Multimodal intervention benefits: Responder analysis of J-MINT PRIME Kanagawa trial.

INTRODUCTION: The J-MINT PRIME Kanagawa trial was an 18-month multimodal intervention (incorporating exercise, nutrition, and metabolic management) for dementia prevention. Because the primary analysis showed no significant benefits, we performed an exploratory responder analysis to identify responsive subpopulations. METHODS: We analyzed the Full Analysis Set comprising 188 participants. Classification and regression tree (CART) analysis, applied to the intervention arm, identified baseline predictors of cognitive improvement. These rules were then applied to the entire cohort to evaluate treatment effects on the Mini-Mental State Examination (MMSE) using fully adjusted mixed-effects models for repeated measures (MMRM). RESULTS: CART identified a "Target Group" (N = 108) characterized by baseline profiles such as an MMSE score < 28 or specific metabolic ranges (e.g., LDL-C < 135 mg/dL). Within this target group, the intervention significantly preserved MMSE trajectories compared with the control group (group &#xd7; time interaction, P = 0.022). In contrast, the Non-Target Group (N = 80), consisting of high-functioning individuals (MMSE &#x2265; 28), exhibited no significant group &#xd7; time interaction. DISCUSSION: Multimodal interventions may effectively preserve global cognition in older adults with sub-threshold cognitive decline. Careful targeting of appropriate populations, while considering potential longitudinal measurement artifacts (e.g., practice effects), is essential. These findings provide a hypothesis-generating framework that warrants external validation in future prevention trials.

Polymorphism of methylenetetrahydrofolate reductase gene (C677T MTHFR) is not a confounding factor of the relationship between serum uric acid level and the prevalence of hypertension in Japanese men.

BACKGROUND: The association between serum uric acid (UA) and the prevalence of hypertension, and the relationship between methylenetetrahydrofolate reductase (MTHFR) polymorphism and hypertension remains unclear. The aim of the present study was to investigate whether the C677T MTHFR mutation genotype (VV) is independently associated with the prevalence of hypertension or blood pressure (BP), and examined any interaction of MTHFR and UA with BP. METHODS AND RESULTS: Participants were randomly selected from all residents (aged 40-69 years) in a rural county of Japan, and the data for the men (n=335) were analyzed. ;Hypertension' was defined as systolic BP >or=140 and/or diastolic BP >or=90 mmHg and/or being administered antihypertensive medication. Serum UA level was independently associated with the prevalence of hypertension (odds ratio (95% confidence interval) =2.7 (1.2-5.9), p=0.047) for the highest tertile of serum UA (>or=398.5 micromol/L (6.7 mg/dl)) vs that of the lowest tertile (<321.2 micromol/L (5.4 mg/dl)), but the MTHFR mutation was not independently associated with prevalence of hypertension or BP. No interaction of the MTHFR mutation and serum UA with BP was found. CONCLUSIONS: The mutation of C677T MTHFR was not independently associated with the prevalence of hypertension or BP levels although serum UA was. Furthermore, the relationship between serum UA and BP was not modulated by the MTHFR mutation in Japanese men.

Integrated electromyography and performance outcomes to inertial resistance exercise.

Integrated electromyography (IEMG) and performance outcomes from resistance exercise may be influenced by gender, repetition order, and contractile mode. A novel strength training apparatus employing inertial resistance operates unlike standard exercise equipment and may therefore evoke different IEMG and performance outcomes. Subjects performed 3-set, 10-repetition calf press workouts on an inertial device while IEMG and flywheel velocity were recorded. Dependent variables were examined with 2 (men, women) x 2 (first vs. 10th repetition) x 2 (concentric, eccentric) analyses of variance. Performance outcomes showed gender-by-repetition and gender-by-contractile-mode interactions, with men's 10th-repetition and men's concentric data, respectively, causing these effects. Medial gastrocnemius (MG) IEMG showed a gender-by-repetition interaction resulting from men's first-repetition data. Greater 10th-repetition performance despite higher first-repetition MG IEMG may result from heightened triceps surae elastic energy utilization and/or maintained cross-bridges during transitions from lengthening-to-shortening actions. Inertial strength training may improve performance outcomes without additional motor unit recruitment.

