[Optimization of diagnostic and therapeutic programs in modern neonatal surgery].

Tendencies in neonatal surgery research are presented. Based on 187 cases, indications to preventive intestinal stomas creation in the newborns and also time of their closure are regarded. Surgical correction of malformation or treatment of acute process in the abdominal organs with intestinal stoma closing is finished to month 2-3 of childs life. The developed differential-diagnostic criteria of functional and organic lesion of ureteropelvic and ureterovesicular segments based on diuretic sonography permitted one to reveal early age pathology of urinary system and to correct it with surgical methods. The developed algorithm of diagnosis and treatment of newborns with eruct syndrome permits one to determine indications to surgery in 10% infants with gastroesophageal reflux.

[Characteristics of antiviral immunity in children with the atopic form of bronchial asthma].

In atopic bronchial asthma, an earlier "encounter" of children with viruses that cause acute respiratory diseases was established as was mass infection of asthma children with respiratory viruses. Disorders of antiviral immunity in children suffering from atopic asthma primarily manifested themselves in overproduction of antibodies among viruses. The leukocyte migration inhibition test has demonstrated that atopic asthma children are sensitized with respiratory viruses. Sensitization to viruses rises with age whereas the presence of the foci of chronic infection promotes ever higher sensitization of the body with respiratory viruses, bringing about inhibition of leukocyte function.

The effect of using T-activin in the therapy of the newborn with suppurative surgical infection.

A total of 156 newborn infants with suppurative surgical infection (SSI) were observed; 73 of them had sepsis and 83 a severe localized process. In 47 patients with sepsis and 34 with localized infection, T-activin was included in complex therapy while the other infants formed the control group. It has been established that T-activin leads to an increase in the quantity of the active population of T-lymphocytes in the peripheral blood and to enhanced functional activity of T-lymphocytes in the newborn with SSI independent of generalization of the process. Bactericidal activity of circulating phagocytes is improved. The clinical course of SSI is less severe with more pronounced positive changes in the symptoms, hospital stay of the children is shortened, lethality is reduced. The effect of T-activin on the dynamic of the indices of the immune state is more marked in a septic process.

Intertypic recombination in poliovirus: genetic and biochemical studies.

Poliovirus strains of type 1 and type 3 carrying genetically mapped ts mutations and differring in growth response to guanidine have been used to infect HeLa cells. With four heterotypic pairs of the mutants, recombinants with the crossover points between the loci coding for the antigenic properties, on the one hand, and for the sensitivity to guanidine, on the other, have been obtained. The recombinants have been identified on the basis of their phenotypic properties and, in particular, of the pattern of inheritance of unselected markers. One recombinant has been characterized by fingerprinting virus-specific polypeptides. It has been found that the capsid proteins (VP2, VP3, and VP1) of this recombinant originate from the type 3 parent, whereas the nonstructural polypeptides (X, 2, and 4) are inherited from the type 1 parent. Implications of the poliovirus intertypic recombination are discussed.

[Gastro-esophageal reflux esophagitis in infants].

On the basis of their clinical experience the authors consider the main cause of gastro-esophageal reflux-esophagitis in children up to three months of age to be dysfunction of the autonomic nervous system against the background of underdeveloped zone of cardia. Maximum conservative treatment is recommended and a complex of pathogenetic therapeutic measures is proposed. The remote results confirm the rightness of the tactics proposed.
