International cooperation to collect obstetric data.

Reported are preliminary results of an international collection of data initiated by FIGO. Data of 33.000 deliveries in 19 countries, five countries each in South America, Africa, and Asia and four in Europe provided a good basis for comparison and called attention to shortcomings in each individual country and to the need for more progress in obstetric treatment. - Sociological and demographic items in a questionnaire bring into focus the importance of both family planning and sexual education. - The authors consider this data collection a preliminary study for modernisation of Tauffer Statistics.

[Different labour behaviours following induction, using oxytocin and prostaglandin F2 alpha (Enzaprost) (author's transl)].

The authors studied the characteristics of labour induced by means of prostaglandin (Enzaprost F). The results were compared to spontaneous childbirths and to labour induced by means of oxytocin. Increased basal tension of the uterus and higher amplitude of uterine contractions were recorded from the prostaglandin group. Contractions were also analysed for variations in shape. Deformation in response to oxytocin was found to be almost identical to that following prostaglandin. Atypical contractions were found to occur more often in women in whom labour was induced for risk to the foetus and in primiparae, though the number of third-type contractions was lower in such cases. The point is made that betamimetics may be very helpful in getting to grips with incoordinate uterine activity. They were found to stop irregular uterine activity, and re-induction then will be accompanied by regular rhythmicity of labour.

Two modifications of the intrauterine membrane contraceptive device.

The intrauterine membrane (IUM) is an intrauterine contraceptive device designed to be compliant with a range of uterine shapes and sizes and with transient dimensional changes resulting from uterine motility. This compliance was expected to contribute to low rates for removal due to bleeding and pain, but the use of the IUM resulted in heavy bleeding among its early users. Consequently, two modifications of the device were made to decrease bleeding. One group of IUMs was modified by using ethylene vinyl acetate to provide a high level of lateral compliance and another was modified by using a tissue-compatible Hydron coating in order to decrease the inflammatory response of the endometrium. One hundred of each of these modified IUMs were tested along with 100 of the standard IUMs in a double-blind study conducted at the Noi Klinika in Debrecen, Hungary. The findings of this study suggested that these two modifications did not improve the over-all performance of the IUM.

Menstrual regulation by mean of endometrial aspiration.

The results of menstrual regulation performed in 181 cases of supposed pregnancy are described. In 166 cases (92%) pregnancy could be interrupted without dilatation of the os uteri and only in 15 cases (8%) was it necessary to dilate the cervical canal by 8.5--9.0 H units. The majority of pregnancies was interrupted by means of vacuum aspiration and only in 15 cases was curettage performed because of a supposed residue. In the postoperative period slight blood discharge occurred in 2 cases and in 1 case the choriodecidual residue had to be removed by curettage one week after vacuum aspiration. The advantage of the method are particularly remarkable in the interruption of the first pregnancy since it can be carried out without an injury to the cervical canal.

The value of bed rest in twin pregnancies.

The effect of bed rest on the duration of pregnancy, the birth weight of newborns, the frequency of premature birth, and perinatal mortality was examined in 491 twin pregnancies. Cases were divided into two groups - women hospitalized from the 24th-26th week of pregnancy until delivery (rested group, 242 patients) and outpatients (unrested group, 249 patients). The premature birth rate was considerably less for patients in the rested group: in this group 41.0% of the newborns were delivered earlier than the 38th week and 42.9% of them weighed less than 2 500 g, whereas in the unrested group 75.0% were delivered earlier than the 38th week and 77.2% weighed less than 2 500 g. In addition, the perinatal mortality rates observed in the two groups were significantly different. Based on these findings, we conclude that the high premature birth rates and subsequent high perinatal mortality may be considerably reduced if twin pregnancies are diagnosed early and patients are hospitalized from the 26th week until delivery.

Fetal arrhythmia during pregnancy and labour.

The significance of fetal arrhythmia presenting during pregnancy and labour has been examined in 68 patients. The different types of rhythm disorders diagnosed included 61 extrasystoles, 6 atrioventricular blocks and 1 auricular fibrillation. Extrasystoles were not associated with acidosis or organic cardiac disease. Atrioventricular block, though not associated with fetal hypoxia, was occasionally associated with congenital cardiac failure.

[Prevention of respiratory distress syndrome prior to delivery using steroids].

Dexamethason in a total dose of 15 mg was given in 125 cases of premature labour 48 hours before delivery to prevent the development of respiration distress syndrome. Clinically, the incidence of RDS was 14,5% (18 cases) in the treated group, while in the control group it was 27,7% (32 cases out of 119). Owing to RDS two prematures died in the treated group, and 14 in the control one. Steroid prophylaxis was applied prenatally with 69 pregnant women at term who were predisposed to give birth to infants developing RDS (retardation, diabetes mellitus, elective section caesareas, etc.). Five neonates developed RDS and one died of hyaline membrane disease. The authors studied the effect of steroids on carbohydrate metabolism and total oestrogen excretion in the urine. From profile examinations of blood sugar it was concluded that no pathological alterations take place in maternal carbohydrate metabolism due to intravenous or intermittent prolonged administration of Dexamethason. After one single large dose (15 mg) of Dexamethason a reduction of 30-40% was noted in total oestrogen excretion. Return to the initial value was noted on the 11th-12th day. No appreciable changes were observed with prolonged intermittently given Dexamethason. On the basis of their observations the authors consider that steroid therapy applied during labour is efficient in preventing neonatal RDS.