Phylogenetic investigation of Dogiel's pericellular nests and Cajal's initial glomeruli in the dorsal root ganglion.

Cajal's initial glomeruli (IG) and Dogiel's pericellular nests (PCNs) were first described from methylene blue preparations of healthy animal tissues around the beginning of the last century. Since that time, although many reports have been published concerning these structures, few have focused on their development and phylogeny in healthy animals. The aim of this study was to examine the phylogenetic development of the sensory neurons in Cajal's IG (also called axonal glomeruli) and Dogiel's PCNs in the dorsal root ganglion (DRG) of the healthy adult frog, chick, rat, and rabbit. The three-dimensional architecture of the neurons was observed in ganglia by scanning electron microscopy after removal of the connective tissue. The neurons in the DRG of fish are known to be bipolar, but DRG neurons in the species examined here were found to be pseudounipolar, with single stem processes. The proportion of neurons having IG or PCNs increased with increasing phylogenetic complexity in the species examined here. Cajal's initial glomeruli, the convolution of the stem process near the parent cell body: In frogs, the ganglia were small and the neuronal stem processes were very short and straight. In chicks, the stem processes were longer; sometimes very long, tortuous processes were observed. However, no neurons with typical IG were observed in either species. Typical IG were observed in rats and rabbits; their occurrence was much more frequent in rabbits. Pseudounipolarization, i.e., the transition from bipolar to pseudounipolar neurons, is thought to save space, limit the length of neuronal processes, and reduce conduction time. However, an explanation of the evolutionary advantage of the IG, which is formed by the excessive prolongation of the stem process, remains elusive. The cytological and electrophysiological importance of IG has been discussed. Dogiel's pericellular nests (PCNs), which resemble balls of yarn made of thin unmyelinated nerve fibers around DRG neurons, have been observed in the DRG of rats and rabbits, but not in frogs or chicks. This interesting structure shows not only ontogenetic development in healthy animals but also phylogenetic development among species. The nerve fibers in the PCNs were less than 1.2 mum in diameter and had some varicosities. An immunohistochemical study using anti-tyrosine hydroxylase (TH) antibody revealed that some PCNs contain TH-positive nerve fibers and varicosities. Such TH-positive PCNs disappear after sympathectomy. These results suggest that the PCNs are made up of autonomic nerve fibers.

Changes in expression of prosaposin in the rat facial nerve nucleus after facial nerve transection.

Prosaposin is the precursor of saposins A, B, C and D, which are activators of sphingolipid hydrolases. In addition, unprocessed prosaposin functions as a neurotrophic factor in the central and peripheral nervous systems by acting to prevent neuronal apoptosis, to elongate neurites and to facilitate myelination. In this study, the expression pattern of prosaposin in the facial nerve nucleus after facial nerve transection was examined by immunohistochemistry and in situ hybridization. Prosaposin immunoreactivity in the neurons on the operated side facial nerve nucleus showed a biphasic pattern: it was significantly increased on day 3 after transection, decreased dramatically on day 7, started to increase gradually on day 14 and reached another peak on day 21 after transection. Significant increases in the levels of prosaposin mRNA were identified in the neurons on the operated side, suggesting that prosaposin was synthesized vigorously by the neurons themselves in the case of facial nerve transection. The diverse changes in prosaposin immunoreactivity during the process of facial nerve regeneration may reflect the diverse neurotrophic activities of prosaposin in facial motoneurons.

The 2.3 genotype of ESRRA23 of the ERR alpha gene is associated with a higher BMI than the 2.2 genotype.

