Examination of the uterine cervix by ultrasound in normal and pathologic pregnancy.

New possibilities of examination of the uterine cervix are provided by sonography in normal and pathologic pregnancy. Basic data of the ultrasonographic anatomy of the non pregnant uterine cervix is presented first: the length of the cervix as well as the diameters at the levels of external and internal os. These data are compared then with those in normal, physiologic pregnancy, and contrasted with those obtained in cases of incompetent cervix. In this group shortening of the cervical length, opening of the internal os and the funnel, or hour-glass-like herniation of the fetal membranes were characteristic findings. The method seems to be suitable for the assessment of the effectivity of cerclage operations for cervical incompetence.

Impact of pregnancy termination on subsequent fertility.

The effects of pregnancy termination on subsequent fertility was studied in a group of 845 patients within a WHO organized multicentre trial. After comparing the index (abortion) and control groups by using the cumulative pregnancy rates per 100 women it was found at 30 months after recruitment to be 96.9 and 98.7, respectively. The difference was not significant statistically.

[Significance of surgical tumor reduction in the therapy of ovarian cancer].

Authors report on their experiences concerning 333 ovarian cancer patients operated on at the Gynecological Department of the Medical University, Debrecen during the last 10 years. They describe the surgical technique applied summarizing in three points its goal: accurate staging, radical tumor debulking, map out residual disease. Characterising the studied population authors give the distribution of patients according to prognostic factors, and postoperative treatment tips. They analyse treatment results according to diameter of residual tumor based on three parameters--the grade of response, duration of remission and surviving. They found that each therapeutic parameter is basicly influenced by the diameter of residual tumor left behind. The best results can be achieved among patients with minimal residual disease. The extent of tumor debulking during the first laparotomy is decising prognosis fundamentally.

[Pretherapeutic chemosensitivity testing of ovarian cancer and possibilities for its clinical use].

Authors report on their experiences concerning in vitro clonogenic assay performed according to Hamburger and Salmon on solid tumor tissue samples (13 cases) and ascites cells (5 cases) obtained from 16 ovarian cancer patient. Out of the 13 solid tumors they got single cell suspension with sufficient number of cells in each case. Colony formation was observed in 10 out of 18 specimens. Dose-response curves were determined after incubation of cells with different concentrations of the following drugs: cis-platin, adriamycin and melphalan. Patients were treated with the CAP combination chemotherapy. The correlation between clinical responses and in vitro sensitivity was evaluated retrospectively. The analysis of the in vitro-in vivo correlation showed that in case of in vitro resistance against two components of the CAP combination the tumor progressed during CAP treatment. In case of in vitro resistance against one component except cis-platin CAP therapy could cause even complete response. By in vitro cis-platin resistance the maximum response with CAP treatment was partial remission.

[Stage distribution and survival rates in cervix cancer at the Gynecologic Clinic of the Debrecen Medical University from 1970 to 1980].

The authors report on the stage distribution and 5-years survival rate of 1158 cervical cancer cases treated from 1970 to 1980 at their center. Comparing these data to those of other institutions according to the 17th volume of Annual Report on the results of treatment in gynecological cancer they found that the stage distribution of their cases is not so favourable as that of other institutions. This unfavourable stage distribution is the consequence of the insufficiency of the mass screening of cervical cancer. Their treatment results characterized by the five year survival rate correspond to the international average.

Effects of hypertensive disorders on the outcome of pregnancy: early findings of maternity care monitoring (MCM) in Debrecen, Hungary.

Debrecen is one of the original testing sites of MCM. This report focuses on all single births having occurred from January, 1978 to June, 1980 (N = 8818). Pregnancy outcome associated with maternal Hypertensive Disorders in Pregnancy (HDP) shall be set against pregnancy outcome among women with No Primary Antenatal Conditions (Control), the respective sample sizes being N = 540 (HDP) and N = 5, 472 women. The findings are given in four sections: (1) Occurrence of HDP; (2) Characteristics other than presence/absence of HDP; (3) Reproductive experience prior to current delivery; and (4) Maternal and infant outcome for the current delivery. A specific look is also given at smoking during current pregnancy. Special attention is called for the primordial importance of prenatal visits, preventive care and proper obstetrical management. The role of the MCM early warning system is emphasized and broader use of the system and rationale is suggested for objective international and intranational comparisons to improve pregnancy outcome for both the mother and infant.
