultrastructural observations are reported in the first case of pagetoid reticulosis (Woringer-Kolopp disease) in Japan. The patient was a 61-year-old woman with multiple skin lesions running a chronic and apparently benign clinical course. Histology of the skin biopsies revealed typical pagetoid appearance of the epidermis due to intraepidermal infiltration of abnormal cells. Ultrastructural investigation showed that the intraepidermal abnormal cells were classified into mycosis fungoides cells, Sézary cells, lymphoblast-like cells, and large blastoid cells and that the mycosis fungoides cells were a major cell population. Intermediate or transitional cells were found between these cells and large blastoid cells were mostly situated in the basal cell layer. By the rosetting assays of the free cell suspensions prepared from the epidermis of the biopsied skin lesions, 93% of the suspended cells were positive for spontaneous rosette formation with sheep erythrocytes. The immunoperoxidase technique demonstrated no cytoplasmic immunoglobulins in almost all the intraepidermal abnormal cells. These results indicate that the intraepidermal abnormal cells are T-lymphocytes. Thus, it is concluded that the present case is a cutaneous T-cell lymphoma of low-grade malignancy showing a prominent epidermotropism. This case is the first description of the disease in Japan.
Change in external appearance, pH values and residual antimicrobial potency of cefotiam dihydrochloride (CTM) upon combination with infusion solutions and other injections was investigated. The combinations of CTM with bromhexine hydrochloride 2 mg/ml, 2 ml, dipyridamole 5 mg/ml, 2 ml and gabexate mesilate 100 mg/vial respectively were found to be incompatible on account of turbidity produced. As for 34 other combinations, no change in their appearance, pH and potency was observed.
