One hundred and eighty patients with acute myocardial infarction (MI) were followed up. The patients were divided into 3 groups: (1) those receiving glyceryl trinitrate (GTN) (n = 43); (2) those on isosorbide dinitrate (ISDN) (n = 66); (3) those on isosorbide-5-mononitrate (IS-5-MN) (n = 71). In all the groups, the drugs were given by long-term continuous oligovolumic infusion. Following 24 hours of administration, 66.7, 47.3, and 17.1% increases in infusion rates were required in 74.4, 72.7, and 26.8% of the patients from Groups 1, 2, and 3, respectively. All the agents produced a pronounced antianginal effect and resulted in alleviated acute left ventricular failure. The in-hospital mortality rates were 16.2, 16.7, and 12.7% in Groups 1, 2, and 3, respectively. GTN, ISDN, and IS-5-MN caused adverse effects in 4.7, 18.2, and 2.8% of the patients from Groups 1, 2, and 3, respectively.

Functional parameters of the heart ventricles, subendocardial morphology of anterior and posterior walls (AW and PW) of the right and left ventricles (RV and LV) and the state of coronary microvasculature were studied in experimental research on chinchilla mature rabbits at early (1, 2, 3 days) and late (after 7 and 30 days) stages after pulmonary artery branches embolism (PABE) which had been established by means of vaseline oil injection (0.3 ml/kg) in marginal ear vein. It was found that the heart muscle responded as a whole to the RV overload in PABE, with the structural and functional changes of both ventricles. LV damage develops in parallel with the restoration of the RV reserve possibilities and is more significant and prolonged in the LV PW. Positive correlation exists between the blood supply level of both ventricles PW and the interventricular pressure changes.

As many as 43 patients with myocardial infarction were under observation. There were 30 men and 13 women aged 36 to 82 years. Of these, 26 patients had large-focal and 17 transmural myocardial infarction. Perlinganit was injected in the form of 0.1% solution by means of a drug-dosing apparatus (DDA-1). The drug infusion lasted 51.1 +/- 4.8 h on the average, the mean infusion rate was 74.5 +/- 3.4 micrograms/min. After 24 hours it was required that in 32 patients the mean infusion rate be increased 1.5-fold. The hemodynamics was assessed with the aid of tetrapolar chest rheography. Well-defined clinical and hemodynamic effects were revealed. In some patients, the clinical signs of left ventricular insufficiency could be eliminated. Contractile function of the left ventricle returns to normal within the first hours of disease. During perlinganit treatment, the antianginal and antiarrhythmic effects were recorded. The data obtained point to the fact that the use of perlinganit for the treatment of acute myocardial infarction is advisable.