ERR alpha (NR3B1) is an orphan nuclear receptor believed to be involved in energy metabolism and obesity. A 23-base pair sequence in the 5'-flanking region of the ERR alpha gene, referred to as ESRRA23, is polymorphic in human chromosomes. Here, we investigated the influence of the ESRRA23 polymorphism on obesity in 703 Japanese individuals by analyzing indices of obesity and related lifestyle factors. The frequency of genotypes was similar to that reported in whites. The 2.3-genotype was associated with a significantly higher BMI than was the 2.2-genotype. An increased number of ESRRA23 repeats may be a genetic factor in human obesity.

Epidemiological evidence that acetaldehyde plays a significant role in the development of decreased serum folate concentration and elevated mean corpuscular volume in alcohol drinkers.

BACKGROUND: Elevated mean corpuscular volume (MCV) is a traditional biological marker for alcohol abuse and alcoholism, but the underlying mechanism is unclear. Three recent epidemiologic studies consistently showed that MCV was elevated by alcohol drinking more markedly among individuals with genetically inactive aldehyde dehydrogenase-2 (ALDH2) (encoded by ALDH2*2 mutant allele) than those with active ALDH2 (encoded by ALDH2*1/2*1 genotype), suggesting that the elevated MCV was etiologically linked to acetaldehyde exposure. The purpose of the present study was to clarify further this relationship by examining the status of folate and vitamin B12. METHODS: The study participants were 159 men who were aged 40 to 69 years and randomly selected from a Japanese rural population. The genetic polymorphism of ALDH2 was determined by PCR-restriction fragment length polymorphism method; data on alcohol drinking and other lifestyles were collected using a structured questionnaire; serum concentrations of folate and vitamin B12 were measured using the protein competitive reaction method, and blood cell counts were measured by routine methods. A multiple linear regression model was used to analyze the data. RESULTS: : The relationship between alcohol drinking and serum folate concentration was significantly different between ALDH2 genotypes, indicating that the reduction of serum folate by alcohol drinking was more marked in men with ALDH2*1/2*2 than those with ALDH2*1/2*1. The relationship between alcohol drinking and elevated MCV was significantly stronger in men with ALDH2*1/2*2 than those with ALDH2*1/2*1 even after adjustment for serum folate and vitamin B12 concentrations. CONCLUSIONS: These findings indicate that acetaldehyde plays a significant role in the development of decreased serum folate concentration and elevated MCV by alcohol drinking.

The relationships between plasma total homocysteine and selected atherosclerotic risk factors according to the C677T methylenetetrahydrofolate reductase gene in Japanese.

BACKGROUND: Variation of plasma total homocysteine (tHcy) levels among individuals is modified by 5,10-methylenetetrahydrofolate reductase (MTHFR) genotypes and selected atherosclerotic risk factors. DESIGN: Cross-sectional study. METHODS: Healthy Japanese men (n=213) and women (n=242) aged 40-69 years were examined. RESULTS: Serum uric acid, hemoglobin, hematocrit, serum creatinine, and men were independently significantly associated with plasma tHcy. The relationship between serum uric acid and plasma tHcy was the strongest in those with the mutant homozygote (VV), but the overall statistical test of interaction was not significant. CONCLUSIONS: Serum uric acid, hematocrit, hemoglobin, creatinine, and men were determinants of plasma tHcy levels. Further investigations are needed.

Transient increase of TUNEL-positive cells on postnatal day 20 in the developing rat olfactory bulb.

In the developing central nervous system, apoptosis plays an important role in the normal organization of the neuronal circuit. The timing of neurogenesis, proliferation, and migration of the neurons in the developing olfactory bulb (OB) is well studied; however, the involvement of apoptosis in this process is not fully understood. In this study, we examined the changes in the distribution and the number of apoptotic cells in the rat OB during embryonic and postnatal periods, by using terminal deoxynucleotidyl transferase-mediated dUTP-digoxigenin nick end-labeling (TUNEL) staining. Although the number of TUNEL-positive cells was relatively small during the embryonic period, it gradually increased after birth, and peaked on postnatal day 20 with statistical significance, especially in the granule cell layer of the main OB. This transient increase of TUNEL-positive cells on postnatal day 20 may be involved in a critical event during maturation of the OB.

