Living retinal nematode (filarial-like) destroyed with photocoagulation.

A motile worm creating tracks upon the pigment epithelium of the retina was observed in each of two patients. The fundus findings resembled pseudoretinitis pigmentosa. Unilateral macular degeneration resulted in one patient. The worm was a nematode, probably a filaria. This report represents the third and fourth intraocular filariae found in the United States. To our knowledge, it is the first report of living, intraretinal filarial-like worms destroyed by photocoagulation.

Large suprasellar aneurysms imitating pituitary tumour.

Two patients had large (20--30 mm) suprasellar aneurysms with clinical findings resembling an expanding pituitary tumour. One aneurysm was giant (30 mm). Its delayed diagnosis for six years led to severe chiasmal compression. The other aneurysm was compressing the optic nerve. Visual improvement resulted after surgery.

Peripheral retinal neovascularization in sarcoidosis with thalassemia.

A black youth with peripheral retinal (sea fan) neovascularization had both sarcoidosis and alpha-thalassemia. The possibility that both of these disease entities were responsible for the observed pathology is explored. Most of the pathology present, the stasis, the hypoxia, and the vasoproliferation, is presumed to be due to the sarcoidosis. No definite relationship was established in this case for the alpha-thalassemia, a rare genetic abnormality of hemoglobin production.

Significance of scintillating scotoma of late onset.

Scintillating hemianopic scotomas are usually caused either by migraine or, more rarely, vertebral-basilar insufficiency. We report two patients in whom this symptom was probably caused by platelet microthrombi in pial vessels of the visual cortex. These patients were similar in that they had scintillating scotomas which began after the age of fifty and abnormal platelet aggregability. Since increased platelet aggregability is associated with cerebral vascular accidents and is treatable, older patients who develop teichopsias should have appropriate hematological tests.

Historical perspectives on photocoagulation.

Benjamin Franklin may share a prominent place in the history of photocoagulation. He devised an uncomplicated experiment showing the effect of color on the absorption of light by solids and its conversion into heat motion of the molecules. His experiment and the related experiments of others leading ultimately to the development of photocoagulation techniques are discussed.
