[Chronic progressive polycythemia and thrombocytosis].

The 73-year-old female patient presented with chronic progressive erythrocytosis diagnosed 5 years ago which now is accompanied by thrombocytosis. She complains about generalised itching, pain in distal limbs and shows marked plethora. Having excluded major causes of secondary erythrocytosis the diagnosis was polycythaemia vera with a typical constellation of symptoms, findings and course of disease. Under treatment with 5-Hydroxyurea and repeated phlebotomy thrombocyte count and haematocrit normalised and the patient recovered her usual vitality. Limb pain, itching and plethora disappeared.

[Extra-articular localization of nodular synovitis: a case report].

INTRODUCTION: The extra-articular nodular synovitis is regarded as a rare case of the common pigmented villonodular synovitis. The following report points out a case with wide extension in a young man's proximal lower leg. CASE-REPORT: A 24-year-old man complained about a non-painful tumor at his left proximal lower leg without any clinical handicap. Some years before he got a fracture of the proximal left tibia caused by an adequate sports trauma. So he had to undergo two operative investigations. The radiographs showed a transparent soft tissue tumor, which displaced the corticalis of the bone for some centimeters. The magnetic resonance imaging did not hint at a malignant process. After operative treatment the diagnosis was confirmed by histology. CONCLUSION: By operative and histological investigation advice could given to a benign tumor. The etiology of the extra-articular nodular synovitis is to be regarded as unknown. In our case a dislocation of synovial cells to extra-articular should be discussed.

[Intermittent bile duct obstruction as the first manifestation of a hepatocellular carcinoma].

HISTORY AND CLINICAL FINDINGS: For two months a 73-year-old woman had been suffering from intermittent colicky upper abdominal pain. She had also noted transient colourless stools and finally jaundice. Suspected cholelithiasis led to her admission for cholecystectomy. She had slight jaundice of skin and sclerae. Physical findings were unremarkable except for pain on epigastric palpation. INVESTIGATIONS: Sonography and computed tomography showed dilatation of the intrahepatic biliary passages and the proximal common bile duct, the latter containing a polycyclic mass that on endoscopic retrograde cholangiography appeared ovular and was surrounded by contrast medium. It was thought to be an intraductal bile stone. TREATMENT AND COURSE: Laparotomy, performed to remove the suspected stone, revealed a floating intraductal tumour, histologically a hepatocellular carcinoma. Further imaging discovered the primary hepatic tumour. CONCLUSION: Although a smooth-walled contrast-sparing mass in the common bile duct usually denotes a stone, other structures may rarely imitate this picture, in particular atypical tumour growth.

[Serous psammocarcinoma of the ovary. Case report and literature review].

We report a rare case of psammocarcinoma which presented as a mass in the lower abdomen of an otherwise asymptomatic woman. Ultrasonographically, the tumor showed extensive calcification and was suspected to be a myoma. DNA image cytometry and assessment of proliferative activity were performed and showed an euploid, low proliferating tumor cell population. This underlines the low malignancy of this type of carcinoma, which is comparable to that of serous borderline lesions of the ovary and explains the benign clinical course.

Hemangiopericytoma of the inner canthus.

PURPOSE: A 31-year-old woman with a mass at the left inner canthus was referred to our hospital. METHODS: Clinical examination failed to establish a diagnosis and the patient underwent an incisional biopsy. RESULTS: Histopathologic examination disclosed a hemangiopericytoma. CONCLUSIONS: Because the behavior of a hemangiopericytoma is unpredictable, this tumor should be included when making a differential diagnosis in lesions of the inner canthus.

[Early pulmonary changes caused by amiodarone].

Confluent infiltrates and interstitial condensates with (sub)segmentary spreading are typical radiological findings in amiodarone pneumopathy. A case is presented where daily amounts of 200-600 mg and a total of 18.3 g amiodarone led to alveolitis with fibrosis within 4 weeks, which is uncommon in the early stage.

[A case of AIDS-associated histoplasmosis in Germany].