Process formation of the renal glomerular podocyte: is there common molecular machinery for processes of podocytes and neurons?

The renal glomerular podocyte exhibits a highly arborized morphology. In comparison with the neuron, which is the best studied process-bearing cell, the podocyte major processes share many cell biological characteristics with neuronal dendrites. Both podocytes and neurons develop microtubule-based thick processes with branching morphology and both have thin actin-based projections (i.e. podocyte foot processes and dendritic spines). Formation of podocyte processes and neuronal dendrites depends on the assembly of microtubules. Because the assembly of microtubules is regulated by phosphorylation of microtubule-associated proteins, inhibition of protein phosphatases abolishes and inhibition of protein kinases promotes process formation. Podocytes and dendrites also share the machinery of intracellular traffic of membranous vesicles, as well as cytoskeletal elements, which is indispensable for the elongation of these processes. Furthermore, these two cell types share expression of various molecules working for signal transduction, transmembranous transport and intercellular contacts. Such common gene expression implies a similar transcriptional regulation in these cells. Concerning the formation of podocyte foot processes and dendritic branches, actin filaments are thought to play a central role in orchestrating the function of various molecules and the regulation of actin assembly is necessary to establish and maintain such sophisticated cellular architecture. The molecular mechanism of foot process formation seems to include Rho family small GTP-binding proteins, which are known to be responsible for the establishment of dendritic branching morphology.

Rho-ROCK signal pathway regulates microtubule-based process formation of cultured podocytes--inhibition of ROCK promoted process elongation.

BACKGROUND: Podocytes, renal glomerular visceral epithelial cells, have two kinds of processes, namely major processes containing microtubules (MTs) and foot processes with actin filaments (AFs). The present study investigated how MTs are organized by the Rho-ROCK signal transduction pathway during process formation of podocytes. METHOD: After induction of differentiation, podocytes of the conditionally immortalized mouse cell line were treated with Y-27632, a specific inhibitor of ROCK, and exoenzyme C3, an inhibitor of RhoA, as well as with forskolin whose effects include inhibition of RhoA, in order to inhibit the Rho-ROCK pathway. RESULTS: Inhibition of ROCK significantly enhanced the formation of thick processes containing MT bundles. Y-27632 promoted process formation even in the presence of latrunculin A which disrupts AFs, strongly suggesting that ROCK directly regulates MT assembly. Treatment with Y-27632 increased MT stability, and stabilized MTs preferentially localized in podocyte processes. Moreover, when treated with a combination of Y-27632 and forskolin, and with Y-27632 and C3 as well, podocytes developed not only MT-based thick processes but also AF-based thin projections. CONCLUSIONS: These data indicate a contribution of ROCK in MT organization to promote podocyte process formation, although it was originally thought to regulate AF assembly. AF-based thin projections seem to be induced mainly by inhibition of RhoA and ROCK. The present study reveals a significant role of the Rho-ROCK signal pathway in the reorganization of both MTs and AFs during process formation of podocytes.

[Thyroid uptake error in four different gamma camera systems].

