[Influence of pyloroplasty and pyloric stenosis on motoric and secretory function of the stomach after selective proximal vagotomy--an experimental study (author's transl)].

In conscious fullgrown minipigs simple SPV alone, SPV and pyloric stenosis and SPV and pyloroplasty were performed. After a liquid test meal the motoric and secretory function of the stomach were examined simultaneously by a modified method of intestinal perfusion and aspiration. After simple SPV initially a marked decrease of gastric volume and normal emptying into the duodenum were found. With additional pyloric stenosis no significant change was found. The pyloroplasty lead to an increase of gastric volume and delayed emptying. The acid secretion after feeding reduced by SPV was not changed significantly neither by pyloroplasty nor by pyloric stenosis. The baseline values of serum gastrin were elevated after SPV as well as after SPV in combination with pyloric stenosis or pyloroplasty. After food stimulation there was a delayed increase of gastrin after SPV which differed from that after SPV with pyloric stenosis or pyloroplasty only during the first hour. These results show that after SPV no further improvement of the motoric and secretory function can be achieved by an additional pyloroplasty. Furthermore these findings permit the conclusion that even after SPV with additional artificial pyloric stenosis no delayed gastric emptying occurs and that there is no negative effect postoperatively on the acid secretion and gastrin production.

[A modified Thomas canula to create a chronic pancreatic fistula in minipigs (author's transl)].

The design of a modified Thomas canula is described. It's possible applications for investigation of exocrine pancreatic function and gastric secretion as well as gastric emptying in minipigs are discussed. The advantages of this modified canula are that it can be simply manufactured, that it is well tolerated by the tissue and that there is no risk for the animal and no need for the construction of a pouch.

[The "open-palm" technic in the treatment of Dupuytren's contracture].

We report our experience in the treatment of 39 patients with Dupuytrens' contracture using the "open-palm" technique (McCASH). In this method the palmar fascia is excised through a transverse incision in the distal plamer crease. The wound is left open. The advantages of this method are: no necrosis of the skin, absence of haematomas, closure of other incisions without tension, early motion with little postoperative discomfort. Based on our experience we recommend this method for a selected group of patients.

[Clinical results by selective proximal vagotomy with pyloroplasty (author's transl)].

The results of selective proximal vagotomy and pyloroplasty for duodenal and gastric ulcer in 464 patients over the last 5 years were evaluated in 438 cases. More than 75% of the patients were observed for 2 years after the operation. More than 25% were emergency cases, the incidence of intraoperative complications like splenic lesions or perforation of the esophagus was 3.2%. Postoperative complications like leakage of the pyloroplasty, peritonitis, hemorrhage from the pyloroplasty or disruption of the laparotomy closure occurred in 14,4%. The overall mortality was 4.6%, the elective mortality 1.6%. Recurrent ulcers were seen after 1/2-2 1/4 years with an overall rate of 3.2%. Over half of these cases required relaparotomy. In 5.3% relaparotomy had to be done for peritonitis, GI-bleeding, bleeding from the lesser curvature of the stomach, ileus or carcinoma. The Pentagastrin stimulated gastric secretion remained constantly reduced for more then 2 years in over 60%. Following the Visick-grading system the results were good in 61-65% of the patients and bad in 16-22% depending on the time of observation.

[Ganglions a rare cause of isolated peripheral nerve lesions].

Extra- and intraneural ganglionic cysts rarely involved peripheral nerves. They are found in the neighbourhood of large joints. The ulnar nerve is affected most often by extraneural cysts at the wrist. Intraneural cysts prefer the peroneal nerve at the tibiofibular joint. Ganglionic cysts as a cause of peripheral nerve damage are often overlooked, mainly because they are not considered as a cause of pain, paresis or sensory changes. Surgical treatment is recommended.

[Cholesteatosis of gallbladder (author's transl)].

741 cholecystektomies were examined for cholesterosis. In 92% gallstones were present and in 8% there were no stones. Cholesterosis of the gallbladder was found in 0,9% of 4417 autopsies. It was combined with gallstones in 25% and in 75% it was not. There was no difference concerning the clinical symptoms between cholesterosis with and without stones in the surgical patients. Cholesterosis without stone presented the same picture as gallstones. In the autopsy material however no typical symptoms could be found indicating either cholesterosis with gallstones or without. In the histological examination 80,5% of the cases with gallstones in the surgical and autopsy group had inflammatory changes of the gallbladder wall whereas only 22% of the cases with stoneless cholesterosis showed these alterations. Fibrosis of the gallbladder wall however was found in 86,1% in cases with cholesterosis without stones and in 88% in cases with stones. From these findings the conclusion is made that cholesterosis is no well defined disease. Clinical symptoms seem to appear only when other factors, such as stones, inflammation or functional disturbances probably because of fibrosis of the wall, become manifest in addition.

[Duckbill fractures of the tuber calcanei, etiology and treatment].

This report is of two cases of tuber calcanei fractures (fractures of group I according to Böhler). In one case the fracture occured after direct trauma, in the other after an uncorrdinated maximal contraction of the triceps muscle. The different origins of this type of fracture are discussed and the reported cases in the literature are reviewed. Both fractures were treated by osteosynthesis. Immediate operation is indicated because problems in the surrounding soft tissues are to be expected.

[Intraneural ganglion in the peroneal nerve].

Report of a case of an intraneural ganglion in the peroneal nerve. Reviewing the literature this rare case of peroneal nerve palsy is discussed. The therapy consists in an operative decompression of the nerve by excision or incision of the ganglion cyst.