In a thirty-year-old patient with AIDS the diagnosis of disseminated histoplasmosis was established via biopsy and culture. The patient had grown up in Argentina, where histoplasmosis is endemic. He had not been in an endemic region during the last two years anteceding the manifestation of systemic histoplasmosis. Accordingly, in patients with a progressive immunodeficiency syndrome, reactivation of a former (possibly inapparent) infection with Histoplasma capsulatum must be considered. Therapy with Amphotericin B lead to a remarkable improvement of clinical, laboratory and sonographic findings. Due to the fact that total eradication of H. capsulatum from the infected host cannot be achieved with any known drug regimen, a life-long follow-up therapy was begun. The patient showed no signs of relapse after a follow-up of 7 months.

[Epidemiological and pathomorphological autopsy findings in AIDS].

The autopsy findings in 41 AIDS patients (1982-1991), including a rather large group of 8 patients without one of the classical risk factors for HIV, showed a much higher rate of opportunistic infections, especially CMV and fungal infections, as well as Kaposi's sarcoma in homosexuals than in the others. There was no decline in the incidence of Kaposi's sarcoma over the last few years. The bone marrow findings, which presented a remarkable high cellularity in these patients, well correlated with the so-called HIV-myelopathy, which is highly suggestive to represent a direct HIV-related damage of the bone marrow.

Acquired Immunodeficiency Syndrome↗

[Fatigue, exertional dyspnea, unsteady gait].

Exertional dyspnea and fatigue dominated the clinical picture of pernicious anemia in a 78 year old man. A disturbed gait indicated concomitant neurologic disorders. A reduced Vitamin B12 level, an abnormal Schillingtest, atrophy of gastric mucosa as well as maturation disturbance of erythrocytes and neutrophils were present as typical signs of the disease. Administration of Cyanocobalamine led to correction of blood values within three weeks.

[Angiofollicular lymphatic hyperplasia with plasmacytoma and polyneuropathy: a case report with immunohistochemical study].

A rare case of a 43-year-old man with polyneuropathy, monoclonal gammopathy, myeloma and Castleman's disease of plasma cell type was diagnosed. Immunohistological examination of the lymph node with Castleman's disease showed polyclonal binding while the myeloma was monoclonal containing IgA with lambda-light chains. At the immunoelectrophoresis IgA/IgA paraprotein was demonstrated. The possible pathogenetical relationship between polyneuropathy and plasmacytoma as well as Castleman's disease will be discussed.

Immunohistochemical localisation of alpha-fetoprotein (AFP) in germ cell tumours: evidence for AFP production by tissues different from endodermal sinus tumour.

20 germ cell tumours have been studied with respect to the presence of alpha-fetoprotein (AFP), using the peroxidase-antiperoxidase (PAP) technique. 6 out of 20 tumours contained elements of endodermal sinus tumour (EST) and were AFP positive. 16 tumours were diagnosed either as pure embryonal carcinomas (6) or as mixed germ cell tumours, containing elements of embryonal carcinoma (10). In 3 of these 16 tumours AFP was localised definitely in the embryonal carcinoma cells; in an additional 6, AFP was also detected but it could not be decided whether AFP was present in embryonal carcinoma cells or in EST cells during early differentiation. In 2 of 7 immature teratomas, AFP was shown to be present in cylindric epithelia. All seminomas (4) studied were AFP-negative. These results show that AFP, which occurs regularly in EST, may also be present in embryonal carcinomas as well as in immature teratomas. Thus, it seems that the immunohistochemical demonstration of AFP by the PAP technique is a suitable method of identifying late stages of embryonal carcinoma or early stages of endodermal sinus tumour during the process of differentiation.

[Morphology of late hepatic lesions after thorotrast (author's transl)].

Late lesions of the liver after longstanding exposure to thorotrast are the following: adaptive and degenerative alterations of hepatocytes, proliferation and atypical morphology of sinusoidal cells, ectasia of sinusoids, portal fibrosis due to inflammatory as well as non-inflammatory changes with transition to complete cirrhosis. Pronounced changes of sinusoidal cells and sinusoidal shape occur most frequently in the immediate vicinity of angiosarcomas. Just as in chronic vinyl chlorid intoxication these changes may represent an early stage of angiosarcoma, or may be due to the fact, that an angiosarcoma is close to the hepatic area, where the biopsy specimen was taken from. However, liver tissue surrounding carcinoma induced by thorotrast usually shows only noncharacteristic alterations, except in cases, where cirrhosis of the liver is present.