The thyroid uptake of (123)I and (131)I is generally measured by a gamma camera system. We evaluated the error in determining thyroid uptake caused by different methods of calculation among four gamma camera systems with various collimators. We first designed an original thyroid phantom that consisted of the thyroid and a body containing various levels of radioiodine activity. The applications for thyroid uptake equipped in two gamma camera systems performed calculations by the automatic method with background counts not subtracted from the capsule counts. When the size of the rectangular region of interest (ROI) for the capsule was set at 10x8 cm (a typical ROI size for the thyroid), percentages of thyroid uptake as calculated by the manual method with background counts subtracted from the capsule counts and thyroid counts were 52% to 57% when the value was set at 55% for (123)I; and 54.2% and 58.7%, respectively, when the value was set at 60% for (131)I. On the other hand, the percentages of thyroid uptake calculated by the automatic method with the application using two gamma camera systems with non-subtraction of background counts from the capsule counts were 46% and 50.5% when the value was set at 55%; and 49.6% when the value was set at 60%. The values calculated by the automatic method were underestimated as a result of background counts that were not subtracted from the capsule counts. When ROI size for the capsule was set at 4x4 cm, which is slightly larger than the capsule size, even thyroid uptake as determined by the automatic method using the application showed a difference of less 2% from the set values. There was no difference in thyroid uptake among the various kinds of collimators, high-resolution collimators, all-purpose collimators, and a suitable collimator for gamma-ray energy of (123)I.

Purification and characterization of Chinese hamster phosphatidylserine synthase 2.

Phosphatidylserine (PtdSer) in mammalian cells is synthesized through the action of PtdSer synthase (PSS) 1 and 2, which catalyze the conversion of phosphatidylcholine and phosphatidylethanolamine, respectively, to PtdSer. The PtdSer synthesis in intact cells and an isolated membrane fraction is inhibited by exogenous PtdSer, indicating that inhibition of PtdSer synthases by PtdSer is important for the regulation of PtdSer biosynthesis. In this study, to examine whether the inhibition occurs through the direct interaction of PtdSer with the synthases or is mediated by unidentified factor(s), we purified a FLAG and HA peptide-tagged form of Chinese hamster PSS 2 to near homogeneity. The purified enzyme, as well as the crude enzyme in a membrane fraction, was inhibited on the addition of PtdSer to the enzyme assay mixture. In contrast to PtdSer, phosphatidylcholine and phosphatidylethanolamine did not significantly inhibit the purified enzyme. Furthermore, PtdSer-resistant PtdSer synthesis was observed on cell-free assaying of the membrane fraction prepared from a Chinese hamster ovary cell strain whose PtdSer synthesis in vivo is not inhibited by exogenous PtdSer. These results suggested that the interaction of PtdSer with PSS 2 or a very minor protein co-purified with PSS 2 was critical for the regulation of PSS 2 activity in intact cells.

Nuclear localization of beta-catenin in vegetal pole cells during early embryogenesis of the starfish Asterina pectinifera.

Recently, beta-catenin has been reported to control the expression of morphogenetic genes through the Wnt signaling pathway in invertebrate embryogenesis. In this study, the distribution pattern of beta-catenin during starfish embryogenesis was investigated using immunohistochemistry. In 16-cell stage embryos, beta-catenin began to accumulate in some nuclei at the vegetal pole. During the early cleavage stage, the cells expressing nuclear beta-catenin increased in number in the vegetal pole region of the embryos, and the beta-catenin signal increased in intensity in each nucleus. At the blastula stage, signal for beta-catenin was also found in the cytoplasm of the cells with nuclear beta-catenin. At the vegetal plate stage, almost all vegetal plate cells expressed beta-catenin in both the nucleus and cytoplasm. When the embryos developed to early gastrulae, cells with nuclear beta-catenin were restricted to the archenteron tip, and the signal gradually faded in later stages. The localization and temporal change of beta-catenin expression suggests that beta-catenin has a pivotal role in archenteron formation in starfish embryos.

Do the ethanol metabolizing enzymes modify the relationship between alcohol consumption and blood pressure?

BACKGROUND: Several cross-sectional studies have examined whether the relationship between alcohol consumption and blood pressure (alcohol-BP relationship) differs among individuals with different aldehyde dehydrogenase-2 (ALDH2) genotypes, but few studies have examined the association with alcohol dehydrogenase-2 (ADH2), and those have yielded inconsistent results. We examined the potential modulatory effects of ADH2 and ALDH2 genotypes on the alcohol-BP relationship in a cross-sectional sample of a Japanese rural community. METHODS AND RESULTS: The study subjects were 335 randomly selected men aged 40-69 years, who lived in Shiso, a Japanese rural county, in 1999 or 2000. The genetic polymorphisms of ADH2 and ALDH2 were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) methods. The frequencies of ADH21/21 (wild-type), 21/22 (superactive heterozygotes), and 22/22 (superactive homozygotes) were 8.4, 34.9 and 56.7%, respectively; and those of ALDH21/21 (wild-type), 21/22 (inactive heterozygotes), and 22/22 (inactive homozygotes) were 52.8, 40.9, and 6.3%, respectively. A multiple linear regression analysis showed that the relationship between alcohol consumption and diastolic blood pressure was significantly stronger in men with ADH21/21 than those with ADH21/22 or 22/22 (adjusted regression coefficient = 0.0392 versus 0.0113 mmHg for + 1 g ethanol/week, P for difference in slope = 0.018). The strength of the alcohol-BP relationship was similar in all of the ALDH2 genotype groups. CONCLUSION: The alcohol-BP relationship was significantly stronger in men with ADH21/21 than in men with ADH21/22 or 22/22 in this Japanese rural population. This finding was exactly the opposite of what one previous study suggested.

Inclusion of eccentric actions on net caloric cost resulting from isoinertial resistance exercise.

Net O(2) uptake was measured from maximal-effort 3-set, 8-repetition seated leg press protocols on an isoinertial ergometer. Subjects (25 women, 9 men) did 2 workouts each exerting concentric-eccentric (CE) and concentric-only (CO) knee extensor forces to measure work and net caloric cost. Significant (p < 0.05) relationships between work and net caloric cost resulted from CE and CO workouts for the male, female, and total subject sample. Two-way repeated-measures analyses of variance showed CE workouts resulted in significantly higher work but not net caloric cost and values. CE workouts likely relied on the knee extensors series elastic element to perform an additional approximately 3,600 J of eccentric work at no additional net caloric cost. Unlike other exercise modes, maximal-effort eccentric actions on the isoinertial ergometer, as done in the current study, provide no additional net caloric cost and is thus safe to administer to populations in whom metabolic cost is a concern.

A multivariate approach to predicting knee extensor performance.

The impact of two predictor variables (estimated knee extensor fast-twitch fiber percentage, body mass) on performance measures (vertical jump power output, leg press peak angular velocity) were examined. Subjects (25 men, 27 women) performed 5 workouts involving 2 vertical jump, leg press, and 50-repetition isokinetic tests (to estimate knee extensor fast-twitch fiber percentage). Multivariate regression determined the following significant (p < 0.05) vertical jump equations: predicted male power output = -59.3464 + 1.566 (estimated knee extensor fast-twitch muscle fiber percent) + 15.7884 (body mass), predicted female power output = 36.1574 + 3.4248 (estimated knee extensor fast-twitch muscle fiber percent) + 9.8633 (body mass). Leg press peak angular velocity equations were insignificant by gender; thus, pooled data yielded the following: predicted leg press peak angular velocity = 18.6187 + 0.235 (estimated knee extensor fast-twitch muscle fiber percent) + 0.3801 (body mass). Body mass explained more variance for each performance measure.

QSAR evaluation of the Ch'an Su and related bufadienolides against the colchicine-resistant primary liver carcinoma cell line PLC/PRF/5(1).

QSAR analysis has been used to identify the essential structural requirements for increasing the inhibitory activities of selected bufadienolides from the Chinese drug Ch'an Su (and other sources) against the primary liver carcinoma cell line PLC/PRF/5 (PLC) and the derived colchicine-resistant line (COL). The variable substituent domain of the proposed pharmacophore of the bufadienolides was investigated using a Comparative Molecular Field Analysis (CoMFA) approach. A model with considerable predictive ability was obtained. In addition, the CoMFA results agreed well with the pharmacophore bufadienolide model for the parent PLC line proposed earlier.
